A Study on Glomerular Filtration Rate Comparisons between Gates Method and Modified Gates Method used in Dynamic Renal Scintigraphy (동적 신장 신티그래피 검사 시 사용되는 Gates 법과 Modified Gates 법의 사구체 여과율 비교에 관한 고찰)

  • Purpose: Glomerular filtration rate is an important index for assessment of renal function, early discovery of renal disease, and progress observation of chronic renal disease patients. In the present study, the objective is to conduct a comparative analysis of differences between Gates and Modified Gates method in dynamic renal scintigraphy based on MDRD (Modification of Diet Renal Disease) formula using blood collection. Materials and Methods: Renal scintigraphy was performed for 45 patients who visited our hospital between November 2010 and August 2011. For 20 patients of those tested, glomerular filtration rates from Gates method and MDRD formula using AGUS equipment, were compared. For the other 20 patients, glomerular filtration rates from Modified Gates method and MDRD formula using INFINIA equipment. Finally, Gates and Modified Gates method were compared for 5 patients who indicated no change in glomerular filtration rates from MDRD formula during progress observation. Results: Glomerular filtration rates from both Gates and Modified Gates method showed a high correlation with those from MDRD formula ($p$<0.01, r=0.903, r=0.867), with a paired difference mean for Gates method of $2.05{\pm}2.54mL/min/1.73m$, and that for Modified Gates method of $25.2{\pm}3.71mL/min/1.73m$. Finally, the values for Gates method and those for Modified Gates method showed a high correlation for the five patients ($p$<0.05, r=0.949), with a paired difference mean of $20.4{\pm}8.84mL/min/1.73m$. Conclusion: Glomerular filtration rates from Gates method, Modified Gates method and MDRD formula showed mutually high correlations. If the tests are performed with recognition for the correlations between Gates and Modified Gates method used in a dynamic renal scintigraphy, then an accurate assessment of renal function is considered possible with an improved diagnostic ability.

  • The glomerular filtration rate (GFR) has been the subject of much research as a key indicator for diagnosing, treating, and monitoring kidney function. The gamma camera method (Gates method) is simple and allows simultaneous acquisition of GFR and renal scintigraphy for each kidney, however its accuracy is inferior. This study aimed to investigate changes in GFR depending on how region of interest (ROI) are set up, which is one of many factors influencing accuracy. GFR was calculated by setting the ROI for each phase of the image acquisition time (Gates-1: 0~1 minutes, Gates-2: 1~3 minutes, Gates-3: 3~27 minutes), and statistical significance was verified based on probability value 0.05 through ANOVA analysis. While there was no statistically significant difference among results from Gates-1, 2, 3 (p=0.481>0.05), overall results from the Gates method tended to overestimate compared to those from the multiple blood sampling-dual exponential (MBSDE) method. When comparing averages between phases, results from Gates-2 were most similar to those from the MBSDE method. Moreover, paired t-test p-values between MBSDE method and phases were as follows Gates-1: 0.021 (p<0.05), Gates-2: 0.280 (p>0.05), and Gates-3: 0.164 (p>0.05) indicating that only Gates-1 had statistically significant differences compared with MBSDE method. Thus, setting ROI around 2~3 minutes is calculated can aid in accurately determining GFR when Gates Method.

Comparison of Glomerular Filtration Rate Using Gates Method and Serum Creatinine (Gates 방법과 혈청 크레아티닌을 이용한 사구체 여과율의 비교)

  • Purpose: Glomerular filtration rate (GFR) is considered as the best overall index for the level of renal function, diagnosis of doubtful kidney disease, progress observation from chronic kidney disease and is measured with the various methods. In this study, We measured standard GFR by Gates method and attempted to compare the result with serum creatinin-based, Cockcroft-Gault(C-G) formula and Modification of Diet in Renal Disease (MDRD) formula. Materials and methods: 217 patients (127 men, 90 women, mean age $51.3{\pm}16.9$) with various renal function were examined. we compared the GFR using $^{99m}Tc$-DTPA (Gates), C-G formula and MDRD formula. Results: Significant correlations were noted between 2 different GFR estimates (from C-G formula: r=0.864, p<0.0001, MDRD formula: r=0.831, p<0.0001) and $^{99m}Tc$-DTPA (Gates) GFR. Average of serum creatinine (Scr) was measured with $3.0{\pm}3.1\;mg/dL$, In patients with normal renal function (Scr<1.5 mg/dL), $^{99m}Tc$-DTPA (Gates) GFR was statistically significant to C-G formula (p<0.0001) and MDRD formula (p<0.0001). In patients with mild to moderate renal insufficiency (1.5$^{99m}Tc$-DTPA (Gates) GFR was not statistically significant to C-G formula (p=0.181) and MDRD formula (p=0.127). In patients with severe renal insufficiency (Scr>4.0mg/dL), $^{99m}Tc$-DTPA (Gates) GFR was statistically significant to C-G formula and MDRD formula (p<0.0001). Conclusions: Glomerular filtration rate using Gates method was closly correlated to C-G formula and MDRD formula. In patients with normal renal function, $^{99m}Tc$-DTPA (Gates) GFR was significantly lower than C-G formula and MDRD formula. In patients with mild to moderate renal insufficiency, $^{99m}Tc$-DTPA (Gates) GFR was simmilar with C-G formula and MDRD formula. In patients with severe renal insufficiency, $^{99m}Tc$-DTPA (Gates) GFR was significantly higher than C-G formula and MDRD formula. None of the three different methods was clearly superior to the others.

  • Controlling spillway gates of dams is a complex, nonlinear, non-stationary control process and is significantly affected by hydrological conditions which are not predictable beforehand. In this paper, control methods based on dynamic fuzzy control are proposed for the operation of spillway gates of dams during floods. The proposed methods are not only suitable for controlling spillway gates but also able to maintain target water level in order to prepare a draught. In the proposed methods, we use dynamic fuzzy control that the membership functions can be varied by changing environment conditions for keeping up the target water level, instead of conventional static fuzzy control. Simulation results demonstrate that the proposed methods based on dynamic fuzzy control produce an accurate and efficient solution for both of controlling spillway gates and maintaining target water level defined beforehand.

  • Numerous water gates have been constructed and are under construction for the control of water level because, due to the topographical characteristics of mountainous area, many small size rivers are developed in Korean peninsula. Among the water gates, movable water gates are more efficient to control water level than the fixed water gates. By the field application of the movable water gates, many problems associated with the fixed water gates have been mitigated. The problems include the bottom water pollution, the change of habitats for the riparian organisms, etc. This is the reason to increase the field application of movable water gates. In the paper we present the result of analytical investigation pertaining to the structural behavior of multistage overturning movable water gate which is one of the movable water gates. In the analytical investigations, the finite element analysis on the constructed water gate has been conducted for illustration purposes and it was found that the multistage movable water gate is much better than the fixed and/or existing movable water gates in the point of structural and environmental performances.

  • Gates for the purpose of drainage are classified following the types of structure as: Radial Gate, Sluice Gate, Rolling Gate, Drum Gate. In many cases of the reclamation project the sluice type of gates are applied. Different from this general trend, however the radial type of gate was adopted in the Saemangeum project. In this case the discharge coefficients which are used for the sluice type of gate was applied. To estimate the correct amount of discharge which will be evacuated through the gates, therefore the proper discharge coefficients should be estimated before the operation of the gates. The discharge coefficients were estimated through the physical hydraulic modeling, and we got the results as: $0.72{\sim}0.84$ for the submerged condition on the both sides of upstream and downstream, $0.62{\sim}0.83$ for the free surface condition on the downtream side only, and $1.04{\sim}1.12$ for the free surface condition on the both sides of upstream and downstream. The discharge coefficients obtained from the experiments are greater than those of the sluice gates in the design criteria. From the results of the study we may expect that in the Saemangeum project the radial gates could evacuate larger amount of discharge than the originally designed discharge, so that we may sure that the Saemangeum gates have enough capability to control the evacuation of water not only in the usual period but also in the flooding season.

New Dynamic Logic Gate Design Method for Improved TFT Circuit Performance

  • We explored a new way of designing dynamic logic gates with low temperature polysilicon thin film transistors to increase the speed. The proposed architecture of logic gates utilizes the structural advantage of smaller junction capacitance of thin film transistors. This method effectively blocks leakage of current through the thin film transistors. Furthermore, the number of transistors used in logic gates is reduced thereby reducing power consumption and chip area. Through HSPICE .simulation, it is confirmed that the circuit speed is also improved in all logic gates designed.

Probability subtraction method for accurate quantification of seismic multi-unit probabilistic safety assessment

  • Single-unit probabilistic safety assessment (SUPSA) has complex Boolean logic equations for accident sequences. Multi-unit probabilistic safety assessment (MUPSA) model is developed by revising and combining SUPSA models in order to reflect plant state combinations (PSCs). These PSCs represent combinations of core damage and non-core damage states of nuclear power plants (NPPs). Since all these Boolean logic equations have complemented gates (not gates), it is not easy to generate exact Boolean solutions. Delete-term approximation method (DTAM) has been widely applied for generating approximate minimal cut sets (MCSs) from the complex Boolean logic equations with complemented gates. By applying DTAM, approximate conditional core damage probability (CCDP) has been calculated in SUPSA and MUPSA. It was found that CCDP calculated by DTAM was overestimated when complemented gates have non-rare events. Especially, the CCDP overestimation drastically increases if seismic SUPSA or MUPSA has complemented gates with many non-rare events. The objective of this study is to suggest a new quantification method named probability subtraction method (PSM) that replaces DTAM. The PSM calculates accurate CCDP even when SUPSA or MUPSA has complemented gates with many non-rare events. In this paper, the PSM is explained, and the accuracy of the PSM is validated by its applications to a few MUPSAs.

Design Method of Current Mode Logic Gates for High Performance LTPS TFT Digital Circuits (LTPS TFT 논리회로 성능향상을 위한 전류모드 논리게이트의 설계 방법)

  • Development of high performance LTPS TFTs contributed to open up new SOP technology with various digital circuits integrated in display panels. This work introduces the current mode logic(CML) gate design method with which one can replace slow CMOS logic gates. The CML inverter exhibited small logic swing, fast response with high power consumption. But the power consumption became compatible with CMOS gates at higher clock speed. Due to small current values in CML, layout area is smaller than the CMOS counterpart even though CML uses larger number of devices. CML exhibited higher noise immunity thanks to its non-inverting and inverting outputs. Multi-input NAND/AND and NOR/OR gates were implemented by the same circuit architecture with different input confirugation. Same holds for MUX and XNOR/XOR CML gates. We concluded that the CML gates can be designed with few simple circuits and they can improve power consumption, chip area, and speed of operation.

GFRs Measured by Gates' Method According to 5 Background Sites: Comparison with GFR Measured by I-125-Iothalamate Method (다양한 배후 방사능 설정에 따른 Cates 법 사구체 여과율의 차이: I-125-Iothalamate 측정법과의 비교)

  • Purpose: The aim was to assess how the background site affects the Gates' glomerular filtration rate(GFR) measurement using Tc-99m-DTPA in correlation with GFR by I-125-lothalamate method. Material and methods: The study populations were 63 adults with 39 men and 24 women aged from 20 to 59 yrs (mean=37.9 yrs). The fellowing five background regions of interest were used in measurement of GFR using Gates' method: 1) lower side of each kidney(subrenal), 2) around each kidney(circumferential), 3) upper side of each kidney(suprarenal), 4) lateral side of each kidney(lateral), 5) between the two kidneys(inter-renal). We also measured GFR using I-125-iothalamate in each subject. The two studies were separated by 1 to 3 weeks. The subjects were divided into two groups by renal depth. Group 1 with renal $depth{\geq}7cm$ and group 2 with renal depth<7cm. We calculated the means and standard deviations of the GFRs measured by two studies. And we statistically analyzed the correlation and differences among GFRs by Gates' method and the GFR by iothalamate method with correlation analysis. Results: The GFRs by Gates' method using suprarenal and inter-renal background correction showed better correlation with the GFR measured by I-125-iothalamate. And GFRs measured by Gates' method showed statistically significant correlation with the GFR measured by I-125-iothalamate in the group with renal depth<7cm. But GFRs measured by Gates' method did not show statistically significant correlation with the GFR measured by I-125-iothalamate in the group with renal $depth{\geq}7cm$. Conclusion: GFRs measured with Gates' method showed higher correlation with the GFR measured by I-125-iothalamate when the regions of interest were plated over the suprarenal and inter-renal backgrounds. And GFRs measured with Gates method showed statistically significant correlation with the GFR measured by I-125-iothalamate in the group with renal depth<7cm.
