병동간호사의 간호업무중단이 업무부담과 환자안전문화인식에 미치는 영향 (The Effect of Work Interruption on Workload and Perception of Patient Safety Culture in Ward Nurses)

  • Purpose:This study aimed to identify the effect of work interruption that influenced workload and perception of patient safety culture on ward nurses. Methods: Participants were 184 ward nurses, with more than 12 months of work experience, from two tertiary hospitals in S city. A descriptive correlational study design was used. Collected data were analyzed using descriptive statistics, t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis test, Pearson's correlation, and hierarchical multiple regression analysis via SPSS version 27.0. Results: The results showed that general wards nurses had a higher degree of work interruption (t=5.632, p<.001) and workload (t=3.603, p<.001) compared to comprehensive nursing care service wards nurses. More interruption in nursing work caused more burden on work (γ=.440, p<.001) and led to lesser perception of patient safety culture (γ=- .199, p=.007). Finally, the regression analysis showed that work interruption had a statistically significant relevance on nurses' workload (F=20.582, p<.001) and perception of patient safety culture (F=8.792, p<.001). Conclusion: To alleviate ward nurses' work interruption and decrease the negative effect on workload and perception of patient safety culture, it is necessary to mediate nurse staffing level and the number of assistants and utilize the environment.

간호사의 손씻기에 대한 지식 및 태도 연구 (A Study on Handwashing Knowledge and Attitudes of Nurses)

  • The purpose of this study is to identify the handwashing knowledge and attitudes of resistered nurses on general wards. Subjects for this study include 182 nurses working in general wards of a hospital in Seoul area. Data were collected by a questionnaire from July 30 to August 10,1996. Statistical analyses were done by the SPSS/PC program. The techniques used in this sudy included frequencies, chi-square test. The results of this study are summarized as follows. 1. 34.6% of the respondents reported that they washed their hand an average of 5 to 7 times during the work day. 2. 38.5% of the respondents reported always washing after each patient contact. Only 9.9% always washed before contact with a patient. 3. 56.6% of the respondents reported a washing time of 10 to 20 seconds, whereas 29.1% washed 21 to 30 seconds. Chi-square tests were employed to determine any differences in handwashing frequency and duration by age, working years, work position. There were no satistically significant differences among the variables. 4. 95.6% of the subjects used water and plain soap during the washing process. The respondents washed the palms of the hands, the backs of their hands and between their fingers(33%). Only 17.6% removed rings before handwashing. Similarly, 11% removed their wristwatch. The majority(96.2%) reported that they used communal textile towel to dry their hands. When asked what method they used to turn off the water faucet after washing, 98.4% reported using direct hand-to-faucet contact. 5. Nursing activities that showed the highest handwashing rates was after wound dressing(22.9%), followed by suctioning(21.4%), injection(21.2%), inserting catheter(18%). 6. Reasons cited for reduced handwashing frequency included being too busy to wash more often(74.7%), no need to wash more often(11.0%) and dry skin caused by frequent handwashing(3.3%). 7. When asked where they obtained their current knowledge of handwashing techniques, they reported professors(73.6%), resisted nurses(14.8%) and supervisor(6.6%). 8. Nursing staff thought that proper handwashing was important factor in decreasing nosocomical infections(72.4%).

노인전문병원의 공간구성과 부문별 면적배분에 관한 연구 (A Study of the Spatial Composition and Departmental Area Distribution in Geriatric Hospital)

  • As the population of the elderly increases drastically year by year in Korea, the more welfare centers for them are greatly needed. There are, however, a small number of public geriatric hospitals currently available. And a limited number of private-founded geriatric centers are under construction throughout the country. And the systematic standards for the spacial composition and departmental area distribution for them are not satisfactorily set up yet. The analytical study of the space composition and area distribution of the hospitals in question shows that publicly-shared spaces, such as day-room and lobby, are much more useful for their communications than private spaces, such as shut-in living rooms. It also shows that in-yards or in-gardens which are located in the center of the wards are very helpful for their comfortable long-term hospitalization. The shared spaces are recommended to be in sight of and within earshot of their caretakers. In relation with out-patient departments, the programs for the local elderly residents, such as the day-care center and recreation facilities are recommended to be prepared for their physical care and emotional treatment at a time by municipal welfare centers rather than by private institutions. The analytic study carried out in terms of departmental area distributions reveals that the areas for the wards are generally wider and areas for out-patient/or diagnosis departments are relatively narrower than those in the general hospitals. These area distributions seem to have come from the considerations of their relatively long-term staying in the centers than general hospitals and their mental stability as well.

Participation and Influencing Factors in the Decision-Making of Life-Sustaining Treatment: A Focus on Deceased Patients with Hematologic Neoplasms

  • Purpose: This study aimed to investigate the involvement of patients who died from hematologic neoplasms in the decision-making process surrounding the withdrawal of life-sustaining treatment (LST). Methods: A total of 255 patients diagnosed with hematologic neoplasms who ultimately died following decisions related to LST during their end-of-life period at a university hospital were included in the study. Data were retrospectively obtained from electronic medical records and analyzed utilizing the chi-square test, independent t-test, and logistic regression. Results: In total, 42.0% of patients participated in the decision-making process regarding LST for their hematologic neoplasms, while 58.0% of decisions were made with family involvement. Among these patients, 65.1% died in general wards and 34.9% in intensive care units (ICUs) as a result of decisions such as the suspension of LST. The period from the LST decision to death was longer when the decision was made by the patient (average, 27.15 days) than when it was made by the family (average, 7.48 days). Most decisions were made by doctors and family members in the ICU, where only 20.6% of patients exercised their right to make decisions regarding LST, a rate considerably lower than 79.4% observed in general wards. Decisions to withhold or withdraw LST were more commonly made by patients themselves than by their families. Conclusion: The key to discussing the decision to suspend hospice care and LST is respecting the patient's self-determination. If a patient is lucid prior to admission to the ICU, considerations about suspending LST should involve the patient input.

타액-알파아밀라제를 이용한 병원간호사의 직무스트레스 측정 (Measurement of Temporal Job Stress for Hospital Nurses using Salivary Alpha-Amylase)

  • As contemporary society has become more complicated, specialized, and segmented, people are experiencing more diverse types of stress. In particular, while several factors associated with job stress have been examined among nurses, who belong to a professional group, the existing research has made no quantitative assessments of stress that reflect temporal differences in individuals. Therefore, the aim of this study is to understand the effects of job stress on alpha-amylase with regard to the working hours of nurses, to assess the variations in jobs stress over time, and provide basic data to improve the quality of nursing services. Ninety nurses working in three shifts in general, emergency, and intensive care wards of a university hospital in D City participated in this study. Salivary alpha-amylase (SAA) was extracted and analyzed at two-hour intervals from 07:00 to 15:00 from nurses on the day shift and from 23:00 to 07:00 from those working the night shift. The SAA level was highest between 23:00 and 01:00 for nurses in general wards ($mean{\pm}S.D.\;39.00{\pm}14.88$) and between 11:00 and 13:00 for those in both intensive care units and emergency wards ($mean{\pm}S.D.\;67.50{\pm}62.93$ and $mean{\pm}S.D.\;39.67{\pm}35.96$, respectively). The characteristic variation in SAA was significant between 23:00 and 01:00 (p < 0.01) and for those in their fifties or older (p < 0.01). The activation ratio of alpha-amylase, a stress reactant, showed an increase when the sympathetic nervous system was activated by mental stress; in addition, job stress was manifested with the effect of awakening at different time segments and at different ages among the nurses. With the aim of raising the level of service based on the nurses maintaining their mental health, it is necessary to focus sharply on the time segment for critical control and to conduct repetitive studies to determine the divisions of eustress critical values as well as to expand the population.

종합병원 다인병실의 치유환경 개선을 위한 건축 계획적 연구 (A Study on the Architectural Planning for Improvement of the Healing Environment in Multi-bed Room of General Hospitals)

  • The purpose of this study is to know how to improve environment of multi-bed room by investigating and analysing factors influencing to patients. 1 measured stress and satisfaction degrees about patient room for 4ㆍ5ㆍ6ㆍ7 men of domestic general hospital wards. The results of research show that patients of 4 bed room appeared the fewest stress and highest satisfaction degree among patients of other room styles. The patients positioned between two beds can give us more stress and fewer satisfaction than others. These results have proven that 4 bed room patients are offering comfortable environment than any other room types as a healing environment. When patient room area are more than 7.3$M^2$ per 1 person, bed interval space is more than 120cm, there is curtain between bed, transit space such as a toilet were established in patient room, patients' satisfaction appeared high.

  • The purpose of this study is to suggest various index concerning the nursing services and traffic efficiency between two types of nursing care system - Team Nursing Method and Functional Nursing Method. Through the investigations on activities of 5 day-duty nurses of ward in each general hospital occupying one method of them. So, it can be suggested that the basic data for the archtectural planning of wards in general hospitals must be considered to accept the hospital environmental requests.

The important result and advantage of the system such as HcCad-System for design and plan of wards are as follows; At first, the system useful for communication medium and instructional tool for ward planners and designers. Secondly, fast and economical evaluation are obtained through the study of experimental simulation. Finally as a new tool in architectural field, system helps to enhance the relationship between computer and human.

임상간호사의 통증관리에 대한 지식과 태도 (Clinical Nurses' Knowledge and Attitudes on Pain Management)

  • The purpose of this study was to understand clinical nurses' knowledge and attitudes on pain management. The subject of the study were 254 nurses working at two hospitals affiliated with a university in Seoul. The questionnaires included four areas: general knowledge on pain, knowledge on the use of analgesia, knowledge and implementation on the pain assessment scales and pain interventions and nurses' general characteristics. The data were analyzed with descriptive statistics, analysis of variance, LSD test and t-test using SPSS statistical package. The results were as follows. 1. The mean score of the general pain knowledge was 61.46 and that of knowledge on the use of analgesia was 52.19. 2. Most nurses(74%) answered with hesitation about injecting narcotic analgesia to patients. 3. The pain assessment scale which nurses knew (57.5%) and used(48.0%) extremely was a simple descriptive scale. 4. The pain intervention which nurses knew (94.5%) and implemented(92.1%) extremely was to inject analgesia. 5. The number of nurses who had learned about pain management was 49 of 254(19.3%). 6. Nurses' knowledge on the use of analgesia was of relevance to having learned pain management, but general pain knowledge was not so relevant. According to this research, I suggest the following. 1. It is necessary to develop an education program with actual practice and intervention which nurses can perform for themselves. 2. It is necessary to continuously educate about pain management in clinical wards.

일 종합병원 병동 간호단위의 간호효율성 평가-자료포락분석의 적용 (Evaluation of the Efficiency of General Nursing Units using Data Envelopment Analysis (DEA))

  • Purpose: The aim of this study was to evaluate the efficiency of nursing units in a hospital using DEA. Methods: Data were collected using the medical information system of a general hospital in a city. Input variables were number of nurses, number of nurse-aides, number of beds, and overhead costs. Output variables included number of admitted patients, rate of bed utilization, satisfaction of discharged patients, and prevention rate of safety accidents and sores. EMS Window version 3.1 was used to measure the efficiency score and descriptive statistics were applied to analyze the general characteristics of variables. Results: The average efficiency score of 18 general nursing units was approximately .99. Nine nursing units had a 1.00 efficiency score. Conclusion: Our findings reveal that nursing units operated very efficiently. To increase efficiency of inefficient nursing units, we recommend results of the DEA slack analysis as a benchmark of the most efficient nursing unit.
