  • Background: This study aimed to compare and evaluate the marginal fit of nanocomposite computer-aided design/computer-aided manufacturing (CAD/CAM) inlays. Three types of nanocomposite CAD/CAM blocks (HASEM, VITA Enamic, and Lava Ultimate) were used as materials. Methods: Class II disto-occlusal inlay restorations were prepared on a typodont mandibular right first molar using diamond rotary instruments. The inlays were fabricated using CAD/CAM technology and evaluated using the silicone replica technique to measure marginal gaps at five locations on each inlay. The data were analyzed by two-way analysis of variance and Tukey post hoc tests (α=0.05). Results: There were no significant differences in the marginal gaps based on the type of nanocomposite CAD/CAM inlay used (p=0.209). However, there was a significant difference in the marginal gaps between the measurement regions. The gingival region consistently exhibited a larger marginal gap than the axial and occlusal regions (p<0.001). Conclusion: Within the limitations of this in vitro study, the measurement location significantly influenced the marginal fit of class II disto-occlusal inlay restorations. However, there were no significant differences in the marginal gaps among the different types of CAD/CAM blocks. Furthermore, the overall mean marginal fits of the class II disto-occlusal inlay restorations made with the three types of nanocomposite CAD/CAM blocks were within the clinically acceptable range.

  • Purpose: Peri-implantitis (PI) is an inflammatory condition associated with the destruction of bone tissue around a dental implant, and diode lasers can be used to treat this disease. In this study, we aimed to evaluate the effectiveness of a 940-nm diode laser for the nonsurgical treatment of PI. Methods: Twenty patients (8 women and 12 men) were enrolled in a split-mouth randomized controlled study. In the control group (CG), mechanical debridement with titanium curettes accompanied by airflow was performed around the implants. The test group (TG) was treated similarly, but with the use of a diode laser. Clinical measurements (plaque index, gingival index [GI], probing pocket depth [PPD], bleeding on probing [BOP], clinical attachment level, and interleukin-1β [IL-1β] in the peri-implant crevicular fluid) were evaluated and recorded at baseline and 3 months. IL-1β levels were determined using the enzyme-linked immunosorbent assay method. Results: The symptoms were alleviated in both groups at 3 months as assessed through clinical measurements. GI, BOP, and PPD were significantly lower in the TG than in the CG (P<0.05). The IL-1β level increased post-treatment in both groups, but this increase was only statistically significant (P<0.05) in the CG. Conclusions: The diode laser enabled improvements in clinical parameters in the periimplant tissue. However, it did not reduce IL-1β levels after treatment. Further studies about the use of diode lasers in the treatment of PI will be necessary to evaluate the effects of diode lasers in PI treatment.

  • Background: Periodontitis and peri-implantitis are diseases caused by pathogenic microorganisms that cause tissue damage and alveolar bone destruction resulting in the loss of teeth and implants. Due to the biological differences in the tissues surrounding the implants, peri-implantitis progresses more rapidly and intensely than periodontitis, underscoring the importance of understanding the characteristics and interactions of pathogenic bacteria. This study aimed to quantitatively analyze the pathogenic microorganisms associated with periodontitis and peri-implantitis in Korean patients and evaluate the correlation between these bacteria. Methods: A total of 98 (52 males and 46 females) were randomly selected and classified into three groups (healthy group [HG]=25; periodontitis group [PG]=31; and peri-implantitis group [PIG]=42). The relative expression levels of 11 pathogenic microorganisms collected from the gingival sulcus fluid were determined using multiplex real-time polymerase chain reaction. Results: Eikenella corrodens, Fusobacterium nucleatum, and Prevotella nigrescens were highly prevalent in the HG, PG, and PIG patients. The results of the relative quantitative analysis of microorganisms showed that all bacteria belonging to the green, orange, and red complexes were significantly more abundant in the PG and PIG than in the HG (p<0.05). Porphyromonas gingivalis in the red complex showed a positive correlation with all microorganisms in the orange complex (p<0.05). Campylobacter rectus in the orange complex showed a significant positive correlation with all microorganisms in the red complex, and with F. nucleatum, P. nigrescens, Prevotella intermedia, and Eubacterium nodatum (p<0.05). Conclusion: P. gingivalis, C. rectus, and F. nucleatum exhibit strong interactions. Removing these bacteria can block complex formation and enhance the prevention and treatment of periodontitis and peri-implantitis.

  • When fabricating definitive implant-supported fixed prostheses in upper and lower completely edentulous patients, it is crucial to get information about the vertical dimension, jaw relationship, implants, abutments and gingival contour. In this case, temporary prostheses were used to take the information. The temporary prosthesis was scanned outside of the mouth so that it increased the efficiency of scanning it. During the scan of the prosthesis, a scan zig which connected both posterior parts of the prosthesis was used to minimize the scan errors. We report this case because we obtained satisfactory functional and esthetic results by using the digital technology.

  • Purpose: The objective of this retrospective clinical study was to provide evidence supporting the adjunctive local application of doxycycline solution or minocycline ointment, in conjunction with drainage, for the treatment of acute periodontal abscesses. Methods: The study included 63 patients who had received treatment for acute periodontal abscesses through drainage supplemented with 1 of 3 types of adjunctive medications during their initial visit (visit 1; baseline): 1) saline irrigation (the control group), 2) 2% minocycline ointment (the TM group), or 3) 300 mg/mL doxycycline irrigation (the TD group). The same adjunctive medication was administered at visit 2, which took place 1 week after visit 1. Probing depth (PD), bleeding on probing (BOP), plaque index, gingival recession, clinical attachment level, and tooth mobility were clinically evaluated at visits 1, 2, and a third visit (visit 3; 4 weeks after visit 1). Statistical significance was considered to be indicated by P values <0.05. Results: By visit 3, all clinical indices and tooth mobility had significantly decreased in each group. At this visit, PD and BOP on the abscess side were significantly lower in the TM and TD groups compared to the control group. The TD group showed a significantly greater improvement than the TM group, with mean PD reductions of 1.09 mm in the control group, 1.88 mm in the TM group, and 2.88 mm in the TD group. Similarly, mean BOP reductions were 45% in the control group, 73.02% in the TM group, and 95.45% in the TD group. Conclusions: Local and adjunctive administration of doxycycline and minocycline in combination with drainage exhibited clinical advantages over drainage alone in improving PD and BOP. Notably, a doxycycline solution of 300 mg/mL was more effective than a 2% minocycline ointment.

  • The aim of this study was to investigate the prevention effect of oral disease on the school-based toothbrushing program(SBTBP) in Daejeon in 30 months. The experimental subjects were the 5th grade's 70 students who have gone to school with the school-based toothbrushing program since the 3th-grade. The control school was located in close geographical area with similar economical status. The questionnaire about oral health knowledge and behavior was done by self-recording. After one dentist examined the dental caries and periodontal status, the gingival crevicular fluid(GCF) was collected with #25 paperpoint for 1 miniutes. Matrix Metalloproteinase(MMP)-9 in GCF was analyzed by ELISA. The SBTBP group had the upper oral health knowledge than the control significantly(p<0.001), but, the SBTBP group had no difference of the oral health behavior from the control. Although the SBTBP group had the lower plaque index than the control significantly(p=0.02), the SBTBP group had no difference of the gingival index, calculus index and the concentration of MMP-9 in GCF from the control. In conclusion, The SBTBP had the effect to reduce the dental plaque and to improve the oral health knowledge. On the other hand, the effect to prevent dental caries and periodontal disease of SBTBP was not clear.

  • The purpose of this study was determine the effects of professional tooth cleaning and plaque control instruction(PT & PCI) on reduction of peri-implantitis. A total 80 implant patients were investigated using O'Leary plague index(PI), L${\ddot{o}}$e & Silness gingival index(GI), implant bone loss(BL). While the PT & PCI was conducted by using 'Watanabe method' after scaling for the experimental group, the engine polishing and a plaque control instruction was operated by rolling method after scaling for the control group. The collected data were analyzed with t-test, ANOVA, paired t-test and stepwise multiple regression. PI and GI of the experimental group significantly decreased than the control group(p<0.05). BL, also tended to decreased in the experimental group than the control group(p=0.155). Multiple regression analysis, the factors that was highly correlated with PT & PCI on the PI and GI. According to the implant characteristics, implant location and duration were closely related to PI(p<0.05). Therefore, the PT & PCI is one of the effective methods to reduce the failure caused by the peri-implantitis. Based on this conclusion, that it would be meaningful if the proposed PT & PCI is applied to the oral health management programs of the implant patients.

  • The purpose of this study was to evaluate the efficacy of an electric toothbrush, with a specially designed orthodontic brush head, compared to a manual toothbrush on the periodontal health of patients with fixed orthodontic appliances Forty patients were randomly divided into two groups. the electric and the manual toothbrush groups, 1 month after attachment of fixed orthodontic appliances. Periodontal status was measured using a plaque index, a gingival index. a bleeding index, a pocket depth and a relative attachment loss, at baseline and after 3 months. 6 months, 9 mouths. and 12 mouths. The Braun Oral-B Plak Control with Ortho OD 15-1 brush head was used as the electric toothbrush while the Butler G.U.M 124 was given as the manual toothbrush. In the manual toothbrush group, the giugival and bleeding indices showed no statistically significant differences, but the plaque index was significantly decreased (P<0.001) and pocket depth as well as relative attachment loss were significantly increased (p<0.01). In the electric toothbrush group. the bleeding index, pocket depth and relative attachment loss showed no statistically significant differences. but the plaque and gingival indices were significantly decreased (p<0.001). In the case of the plaque. giugival and bleeding indices, there were no statistically significant differences between the electric and the manual toothbrush groups. On the contrary, in the case of pocket depth and relative attachment loss, there were statistically significant differences between the electric and the manual toothbrush groups: an increase in the manual toothbrush groups unlike the electric toothbrush group which kept the same state (P<0.05) These findings suggest that an electric toothbrush is useful for orthodontic patients with fixed appliances.
