Background: Elderly patients visiting pain clinic may be at greater risk of misunderstanding the explanation because of age-related cognitive decline. Video instruction may provide a consistent from of teaching in a visual and realistic manner. We evaluated the effect of educational video on the patient understanding and satisfaction in a group of geriatric patients visiting pain clinic. Methods: Ninety two patients aged more than 60 years old who were scheduled for transforaminal epidural block were recruited. After exposure to either video or paper instruction process, each patient was asked 5-item comprehension questions, overall satisfaction and preference question. During follow-up period, number of outpatient referral-line call for further explanation was counted. Results: We observed significantly better comprehension in the video education compared with paper instruction (P < 0.001). Patient satisfaction was also higher in the video group (P = 0.015), and patients visiting pain clinic were more preferred video instruction (P < 0.001). Proportion of referral-line call for further explanation were similar (P = 0.302). Conclusions: Video approach to instruction process before consent improves treatment comprehension in geriatric patient visiting pain clinic.
Objective : Although traumatic brain injury (TBI) occurs in people of all age groups, the elderly population is at a particular risk. The proportion of elderly population in the society is markedly increasing and Korea is one of the most rapidly aging societies. Here, we analyzed the data from 904 patients older over 65 years who were registered in the Korean Neuro-Trauma Data Bank System (KNTDBS). Methods : The Korean Society of Neurotraumatology recorded data from 20 institutions between September 2010 and March 2014. This retrospective study examined the clinical epidemiology, sex difference, outcome epidemiology, sociodemographic variables, and outcomes in the geriatric population related to TBI based on data from the KNTDBS. Results : The study included 540 men and 364 women. The age distributions in the male and female groups were statistically significantly different. The most common cause of trauma was a fall and diagnosis was acute subdural hematoma. The incidence was the highest in men aged 80-84 years and in women aged 75-79 years. The most common time of arrival to hospital after TBI was within 1 hour and 119 rescue team provided first aid earliest to patients with TBI. The mortality rate stratified according to the cause of trauma was significantly different, with mortality rates of 3.77% in fall and 11.65% in traffic accident. The mortality rates according the severity of brain injury, Glasgow Coma Scale score, and treatment were statistically significant. Conclusion : To our knowledge, this study is the first to focus on elderly patients with TBI in Korea and particularly investigate mortality and characteristics related to TBI-related death based on data from the KNTDBS. Although the study has some limitations, our results may be used to obtain useful information to study targeted prevention and more effective treatment options for older TBI patients and establish novel treatment guidelines and health polish for the geriatric population.

Subjects and Methods: Patients over 65 years old who underwent CI at implant center in Bozyaka Training and Research Hospital were included in this study (n=54; 34 males and 20 females). The control group was patient over 65 years old with normal hearing (n=54; 34 males and 20 females). We administered three questionnaires [World Health Organization Quality of Life-BREF (WHOQOL-BREF), World Health Organization Quality of Life-OLD (WHOQOL-OLD)], and Geriatric Depression Scale (GDS) to evaluate the QoL, CIrelated effects on activities of daily life, and social activities in all the subjects. Moreover, correlations between speech recognition and the QoL scores were evaluated. The duration of implant use and comorbidities were also examined as potential factors affecting QoL. Results: The patients had remarkable improvements (the mean score of postoperative speech perception 75.7%) in speech perception after CI. The scores for the WHOQOL-OLD and WHOQOL-BREF questionnaire responses were similar in both the study and control groups, except those for a two subdomains (social relations and social participation). The patients with longer-term CI had higher scores than those with short-term CI use. In general, the changes in GDS scores were not significant (p<0.05). Conclusions: The treatment of hearing loss with CI conferred significant improvement in patient's QoL (p<0.01). The evaluation of QoL can provide multidimensional insights into a geriatric patient's progress and, therefore, should be considered by audiologists.

Objective : The aims of this study were to investigate the prevalence of co-morbid depression and related factors of depression in mild cognitive impairment (MCI) patients. Methods : Nine hundred and six MCI individuals were included in this study. Depression was defined as major and minor depressive disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV). Depression-related demographic and clinical factors were also explored through multivariate logistic regression analysis. Results : In MCI patients, the prevalence of depression as defined according to the DSM-IV diagnostic criteria was 1.8% for major depressive disorder, 11.4% for minor depressive disorder, and overall 13.2% for both. Multivariate logistic regression showed that increased prevalence of overall depression was associated with female gender and non-amnestic subtype of MCI. Conclusion : Our findings based on a large number of MCI subjects who visited memory clinic indicated that more than ten percent of MCI patients have comorbid depression. In addition, female and non-amnestic subtype of MCI patients seems to be more vulnerable to depression.
본 연구는 근 골격계 질환을 앓고 있는 60대 이상 노인환자 관리를 위한 기초자료로 삼고자 2012년 1월 1일부터 2012년 12월 31일 지방 소재의 노인요양병원을 내원한 환자 2,500명의 척추 및 상지, 하지, 관절부의 일반 촬영건수 5,042건을 분석하였다. 노인연령은 보로디 연령구분법을 따라 3그룹으로 분류하였다. 병원에 내원한 노인 환자의 대부분이 척추질환과 하지관절질환에 특히 많이 노출되어 있었고. 어깨, 무릎, 요추 촬영 검사 건수가 절반 이상을 차지하고 있으며 공통적으로 연령대별로 1분기 건수가 가장 많았고 2, 3분기에 약간 감소세를 유지하다가 4분기에는 1분기에 비해 감소된 촬영 건수를 나타냈다. 앞으로 더 많은 노인을 대상으로 내원목적과 질환 발생 원인을 분석하고 예방대책을 강구하는 노력이 추가적으로 진행된다면 연구결과를 토대로 노인요양병원의 질환예방교육과 더불어 노인행동 계획을 세우고 농촌지역의 의료자원을 효율적으로 재분배하는데 기초자료로 활용될 것으로 기대된다.
Purpose: The first purpose of this study was to determine the effect of reciprocal inhibition that influence changes in pain when applied to patients with culf cramps. The second purpose of this study was to determine the effect of manual therapy on iliaosacral joint that influence changes number of occurrences when applied to patients with frequent nocturnal culf cramps. Methods: The first study using reciprocal inhibition techniques to 20 patients with calf cramps and saw the change in pain. The second study using manual therapy on iliaosacral joint to 2 patients with frequent nocturnal culf cramps and saw the change in number of occurrences during the week. Results: The pain and number of occurrences were significantly difference between pre-treatment and post treatment. Conclusion: Based on the results of this study, we found that reciprocal inhibition decreased pain and that manual therapy on iliaosacral joint decreased number of occurrences.
Purpose : The purpose of this study was to identify the effect of recovery of disability on post-stroke depression in stroke patients. Methods : Fifty patients with stroke were participated in this study. The stroke impact scale(SIS) was used to assess the recovery of disability according to stroke and short form of geriatric depression scale(SGDS) was used to assess the poststroke depression(PSD). Results : The suspicious for depression were exhibited in 38%(n=19) and patients with depression were 58% (n=26) of 50 patients with stroke. In correlation between recovery of disability and depression, emotion and recovery of stroke negatively correlated with depression(p<.05). Conclusion : These results demonstrates that patients with stroke need to manage and treat emotion for the prevention and management of PSD.
As Korea has just turned into an aging society with the increase of average life expectancy, and the main causes of deaths is cancer and other chronic diseases. And this corresponds to a dramatic increase in medical expenses for the aged. To curve this problem, the hospice care can be an effective alternative, which can provide patients with both quality service and intensive care to help ensure high quality life for the patients. To demonstrate the economical effect of hospice services, a comparative study on the media expenses of geriatric hospitals and general hospitals, which bear similarities in common regarding the characteristics of their patients, is performed. Thus the results of the study can serve as a quantitative indication for the management of hospice services.
A majority of patients who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but gradual return of sensation that is functional and tolerable, if not the same as before the injuries. However, long-term effects of such injuries are aggravating for many patients, and a few patients experience significant suffering. In some of these patients, posttraumatic symptoms become pathological and are painful. The predominant painful components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning and aching causalgiaform pain, and (4) phantom pain. This is a case report of conservative management of traumatic neuralgia and neuritis as part of posttraumatic pain syndromes in geriatric patients who have undergone the teeth extraction and alveoloplasty.
