유방 염증성 질환의 세침흡인소견 (Fine Needle Aspiration Cytologic Findings of Inflammatory Breast Diseases)

  • Fine needle aspiration of the breast is an important diagnostic tool in malignant lesions, but is also useful in differentiation of inflammatory breast diseases mimicking carcinoma clinically and radiologically. Recently, the authors have experienced eight biopsy-proven cases of chronic inflammatory diseases of the breast, which consisted of 4 cases of duct ectasia, 2 cases of fat necrosis, and a case of tuberculous mastitis and granulomatous mastitis respectively. Their cytologic features mainly based on the components and the relative frequency of inflammatory cells were evaluated for differential diagnosis of chronic inflammatory breast diseases. The results are as follows; 1. In cases of duct ectasia, varying amount of neutrophils, mononuclear leukocytes, histiocytes and multinucleated giant cells were intermixed with benign epithelial cell clusters. 2 Abundant fat tissue fragments were diagnostic for fat necrosis. Histiocytes and mononuclear cells were main components but not rich, and neutrophils and giant cells were infrequently observed. 3. Characteristic granulomas composed of epithelioid cells, mononuclear leukocytes and Langhans' type giant cells and lymphocytic infiltrates were conspicuous in tuberculous mastitis, and occasionally neutrophils, necrotic materials and epithelial cell clusters were found 4. In granulomatous mastitis, epithelioid cell granulomas were also noted but numerous neutrophils and histiocytes were intermingled within or outside the granulomas.

Bayesian Value of Information Analysis with Linear, Exponential, Power Law Failure Models for Aging Chronic Diseases

  • The effective management of uncertainty is one of the most fundamental problems in medical decision making. According to the literatures review, most medical decision models rely on point estimates for input parameters. However, it is natural that they should be interested in the relationship between changes in those values and subsequent changes in model output. Therefore, the purpose of this study is to identify the ranges of numerical values for which each option will be most efficient with respect to the input parameters. The Nonhomogeneous Poisson Process(NHPP) was used for describing the behavior of aging chronic diseases. Three kind of failure models (linear, exponential, and power law) were considered, and each of these failure models was studied under the assumptions of unknown scale factor and known aging rate, known scale factor and unknown aging rate, and unknown scale factor and unknown aging rate, respectively. In addition, this study illustrated developed method with an analysis of data from a trial of immunotherapy in the treatment of chronic Granulomatous disease. Finally, the proposed design of Bayesian value of information analysis facilitates the effective use of the computing capability of computers and provides a systematic way to integrate the expert's opinions and the sampling information which will furnish decision makers with valuable support for quality medical decision making.

악하선에 발생한 방선균증 (Actinomycosis of Submandibular Gland)

  • Actinomycosis is defined as a chronic, specific, suppurative, granulomatous disease caused mainly by the anaerobic, gram positive organism, Actinomyces israelii. Actinomycosis in the salivary gland is a rare disease that is caused by an inhabitant of the normal flora. We report the case of the actinomycosis of submandibular gland. A 53-year old man presented with the swelling on left submandiblar area. The lesion was not painful but had been increasing for about 10 days. In the CT view, the internal portion of the mass showed homogeneous moderate signal. The mass had continuities with the inferior portion of the left enlarged submandibular gland. In the MRI, there was a mass that showed a buldging pattern inferiorly in the left submandibular gland without bony invasion sign. The biopsy shows the colony of special organism. Many filaments are discovered with clubbed ends diffused from center of colony. We diagnosed this disease as actinomycosis in the submandibular gland by the postoperational biopsy.

일차성 면역결핍질환의 최신 지견 (Recent advance in primary immune deficiency disorders)

  • The immune system is comprised of cells and molecules whose collective and coordinated response to the introduction of foreign substance is referred to as the immune response. Defense against microbes is mediated by the early reaction (innate immunity) and the late response (adaptive immunity). Innate immunity consists of the epithelial barrier, phagocytes, complement and natural killer cells. Adaptive immunity, a more complex defense reaction, consists of activation of later-developed lymphocytes that, when stimulated by exposure to infectious agents, increase in magnitude and defensive capabilities with each successive exposure. In this review we discuss recent advances in important primary immune deficiency disorders of innate immunity (chronic granulomatous disease, leukocyte adhesion deficiency) and adaptive immunity (severe combined immune deficiency, Wiskott- Aldrich syndrome).

호흡기 질환 환자로부터 분리한 Scophulariopsis brevicaulis에 관한 연구 (Studies on the Scophulariopsis brevicaulis Isolated from the Respiratory Disease Patients)

  • It has been reported that the pulmonary mycosis generally results from saprophytic colonization of pre-existing lung cavities usually due to pulmonary tuberculosis etc. Culture of homogenized tissues yield pure colony of slightly brown on the Sabouraud dextrose medium. The fungus was identified as the Scophulariopsis brevieaulis which had been rarely reported in pulmonary fungal infection. Histopathological examination of excised tissues revealed a chronic granulomatous inflammation and many branched hyphal mass. Susceptibility tests were tested with ketoconazole, griseofulvin, nystatin, and amphotericin B. Of these antifungal agents, ketoconazole and griseofulvin proved to be most effective and minimum inhibitory concentration was $10{\mu}g/ml,\;50 {\mu}g/ml$, respectively.

Empyema Necessitatis in a Patient on Peritoneal Dialysis

  • Empyema necessitatis is a rare complication of an empyema. Although the incidence is thought to be decreased in the post-antibiotic era, immunocompromised patients such as patients with chronic kidney disease on dialysis are still at a higher risk. A 56-year-old woman on peritoneal dialysis presented with an enlarging mass on the right anterior chest wall. The chest computed tomography scan revealed an empyema necessitatis and the histopathologic findings revealed a granulomatous inflammation with caseation necrosis. The patient was treated with anti-tuberculous medication.

수부 굴곡건에 발생한 결핵성건초염 (Tuberculous Tenosynovitis of Flexor Tendon of the Hand)

  • Purpose: The sheath of tendon is uncommon site of tuberculous involvement as compared to other parts of the body. Especially, tuberculous tenosynovitis affecting flexor tendon of the hand is a rare condition. In recent years, furthermore, the incidence of tuberculosis is increasing in our country. Tuberculous tenosynovitis is a chronic, slowly destructive disease, which is difficult to diagnosis before operation, but can be definitively diagnosed by the pathologic microscopic examination. Early detection and surgical excision combined with antituberculous medication is important. We report a rare case of tuberculous tenosynovitis of the flexor tendon of the hand. Method: A 12-year-old woman presented with a painless, nontender mass on palmar side between distal interphalangeal joint and proximal area of metacarpophalangeal joint of the left third finger. We had surgical excision of the involved flexor tendon sheath and studied histopathologically. Result: The histopathological findings were chronic granulomatous inflammation with caseating necrosis consistent with tuberculosis. We started antituberculous medication. Conclusion: Tuberculous tenosynovitis is a rare condition, especially involving on the flexor tendon of the hand. But because of increasing tendency of tuberculosis, it is important to differentiate it from other tumors of the hand.

  • Actinomyces is a part of the normal oral flora, but under certain circumstances it may become pathogenic. Actinomycosis is a chronic granulomatous infective disease caused by microaerophilic Gram-positive bacteria of the genus actinomyces. It can involve almost any system, but principally affects the head and neck. Because the lesions in the submandibular region and the angle of the jaw give the face a swollen, indurated appearance, actinomycosis of mandible can be easily misdiagnosed in its acute or early state of infection. In these cases the disease usually presented as a swelling suggestive of an abscess or mimicking a neoplasm. The yield from standard cultures was poor and repeated sampling and anaerobic culture may be needed to obtain a positive culture. So actinomycosis should always be considered in a differential diagnosis of all infections of the cervicofacial area. Diagnosis of actinomycosis is made based on the histopathology, the clinical presentation and past dental history. We experienced a case of actinomycosis in the masseter muscle and present the case with review of literature.

만성육아종질환 환자 단핵구에서 TNF-α 자극에 의한 IL-8과 IL-10의 발현 양상 (TNF-α stimulated IL-8 and IL-10 expression in monocytes from patients with chronic granulomatous disease)

  • 목적: 만성 육아종 질환은 백혈구 내에서 반응성 산소 대사물을 생성하는 효소인 NADPH oxidase의 유전적인 결함으로 인하여 반복적이고 치명적인 세균 감염과 진균 감염이 발생되는 유전성 면역 결핍 질환이다. 만성적인 염증 상태의 원인과 염증성 육아종 형성의 기전은 잘 알려져 있지 않으나 만성육아종질환의 염증 조절 부전과 관련이 있을 것으로 생각된다. 본 연구는 만성육아종질환 환자의 단핵구에서 $TNF-{\alpha}$의 자극으로 발현되는 IL-8과 IL-10을 측정하여 염증성 시토카인과 항염증성 시토카인의 발현 양상과 만성육아종질환 환자의 만성적인 염증 상태와의 관련성을 알아보고자 하였다. 방법: 만성 육아종 질환 환자 8명과 건강 대조군 10명의 말초 혈액에서 magnet cell isolation kit를 이용하여 단핵구를 분리하고, $TNF-{\alpha}$ (10 ng/mL)로 6시간 동안 자극한 후 실시간 정량적 중합효소 연쇄반응과 ELISA법으로 IL-8과 IL-10의 발현 정도를 측정하였고, 건강 대조군의 단핵구에 항산화제를 전처리한 후 $TNF-{\alpha}$로 자극하여 IL-8과 IL-10의 발현 정도를 측정하였다. 결 과: 만성육아종질환 환자에서 $TNF-{\alpha}$ 자극에 의한 IL-8의 발현 양상은 건강 대조군과 달리 최고 발현치가 60분 정도 늦게 관찰되었고, IL-8의 발현도 자극 시간이 경과하여도 지속되었다. 그리고 건강 대조군의 단핵구에 항산화제를 처리하여 만성육아종질환과 유사한 조건하에서 IL-8을 측정하였을 때 항산화제를 처리한 군에서 처리하지 않은 대조군에 비하여 IL-8의 발현이 증가하였고, catalase와 apocynin을 처리한 군에서 DPI를 처리한 군보다 IL-8의 발현이 증가하였다. $TNF-{\alpha}$ 자극에 의한 IL-10의 발현도 IL-8의 발현 양상과 유사하게 지연 반응과 과발현이 관찰되었다. 그러나 건강 대조군의 단핵구에 항산화제를 처리하였을 때의 IL-10 발현은 처리하지 않은 군과 큰 차이를 보이지 않았다. 결 론: 만성육아종질환 환자의 단핵구에서 $TNF-{\alpha}$ 자극에 의하여 염증성 시토카인인 IL-8과 항염증성 시토카인인 IL-10의 지연 반응과 과발현이 동시에 관찰되었으며, 건강 대조군의 단핵구에 항산화제를 처리하였을 때 IL-8의 발현 증가가 관찰되었다. 이는 만성육아종질환의 만성 염증 상태가 시토카인의 생성 조절의 부전과 연관이 있고, NADPH oxidase의 결함에 의한 반응성 산소대사물의 생성 부전과도 연관되어 있음을 알 수 있다.
