고환에서 발생한 종자세포암의 폐전이 절제 - 1예 보고 - (Pulmonary Metastasectomy from Testicular Germ Cell Tumor - A case report-)

  • 이 보고는 고환종자세포암 치료 후 폐전이를 폐절제술을 시행하여 성공적으로 치료한 증례이다. 항암치료 후 양전자방출단층촬영상 FDG의 유의한 섭취는 없었고 잔존종양을 원발 병소와 조직학적 비교를 위해 전이암을 제거하였다. 본 증례는 잔존종양에 생존세포가 관찰되지 않아 부가적인 항암치료는 시행하지 않았으며, 현재 술 후 6개월로 현재 재발이나 합병증 없이 잘 지내고 있다.

miRNA-1297 Induces Cell Proliferation by Targeting Phosphatase and Tensin Homolog in Testicular Germ Cell Tumor Cells

  • To investigate the role of miR-1297 and the tumor suppressor gene PTEN in cell proliferation of testicular germ cell tumors (TGCT). MTT assays were used to test the effect of miR-1297 on proliferation of the NCCIT testicular germ cell tumor cell line. In NCCIT cells, the expression of PTEN was assessed by Western blotting further. In order to confirm target association between miR-1297 and 3'-UTR of PTEN, a luciferase reporter activity assay was employed. Moreover, roles of PTEN in proliferation of NCCIT cells were evaluated by transfection of PTEN siRNA. Proliferation of NCCIT cells was promoted by miR-1297 in a concentration-dependent manner. In addition, miR-1297 could bind to the 3'-UTR of PTEN based on luciferase reporter activity assay, and reduced expression of PTEN at protein level was found. Proliferation of NCCIT cells was significantly enhanced after knockdown of PTEN by siRNA. miR-1297 as a potential oncogene could induce cell proliferation by targeting PTEN in NCCIT cells.

두개내 생식세포종양 유발 중추성 요붕증 환아의 한약 치험 1례 (A Case Report of Intracranial Germ Cell Tumor-Induced Central Diabetes Insipidus in a Child Treated with Korean Herbal Medicine)

  • Objective: The purpose of this study is to report the case of intracranial germ cell tumor-induced central diabetes insipidus (CDI) in a child treated with Korean herbal medicine. Methods: A nine-year-old female patient diagnosed with intracranial germ cell tumor-induced CDI suffering from polyuria, polydipsia, and headache was hospitalized. Nocturia frequency and 24-hour urine volume were assessed. Results: The patient was treated with Korean medicine, including Nokyong-hwan. As a result of the 6-day inpatient treatment, nocturia frequency was decreased 3-4 times to 0-1 time, and 24-hour urine volume was decreased. Conclusion: Korean medicine, including Nokyong-hwan, may be considered an optional treatment for releasing the symptoms of intracranial germ cell tumor-induced CDI. Further studies are needed to confirm this finding.

  • Sacrococcygeal teratoma (SCT) is an extragonadal germ cell tumor (GCT) that develops in the fetal and neonatal periods. SCT is a type I GCT in which only teratoma and yolk sac tumors arise from extragonadal sites. SCT is the most common type I GCT and is believed to originate through epigenetic reprogramming of early primordial germ cells migrating from the yolk sac to the gonadal ridges. Fetal SCT diagnosed in utero presents many obstetrical problems. For high-risk fetuses, fetal interventions (devascularization and debulking) are under development. Most patients with SCT are operated on after birth. Complete surgical resection is the key for tumor control, and the anatomical location of the tumor determines the surgical approaches. Incomplete resection and malignant histology are risk factors for recurrence. Approximately 10-15% of patients have a tumor recurrence, which is frequently of malignant histology. Long-term surveillance with monitoring of serum alpha fetoprotein and magnetic resonance imaging is required. Survivors of SCT may suffer anorectal, urological, and sexual sequelae later in their life, and comprehensive evaluation and care are required.

  • 종격동에 발생하는 생식세포종양에서 미성숙 기형종과 정상피종이 혼합된 경우는 매우 드물며 예후는 좋지 않은 것으로 알려져 있다. 저자들은 객혈을 주소로 내원한 환자에서 수술로 확진한 미성숙 기형종과 장상피종이 혼합된 생식세포 종양의 1예를 경험하였기에 보고하는 바이다.

두개내 악성 생식세포종양의 압착도말 세포학적 소견 (Cytologic Features of Intracranial Germ Cell Tumors in Crush Preparation)

  • Intraoperative cytologic examination of intracranial tumors using crush preparation provides useful information in operative decision making. The diminutive nature of many biopsy specimens, particularly those obtained by stereotactic neurosurgical procedures emphasizes the importance of combining the cytologic smear method with conventional frozen section interpretation. The great advantage of the cytologic smear method resides in its suitability for the study of minute fragments of tissue, allowing retention of the majority of the specimen for optimal processing. We present the cytologic features of 3 cases of intracranial germ cell tumors(2 germinomas and 1 endodermal sinus tumor), using crush preparation during intraoperative diagnosis and compare them with histologic findings. The cytologic features of the germ cell tumors were similar to those of the respective gonadal counterparts. The cytologic differential diagnosis of both types of germ cell tumors is described.

난소 악성 생식세포종의 전산화 단층촬영 소견 (Computed Tomographic Findings of Malignant Ovarian Germ Cell Tumors)

  • 영남대학교 의과대학 부속병원에서 난소 악성 생식세포종으로 확진된 12례를 대상으로 CT 소견을 분석하여 다음과 같은 결론을 얻었다. 내배엽동 종양은 낭성 종괴내 일부 고형조직과 격막이 혼재되어 있는데 반하여 미성숙 기형종은 비교적 특징적인 지방조직과 석회화가 보였고, 미분화 세포종은 비록 1례지만 전형적인 낭성 종괴가 없는 고형 조직으로 충만하였다. 그러나 혼합 생식 세포종은 2종이상의 생식 세포종이 혼합된 종양이므로 특징적인 CT 소견없이 혼합된 종양들의 종류에 따라 다양하게 나타날 것으로 생각되었다. 또한 모든 종양들의 크기는 11cm에서 33cm의 비교적 컸다. 결론적으로 젊은 여성에서 큰 난소 종양이 있을 때 CT 영상에서 그 조직성상을 분석하면 그들의 감별진단이 가능하리라고 생각되었다.
