Thymectomy for the Myasthemia Gravis Patient (중증 근무력증 환자의 흉선 절제술)

  • Backgroun: Thymectomy was known as an effective treatment modality of myasthenia gravis. In this paper, we analyzed the result of thymectomy and the factors affecting the postoperative system improvement. Material and Method: We analyzed the medical records of 50 patients who received the thymectomy for myasthenia gravis from January 1997 to December 2001. Result: 39 patients showed sympton improvement. The effect of thymectomy as a treatment is 78%. There was no statistically significant correlation between postoperative improvement and Sex, Age, the Weight of thymic tissue, preoperative symptom duration, and preoperative mestinon dosage. However, the thymic pathology and low grade preoperative symptoms were affecting the postoperative prognosis. Conclusion: Thymic hyperplasia showed good prognosis compared to thymoma. Low grade preoperative symptoms (Group I or IIA) also showed good prognosis. So, early thymectomy is recommendable for the good treatment results of myasthenia gravis.

A Clinical Analysis on Traumatic Subarachnoid Hemorrhage (두부외상 후 발생한 지주막하 출혈에 대한 임상분석)

  • Objective : Many authors suggest that patients with traumatic subarachnoid hemorrhage(tSAH) visible on first CT after heve injury had a significantly worse prognosis than patients who do not. The aim of this study is to identify patients with tSAH who present with a bad prognosis by reviewing their clinicoradiological features and plan appropriate treatments. Patients and Methods : We reviewed and analysed the factors that influenced discharge outcomes in 172 patients with tSAH for a 3-year period. The outcome was divided into good(good recovery and moderate disability of glasgow outcome scale) and good(severe disability, vegetative state and death). Results : A regression analysis of statistical significant factors(p<0.05) among the clinical and CT features ranked them by descending order of contribution to Glasgow Outcome Scale(GOS) scores at the time of discharge from acute hospitalization as follows 1) clinical : admission Glasgow Coma Scale(GCS), hypotension, CT grade, abnormal APTT, skull fracture, hyperglycemia(>160mg/dl), hypoxia, operation, 2) CT : basal cistern effacement(BCE), mass lesion, cortical sulcal effacement(CSE), midline shift. Conclusion : We have also experienced that the CT grading scale proposed by Green et al is a simple and useful prognostic factor. The authors believe that the patients with high CT grade need adjuvant therapies as of well surgery but it seems mandatory to consider early identification and correction of hypotension, hyperglycemia, and hypoxia in emergency setting.

  • Purpose; To find out the best imaging parameters for the diagnosis of disc in MRI imaging. Materials and methods; The diagnostic quality of the disc among the Tl, PD and T2 images of same patients02 joints, 223 images) was compared by visual(I-IV grades) and gray level measurement (pre- and infra-discal area) method. Results; PD images showed best results with 43.7% of the images belonging to grade III (good) and with statistically significant higher difference of the gray levels at pre- and infra-discal areas. But there were no grade N (excellent) images. Conclusions; PD images are best method among Tl. PD and T2 images in diagnosing the disc but since there were no excellent images further imaging parameters should be studied for better images.

Numerical Simulation of Steel Mixing during Sequential Casting of Dissimilar Grades in the Continuous Caster (연속주조시 강종 혼합에 관한 수치해석적 연구)

  • In order to investigate the mixing of dissimilar grades during the arbitrary grade transition in bloom caster, a computational model has been developed. The model is fully transient and consists of two sub models, which account for mixing in the bloom tundish, mixing in the strand. The developed model was verified using concentration histories measured on 1 : 1 scale bloom tundish water model. The result of numerical model showed good agreement with the experimental results of water model. By using this numerical model, the mixing of dissimilar grades in bloom caster has been simulated. As that result, the characteristics of the steel mixing in the bloom tundish and strand was showed and the amount of the intermixed grade bloom was predicted.

Natural Dyeing & Antimicrobiology of Nylon Fabrics (나일론의 천연염색과 염색포의 항균성)

  • Nylon fabrics and stocking were dyed with artemisia dyeing solution, dyed with the natural dyestuffs that were manufactured from chestnut hulls by boiling in 0.5% NaOH solution and powdering in freeze dryer, and dyed with gromwell dyeing solution. Dyeabilities on nylon fabrics and stocking were good because of being amine group. All the dyed fabrics & stocking showed excellent in color fastness to laundering, and as poor as grade 1 to light fastness. But Light fastness of them could be improved to grade 3 or 4-5 by treatment with gallic acid and by repeated dyeing. Antimicrobial activities of the fabrics dyed with artemisia and gromwell against Staphylococcus aureus and Candida albicans were excellent in the aspect of bacteriostatic and fungistatic. There were not any distinct differences between mordant and non-mordant fabrics.

  • Because Self-Adhesive Waterproofing sheet has good constructability, it has been widely used. Also Self-Adhesive Waterproofing sheet is researched based on variety of materials and various of forms. However, producers have recognized about only pass KS quality standard. So, Self-Adhesive waterproofing sheet has been limited about improvement of performance. Therefore, in this research, proposed guideline of Self-Adhesive Waterproofing sheet for disinterested performance grade using probability theory and quality management theory.

Dyeability and Antibacterial Activity of the Fabrics Using Balsamine Extracts (봉숭아 추출액의 염색성과 염색물의 항균성에 관한 연구)

  • Dyeing liquor was extracted from flowers, leaves and stems of balsamine. Dyeing properties, colorfastness and antibacterial activities of the fabrics dyed with the balsamine extracts were investigated at a variety of dyeing conditions of differing pH. time and temperature. Wool, silk and nylon fabrics showed better dyeability than cellulosic fabrics, i.e. cotton, linen and rayon. Especially, nylon fabrics showed the highest K/S values. The dye-uptake was achieved to the highest degree during the first ten minutes of dyeing procedure. All fabrics were dyed well at $40^\circ{C}$ with no pH adjustment. The pH of unadjusted balsamine extracts was 4.6. Their colorfastnesses to washing, drycleaning and perspiration were good as 4 or 4-5 grade. but the light fastnesses were poor as 1-2 grade. While, the antibacterial activities to Staphylococcus aureus and Klebsiella pneumoniae were excellent with 99.9% of colonies reduction ratio. Their excellent antibacterial activity was kept well after ten times washing.

  • Activated carbon fibers (ACFs) were prepared from cost effective commercial textiles through stabilization, carbonization, and subsequently activation by carbon dioxide. ACFs were characterized for surface area and pore size distribution by physical adsorption of nitrogen at 77 K. ACFs were also examined for various surface characteristics by scanning electron microscopy, Fourier transform infrared spectroscopy, and CHNO elemental analyzer. The prepared ACFs exhibited good surface textural properties with well developed micro porous structure. With improvement in physical strength, the commercial textile grade acrylic precursor based ACFs developed in this study may have great utility as cost effective adsorbents in environmental remediation applications.

  • Dysphagia generally has a good prognosis after stroke involving cerebral hemisphere or braibstem, but it could have serious consquence with dehydration leading to haemoconcentration, renal failure and aspiration leading to pneumonia. This preliminary report was written for main report. The main report will be written to give an objective guide post of management and treatment in stroke patient with dysphagia. The objective guide posts were follows as, 1. Close examination for relationship between dysphagia in stroke patients and other neurological deficits 2. Influence on the prognosis of dysphagia in stroke patients by the grade of dysphagia at admission time. 3. The realtionship between the site and size of brain damage and the grade and prognosis of dysphagia 4. The frequency and prognosis of dysphagia in stroke patients according to sex and age. 5. The rate and speed of recovery with dysphagia after stroke. 6. Frequency and process of complications, aspiration pneumonia, weight loss, sore, in stroke patient group with dysphagia.

Saenggangeonbi-tang's Effect on Alcoholic Liver Cirrhosis - 1 Case (생간건비탕가감을 투여한 알콜성 간경변증 환자 치험 1례)

  • We treated a 53 year-old male patient. He had alcoholic liver cirrhosis and diabetes. The main symptoms were fatigue, ascites, edema, anorexia, indigestion, constipation, and abdominal pain. We diagnosed him as 'the retention of pathogenic damp-heat in the liver and gall bladder’ and adminstered Saenggangeonbi-tang three times a day. We observed clinical symptoms, liver function test(AST, ALT, ${\gamma}-GT$, ALP, total bilirubin), nutritional state(total protein, albumin) and capillary blood glucose. After treatment, all the main symptoms disappeared and the patient's nutritional state has improved. Child's grade using the improvement index in liver cirrhosis has changed from B to A. Although the very mechanism of Saenggangeonbi-tang was not proven, we could suggest that Saenggangeonbi-tang is not toxic to the liver and has a good effect on alcoholic liver cirrhosis with diabetes mellitus.
