A Study on the Development of Computerized Program for the Standardized Nursing Documentation Form;Based on A University Hospital (전산화된 표준간호서식 프로그램 개발에 관한 연구;일 대학병원 중심으로)

  • Purpose: This study was designed to develop a computerized program for patient classification and record of basic nursing care. Method: The study was carried out from February to May 2004 at the four general wards in A university hospital using three steps; investigation of needs for program development, program design and development, and program application and evaluation. Result: Results of this study showed that users were mostly satisfied with the program and set a high value on the usefulness and performance of this program. Accordingly it was found that this program could be of help to assist nurses in classifying the patients and taking records of their nursing service. Conclusion: The program is likely to contribute to enhance the efficiency in providing appropriate and effective nursing service to the patients by easily identifying the overall particulars of patients and improving repeated manual works. And Record of basic nursing care is a part of several nursing records. As such the continual development on the computerized program for various fields of nursing service is needed.

An Analysis of Studies on Clinical Nurses' Job Stress (임상간호사의 직무스트레스에 관한 국내논문분석)

  • Purpose: To understand the recent trend of studies for nursing job stress, we collected Korean articles about job stress of clinical nurses. Method: Seventy two articles were selected from the database of "richis", Korean journals of related academic society, and dissertation papers, which are presented from 1981 to 2002. Result: The trend shows that the number of researches for the job stress of clinical nurses are increasing from the year of 1996. Most of the researches are for dissertation papers (75.0%), have done surveys (90.3%), and used questionnaires (94.4%). The nurses who were studied have their jobs in general wards of hospitals (50.6%). And 36.1% of the researches used the tool of Kim and Gu (1984) to measure job stress. These researches focused mainly on job satisfaction, burnout and coping successively. Conclusion: As only 6 out of 72 researches examined in this study have experimental design (8.3%), it can be said that more experimental studies are needed in this area. And we might also consider to use meta-analytic techniques for further studies, because these researches have used very similar tools for the same subject.

Development and Feasibility of a Video Recording-Based Standardized Handoff Program for Ward Nurses: A pilot study (비디오 녹화 기반 병동 간호 인수인계 프로그램 개발 및 적용가능성 : 예비연구)

  • Purpose : This study develop a Video Recording-Based Standardized Handoff (VRSH) program at shift change for ward nurses. Methods : The study was conducted in five medical, three surgical, and one comprehensive nursing care service wards affiliated with a secondary general hospital. In this methodological study, the VRSH program was developed between April and December, 2017. It is noted that 65 nurses who were involved in the VRSH program participated in this study. Results : In line with the modified Situation, Background, Assessment, Recommendation tool, the VRSH program consisted of three phases. In the VRSH program, the average time for handoff duration was 3-5 minutes per patient. More than 90% of the ward nurses were satisfied with the VRSH program since it benefited them by reducing overtime work and improving the performance, as well as effective communication, of nurses. The content analysis of nurses' VRSH program experience, revealed three categories and eight sub-categories. Conclusions : This study provides evidence that the VRSH program improves effective nursing performance and, the handoff communication and relationships between nurses. Future studies on large sample sizes and multiple settings are required to substantially evaluate the impact of the VRSH program on clinical outcomes.

A Study on Psychological Care for Hospitalized Patients (입원 환자에 대한 심리 간호 연구)

  • The purpose of this study were to find out the perceived importance of psychological care, to investigate the nurses perception on the quantity of psychological care activities, and the correlation between the structual variables. 206 registered nurses from general wards of 3 university hospitals in Seoul were randommly sampled. Data were gathered by check-list on the perceived importance and perceived quantity and were analysed by the frequency, percentile scores, ANOVA and Correlation-coefficient. Results are as follows 1. Majority of subjects(96.6%) responded to the psychological care for hospitalized patients being very, and most important nursing care activity Two-third(66%) revealed to be give more or less satisfactory amount of psychological care-to the hospitalized patients. 2. Heavy work-load(79%), personal factors of(33.0%) nurses and the health team and factors of nurse-patient relationship (20.9%) revealed to be the major causes hindering psychological care. 3. To improve quality and the quantity of psychological care, personal and professional development of nurses and the health team(56.8%), improvement of nurse-patient relationships (49.03) and improvement of nurse patient ratio(45%) were suggested. 4. The perceived quantity of psychological care and the educational status revealed significant correlation(p<.005). The perceived quantity and position in nursing revealed significant correlation (p<.01). 5. No significant correlation was revealed between the perceived importance and the perceived quantity of the psychological care.

A Study on Developing Free Size Patient Clothing (Free size 환자복 개발을 위한 실험적 연구)

  • Background : Hospitals in Korea are in trouble at ecomomic matters because of low cost of medical insurance, and consumers' demand for high quality. The management of inpatient clothing is very difficult. Especially the management of patient's clothing size is very difficult because the case mix of patients in wards is very different from the stocked clothing size. Because of the economic matters, the ward does not prepare enough size of clothing. The nurses in ward are in trouble with managing the clothing. So the nurses want free size patient clothing for ease management of clothing size. Method : The experimental inpatinets clothing is designed for in 170cm height refer to Korean standard. The fit of 8 subjects from 150 cm to 185 cm wearing experimental were evaluated by clothing specialists, nurses and patients. Result : 75.8% of evaluators like free size clothing. This results show that the fit of subjects from 160cm to 180cm was good in general but subjects with 150cm, 155cm, 185cm height showed poor fit.

The Risk Factors Related to Early Readmission to the Intensive Care Unit. (중환자실 조기 재입실 관련 위험요인)

  • Purpose : The purpose of this study was to identify status and characteristics of patients who have been readmitted to ICU, and to analyze risk factors associated with the readmission to ICU within 48hours. Method: Data were collected from patient's electronic medical reports from one hospital in B city. Participants were 2,937 patients aged 18 years old or older admitted to the ICU. Data were analyzed using odd ratios (ORs) from multivariate logistic regressions. Results: 2.2% of the 2,937 patients were early readmitted to ICU. Risk factors for early readmission to ICU were existence of respiratory disease, use of mechanical ventilator, and duration of hospitalization (longer). Conclusion: The assessment on the respiratory system of the patient who will be discharged from the ICU was identified as an important nursing activity. Therefore, the respiratory system management and education should be actively conducted. In addition, early ICU readmission may be prevented and decreased if a link was built to share the information on patient condition between the ICU and general wards.

A Study of the Spatial Composition and the Facility Criteria of In-patient Rooms in General Hospitals after MERS 2015 (2015년 메르스 사태 이후 신축된 종합병원 병동부의 공간구성 특징 및 입원실 세부시설기준에 관한 연구)

  • Purpose: This study is to address the spacial composition of a standard ward and bedroom size for sake of infection control and efficient medical service. Methods: Spacial composition of a standard ward has been proposed by comparative analysis of 5 big hospitals' wards. Bedroom sizes have been explored on the ground of Health care facility regulations from Korea, USA, Australia and Canada. Of course, Literature and field survey have been conducted in order to draw out various bedroom sizes. Results: 16 basic and some other additional spaces have been proposed for the composition of hospital standard ward. Area of Single bedroom is $11.6m^2$, and that of multi-beded room is $7.4m^2$. Bed to bed Clearance is 1.5m, spacing between bedsize and hard wall is 0.9m in 1~2 beded room, 0.75m in 4-beded room. Space clearance between Foot side of bed and curtain is proposed as 0.3m and additional 0.9m is necessary for the circulation. Implications: The result of this study can be applied to the new cons.

Delirium Experience of the Intensive Care Unit Patients (중환자실 환자의 섬망 경험)

  • Purpose: The study aimed to understand the delirium experience of intensive care unit (ICU) patients. Methods: We performed a qualitative study using Colaizzi's phenomenological method. Eleven patients, who experienced delirium according to the Confusion Assessment Method for ICU, participated after transferring to general wards from the ICU. Individual in-depth semi-structured interviews ranging from 30 minutes to 2 hours in length were conducted between November 2018 and August 2019. Results: Nine themes and four theme clusters emerged. The four theme clusters were: 1) "Overwhelmed by fear," which describes the experience of a patient close to death and the feeling of difficulty in understanding disorganized thinking; 2) "Anxious about not understanding the situation," which means that patients' sense of time and space were disordered in the ICU; 3) "Being deserted," which indicates the feeling of being separated from others and yourself; and 4) "Resistance to protect my dignity," which indicates that the dignity and autonomy of an individual in the patient's position at the ICU, are ignored. Conclusion: Nursing interventions are needed that would enable patients to maintain orientation and self-esteem in the ICU. In addition, healthcare providers need to provide information about the unfamiliar environment in the ICU in advance.

Comparison of Standards for healthcare Facilities and Environmental Investigation to Analyze Guidelines and Current Status of Healthcare Facilities (의료시설 관련 기준 비교와 환경 조사를 통한 의료시설 지침 및 현황 분석)

  • Purpose: This study aims to analyze and supplement the standards related to healthcare facilities, negative pressure isolation wards, and emergency treatment facilities. In addition, through environmental investigations, analysis of emergency remodeling cases centered on the structural and HVAC characteristics of healthcare facilities is conducted. Methods: Domestic and foreign standards related to healthcare facilities were analyzed. Field investigations and architectural drawing analysis of general and emergency treatment facilities were conducted. Results: Healthcare facilities have different space classifications and air conditioning methods depending on the site situation. Emergency treatment facilities are classified into cases where the HVAC system is remodeled and portable negative pressure unit is installed, and some facilities did not meet the standards for differential pressure and air change rate. Implications: When developing emergency remodeling technology, remodeling and safety evaluation guidelines, it is considered possible to propose clearer guidelines for emergency remodeling treatment facilities for infectious diseases in Korea by referring to the results of this study.

Comparison of Perceived Nurses' Cultural Competence of Nurses and Foreign Patients (간호사의 문화적 역량에 대한 외국인 환자와 병원 간호사의 인식 비교)

  • Purpose: This study aimed to examine the differences in Korean nurses' cultural competence perceived by nurses and foreign patients in a general hospital. Methods: The study participants were recruited from outpatient clinics and wards in H General Hospital located in Busan. Cultural competence was measured by the Cultural Competence Scale for Korean Nurses, consisting of 4 subscales: cultural perception, cultural knowledge, cultural sensitivity, and cultural skills. Data were collected from March 31 to April 30, 2014 by distributing and collecting a self-administrated questionnaire set. Data of the 90 foreign patients and 90 nurses were analyzed using PASW statistics 18.0 for independent t-test and ANOVA. Results: The level of cultural competence of Korean nurses perceived by foreign patients was significantly higher than that of nurses. On the cultural awareness dimension, nurses rated themselves more highly than foreign patients did while ratings on the dimensions of cultural knowledge and cultural skill were the opposite. Conclusion: This study indicates that there was a statistically significant difference in the perception of nurses' cultural competence between nurses and foreign patients. The findings can be used for developing education programs to enhance nurses' cultural competence, integrating the perspectives of foreign patients.
