Correlations between Gastroscopy Results and Zone-5 Data from the 7-zone-Diagnostic System (7구역진단기의 5구역 정보와 위내시경 소견의 상관성에 관한 연구)

  • Objectives : The 7-zone-diagnostic system is a device for predetermining bodily locations by measuring the energy of the living body. The purpose of this study is to examine correlations between gastroscopy results and zone-5 data from the 7-zone-diagnostic system. Methods : This study was carried out with data from gastroscopy procedures. It involved 115 patients who had been diagnosed with gastritis, esophagitis, or gastric ulcers. These patients were divided into three groups according to the different patterns of zone-5 factors AA, FL2 and FL1. Group A was made up of patients for whom the red bar graph of zone-5 was higher than the normal range for factor AA. Group B was made up of patients for whom the red bar graph of zone-5 was within the normal range for factor AA. Group C was made up of patients for whom the red bar graph of zone-5 was lower than the normal range for factor AA. Group D E and F were made up of same way as group A B and C for factor FL2. Group G H and I were made up of same way as group A B and C for factor FL1. After the collection of gastroscopy results and data on zone-5 from the 7-zone-diagnostic system, the data was analyzed statistically. Results : 1. Group D had a higher ratio of medication than group E, and this result was statistically significant. 2. Group D G had a higher ratio of medication than group E G and group E H, and this result was statistically significant. 3. Group G had a higher ratio of esophagitis than group H, and this result was statistically significant. 4. A Group B H had a higher ratio of esophagitis than group B G, group C G, and group C H, and this result was statistically significant. 5. The medication group had a high score on the function index than the non-medication group, and this result was statistically significant. Conclusions : This study suggests that there is a slight correlation between gastroscopy results and zone-5 data from the 7-zone-diagnostic system. The research resulted in significant data that are helpful for diagnosing digestive system problems through the use of the 7-zone-diagnostic system.

The Effects of Video-audio Information Provision on Physical Discomfort, Anxiety, and Nursing Satisfaction of the Clients for Gastroscopy (동영상 정보제공이 위내시경 대상자의 신체적 불편감, 불안 및 간호 만족도에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of video-audio information provision on physical discomfort, anxiety and nursing satisfaction of the clients for gastroscopy. Methods: The study design was nonequivalent control group pre-post test design. The subjects were 50 patients who visited H hospital health examination center for gastroscopy. Video-audio information developed by the authors was used as educational material for the treatment group. The data were collected between September 15 and November 15, 2010. The study instruments were the State-Trait Anxiety Inventory, the Physical Discomfort Scale, and the Nursing Satisfaction Scale. Results: The level of anxiety and physical discomfort in the treatment group were not significantly different from that in the comparison group (t=-0.28, p=.781; t=-0.34, p=.741). The level of clients' satisfaction with nursing care in the treatment group was significantly higher than in the comparison group (t=-4.12, p<.001). Conclusion: Use of video-audio information was effective in the increase in satisfaction with care. Therefore, it could be useful in the nursing practice, and be utilized as a way of nursing intervention to improve nursing satisfaction.

Usefullness of CT Gastrography and Vurtual Gastroscopy using Computed Tomography in Detection of Gastric Cancer (위암 진단에 있어서의 CT 위장 조영술과 상부위장관 조영술과의 비교)

  • Purpose: Advancement of computed tomography (CT) hardware and software has allowed thin section scanning and reconstruction of fascinating 2-dimentional (2D) and 3- dimentional (3D) images. Especially, the reconstruction of 3D images of gastrointestinal tract has been used in the detection and diagnosis of pre-malignant and malignant diseases. To compare the efficacy of CT gastrography with conventional upper gastrointenstinal series (UGIs) in gastric cancer patients. Materials and Methods: During Nov. 2002 and Mar. 2003, twenty-seven patients who had gastric cancer received both double contrast upper GI series and CT gastrography prior to radical surgery. Among these patients, nineteen had early gastric cancer (EGC) and 8 had advanced gastric cancer (AGC). Fifteen patients were male and 12 were female. The mean age was 54 yrs (range, $27\∼75$ yrs). The patients were placed on NPO and Stomach was distended with gas in fasting state prior to CT scanning. Double contrast upper GI series were performed as routine manual. CT scan was conducted in all patients using 8 or 16-channel multidetector CT in this study. The collimation and reconstruction for CT scanning were set at 2.5 mm and 1.25 mm, respectively. CT scanning was performed in the supine position. For image processing, CT gastrography, in which raysum and surface rendering images were constructed, virtual and 2D image in coronal and sagittal images were performed. The detectability of gastric cancer was assessed between UGIs and CT gastrography. Results: In AGCs, the detection rate of cancer using CT gastrography and virtual gastroscopy was higher than EGC cases. However, CT gastrography and virtual gastroscopy showed less favorable results than UGIs. Even though only a small number of cases had been studied, we might conclude that CT gastrography and virtual gastroscopy could replace UGIs in the detection of AGC cases. Conclusion: The detection rate used with CT gastrography and Virtual gastroscopy is not better than that of UGIs in early gastric cancer, however, in advanced gastric cancer cases, it is nearly equal to that of UGIs.

Two Cases of Chronic Gastritis Treated with Wihwa Herbal-acupuncture (위화(胃火) 1호(號)로 호전(好轉)된 만성위염(慢性胃炎) 환자(患者) 치험(治驗) 2례(例))

  • Objective: This study was designed to investigate the effect of Wihwa herbal-acupuncture on chronic gastritis. Methods: Wihwa herbal-acupuncture was administered two times a week. The following points were selected: CV12(中脘), CV13(上脘), S36(足三里), Liv3(太衝). We checked pain rating score(P.R.S.), visual analogue scale (V.A.S.) and Gastroscopy. Results: After 6 weeks of treatment, the patient showed that clinical symtoms was disappeared, Lee's P.R.S. changed from 77 to 8 and Park's P.R.S. from 55 to 0 and gastric erosion disappeared on gastroscopy. Conclusions: These results suggest that and Wihwa herbal-acupuncture has good effect on chronic gastritis. But further studies are required to concretely prove the effectiveness of Wihwa herbal-acupuncture for treating chronic gastritis.

The Effect of Music Therapy on Reduction of Anxiety before Gastroscopy (음악요법이 위내시경 대상자의 검사전 불안에 미치는 효과)

  • Purpose: This study examined the effect of music therapy on the pre-exam anxiety of gastroscopy clients. Method: The research period is from June 17 2002 to August 30 2002, and the subjects were 60 individuals, 30 for the experimental and 30 for the control group among adult clients. The data were analyzed by using the SPSS program. Result: The results of this study were as follows: The score of the experimental group was lower than the score of the control group for the situational anxiety (t=-12.56, p=0.000) and systolic blood pressure (t=-3.97, p=0.000) before the examination was supported. The vital sign was partially supported. There was no significant difference(t=-1.05, p=0.297) in the diastolic blood pressure (t=-1.05, p=0.297) and pulse rate(t=-1.30, p=0.199) before the examination of the experimental group and lower than the score of the control group. Conclusion: Music therapy could be useful in the nursing practice, and be utilized as a way of nursing intervention to reduce anxiety of the patients in specific clinical situations. Finally, it is recommended that further studies be conducted on this therapy by utilizing different musical selections.

Mean Sojourn Time of Preclinical Gastric Cancer in Korean Men: A Retrospective Observational Study

  • Objectives: This retrospective cohort study aimed to estimate the mean sojourn time (MST) of preclinical gastric cancer in Korean men. Methods: The subjects consisted of voluntary male screenees aged 40 to 69 years who underwent subsequent screening gastroscopies after testing negative at a baseline screening performed between January 2007 and December 2011. A new case was defined if gastric cancer cells were present in the biopsy specimens obtained from gastroscopy. The follow-up period was calculated as the number of person-years between the date of baseline screening gastroscopy and positive findings at a subsequent screening. The MST was calculated using transition rates of gastric cancer to determine the best screening interval. Results: Of the 171 979 voluntary male screenees, 61 688 (36%) underwent subsequent screening gastroscopies between January 2007 and December 2011. A total of 91 incident cases were found during 19 598 598 person-years of follow-up. The MST of gastric cancer was 2.37 years (95% confidence intervals, 1.92 to 2.96), and those aged 40 to 49 years had a shorter MST than those 50 to 69 years did. Conclusions: These findings support the 2-year interval of screening recommended by the nationwide gastric cancer screening program in Korea. Further studies for the age-specific MST among women are needed.

Improved Detection of Helicobacter pylori Infection and Premalignant Gastric Mucosa Using Conventional White Light Source Gastroscopy

  • Background: The gold standard diagnosis of H. pylori related gastritis is evidence of bacteria on histopathological examination of gastric mucosa. Our aim was to study the correlation between gastric mucosal morphology and histopathological severity of H. pylori related gastritis. Materials and Methods: Division was made on morphological features into:Type 1, showing regular arrangement of red dots; Type 2, showing cleft-like appearance; Type 3, with a mosaic appearance; and Type 4, having a mosaic appearance with focal or diffuse hyperemia. Results: Types 1 and 2 gastric mucosal morphologies were statistically significant in predicting an H. pylori negative status (137/145, p<0.01), while Types 3 and 4 were significant a positive status (139/155, p<0.01). The sensitivity, specificity, positive and negative predictive values of Type 3 and 4 morphologies for predicting H. pylori positive were 94.6%, 89.5%, 89.7% and 94.5%, respectively, with a good correlation with inflammation grading (p<0.01). Conclusions: Our study suggests that gastric mucosal morphology can be reliably identified using conventional white light source gastroscopy with good correlation between findings and inflammation grading.

A Systemic Review of Studies for Traditional Chinese Treatment of Intestinal Metaplasia - Research using CNKI (장상피화생(腸上皮化生) 치료에 대한 최신 임상 연구 동향 - CNKI 검색을 중심으로)

  • Objectives: The purpose of this research is to investigate recent clinical studies of Intestinal Metaplasia (IM) based on a search of the China National Knowledge Infrastructure (CNKI). Methods: We investigated recent clinical studies of Oriental medicine therapies in traditional Chinese medical journals for IM through a CNKI search. 20 clinical articles published from 2010 to 2016 were analyzed. This study examined the authors, publication years, types of studies, criteria for diagnosis and evaluation, periods, purposes of study and articles classified by methods of treatment. Results: Most of articles were classified as randomized controlled trial. IM was diagnosed using gastroscopy, pathological examination and clinical symptoms in a high proportion of articles. The criteria for evaluation most frequently used were the effective ratios of clinical symptoms, gastroscopy and pathological tests. The methods of treatment in all studies were Chinese herb medicine. Acupuncture, acupoint injection therapy, and embedding therapy were also used in several studies. Conclusion: In this study, the Chinese treatment for IM was markedly effective. To improve the treatment of IM in Korea, many clinical studies and case reports are required, based on Chinese medicine.

  • Objective: Case report on the improvement of intestinal metaplasia through ortho-cellular nutrition therapy. Methods: A Korean woman in her 50s who is suffering from bloating, abdominal distension and frequent eructation after meals. Results: Symptoms improved one year after the introduction of nutrition therapy, and patient was diagnosed as fully recovered from reflux esophagitis, atrophic gastritis, and intestinal metaplasia through gastroscopy. Conclusion: In patients with complex gastrointestinal problems, the application of nutrition therapy may help improve and treat symptoms.

Improved Detection of Helicobacter pylori Infection and Premalignant Gastric Mucosa Using "Site Specific Biopsy": a Randomized Control Clinical Trial

  • Background: Helicobacter pylori infection and premalignant gastric mucosa can be reliably identified using conventional narrow band imaging (C-NBI) gastroscopy. The aim of our study was to compare standard biopsy with site specific biopsy for diagnosis of H. pylori infection and premalignant gastric mucosa in daily clinical practice. Materials and Methods: Of a total of 500 patients who underwent gastroscopy for investigation of dyspeptic symptoms, 250 patients underwent site specific biopsy using C-NBI (Group 1) and 250 standard biopsy (Group 2). Sensitivity, specificity, and positive and negative predictive values were assessed. The efficacy of detecting H. pylori associated gastritis and premalignant gastric mucosa according to the updated Sydney classification was also compared. Results: In group 1 the sensitivity, specificity, positive and negative predictive values for predicting H. pylori positivity were 95.4%, 97.3%, 98.8% and 90.0% respectively, compared to 92.9%, 88.6%, 83.2% and 76.1% in group 2. Site specific biopsy was more effective than standard biopsy in terms of both H. pylori infection status and premalignant gastric mucosa detection (P<0.01). Conclusions: Site specific biopsy using C-NBI can improve detection of H. pylori infection and premalignant gastric mucosa in daily clinical practice.
