홍수 위험도 척도 및 예측모형 연구 (Study on Measurement of Flood Risk and Forecasting Model)

  • There have been various studies on measurements of flood risk and forecasting models. For river and dam region, PDF and FVI has been proposed for measurement of flood risk and regression models have been applied for forecasting model. For Bo region unlikely river or dam region, flood risk would unexpectedly increase due to outgoing water to keep water amount under the designated risk level even the drain system could hardly manage the water amount. GFI and general linear model was proposed for flood risk measurement and forecasting model. In this paper, FVI with the consideration of duration on GFI was proposed for flood risk measurement at Bo region. General linear model was applied to the empirical data from Bo region of Nadong river to derive the forecasting model of FVI at three different values of Base High Level, 2m, 2.5m and 3m. The significant predictor variables on the target variable, FVI were as follows: ground water level based on sea level with negative effect, difference between ground altitude of ground water and river level with negative effect, and difference between ground water level and river level after Bo water being filled with positive sign for quantitative variables. And for qualitative variable, effective soil depth and ground soil type were significant for FVI.

고위험 신생아 어머니의 상황의미와 모성자존감 (Situational Meaning and Maternal Self-esteem in Mothers with High Risk Newborn)

  • Purpose: This study was designed to explore the relationship of situational meaning with maternal self-esteem in mothers with high risk newborn. Method: The subjects of this study were 82 mothers with high risk newborn. Data were collected using a translated Family Meaning Attribution Scale and Maternal Self-Report Inventory. Data were analyzed using descriptive statistics, t-test, Pearson Correlation Coefficients and Stepwise Multiple Regression. Result: The average score of the situational meaning in high risk newborn mothers was 64.0l(possible score is between 0-96) and the average score of each item was 1.98. The average score of the maternal self-esteem in high risk newborn mothers was 81.96(possible score is between 26-104) and the average score of each item was 3.15. No significant differences were found in situational meaning according to general characteristics except whether it was a planned pregnancy or not. No significant differences were found in maternal self-esteem according to general characteristics except disease or admission experience during pregnancy. There was significant positive correlation between situational meaning and maternal self-esteem. Conclusion: It is necessary for nurses to provide high risk newborn mothers with care for improving situational meaning that is attributed to the mothers. It can be helpful to improve maternal self-esteem and in the end it will facilitate the maternal transition in mothers with high risk newborn.

초기 청소년의 건강위험행위에 영향을 미치는 요인 (Factors Influencing Health Risk Behaviors in Early Adolescents)

  • Purpose: This study was to identify variables influencing health risk behaviors in adolescents on the basis of the PRECEDE model. Methods: Data were collected from 434 middle school students in Chungbuk Province for two weeks in July 2008. The instrument for this study was a self-reported questionnaire. The SPSS/WIN 12.0 program was used for data analysis, which included t-test, one way ANOVA, Scheffe's test, Pearson's correlation coefficients, and stepwise multiple regression analysis. Results: Adolescents' level of health risk behaviors ($47.32{\pm}7.98$) was below medium. Physical inactivity ($6.73{\pm}2.23$) and Internet addictive behavior ($6.36{\pm}2.11$) were major health risk behaviors. The perceived level of school health education ($30.74{\pm}9.84$) was low. Family function and general self efficacy explained 14.3% of variance in health risk behaviors. Family function was a major predictor of health risk behaviors ($R^2$=.108, p=.000). However, the perceived level of school health education was not a significant predictor of health risk behaviors. Conclusion: For the prevention of health risk behaviors in adolescents, it is necessary that the family and the school collaborate with each other in educational strategies in order to strengthen influential factors such as family function and general self-efficacy.

우리나라 환경문제의 인지 위해도와 정부규제의 필요성에 관한 연구 (Risk Perception and Need to Regulate towards Environmental Problems in Korea)

  • A substantial literatures on environmental risk perception have emerged since the late 1960s. Most these works focus on how people form risk perception, on what people believe, on difficulties in communicating information about health, safety, or environmental risks to non-experts. In this study, questionnaire surveys were conducted to sample from stakeholders(general public, environmental specialist, governmental official, non-government officials, journalist) during March and Aprils, 2000. Total number of responses was 1,803 including 773 persons of general public, 353 experts, 390 governmental officials, 111 journalists and 176 NGO members. Risk perception on 26 environmental issues were statistically analyzed to relate with the need to regulate each issues, interest and knowledge, experience of hazard, satisfaction in environmental situation, etc. This research aims to aid risk analysis and policy-making by providing a basis for understanding and anticipating group responses to environmental issues and improving the communication of risk information among general public, lay-people, technical experts, and decision-makers. This study concludes that those who are in charge of promoting and regulating health and safety of citizens should understand how people perceive about and respond to environmental risk. Without such understanding, well-intended environmental policies of governments would be ineffective.

Pattern of Shisha and Cigarette Smoking in the General Population in Malaysia

  • Background: Smoking is a primary risk factor for cancer development. While most research has focused on smoking cigarettes, the increasing popularity of shisha or water pipe smoking has received less attention. This study measured the prevalence and risk factors for shisha and cigarette smoking and related knowledge. Materials and Methods: This cross-sectional analytical study was conducted in Shah Alam, Malaysia. Participants aged ${\geq}18years$ were selected from restaurants. Data regarding demographic variables, smoking patterns, and knowledge about shisha smoking were collected in local languages. Logistic regression was performed to assess risk factors. Results: Of 239 participants, 61.9 % were male and 99.2% revealed their smoking status. Some 57.4% were smokers: 50.7% only cigarettes, 5.9% only shisha and 42% both. Mean age of starting cigarette smoking was $17.5{\pm}2.4years$ and for shisha smoking $18.7{\pm}2.0years$. In a univariate model, male gender, age 33-52 years and monthly income > MYR 4,000 increased the risk and unemployment and being a student decreased the risk. In a multivariate model, male gender increased the risk of smoking, while being a student decreased the risk, adjusting for age and income. The perception of shisha being less harmful than cigarettes was present in 14.6% and 7.5% had the opinion that shisha is not harmful at all, while 21.7% said that it is less addictive than cigarettes, 39.7% said that shisha did not contain tar and nicotine, 34.3% said that it did not contain carbon monoxide and 24.3% thought that shisha did not cause health problems. Conclusions: Prevalence of shisha and cigarette smoking is high in the general population in Malaysia and knowledge about shisha smoking is relatively low. The findings of our study might have implications for understanding similarities and differences in incidence of shisha and cigarette smoking in other cultural/geographic regions.

병원경영의 위험요인이 운전자본 관리에 미치는 영향 (Effect of Risk Factors on the Management of Working Capital in Hospital Management)

  • 본 연구는 우리나라 종합병원을 대상으로 경영의 위험요인이 운전자본 관리에 어떻게 영향을 미치는지를 분석하였다. 자료는 의료기관회계정보공시 시스템을 이용하여 271개 종합병원 3개연도(2016년, 2017년, 2018년)의 회계정보를 이용하였다. 도입변수는 종속변수로 운전자본 수준과 현금순환주기, 독립변수로 운영위험과 시장위험, 통제변수로 운전자금 구성요인(현금, 매출채권, 재고자산, 매입채무)을 선정하였다. 연구결과, 우리나라 종합병원들은 운영위험이 낮을수록 운전자본 수준은 높았으며, 운전자본 결정에는 운영위험, 현금, 재고자산, 매입채무가 작용하는 것으로 확인되었다. 그리고 시장위험(의료이익률)이 낮을수록 현금순환주기가 높았다는 것을 알 수 있었다. 따라서 의료기관들도 운영의 특수성을 고려하여 경기대응능력을 갖출 수 있도록 운전자본 결정요인으로 확인된 운영위험, 현금, 재고자산, 매입채무의 적절한 관리방안에 대한 검토의 필요성이 제기된다.

Expression of Pituitary Tumor Transforming Gene 1 is an Independent Factor of Poor Prognosis in Localized or Locally Advanced Prostate Cancer Cases Receiving Hormone Therapy

  • We investigated the prognostic value of pituitary tumor transforming gene 1 (PTTG1) expression according to clinicopathological features among localized or locally advanced prostate cancer cases receiving hormone therapy. A retrospective study involved 64 patients receiving combined androgen blockade treatment was performed. PTTG1 expression was determined by immunohistochemical staining using initial needle biopsy specimens for diagnosis. Associations of PTTG1 with various clinicopathological features and disease-free survival were examined via uni- and multivariate analyses. No association between PTTG1 expression and clinical T stage, Gleason score, pretreatment PSA levels, risk groups was found (p =0.682, 0.184, 0.487, 0.571, respectively). Univariate analysis revealed that increased PTTG1 expression, T3 stage and high risk group were associated with increased risk of disease progression (p =0.000, 0.042, and 0.001), and high PSA level had a tendency to predict disease progression (p =0.056). Cox hazard ratio analysis showed that PTTG1 low expression (p =0.002), PTTG1 high expression (p =0.000) and high risk group (p =0.0147) were significantly related to decreased disease-free survival. In conclusion, PTTG1 expression determined by immunohistochemical staining in needle biopsy specimens for diagnosis is a negative prognostic factor for progression in localized or locally advanced prostate cancer receiving hormone therapy.

Do Long Term Cancer Survivors Have Better Health-Promoting Behavior than Non-Cancer Populations?: Case-Control Study in Korea

  • Background: We compared the health-promoting behavior of long-term cancer survivors with those of the general population to identify necessary behavioral interventions to reduce the health risk among cancer patients. Materials and Methods: We used data from the 2007 and 2012 Korea National Health and Nutrition Examination Surveys (KNHANES IV [2007~2009] and KNHANES V [2010~2012]) on smoking status, alcohol use, physical exercise, and disease screening. We compared long-term cancer survivors with members of the general population; the controls were matched by propensity score matching. A multiple logistic regression model was used to investigate the association between cancer status and health-promoting behavior. Results: Long-term cancer survivors had a lower risk of smoking than the general population controls (OR: 0.42, 95%CI: 0.25-0.71). In addition, the long-term cancer survivors had a lower risk of alcohol use than the general population controls (OR: 0.70, 95%CI: 0.50-0.98). However, in terms of physical exercise and disease screening, no statistically significant differences were detected (physical exercise OR: 1.01, 95%CI: 0.75-1.35; disease screening OR: 1.27, 95%CI: 0.93-1.74). All covariates were adjusted. Conclusions: The long-term cancer survivors had a much lower risk of smoking and alcohol use than the general population controls. However, almost no differences in physical exercise and screening for cancer recurrence or secondary disease were detected between the long-term cancer survivors and general population controls. To reduce the health risks and challenges facing long-term cancer survivors, interventions to encourage physical exercise and screening for cancer recurrence and secondary disease should be implemented.

Pancreatic Fistula after D1+/D2 Radical Gastrectomy according to the Updated International Study Group of Pancreatic Surgery Criteria: Risk Factors and Clinical Consequences. Experience of Surgeons with High Caseloads in a Single Surgical Center in Eastern Europe

  • Purpose: Incidence, risk factors, and clinical consequences of pancreatic fistula (POPF) after D1+/D2 radical gastrectomy have not been well investigated in Western patients, particularly those from Eastern Europe. Materials and Methods: A total of 358 D1+/D2 radical gastrectomies were performed by surgeons with high caseloads in a single surgical center from 2002 to 2017. A retrospective analysis of data that were prospectively gathered in an electronic database was performed. POPF was defined and graded according to the International Study Group for Pancreatic Surgery (ISGPS) criteria. Uni- and multivariate analyses were performed to identify potential predictors of POPF. Additionally, the impact of POPF on early complications and long-term outcomes were investigated. Results: POPF was observed in 20 patients (5.6%), according to the updated ISGPS grading system. Cardiovascular comorbidities emerged as the single independent predictor of POPF formation (risk ratio, 3.051; 95% confidence interval, 1.161-8.019; P=0.024). POPF occurrence was associated with statistically significant increased rates of postoperative hemorrhage requiring re-laparotomy (P=0.029), anastomotic leak (P=0.002), 90-day mortality (P=0.036), and prolonged hospital stay (P<0.001). The long-term survival of patients with gastric adenocarcinoma was not affected by POPF (P=0.661). Conclusions: In this large series of Eastern European patients, the clinically relevant rate of POPF after D1+/D2 radical gastrectomy was low. The presence of co-existing cardiovascular disease favored the occurrence of POPF and was associated with an increased risk of postoperative bleeding, anastomotic leak, 90-day mortality, and prolonged hospital stay. POPF was not found to affect the long-term survival of patients with gastric adenocarcinoma.

Clinical implications of the newly defined concept of ventilator-associated events in trauma patients

  • Purpose: Ventilator-associated pneumonia is the most common nosocomial infection in patients with mechanical ventilation. In 2013, the new concept of ventilator-associated events (VAEs) replaced the traditional concept of ventilator-associated pneumonia. We analyzed risk factors for VAE occurrence and in-hospital mortality in trauma patients who received mechanical ventilatory support. Methods: In this retrospective review, the study population comprised patients admitted to the Jeju Regional Trauma Center from January 2020 to January 2021. Data on demographics, injury characteristics, and clinical findings were collected from medical records. The subjects were categorized into VAE and no-VAE groups according to the Centers for Disease Control and Prevention/National Healthcare Safety Network VAE criteria. We identified risk factors for VAE occurrence and in-hospital mortality. Results: Among 491 trauma patients admitted to the trauma center, 73 patients who received ventilator care were analyzed. Patients with a chest Abbreviated Injury Scale (AIS) score ≥3 had a 4.7-fold higher VAE rate (odds ratio [OR], 4.73; 95% confidence interval [CI], 1.46-17.9), and those with a glomerular filtration rate (GFR) <75 mL/min/1.73 m2 had 4.1-fold higher odds of VAE occurrence (OR, 4.15; 95% CI, 1.32-14.1) and a nearly 4.2-fold higher risk for in-hospital mortality (OR, 4.19; 95% CI, 1.30-14.3). The median VAE-free duration of patients with chest AIS ≥3 was significantly shorter than that of patients with chest AIS <3 (P=0.013). Conclusions: Trauma patients with chest AIS ≥3 or GFR <75 mL/min/1.73 m2 on admission should be intensively monitored to detect at-risk patients for VAEs and modify the care plan accordingly. VAEs should be closely monitored to identify infections early and to achieve desirable results. We should also actively consider modalities to shorten mechanical ventilation in patients with chest AIS ≥3 to reduce VAE occurrence.
