A SURVEY OF DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA AT CHONBUK NATIONAL UNIVERSITY DENTAL HOSPITAL BETWEEN 2014~2017 (2014년부터 2017년까지 전북대학교 치과병원에서 외래 전신마취 하에 치과치료를 받은 증례에 관한 연구)

  • There are increasing number of patients having dental treatment under outpatient general anesthesia. There were researches that studied patients treated under outpatient general anesthesia in Chonbuk National University Dental hospital between 2005~2013. However, there has been an absence of research since 2013. Therefore, the aim of this research is to study cases regarding outpatient general anesthesia visiting Chonbuk National University hospital between 2014~2017 and analyze the trends of these patients. The medical records of 409 cases from January, 2014 to December, 2017 were reviewed. From the patients' charts, data on sex, residence, visiting dates, disability status, and types and number of treatments was collected and investigated. The number of cases has increased from 86 in 2014 to 140 in 2017. Among 409 cases, 275 cases(67.2%) had disabilities, with mental disabilities consisting most of the disabilities. 192 cases were aged under 20 years old mostly with no disabilities, 142 cases were 20~39 years old mostly with mental disabilities, and 75 cases were 40 years old or more mostly with physical disabilities. The conservative treatment was performed most frequently in both disabled and non-disabled patient groups. There was an increasing trend in the number of patients treated under outpatient general anesthesia, and more than half were disabled patients. In order to meet this demand, Chonbuk National University Dental Hospital increased the number of general anesthesia pracrice. To follow this trends, the implementation of safe outpatient general anesthesia system and cooperation among teams is needed to improve the quality of treatments.

THE EFFECT OF DENTURE PLACEMENT ON NUTRITION STATUS AND QUALITY OF LIFE IN THE URBAN ELDERLY (총의치 시술이 도시노인의 영양 및 삶의 질에 미치는 효과)

  • Purpose : This study was conducted to evaluate the effect of denture placement on nutrition status and quality of life in the urban elderly Material and method : The first survey was conducted from 45 targeted elderly people(25 experimental group, 20 control). In the first survey, information on general characteristics, chewing ability, food intake and nutrient intake, anthropometric measurement, and degree of satisfaction in life were obtained. The second survey was conducted to evaluate the effect of denture placement with same method as the first survey. Results : Before denture placement, there are no statistical significant differences (in general characteristics, chewing ability, nutrient intake, anthropometric measurement, and degree of satisfaction in life) between experimental group and control. But after denture placement, survey shows that there is statistically significant difference in chewing ability between two groups. The experimental group have higher intake of energy, protein, carbohydrate, iron, and vitamin C. in nutrient intake. And after denture replacement, there is significant improvement in degree of satisfaction in life. After denture placement there are significant changes in agitation and attitude toward own aging according to Lawton's factor classification. Conclusion: In short, this study shows that chewing ability improvement of the elderly has strong positive effects to their food intake, nutrient status and quality of life.

A Study on Gender Difference in Assessing Emotional Design Characteristics of General Hospital's Wards (사용자 성별에 따른 종합병원 병동부 감성디자인 특성 평가에 관한 연구)

  • Purpose: This research was for understanding the attitudinal difference by gender towards emotional design through questionnaire survey with female and male inpatients on environmental characteristics of wards in general hospitals. Methods: The survey was conducted by questioning inpatients at two general hospitals on the importance rating on emotional design elements of patient rooms, lounges, and hallways. Eighty questionnaires were returned and used for data analyses through SPSS windows version 15.0 statistic package program. Results: 1) In general, female patients considered emotional design of wards as more important than male patients and the most outstanding difference was indicated for patient rooms among patient rooms, lounges, and hallways. For patient rooms the comfortability index was rated as the most important to both female and male inpatient groups, and for lounges and hallways the safety index was evaluated as the most important to both gender groups. 2)For lounges, while male patients rated 'prevention of infection' important among safety relating items, women considered 'accident prevention' more important. It is inferred that female patients have more safety needs and anxiety about physical injury or accidents than male ones do. Implications: It is considered that there need to be further succeeding in-depth studies, e.g. research interviews with inpatient;s family members or other caregivers as well as patients themselves.

  • The purpose of this study is to survey the status of oral care of adult patients with disabilities and satisfaction degree of outpatient general anesthesia for dental treatment. 30 adult patients who underwent general anesthesia for dental treatment at clinic for the disabled in Seoul National University Dental Hospital enrolled for telephone survey. Many patients (83.3%) had mental problems to do social communication. And someone of family members (mainly mother) had cared for the patient but there was only one patient who got regular oral care. The satisfaction level of outpatient general anesthesia and nursing care for dental treatment was very high(100%).

  • Many disabled patients need extensive dental treatment because they have much difficulty in maintaining their oral hygiene. However, because they are not cooperative and not manageable, they require physical restraints, drug induced sedation or general anesthesia. General anesthesia is useful in control of the patients who cannot be treated in other ways. Additionally, general anesthesia provides more safe environment for medically compromised patients. The aim of the present study was to investigate a kind of disorder, sources of referral, preoperative oral condition, experience of dental visit, and performing general anesthesia or not of 930 disabled patients in Kyung-Hee University dental hospital for 30 years.

Comparison of Experiences of Ethical Dilemma between Intensive Care Unit and General Unit Nurses regarding Treatment Decisions and Confidentiality (의료처치 및 환자 비밀유지에 대한 중환자실 간호사와 일반병동 간호사의 윤리적 갈등 경험 비교연구)

  • Purpose : This study compares experiences of ethical dilemma between nurses working in intensive care units and those in general units under specific situations of treatment decisions and confidentiality. Method : This cross-sectional descriptive study utilizes the self-report survey method. The survey questionnaires were completed by 50 and 52 nurses working in intensive care units and general units, respectively. The instrument, which consisted of 16 items of ethical dilemma situations about treatment decision and confidentiality, was used. The mean scores for each item were compared between the two groups. Results : The study found no differences in terms of age, gender, education level, clinical experience in years, and being educated on healthcare ethics. For 9 out of 16 items, the mean scores of nurses in intensive care units were significantly higher than those of nurses in general units. Conclusions : Nurses in intensive care units experienced ethical dilemmas regarding treatment decisions and confidentiality more often than those in general units. This study emphasizes the need to establish strategies for improving the ethical competence of critical care nurses.

A Comparative Study on the Operation Status of Comprehensive Nursing Care Ward (간호·간병통합서비스 제공 병동의 운영현황 비교연구)

  • Purpose: The purpose of this study was to contribute to the activation of comprehensive nursing care services by comparing and analyzing the working environment and operational status of comprehensive nursing care wards with general wards. Methods: A questionnaire survey was conducted at hospitals operating comprehensive nursing care services. We collected data on patient characteristics, workforce, working environment, and work support facilities of 40 hospitals voluntarily participating in the survey. The collected data were analyzed by dividing it into an comprehensive nursing care ward and a general ward. Results: Compared with the general ward, the comprehensive nursing care ward had fewer hospitalized patients, but the severity of the patients and the need for nursing were high. The number of nurses, assistant nurses, and ward support workers per patient was higher in the comprehensive nursing care ward than in the general ward, and the implementation rate of night shift fixed system, the fixed shift system, and the preceptor system were higher. In terms of structural environment, comprehensive nursing care wards had more room for interviews. Conclusion: The comprehensive nursing care ward had more nursing staff and provided a better working environment than the general ward, but required additional support for the structural environment.

Associations between Airborne Manganese and Blood Manganese in the Korean General Population according to KNHANES 2008-2009 (한국인의 혈중 망간농도와 공기중 망간농도의 관련성)

  • The objective of this study was to evaluate associations between airborne manganese and blood manganese in a general population of South Korean adults. The concentrations of airborne manganese in total suspended particulate (TSP) were calculated from data obtained from ambient air-monitoring stations (AAMSs) located in South Korea. Blood manganese data obtained Korean National Health and Nutrition Examination Survey (KNHANES) using a rolling sampling design involving a complex, stratified, multistage, probability cluster survey of a representative sample of the non-institutionalized civilian population of South Korea. Airborne manganese geometric means was 46.10 $ng/m^3$, blood manganese geometric means were 1.19 ${\mu}g/d{\ell}$ for male and 1.40 ${\mu}g/d{\ell}$ for female. In multiple linear regression analysis of log transformed blood manganeseas a continuous variable on airborne manganese, after adjusting for covariates including gender, age, job, smoking and drinking status, education level, BMI (body mass index). Airborne manganese was positively associated with blood manganese with statistical significance. The present study confirms that airborne manganese is a possible contributor to the increase of blood manganese in the adult general population.

A Study on a Bycatch of the Gape Net in Jindo Area of Jeollanam-do (전남 진도지역 낭장망 어업에 있어서 혼획 연구)

  • In order to survey the conditions of bycatch and discards for a gape net, a series of filed surveys was carried out total 10 surveys on two gape nets operated in the coastal area of Jindo during July to November 2008. The all catches in each survey were measured the body length, weight after sorted species. The analysis of catch data was conducted to separate 'first operation' and 'general operation'. In this study, the first operation defined as hauling the fishing gears that soaked over 10 hours because of setting on the previous day. The general operation means the haul of fishing gears conducted at intervals of one of or two hours after first operation. The bycatch in this study is defined as the other species except for anchovy, and bycatch rate refers to the proportion of bycatch to total catch. From the survey results, the average bycatch rates on first operations were 50% and 51% for Masa-ri and Supum-ri areas in Jindo, respectively. Also, the average bycatch rates on general operation were 2% and 3% for Masa-ri and Supum-ri areas in Jindo, respectively. The proportion of anchovy to total catch caught represented 75.5%. It needs the research on the selective fishing technology to reduce the bycatch on the gape net fishery.

Relations between oral health status and subjective oral health recognition in Korean adolescents (한국 청소년의 구강건강 실태와 주관적 구강건강인식의 관련성)

  • Objectives: The purpose of the study is to investigate the relations between oral health status and subjective oral health recognition in Korean adolescents. Methods: The survey data were extracted from the 2012 Korea National Oral Health Survey from June to November, 2012. The survey consisted of oral checkup data and the oral health interviewing data of 9,981 adolescents(5,335 male, 4,646 female) by proportional distribution. The questionnaire included general objective oral health status and subjective oral health recognition. The subjective oral health recognition consisted of general characteristics of the subjects, oral health status, oral health behavior, and subjective oral health recognition. Results: According to multiple regression analysis for the relations between general characteristics and subjective oral health recognition, subjective health recognition, the influencing factors were gender, age, the number of tooth brushing per day, intake of snacks and carbonated drinks, regular oral examination, decayed teeth (DT), missing teeth (MT,) and DMFT. Conclusions: The routine oral checkup is the most important method to improve the oral health management in the adolescents. The continuing oral health improvement project must be implemented based on the law and will keep the adolescents in good oral health status.
