Purpose: The purpose of this study was to investigate symptoms experienced by patients who reported fever at the emergency room (ER) and to identify any existing cluster of symptom related to fever. Methods: The study used a retrospective and descriptive design with secondary data analysis. Data were abstracted from 665 medical records of patients with fever who visited ER from September 1, 2015 to August 31, 2016 at a tertiary hospital. Results: The most frequently reported symptoms included cold (43.9%), myalgia (24.1%), headache (16.2%), general weakness (15.3%), respiratory symptoms (12.3%), gastrointestinal (GI) symptoms (12.0%), mental change (4.5%), sweating(1.8%), and warmth (0.9%). Analysis of the symptoms related to fever revealed seven symptom clusters; Cluster 1 (n=190) included cold (100%) and myalgia (28.9%); Cluster 2 (n=37), headache (100%) and myalgia (32.4%); Cluster 3 (n=33), GI symptoms (100%), general weakness, headache, and cold; Cluster 4 (n=34), cold (100%), myalgia, headache, and respiratory symptoms; Cluster 5 (n=241), respiratory symptoms (10.8%); Cluster 6 (n=76): myalgia (75.0%) and general weakness, and Cluster 7 (n=54), cold (87.0%), general weakness, and respiratory symptoms. Conclusion: The results of this comprehensive symptom assessment are hoped to be helpful in developing better symptom management for ER patients with fever than before. Further research is warranted to verify the symptom clusters of this study in different clinical settings.

Infectious mononucleosis is Epstein-Barr virus (EBV) inducing a self-limiting clinical syndrome characterized by fever, sore throat, hepatosplenomegaly, and generalized lymphadenopathy. Gastrointestinal symptoms of EBV infection are nonspecific and occur rarely. EBV inducing acute gastrointestinal pathology is poorly recognized without suspicion. Careful consideration is needed to diagnose gastric involvement of EBV infection including gastric lymphoma, gastric cancer, and gastritis. A few recent cases of gastritis associated with EBV infection have been reported in adolescents and adults. However, there is no report of EBV-associated gastritis in early childhood. We experienced a rare case of 4-year-old girl with EBV gastritis confirmed by in situ hybridization.
Vomiting in pediatric patient is frequently encountered problem in emergency room or outpatient clinic. In differential diagnosis, age of the patient or accompanying symptoms should be considered in the differential diagnosis. Accurate diagnosis is very important because surgical treatment is necessary in some of the conditions. Imaging diagnosis of conservative modality such as upper gastrointestinal series or colon study is still important radiological examination in the initial differential diagnosis, but recently ultrasonography offers accurate diagnosis in many situations. The cause of vomiting in pediatric are diverse according to the age group :neonatal sepsis, necrotizing enterocolitis, or hypertrophic pyloric stenosis in neonates : gastroesophageal reflux, viral enteritis, or intussusception in infant: midgut volvulus, appendicitis, metabolic disorders, or increased intracranial pressure also an be the cause. knowledge of radiological findings of normal gastrointestinal tract is important to recognize abnormalities. A discussion of radiological findings in variable surgical conditions to present as vomiting in pediatric patients is offered.
Even in the absence of any specific abnormal pathologic findings of the gastrointestinal tract, many patients still suffer from : fullness, anorexia and postprandial abdominal pain. As these symptoms are similar to visceral origin pain, many physicians focus on the discovery of pathologic abnormality of the gastrointestinal tract. At our Yoido Pain Clinic, after diagnosing myofascial pain syndrome, we treated 64 patients by trigger point injection and physical therapy on abnormal abdominal muscle, from June 1993 to April 1995. Most patients' conditions improved after these treatments.
Chronic recurrent abdominal pain is a common manifestation in children. Functional abdominal pain is the most common cause of chronic abdominal pain and can be diagnosed properly by the physician without the requirement of specific evaluation when there are no alarm symptoms or signs. Functional abdominal pain is categorized as functional dyspepsia, irritable bowel syndrome, functional abdominal pain, abdominal migraine, and aerophagia, according to the Rome II criteria for pediatric functional gastrointestinal disorders. New concepts on the pathogenesis of functional abdominal pain include brain-gut interaction, visceral hypersensitivity, gastrointestinal dysmotility, inflammation, autonomic dysfunction, genetic predisposition, and triggering factors including psycho-social stress.
Objective: This case presents the effects of Gyejibanhasaenggang-tang, acupuncture, and moxibustion on functional dyspepsia.Method: A patient was treated with herbal medicine, acupuncture, and moxibustion. Functional dyspepsia-related quality of life (FD-QoL), the gastrointestinal symptom rating scale (GSRS), and the gastrointestinal scale (GIS) were assessed for evaluation.Results: Symptoms of functional dyspepsia improved after the 22 days of Gyejibanhasaenggang-tang treatment.Conclusion: This case suggests that the combination of Gyejibanhasaenggang-tang, acupuncture, and moxibustion could be effective in treating functional dyspepsia.
Culture forms an integral aspect of environmental factors which influences disease presentation and clinical outcomes in functional gastrointestinal disorders (FGIDs). In this review, the role of culture in FGIDs in the East is briefly explored with regards to symptom presentation and diagnostic issues, lifestyle and cultural habits, epidemiology, and healthcare seeking behavior. In both functional dyspepsia and irritable bowel syndrome, symptom presentation and disease sub-typing in Asians are known to differ from their Western counterparts, possibly relating to cultural dietary practices and from cultural perception of symptoms. Dietary patterns, together with defecating practices are explored as factors contributing to a lower prevalence of constipation in the East. An urban-rural difference in the prevalence of FGIDs in Asia is attributed to a change in dietary patterns in rapidly developing urban communities, together with an increased level of psychological morbidity. Lastly, cultural attitudes towards traditional/local remedies, variation in healthcare systems, anxiety regarding organic disease, and religious practices have been shown to influence healthcare seeking behavior among FGID patients in the East.
The incidence of gastric mucosa-associated lymphoid tissue (MALT) lymphoma is increasing worldwide, but the diagnosis is difficult. Most patients are asymptomatic or complain of nonspecific gastrointestinal symptoms. As the endoscopic features of gastric MALT lymphoma are variable and nonspecific, the possibility of this condition may be overlooked during esophagogastroduodenoscopy, and it remain undiagnosed. Therefore, this condition needs to be considered when an abnormal mucosa is observed during this procedure. Biopsy performed during endoscopy is the primary diagnostic test, but false negative results are possible; large numbers of samples should be collected from both normal and abnormal mucosae. Endoscopic ultrasonography is useful to assess the depth of invasion and to predict the treatment response. After treatment, follow-up tests are required every 3 months until complete remission is achieved, and annually thereafter. Early diagnosis of gastric MALT lymphoma is difficult, and its diagnosis and follow-up require wide experience and competent endoscopic technique.
Objective: A case report on improving the chronic indigestion and fatigue of patients with cholelithiasis by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A Korean female in her 40s who had a very low quality of life due to chronic indigestion and severe fatigue caused by her diagnosed cholelithiasis. Results: The practice of Ortho-Cellular Nutrition Therapy (OCNT) improved fatigue and problems in gastrointestinal functions. Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) can be effective in relieving the symptoms of patients with problems in gastrointestinal functions.
목 적: HS 자반증 환아에서 선행 질환, 위장관 증상 유무, 그리고 검사 소견의 차이에 따른 임상 경과의 차이가 있는지를 알아보고자 본 연구를 시행하였다. 방 법: 1996년 1월부터 2006년 4월까지 한양대학교병원 소아과에 입원하였던 HS 자반증 환아 중 피부 증상 외 다른 증상 없이 위장관 증상을 호소하는 환아와 위장관 증상을 호소하지 않은 환아 104명을 대상으로 급성 감염 병력, 입원 기간, 치료, 재발 여부를 조사하였고, 말초 혈액 검사, 대변 잠혈 반응 검사, 복부 초음파 검사, 위장관 내시경 검사를 시행하여 그 결과를 비교 관찰하였다. 결 과: HS 자반증 환아 104명 중 위장관 증상이 있는 환아는 66명(63.5%)이었고, 위장관 증상이 없는 환아는 38명(36.5%)이었다. 위장관 증상 유무에 관계없이 6~10세 연령군이 가장 많았고, 위장관 증상이 있는 군에서는 남아가 더 많았으나 위장관 증상이 없는 군에서는 성별 차이가 없었다. 위장관 증상으로는 복통 57명(54.8%), 구토 21명(20.2%), 혈변 5명(4.8%), 설사 3명(2.9%), 오심 3명(2.9%)으로 복통이 가장 많았다. 또한 위장관 증상이 하나인 경우가 46명(44.2%)이었고, 2개 이상을 동반하는 경우가 20명(19.2%)이었다. 위장관 증상이 많은 환아와 위장관 내시경 검사를 시행한 군에서 입원 기간이 길었으며, 스테로이드 치료 시행 예와 질병의 재발이 더 많았고, 대변 잠혈 반응 검사에서 통계학적으로 의의가 있었다(p<0.05). 재발 환자와 재발하지 않은 환자와의 비교에서는 통계학적 의의가 관찰되지 않았다. 87명에서 복부 초음파 검사를 시행하였고, 6명(6.9%)에서 소장벽이 두꺼워진 소견이 관찰되었다. 위장관 내시경 검사상 병변의 양상에 따른 빈도는 급성 출혈성 위염과 급성 출혈성 십이지장염 6명(40.0%), 십이지장 궤양 3명(20.0%), 급성 출혈성 위염과 십이지장 궤양 3명(20.0%), 십이지장염과 출혈성 대장염 2명(13.3%), 출혈성 대장염 1명(6.7%) 순으로 관찰되었고, 급성 출혈성 위염과 급성 출혈성 십이지장염이 동반된 경우가 가장 많았다. 치료는 30명(28.8%)에서 시행되었고, 재발은 6명(5.7%)에서 관찰되었으나 모두 1회였고, 추적 관찰이 되지 않은 3명을 제외하고는 치료한 환아와 치료하지 않은 환아 모두 증상이 호전됨을 알 수 있었다. 결 론: HS 자반증 환아에서 적극적인 위장관 점막병변에 대한 검토가 잘 이루어질 경우 환아의 진단과 치료에 도움이 될 것이라 생각된다.
