한국에서의 담뱃갑 경고그림 도입과정 분석연구: 국회 보건복지위 회의록 분석을 중심으로 (Introduction Process of the Tobacco Graphic Health Warning Law in Korea: Analysis on the National Assembly Minutes)

  • Graphic health warning on the tobacco product package is a cost-effective tobacco control policy to convey information on harmful effect of tobacco use to health, and it is known not only to motivate smokers to quit but also to deter adolescents from start smoking. In case of Korea, amendments to National Health Promotion Act requiring implementation of graphic health warning had been submitted 13 times, from 2002 to May 2015. In May 2015, the amendment had been approved by the National Assembly and it enters into force on December 23, 2016. This research analyzed the discussions from Health and Welfare Committee of the National Assembly during the implementation of the graphic health warning in order to study decision making process of legislators. Study found that there was a shift from a general opposition on implementing graphic health warning at first to a harsh conflict over relaxation of the regulation once discussing the implementation in earnest. Particularly, while the group supporting the implementation of the graphic health warning or opposing relaxation advocated the amendment with scientific and knowledge-based evidences including the World Health Organization Framework Convention on Tobacco Control, the group opposing the adoption of the amendment itself or suggesting relaxation tended to defend their position with empathy on smokers or tobacco industries.

특성화 고등학교 흡연 청소년의 담뱃갑 경고그림 위협인식, 심리적 저항이 금연의도에 미치는 영향 (The influence of threat perception and psychological reactance to cigarette warning labels on intention to quit smoking among smoking adolescents -with focus on industrial specialized high school students)

  • Purpose: The aim of this study was to investigate the influence of threat perception and psychological reactance to graphic health warnings on cigarette packages on intention to quit smoking among smoking adolescents. Methods: The participants were 185 smoking adolescents attending two industrial specialized high schools, who had witnessed graphic health warnings on cigarette packages within the previous 30 days. Collected data were analyzed using descriptive statistics, the t-test, one-way analysis of variance, Pearson's correlation coefficients, and hierarchical multiple regression analysis using SPSS/WIN Ver. 25.0. Results: The results showed that participant threat perception (𝛽=.14, p=.037) after witnessing a cigarette warning message had a positive influence on intention to quit smoking. On the other hand, psychological reactance (𝛽=-.23, p=.001) showed a negative influence on intention to quit smoking. Conclusion: When designing and developing a message for smoking adolescents, customized messages are required to reflect teenagers' characteristics and raise threat perception and lower psychological reactance. In addition, we propose a future study be conducted to determine the influences of various psychological determinants, including self-efficacy and skills, on threat perception and psychological reactance to graphic health warnings on cigarette packages among smoking adolescents.

Potential Impact of Graphic Health Warnings on Cigarette Packages in Reducing Cigarette Demand and Smoking-Related Deaths in Vietnam

  • Two years after implementation of the graphic health warning intervention in Vietnam, it is very important to evaluate the intervention's potential impact. The objective of this paper was to predict effects of graphic health warnings on cigarette packages, particularly in reducing cigarette demand and smoking-associated deaths in Vietnam. In this study, a discrete choice experiment (DCE) method was used to evaluate the potential impact of graphic tobacco health warnings on smoking demand. To predict the impact of GHWs on reducing premature deaths associated with smoking, we constructed different static models. We adapted the method developed by University of Toronto, Canada and found that GHWs had statistically significant impact on reducing cigarette demand (up to 10.1% through images of lung damage), resulting in an overall decrease of smoking prevalence in Vietnam. We also found that between 428,417- 646,098 premature deaths would be prevented as a result of the GHW intervention. The potential impact of the GHW labels on reducing premature smoking-associated deaths in Vietnam were shown to be stronger among lower socio-economic groups.

청소년 흡연자의 금연시도 관련 요인 (Factors Related Smoking Cessation Attempts among Teenage Smokers)

  • 이 연구는 흡연 청소년을 연구대상으로 하여 흡연에 대한 부정적 인식에 기여할 것으로 예상되는 담뱃갑 경고 그림과 금연 시도와의 연관성을 분석하였다. 2018 청소년 건강행태 온라인 조사 자료를 활용하였으며, 현재 흡연자인 청소년 3,722명을 연구 대상으로 선정하였다. 변수 측정을 위해 인구사회학적, 건강관련, 흡연 관련 요인들을 보정한 후 다변수 이항 로지스틱 회귀분석을 시행하였다. 현재 흡연자인 청소년의 담뱃갑 경고 그림 인지율은 84.7%였으며, 그 중 금연 시도율은 72.8%이었다. 다변수 이항 로지스틱 회귀분석 결과, 청소년 흡연자의 금연시도와 담뱃갑 경고그림 인지 여부는 유의한 관련이 있었으며, 학년, 주관적 학업성적, 스트레스 인지, 담배 구매 용이성도 유의한 변수로 나타났다. 이러한 결과를 토대로 향후 흡연 청소년들이 쉽게 인지할 수 있는 담뱃갑 경고그림 제작이 이루어져야 할 것이다.

연결망 분석을 활용한 우리나라 금연연구 동향분석 (A Social Network Analysis of Research Key Words Related Smoke Cessation in South Korea)

  • Background: The purpose of this study is supposed to figure out the keyword network from 2009 to 2018 with social network analysis and provide the research data that can help the Korea government's policy making on smoking cessation. Methods: First, frequency analysis on the keyword was performed. After, in this study, I applied three classic centrality measures (degree centrality, betweenness centrality, and eigenvector centrality) with R 3.5.1. Moreover, I visualized the results as the word cloud and keyword network. Results: As a result of network analysis, 'smoking' and 'smoking cessation' were key words with high frequency, high degree centrality, and betweenness centrality. As a result of looking at trends in keyword, many study had been done on the keyword 'secondhand smoke' and 'adolescent' from 2009 to 2013, and 'cigarette graphic warning' and 'electronic cigarette' from 2014 to 2018. Conclusion: This study contributes to understand trends on smoking cessation study and seek further study with the keyword network analysis.

Changes in Vietnamese Male Smokers' Reactions Towards New Pictorial Cigarette Pack Warnings Over Time

  • Printing of pictorial health warnings (PHWs) on cigarette packages became obligatory by the Vietnam Law on Prevention and Control of Tobacco Harm in May 2013. Literature from high-income countries suggests that PHWs motivate smokers to quit smoking although their long-term effects have been questioned due to reduction of impact over time. This study aimed to assess the salience of PHWs and smokers' reactions towards PHWs over time. In May 2014 and May 2015, a cross-sectional questionnaire-based household survey was administered to respectively 1,462 and 1,509 Vietnamese male smokers aged 18 to 35. The result showed that salience of the PHWs 2 years after the implementation was higher than at the point of 1 year after the implementation. The proportion of respondents who tried to avoid noting the PHWs was reduced from 35% in wave 1 to 23% in wave 2. However, "Tried to avoid looking/thinking about the PHWs" increased 1.5 times the odds of presenting quit intention compared to those respondents who did not try to avoid looking/thinking about the PHWs (OR=1.5; 95%CI: 1.2-2.0). In conclusion, avoidance regarding PHWs may not work as a barrier when aiming at a higher level of quit intention. Salience of the PHWs may increase in the period shortly after their introduction onto packs but can be expected to decrease with time. In other words, it might be advisable to change or renew PHWs after a period of implementation to maintain their beneficial effects.
