Sulcus Vocalis Treated with Autologous Collagen Injection (자가 콜라겐주입술을 이용한 성대구증환자의 치료)

  • Background and Objectives : Despite of numerous treatment modalities for glottic insufficiency, it still remains unresolved problem. Recently, autologous collagen injection was introduced as a new treatment for glottic insufficiency. This study was attempted to evaluate the efficacy of this treatment modality. Materials and Methods : Ten patients diagnosed as symptomatic sulcus vocalis with glottic insufficiency and agreed with this procedure were selected. Injectable collagen was obtained from the patient's dermis. It was injected into the pathologic area of the vocal fold. After injection, 4 of 10 patients were 1311owe4 up more than 5 months, and their voice were evaluated prior to injection and at 5 months after injection, using subjective, perceptual, aerodynamic, acoustic and videostroboscopic assessments. Results : In the aerodynamic assessment, 2 of 4(50%) patients was improved in MFR(mean flow rate), but no change was noted in MPT(maximum phonation time). In the acoustic assessment, no definite improvement was noted. In the subjective and perceptual assessments, 2 of 4(50%) patients was improved. In the videostroboscopic assessment, chink was reduced but still remained. Serious complication was not noted. Conclusion : Autologous collagen injection was reported as ideal treatment for glottic insufficiency, but our results were relatively unsatisfactory. But considering that our study is preliminary, it is too hasty to determine the efficacy of autologous collagen injection. We think that further study is required.

Comparison of bimanual laryngoscopy, backward-rightward pressure, and cricoid pressure in difficult airway management: A manikin study (어려운 기도를 가진 마네킹에게 양손후두경법, 우측-후방압박법과 반지연골압박법 효율성 비교)

  • Purpose: The purpose of this study was to establish novice user guidelines for efficient external laryngeal manipulation for intubation in difficult airway management. Methods: This study included 59 pre-qualified junior and senior emergency medical service students. The participants were instructed at random to intubate a manikin equipped with a cervical collar, thus simulating a difficult airway, using three types of external laryngeal manipulation: bimanual laryngoscopy, backward-rightward pressure, and cricoid pressure. The resultant intubation time and glottic view grade scores were estimated. Results: Intubation time was longest using the bimanual manipulation method, followed by cricoid pressure and backward-rightward pressure. A low Cormack-Lehane glottic view score was obtained regardless of the assisted compression method used. Conclusion: Backward-rightward pressure may be the most efficient method of external laryngeal manipulation on the basis of the intubation time and improvement in glottic view.

Local Control after Radiation Therapy for T1-T2 Glottic Carcinoma (T1-T2 성문암의 방사선 치료 성적)

  • Purpose : To assess the effectiveness and problems of the primary radiation therapy and salvage surgery in a series of patients affected by T1-T2NO glottic cancers treated from 1985 to 1991 at the Pusan National University Hospital. Materials and Methods : From 8/85 to 12/91,34 patients affected by early glottic carcinoma histologically proven were treated with curative radiation therapy, Distribution of patients according to T stage was 30 for T1 and 4 for T2. Male to female ratio was 33:1. Age of patients ranged from 31 to 73 with mean age of 58 years. All of the patients were treated with radical radiation with total tumor dose of 63-75. 3Gy(median 68.2Gy), of 5 weekly fractions of 1.8-2Gy and with 6MV photon beams through two laterally opposed fields. Results : The overall 5-year local control rates were $74\%$(8/30) for Tl, and $25\%$(3/4) for T2. The main cause of failure was progression or recurrence in T(10/11). One failures were observed in T and N at the same time. Of these 11 patients, 9($81\%$) were salvaged with surgery, After surgical salvage of radiation failures, the 5-year survival rates were $96\%$ for T1 and $75\%$ for T2. Among the survivors, $73\%$ of T1 and $33\%$ of T2 were able to preserve the larynx. Conclusion : It can be concluded that radiotherapy is the first choice in the treatment of glottic T1 carcinoma.

Glottic Closure Reflex in an Anesthetized and Awake Canine Model (개의 성문폐쇄반사에 미치는 중추조절의 영향에 관한 연구)

  • Background and Objectives : Sphincteric function of the larynx, essential to lower airway protection, is most efficiently achieved through strong reflex adduction by both vocal cords. We hypothesize that central facilitation is an essential component of a bilateral adductor reflex and that its disturbance could result in weakened sphincteric closure. Materials and Method : Seven adult 20kg dogs underwent evoked response laryngeal electromyoraphy under 0.5 to 1.0 MAC isoflurane anesthesia. The internal branch of superior laryngeal nerve was stimulated through bipolar platinum-iridium electrodes and recording electrodes were positioned in the ipsilateral and contralateral thyroaryteonoid muscles. Results : Consistent threshold responses were obtained ipsilaterally under all anesthetic levels. However, contralateral reflex responses disappeared as anesthetic levels approached 1.0 MAC. Additionally, at 0.5 MAC, late responses (R2) were detected in one animal. Conclusion : Alteration of central facilitation by deepening anesthesia abolishes the crossed adductor reflex, predisposing to a weakened glottic closure response. Precise understanding of this effect may improve the prevention of aspiration in patients emerging from prolonged sedation or under heavy psychotropic control.

  • Transoral $CO_2$ Laser Microsurgery of glottic Carcinoma is replacing external approaches. The qualify of life after surgery for laryngeal cancer may be as important as complete resection of the tumor for patient. Transoral $CO_2$ laser cordectomy for the management of early laryngeal cancer has advantages with regard to oncological results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. Moreover, transoral laser surgery can be a useful choice as a salvage surgery in radiation therapy failed early glottic cancer. A classification of laryngeal endoscopic cordectomies which included eight different types was described by the European laryngological Society in 2000. We will also introduce type VI which was newly proposed recently.

  • The preservation of the voice-producing mechanism is an important feature in the management of laryngeal cancer by radiotherapy. But, radiation therapy has certain side effects such as mucositis, tissue edema, necrosis and fibrosis which could effect on normal voice production. Several subjective studies that used questionnaires and auditory perceptual judgements of voice have been interpreted to mean that radiation results in a normal or near-normal voice. Objective evidence of the status of vocal function after radiation treatment, however, is still lacking. We analyzed the changes that occur in voice parameters in a group of patients undergoing radiation therapy, in order to determine the effect of radiation on voice quality. In this study acoustic, aerodynamic measures of vocal function were used to determine the characteristics of voice production. We found that voice parameters in early glottic cancer changed meaningfully comparing to normal larynx with or without radiation and radiation therapy has an little effect on normal larynx.

Occult Metastatic Rate of Laryngeal Cancer Predicted by Elective Neck Dissection (후두암종에서 예방적 경부청소술로 확인한 경부 잠재전이율)

  • Background and Objectives: Neck metastasis is one of the most important prognostic factor in head and neck squamous cell carcinoma. Recently, elective neck dissection has been widely accepted for accurate pathologic staging and elective treatment of neck. Occult metastasis rate of laryngeal cancer varies widely depending upon authors. To evaluate the rate and characteristics of occult metastasis and efficacy of the elective neck dissection in clinical N0 laryngeal cancer, we performed this study. Materials and Method: Seventy two patients (supraglottic cancer: 19 cases, glottic cancer: 53 cases) who underwent surgery for laryngeal cancer as an initial treatment from 1993 to 2002 were evaluated. All was underwent elective neck dissection at the time of surgery for the primary treatment. The record of patients and pathologic report were reviewed retrospectively. Results: Occult metastasis rate of supraglottic and glottic cancer were 42.1% (8/19) and 9.4% (5/53), respectively. According to T stage, the occult meastasis of supraglottic and glottic cancer was 20%, 0% in T1, 36.4%, 0% in T2, 100%, 40% in T2, 100%, 20% in T4, respectively. Occult metastasis was mostly confined within level II (69.2%), III (76.9%), IV (23.1%). Conclusion: Based on ours results, elective neck dissection might be needed in treating of clinically N0 all supraglottic cancer and advanced T3, T4 glottic cancer.

Clinical Analysis of $T_1$ Glottic Cancer (병기 $T_1$ 성문암 30예의 임상적 고찰)

  • Laryngeal cancer is one of the most frequent cancers in the head and neck area. Clinical analysis was performed on 30 cases of treated $T_1$ glottic cancer patients. Early glottic cancer occured most frequently in male in their sixties and all were well or moderately well differentiated squamous cell carcinoma. Overall five year survival rate was 81.5% and there was no significant difference between stages or the differentiation of the diseases. Recurrence at the primary site, especially anterior commissure, was common, thus careful pre- and postoperative follow up evaluation is recommended.

Intensity-modulated radiation therapy in early stage squamous cell carcinoma of the larynx: treatment trends and outcomes

  • Purpose: Definitive radiotherapy remains a primary treatment option for early stage glottic cancer. Intensity-modulated radiation therapy (IMRT) has emerged as the standard treatment technique for advanced head and neck cancers, whereas three-dimensional conformal radiotherapy (3D-CRT) has remained standard for early glottic cancers. We used the National Cancer Database (NCDB) to identify predictors of IMRT use and effect on outcome in these patients. Materials and Methods: We queried the NCDB from 2004-2015 for squamous cell carcinoma of the glottic larynx staged Tis-T2N0 treated with radiation alone. Logistic regression was used to identify predictors of IMRT. Cox regression was used to identify factors predictive of overall survival. Propensity matching was conducted to account for indication bias. Results: We identified 15,627 patients, of which 11% received IMRT. IMRT use rose from 2% in 2004 to 16% in 2015. Predictors of IMRT include: increased comorbidity, T2 stage, urban location, chemotherapy, treatment at an academic center, and later treatment year. Predictors of improved survival were female gender, higher income, lower stage, no chemotherapy, academic facility, and more remote year. There was no difference in survival between 3D-CRT and IMRT across all stages. Conclusions: The rate of IMRT use for early stage glottic laryngeal cancer has increased over time. There was no difference in outcome in patients receiving IMRT versus 3D-CRT across the cohort.

  • An anterior glottic web can be congenital or develop as a result of trauma. The congenital webs indicate failure of normal separation of the two vocal cord primordia and the cicatrical membrane caused by accidental and operative trauma. When the web is small it rarely produces significant sympto-matology but larger webs may produce stridor as well as hoaresness in various degrees. Achievement of epithelialization of the anterior commisure after excision of a web, thus preventing adhesions from forming between the denuded vocal cords, is the principle underlying the surgical treatment of anterior glottic webs. Recently the authors experienced a case of anterior glottic web which occured in a 23 year old man after repeat removal of a papilloma several times in child-hood. The patient received treatment of a web excision followed by placement of a gold keel between the cords in the anterior glottis.
