An Analysis of Nurse Staffing Level and Nursing Performance in the General Unit (일반병동 간호인력 확보수준과 간호업무성과 분석)

  • Purpose: To examine the relationship between nurse staffing level and nurse perception of nursing performance. Method: The subjects of this study were 724 nurses from 70 medical or surgical wards in 23 hospitals from February to March 2006. A self-reporting questionnaire, which was developed by the researcher through a preceding study, was used. In data analysis, SAS program was utilized for descriptive statistics, t-test, ANOVA, and Tukey test. Results: The mean score of the nursing performance was 53.3. The mean of bed-to nurse ratio was 3.39:1. The study revealed a negative correlation between bed-to nurse ratio and the nurses' perception of nursing performance. There was a significant relationship between above 3.5:1 and below 2.5:1(F=4.59, p=0.010). Conclusion: We found that the higher the nurse staffing, the better the nursing performance. It is recommended that future research measures nurses' performance and nursing organizational performance using objective measurement tools including clinical indicators.

Factors Influencing Relocation Stress Syndrome in Patients Following Transfer from Intensive Care Units (중환자실 환자의 일반 병동 전실 시 스트레스 영향요인)

  • Purpose: The purpose of this study was to identify the levels of relocation stress syndrome (RSS) and influencing the stress experienced by Intensive Care Unit (ICU) patients just after transfer to general wards. Methods: A cross-sectional study was conducted with 257 patients who transferred from the intensive care unit. Data were collected through self-report questionnaires from May to October, 2009. Data were analyzed using the Pearson correlation coefficient, t-test, one-way ANOVA, and stepwise multiple linear regression with SPSS/WIN 12.0. Results: The mean score for RSS was $17.80{\pm}9.16$. The factors predicting relocation stress syndrome were symptom experience, differences in scope and quality of care provided by ICU and ward nursing staffs, satisfaction with transfer process, length of stay in ICU and economic status, and these factors explained 40% of relocation stress syndrome (F=31.61, p<.001). Conclusion: By understanding the stress experienced by ICU patients, nurses are better able to provide psychological support and thus more holistic care to critically ill patients. Further research is needed to consider the impact of relocation stress syndrome on patients' health outcomes in the recovery trajectory.

  • Social Media is the open online tool and media platform for sharing and participation of users opinion, experience, viewpoiont, so general situation that is one-side flowing from production to consume doesn't act, and while use of two-way, user create contents use of sharing and participation. This social media include Blog, Social Network Service(SNS), Wiki, User Create Contents(UCC), Micro Blog, 5 types. In broad terms, Social Learning is self-learning that user sharing with coperation and collective intelligence through Social Media, and in few wards Social Learning is learning for Social Media. In this research, we define Social Media and Social Learning, and research of method of use of Elementary Education.

A Review of Research on Hospital Nurses' Turnover Intention (병원간호사 이직의도에 관한 문헌 분석)

  • Purpose: The purpose of this study was to review articles, thesis and dissertation on turnover intention of hospital nurses in order to identify overall trends in turnover intention of nurses and suggest strategies for reducing turnover intention. Method: Thirty research papers on turnover intention of nurses were reviewed. Result: These papers were journal articles and thesis regarding nurses who worked in general wards, intensive nursing units, and emergency rooms in hospital. The tools used to measure turnover intention were Lawler (1983), Mobley (1982), and Lee (1995), and others. Most of tools were composed of 1-11 items, with 4-7 point Likert scales. Reported reliability was .55 to .91. The mean score for turnover intention was between 2.40 and 3.85. The important results revealed that job satisfaction, organizational commitment, and stress affected turnover intention of hospital nurses. Conclusion: The findings from this review indicate that to reduce turnover intention for hospital nurses, a nurse manager should increase job satisfaction, organizational commitment, and reduce stress by developing a good culture and work climate. Moreover it is necessary to assess stress and job satisfaction of nurses, and to manage nurses' welfare, including night duty policy, salary, and career ladder.

Development of Tablet PC-Based Multimedia Educational Contents for Patients and Their Family Undergoing Thoracic Surgery (흉부수술 환자와 가족을 위한 태블릿 PC기반 멀티미디어 교육자료 개발)

  • Purpose: This study aims to develop Tablet PC-based multimedia educational contents on the procedures from surgery preparation to recovery for hospitalized patients who are going to undergo thoracic surgery and their families. Methods: The educational contents were created based on literature review and a survey of needs in patients and their families who were admitted to a hospital in Seoul from Dec. 2013 to May 2014. The contents were developed in conjunction with a film production company. The educational materials including texts, images, sounds, and videos were developed appling the ADDIE (analysis, design, development, implementation, and evaluation) model. Results: Two multimedia educational contents were developed for the patients with coronary artery bypass graft or lung surgery. Each content consisted of six subdivisions of self-learning for 20 minutes. The developed educational contents were completed after testing content validity by 30 medical experts. Conclusion: This study recommends that the Tablet PC-based educational contents for thoracic and cardiovascular patients and their families be used in general wards and intensive care units to relieve their anxiety before surgery and reduce postoperative complications.

Two Cases of Contact Urticaria Syndrome from Cefotiam in Nurses (Cefotiam에 의한 접촉 담마진 증후군 2례)

  • Contact urticaria describes a wheal and falre response elicitied within 30-60 minutes after cutaneous exposure to certain agents. Contact urticaria encompasses a number of different clinical manifestations and the symptoms which can vary from the mildest forms of burning, stinging and itching sensation to life-threatening anaphylaxis referred to as contact urticaria syndrome. Cefotiam is one of the most popular second generation cephem antibiotic used in korea. Since 1975, contact urticaria due to cephalosporins has been reported, and also, in japan, this reported from about last 15 years. Recently we experienced three nurses working at the general wards with showing contact urticaria and anaphylaxis after occupational exposure to cefotiam antibiotics.

Safety Management Status among Nurses Handling Anticancer Drugs: Nurse Awareness and Performance Following Safety Regulations

  • This study identified the actual conditions for safe anticancer drug management among nurses and the relationship between level of awareness and performance of anticancer drug safety regulations in terms of preparation, administration, and disposal. The respondents were 236 nurses working with chemotherapy in wards and outpatient clinics in five hospitals in and near Seoul. Safety regulations provided for the anticancer drug the Occupational Safety Health Administration (OSHA, 1999), as modified for an earlier study, were used. The results showed that the level of awareness and performance on the anticancer drug safety regulations indicate their preparation ($3.38{\pm}0.55$, $2.38{\pm}0.98$), administration ($3.52{\pm}0.46$, $3.17{\pm}0.70$), general handling and disposal ($3.33{\pm}0.54$, $2.42{\pm}0.90$) on a scale 0 to 5. Also, there were significant differences in job positions, work experience, type of preparation, and continuing education and a positive relationship between the level of awareness and nursing performance. Thus, nurses should receive continuing education on the handling of anticancer drugs to improve the level of performance following safety regulations.

A Study of the Head Nurse's Leadership and the Organizational Effectiveness as Perceived by Staff Nurses (간호사가 지각한 수간호사의 리더십과 조직유효성에 관한 연구)

  • Purpose: This descriptive study was designed to find out the relation between nurses' perceptions of leadership and organizational effectiveness. Method: The data was collected from nurses in 451, a tertiary hospital located in B city by means of structured questionnaires. The data was analysed by t-test. ANOVA, Pearson correlation coefficient. Results: The average age of the subjects was 29.1. Regarding the marital status, 68.1% were single. Regarding the nursing unit, 42.6% were working for the general surgical wards. The total period of clinical career was on average 73.7 months. Regarding the period in current ward, 51.0% were 1-less than 3 years. There was a positive correlation with transformational leadership, transactional leadership, job satisfaction, and organizational commitment, whereas the turnover intention had a negative correlation with transformational leadership and transactional leadership. Conclusion: The nurses' different perceptions of transformational leadership, transactional leadership, and organizational effectiveness have strong correlative relationships with each other. This finding suggests the importance of the head nurse's leadership in increasing organizational effectiveness.

Effects of Workplace Bullying, Nursing Work Environment, and Reality Shock on Turnover Intention of Clinical Nurses (임상 간호사의 괴롭힘, 간호근무환경, 현실 충격이 이직 의도에 미치는 영향)

  • The purpose of this study was to identify the relationship between bullying, nursing work environment, reality shock and factors of influence on turnover intension. Data were collected from 381 nurses who were under the charge nurse at a hospital with more than 600 beds in Seoul. The collected data were analyzed using the SPSS24.0 program. The mean score for turnover intension was 3.61 ± 0.89. The intension of turnover was correlated with bullying in the workplace (r=0.197, ⍴=.001), the nursing work environment (r=-.409, p=.001), and reality shock (r=.572, p=.001). Variables affecting turnover intension were the work department(β=-0.018, ⍴=.049), working environment(β=-0.424, ⍴<.000), and reality shock(β=0.654, ⍴<.000). The total amount of explanation for turnover intension was 37.3%. The degree of turnover intension of nurses was affected by work in general wards, the experience of workplace bullying and the nursing work environment. To reduce turnover intension, nursing work needs to be analyzed and efforts are needed to establish an effective communication system.

  • Purpose: This study was designed to develop a computerized program for patient classification and record of basic nursing care. Method: The study was carried out from February to May 2004 at the four general wards in A university hospital using three steps; investigation of needs for program development, program design and development, and program application and evaluation. Result: Results of this study showed that users were mostly satisfied with the program and set a high value on the usefulness and performance of this program. Accordingly it was found that this program could be of help to assist nurses in classifying the patients and taking records of their nursing service. Conclusion: The program is likely to contribute to enhance the efficiency in providing appropriate and effective nursing service to the patients by easily identifying the overall particulars of patients and improving repeated manual works. And Record of basic nursing care is a part of several nursing records. As such the continual development on the computerized program for various fields of nursing service is needed.
