국민건강증진을 위한 보건교육 전문인력 활용방안 (Utilization of Health Education Professionals for National Health Promotion Program)

  • The National Health Promotion Act passed in 1995 was a milestone for initiating a national and local health promotion program in Korea. And since then local governments and health centers have been developing and providing health promotion programs for the community population. To apply the effectiveness of community health promotion program, it is important to understand the key issue related to health education and the role of health education personnel. The purpose of this study was to define the responsibility and competency of health education specialist, and to develop the activity areas of health promotion program in Korea. Those who provide the service for health promotion and health education should be properly qualified and professionally trained. However, the skills and responsibilities of those who are in charge of providing health education program have not yet been clearly defined in Korea because the areas of health promotion and health education are composed of multi-academic fields. In case of United States, health education specialist is being developed through professional preparation in colleges and graduate schools, and certified through the examination. Also health education specialist is in charge of the planing, implementing and evaluation of health education program in school, hospital, health center, workplace and health food company. Therefore it is important to develop the programs to train and certify health education specialist. Also to extend the activity areas, the government should support continuously program development for health promotion and health education personnel.

대법원 판례로 살펴본 무면허 한방의료행위의 법리 (The Legal Aspect of Supreme Court Cases on the Unlicensed Medical Practice of Korean Medicine)

  • Background and Aim : Health care and cosmetics as well as quality of life is now a matter of concern and many categories of complementary and alternative medicine fall into the territory of the medical practice of Korean medicine. Accordingly, penalties are being taken for unlicensed medical practices of Korean medicine in so called complementary and alternative medicine area. There is a possibility of violating the law for the public part because it is not clearly stipulated in the law as to what is a licensed medical practice. Materials and Method : The significance of the Medical Service Act and the Act on Special Measures for the Control of Public Health Crimes were reviewed, and the related supreme court cases were discussed upon the legal aspect of processing the unlicensed medical practice of Korean medicine. The legal information was provided from the National Law Information Center of the Ministry of Government Legislation, and the information websites of the Supreme Court and the Constitutional Court. Results : The concept of medical practice, which is essential in judging the case of unlicensed medical practice, is 'prevention and treatment of diseases through diagnosis, examination, prescribing, medication, or surgical procedures based on medical expertise', and the 'acts that may result in harm and injury of health unless performed by a medical person'. With respect to the medical practice of Korean medicine, the concept includes 'prevention and treatment of diseases using the principle of traditional Korean Medicine'. Conclusions : The concept of medical practice should be clearly stipulated in the law for the control over the unlicensed medical practices of Korean medicine. And it is important to move from the current concept of medical person-oriented medical practice emerging from the national system of healthcare control, to a concept that can accept the era of health managing-oriented medical environment and the co-governance of the healthcare providers and consumers for the future.

Current status of opioid prescription in South Korea using narcotics information management system

  • Background: Recognizing the seriousness of the misuse and abuse of medical narcotics, the South Korean government introduced the world's first narcotic management system, the Narcotics Information Management System (NIMS). This study aimed to explore the recent one-year opioid prescribing patterns in South Korea using the NIMS database. Methods: This study analyzed opioid prescription records in South Korea for the year 2022, utilizing the dispensing/administration dataset provided by NIMS. Public data from the Korean Statistical Information Service were also utilized to explore prescription trends over the past four years. The examination covered 16 different opioid analgesics, assessed by the total number of units prescribed based on routes of administration, type of institutions, and patients' sex and age group. Additionally, the disposal rate for each ingredient was computed. Results: In total, 206,941 records of 87,792,968 opioid analgesic units were analyzed. Recently, the overall quantity of prescribed opioid analgesic units has remained relatively stable. The most prescribed ingredient was oral oxycodone, followed by tapentadol and sublingual fentanyl. Tertiary hospitals had the highest number of dispensed units (49.4%), followed by community pharmacies (40.2%). The highest number of prescribed units was attributed to male patients in their 60s. The disposal rates of the oral and transdermal formulations were less than 0.1%. Conclusions: Opioid prescription in South Korea features a high proportion of oral formulations, tertiary hospital administration, pharmacy dispensing, and elderly patients. Sustained education and surveillance of patients and healthcare providers is required.

군수품의 적정 품질보증형태 분류를 위한 제언 (Suggestion for Proper Quality Assurance Type Classification Criteria of Military Supplies)

  • 군수품의 품질보증을 담당하는 국방기술품질원에서는 크게 4가지 형태로 군수품을 구분하여 품질보증 활동을 수행하고 있다. 이러한 군수품에 대한 품질보증형태 분류는 품질보증기관의 입장에서만 결정되었기에 다른 이해관계자라 할 수 있는 방위사업청, 제조업체 및 수요군에서 요구되는 분류 기준과 상이할 수 있다. 이에 본 논문에서는 군수품에 대한 효율적인 조달 및 생산, 품질보증을 위한 적정 군수품 품질보증 형태 분류 기준을 제시하고자 하였다. 이를 위해 군수품에 대한 정부 품질보증기관인 기품원 뿐만 아니라 정부 조달물자의 품질확인을 담당하는 한국조달연구원의 품목 분류제도에 대해 살펴보았다. 또한 방사청 등 계약 및 제조, 품질보증, 수요군 등 모든 관련기관 입장에서의 적절한 군수품 분류기준을 정의하고 분석하였으며, 현 기준에 따른 군수품 분류 데이터 현황을 파악하였다. 아울러 제시된 품질보증형태 분류 기준을 활용한 적용사례 역시 제시하였다. 끝으로 본 연구에서 제시된 품질보증 형태 분류 기준을 적용한 차등화된 정부 품질보증 활동을 통해 업무 효율성 개선을 통한 군수품 품질향상에 기여할 것으로 판단된다.

2008 한국 성인을 위한 식생활목표와 식생활지침 (2008 Dietary Goals and Dietary Guidelines for Korean Adults)

  • Recently the Ministry of Health and Welfare, Republic of Korea, announced the “Dietary Guidelines for Korean Adults (DGKA)”, which includes ten Dietary Goals, six Dietary Guidelines, and twenty-three Action Guidelines. DGKA are developed as the revision of the 2003 Dietary Guidelines for Koreans, targeting adult population. Dietary Guidelines are developed for general purpose as well as for different age groups. They are revised periodically to accommodate changes in diet and health problems of the population. The process of developing new DGKA can be summarized as 1) selection of focus areas, 2) analysis and review of available data for each area selected, and 3) derivation of guidelines based on the analyzed data, and 4) finalizing the guidelines after open discussions among the experts and general public. Five focus areas were selected by examining the Nutrition Goals of the Health Plan 2010 of Korea, soliciting proposals from the experts in the related fields, and reviewing existing and international guidelines. Five areas selected were 1) adequate intake of nutrients and foods, 2) balance of energy intake and physical activities, 3) alcohol intake, 4) food security and nutrition service, and 5) food safety. Adequacy of nutrient and food intakes of the Korean adult population was assessed using 2005 Korea National Health and Nutrition Examination Survey (KNHANES) data. Newly developed Dietary Reference Intakes for Koreans were used as reference values to assess the prevalence of inadequacies and excesses in nutrient intakes. Energy balance was examined with energy intake of 2005 KNHANES survey and results of physical activity questionnaire in the survey. Alcohol intake was also examined using 2005 KNHANES results of dietary intakes as well as the results of questionnaire survey on alcohol intakes. Food security, nutrition services, and food safety were analyzed using various government data and published results on the issues. Ten Dietary Goals and six Dietary Guidelines were developed after data analysis and were subjected to reviews of experts and general public. The final DGKA are: 1) Eat a variety of foods from each food group, 2) Increase physical activity and maintain healthy weight, 3) Eat proper amount of clean foods, 4) Avoid salty foods and try to eat foods with bland taste, 5) Avoid foods with high fat contents and deep-fried foods, and 6) When you drink alcohol, limit the amount. Twenty-three action guidelines are developed in order to achieve these guidelines in actual diet and life among the population. The government is disseminating the guidelines with “337” slogan and emblem. “337” indicates everyone should practice “3” guidelines of promoting good eating practice, “3” guidelines to limit or decrease in your diet, and you should practice them for “7” days a week. The guidelines will be useful in promoting healthy food habits and good nutritional status which will result in decrease nutrition related health problems in Korea.

국내 경제활동 인구의 직업유형별 적정수면과의 연관성 (Association between job types of economically active population and sleep appropriateness among South Koreans)

  • Background: As of 2016, average Koreans sleep 7 hours and 42 minutes, the lowest figure among Organization for Economic Cooperation and Development(OECD) countries, and the number of people with sleep disorders reached 561,000. Accordingly, the government has promoted the provision of 'Multiple Sleep Test' to strengthen the diagnosis service for patients with 'sleep disorder' in july 2018. As a result, healthcare costs for patients with sleep disorder is on the rise every year. In this study, we utilized 'Appropriate Sleep' criteria of United States's National Sleep Foundation(NSF) then investigated Korean's sleep pertinence using 「7th National Health and Nutrition Survey for 2016-2018」 by different occupational type, demographic characteristics, socio-economic characteristics, and health behaviors. Methods: We performed descriptive analysis to examine differences of sleep appropriateness by various sample characteristics. Multivariate logistic regression models were used to examine sleep appropriateness by occupational type and other variables. We also analyzed subgroup models to investigate. Results: As a result, a total of 1,948 (18.37%) study subjects experienced in-appropriate sleep. Results of the Multivariate logistic regression analysis revealed that blue color group had a higher odds ratio (OR) for experiencing in-appropriate sleep (OR=1.179). In addition, the odds ratio of experienced in-appropriate sleep among the elderly aged 70 and over was 2.698, and the odds ratio of the overstressed group was 1.299. Furthermore, sub-group analysis showed that blue color job of female(Or=1.334), high school or below(OR=1.404), divorce/death/separation(OR=2.039), 25%ile-50%lie income group(OR=1.411) more likely experienced in-appropriate sleep. Conclusion: Growing sleep disorder patients and related health care costs are expected. Government should apply detailed 'total periodic sleep disorder management policy' including pre-consultation, examination, diagnosis, treatment, post-consultation, self-management especially to vulnerable population that this study found.

사회안전망과 지역사회주민의 안전생활만족의 관계: 사회안전망 척도개발과 적용 (The Relationship Between Social Security Network and Security Life Satisfaction in Community Residents: Scale Development and Application of Social Security Network)

  • 본 연구의 목적은 사회안전망 측정도구를 개발하여 타당도와 신뢰도를 검증하고 이를 적용 하여 지역사회주민의 안전생활만족과의 관계를 규명하는데 있다. 이 연구는 2013년 서울시 시민들을 모집단으로 설정하여 집락무선표집법을 이용해 최종분석에 사용한 자료는 203명이다. 사회안전망 측정도구는 문헌고찰, 개념적 정의 및 설문지 초안 작성, 전문가 회의, 예비검사 및 본 조사, 설문지의 타당도 및 신뢰도 검증 등의 과정을 통하여 개발되었다. 이 연구에서는 설문지의 타당도를 검증하기 위해 전문가 회의를 하였으며, 탐색적 요인분석을 통해 범죄예방설계, 거리 CCTV시설, 지역자율방범활동, 지자체안전교육, 경찰치안서비스, 민간경비서비스 등의 6개 요인을 추출 하였다. 통계처리는 SPSSWIN 18.0 프로그램을 활용하여 빈도분석, 일원변량분석, 요인분석 및 신뢰도 분석, 상관분석, 다중회귀분석 등을 실시하였다. 결론은 다음과 같다. 첫째, 사회안전망 척도의 타당도는 매우 높다. 즉, 사회안전망 구성 요인은 범죄예방설계, 거리 CCTV시설, 지역자율방범활동, 지자체안전교육, 경찰치안서비스, 민간경비서비스 등으로 탐색되었으며, 범죄예방설계 요인의 설명력이 가장 높다. 둘째, 사회안전망 척도의 신뢰도는 매우 높다. 즉, 사회안전망은 문항과 영역간, 문항과 전반적 사회안전망간의 상관정도가 매우 높으며, 내적일관성 신뢰도 Cronbach's ${\alpha}$값이 .865 이상이다. 셋째, 사회안전망은 지역사회주민의 안전생활만족에 영향을 미친다. 즉, 범죄예방설계, 경찰치안서비스가 체계적으로 구축 된다면 시민들의 사회불안감은 감소한다.

공공임무용 드론의 효과적 활용방안에 관한 연구 - 실종자 수색용 드론을 중심으로 - (A Study on the Effective Utilization of Public Service Drones - Focused on Search and Rescue Drones -)

  • 군사용으로 개발되었던 드론은 취미, 레저뿐만 아니라 방송촬영, 공공분야까지 사용분야가 급속도로 확대되고 있으며, 네팔로 봉사활동을 갔다가 눈사태로 실종된 한국인 교사 4명을 찾기 위해 수색용 드론이 투입되기도 하였다. 정부는 드론 산업을 육성하기 위하여 정책적으로 공공분야에서의 드론의 활용을 장려하고 있으며, 향후 시장도 확대될 것으로 예상된다. 본 연구에서는 공공부문에서 드론의 활용이 확대되고 있는 시점에서 공공기관의 드론 활용에 대한 개선방안과 구체적 이용방향을 제시하고자 하였으며, 이를 위하여 관련 전문가에 대한 인터뷰, 공공기관의 드론 운용규정 분석 및 경찰의 실종자 수색용 드론 운용사례를 검토하였다. 우선 드론의 도입 이전에 전문성을 갖춘 인력을 확보하고 충분한 검토를 통해 드론의 사양 등을 결정하고, 드론의 도입에 있어서도 면밀한 검사가 이루어진 후에 임무에 투입이 되어야 할 것이다. 다음으로 드론 운용에 대한 근거규정을 마련하고 이에 따른 매뉴얼을 작성할 필요가 있다. 마지막으로 드론 비행은 추락의 위험성이 있으며, 특히 공공부문에서의 드론은 목적 달성을 위하여 취미용 드론에 비하여 대형화 전문화되어 있기 때문에 드론 운영의 전담인력 확보가 필수적이다. 본 연구는 경찰청의 실종자 수색용 드론에 대해서만 구체적인 분석을 실시하였으며, 인터뷰 대상자도 적다는 한계가 있다. 후속 연구에서는 여러 공공기관을 대상으로 분석을 실시할 필요가 있으며, 인터뷰 외에도 설문조사가 각종 자료나 통계 등의 분석이 이루어져야 할 것이다.

병원사업에 있어서 "필수유지업무"에 관한 법리적 검토 (Legal review on essential business of hospital business)

  • This paper is to discuss essential business of hospital business. While the labor world and ILO made continuous recommendation for improvements towards the compulsory arbitration system along with the controversy over unconstitutionality of the system, the Constitutional Court ruled that the system is constitutional on December 23, 1996(90hunba19) and on May 15, 2003 (2001hunga31). Despite this decision from the Constitutional Court, there has been much controversy over whether the compulsory arbitration system infringes the rights of collective action against the principle of trade union & labor relations adjustment which allows Commissioner of the Labor Relations Commission to decide on submission of arbitration by virtue of his/her authority in case where industrial disputes take place in the essential public-service businesses. The revision on the above provision was closely examined from the year 2003 and an agreement was made on the abolition of the compulsory arbitration system and the introduction of essential business with a grand compromise among labor unions, employers and the government on September 11, 2006 followed by revision(Essential business system enacted on January 1, 2008) of the Trade Union & Labor Relations Adjustment Act on December 30 in the same year. Accordingly, in order to perform the essential business, parties to labor relations must have an agreement or obtain a decision by the Labor Relations Commission before taking industrial actions. This paper firstly examined the concept of essential public-service businesses and essential business, legal meaning of essential business, procedures for making agreement and decision and legal effects. Secondly it intensively explored a theory against the principle of the legality which was raised from some part of society. In other words, it is claimed that a theory against the principle of the legality is not consistent with the rule of legislation and some abstract wording is against void for vagueness doctrine because part of crime constitution requirements is delegated to the Presidential Decree or to consultation among parties to labor relations. But analysis on the rule of legislation and void for vagueness doctrine reflected in the decision by the Constitutional Court led that argument for a theory against the principle of the legality is not reasonable. Close examination was done on a formal act of essential business agreement and necessity of prior agreement before submission of decision to the Labor Relations Commission which might have difficulties in performing work. In addition, an example agreement on hospital essential business is attached to help you understand this paper better.

경산(經山) 정원용(鄭元容)의 문학론(文學論) 고찰 (Study on Literature theory of Gyeong-san(經山) Jeong Won-yong(鄭元容))

  • 경산(經山) 정원용(鄭元容)(1783-1873)은 1802년(순조 2)에 과거에 급제하여 약 70여 년간 관직생활을 역임하였다. 정원용은 뛰어난 행정능력으로 세도정치기의 상황에서도 중요한 위치를 차지하였으며, 뛰어난 문장으로 당대의 여러 문인들에게 인정을 받았다. 정원용은 당대 정치와 학술 그리고 문예 방면에서 영향력이 있었고 방대한 저술들을 남겼지만, 그에 대한 연구 성과는 소략한 편이다. 이에 본고에서는 정원용의 문학 전반을 살펴보기 위한 기초 작업으로써 먼저 그의 문학론(文學論)이 어떠하였는지를 고찰하였다. 정원용이 지니고 있었던 문학론(文學論)을 고찰한 결과, 크게 세 가지 측면을 확인할 수 있었다. 첫째, 정원용은 모방에 대한 부정적인 시각을 견지하고 있었다. 정원용은 문장을 지을 때 다른 사람의 작품과 구절을 모방하는 태도에 대해서 부정적인 시각을 가지고 있었다. 이에 대해 자신의 역량을 헤아리지 않고 무조건적으로 고인(古人)을 따르려고 하는 태도에 반대하고 자신의 뜻을 드러내야 한다고 역설하였다. 이유원은 정원용의 문장에 대해서 평가하며 자신만의 색채로 일가를 이루었다고 하였고, 정원용은 김조순의 시에 대해서 언급하며 그가 다른 사람의 작품과 구절을 베끼지 않고 스스로 일가를 이루었다는 점을 높이 평가하였다. 이를 통해서 정원용이 문장을 지을 때 모방하는 것에 대해서 부정적으로 생각했다는 것을 알 수 있다. 둘째, 정원용은 시를 지을 때에 眞意를 추구하였다. 정원용은 두계(荳溪) 박종훈(朴宗薰)(1773-1841)에게 준 서찰에서 '공교롭기를 구하다가 도리어 진의(眞意)에 해가 되어서는 안 된다[위불가구공이반해진의야(謂不可求工而反害眞意也)]'라고 하였는데, 이를 통해 정원용이 시를 수식하는 데에 뜻을 두고 이에 힘쓰기 보다는 정(情)에서 발한 바를 꾸미지 않고 자연스럽게 시에 드러내는 것을 중시했음을 알 수 있다. 정원용은 시를 지을 때 다른 사람의 것을 베끼지 않고 자신의 뜻을 펼치고자 하였고, 또 뜻에서 발한 것을 억지로 꾸미고자 애쓰지 않았다. 다른 사람의 것을 모방하고자 한다면 자신의 뜻을 담아내기보다는 겉을 꾸미는 데에 그치게 될 것이기 때문이었다. 그가 모방에 대해서 부정적으로 인식하고 또 진의(眞意)를 추구했던 것은 서로 긴밀한 관계가 있다고 할 수 있다. 마지막으로 정원용은 기록을 중시하였다. 그는 지방관으로 부임할 당시 일어났던 사건들과 공문(公文)을 기록하고 부임지에서 지었던 시문(詩文)을 편찬하였으며, 우리나라 제도에 참고가 되는 전장제도(典章制度)를 집성한 책을 저술하고, 평생을 일기로 기록하는 등 다양한 방면에서 200여 편에 달하는 많은 저술들을 남겼다. 이러한 정원용의 기록정신은 그의 관직생활과 연관이 깊다고 할 수 있다. 정원용은 평생을 관직에 있었던 사람으로 자신의 경험이 나라를 경륜하는데 도움이 되기를 바라는 목적에서 기록을 매우 중요하게 생각했던 것이다. 그리고 이러한 기록행위를 통해서 자신 또한 훗날 도움을 받고자 하였기에 방대한 기록물들을 남기게 된 것으로 보인다.
