합성의약품으로 호전되지 않은 소화기 증상 환자(KCD K-코드)에 대한 반하사심탕 제제 병용의 효과 : 후향적 차트 분석 (Effectiveness of Additional Administration of a Banha-sasim-tang Formulation to Patients with Gastrointestinal Symptoms (KCD K-code) Not Improved by Western Medicines: A Retrospective Chart Review)

  • Objectives: The aim of this study was to evaluate the effectiveness of additional administration of Banha-sasim-tang formulation to patients with gastrointestinal symptoms that did not improve with Western medicines. Methods: The patients who met the inclusion and exclusion criteria were analyzed retrospectively. A paired t-test was performed on the Nepean Dyspepsia Index-Korean version (NDI-K) scores before and after administering the Banha-sasim-tang formulation. Results: A total of 27 subjects were included. The mean duration of taking the Banha-sasim-tang formulation was 15.6±3.1 days. The most commonly used Western medicine before taking the Banha-sasim-tang formulation was rebamipide. The means of the NDI-K score were 57.4±21.8 before administering Banha-sasim-tang formulation and 34.9±16.5 after administration, and the difference was statistically significant (p<0.0001). No serious side effects were encountered while taking the Banha-sasim-tang formulation. Conclusions: For patients whose gastrointestinal symptoms do not improve with Western medicines, additional administration of Banha-sasim-tang formulation may be an option. However, as this study is a retrospective chart review, prospective, large-scale, double-blind studies are needed in the future.

Incidentally Discovered Solitary Gastrointestinal Polyp with Pathological Significance in Children: Four Case Reports

  • Most solitary gastrointestinal (GI) polyps in children are either inflammatory or hamartomatous. Solitary hyperplastic polyp, sentinel polyp and solitary adenomatous polyp have been occasionally diagnosed in adults, but very rarely reported in Korean children. We recently came across a case with adenomatous polyp in the colon, a case with hyperplastic polyp beneath the gastroesophageal junction, a case with hyperplastic polyp in the prepyloric area, and a case with sentinel polyp in the distal esophagus, which are unusual pathologic types in children. These mucosal lesions were diagnosed incidentally during elective endoscopic examinations for GI symptoms. Most polyps do not cause significant symptoms, so the diagnosis might be delayed, especially in children, in whom GI endoscopy is not commonly performed for screening purpose as in the adults.

건칠약침을 포함한 한방치료 후 호전된 소화불량 환자 5례에 대한 증례보고 (Case Report of Five Patients with Dyspepsia Improved after Korean Medicine Treatment, Including GeonChil Pharmacopuncture)

  • Objectives: This study reports the effects of GeonChil (Rhus verniciflua stokes) pharmacopuncture in patients with dyspepsia. Method: We treated all five patients with GeonChil pharmacopuncture, acupuncture, and moxibustion and evaluated them using the Korean Gastrointestinal Symptom Rating Scale (KGSRS), which is used to assess overall gastrointestinal symptoms. Result: After treatment, the KGSRS showed improvement in all five patients. Marked improvement was seen especially in abdominal pain, heartburn, eructation, and increased flatus symptoms. Conclusion: GeonChil pharmacopuncture may represent a new treatment for the improvement of dyspepsia.

당뇨병 환자를 위한 고식이섬유 보충물의 개발을 위한 연구(I) -해조류 투여가 당뇨쥐의 장기능과 증세호전도에 미치는 영향- (A Study on the development of high-fiber supplements for the diabetic patients (I) -Effect of Seaweed Supplementation on the Gastrointestinal Function and Diabetic Symptom Control in Streptozotocin-induced Diabetic Rats-)

  • The present study was conducted to evaluate the usefulness of four kinds of seaweeds (mixture of purple laver & sea lettuce, sea tangle, sea mustard, agar agar) as a high- fiber supplment in the therapeutic deit for the diabetic patients. Seven groups of normal and streptozotocin-induced diabetic rats were fed dietary fiber-free control diet or one of experimental diets containing 7% of one of the four seaweeds for 6 weeks. The effect of seaweeds supplementation on the body weight change, gastrointestinal function, and the control of diabetic symptoms were examined and compared with the effect of fiber-free diet or pectin diet used as references. The body weight gains of all the diabetic groups were significantly suppressed compared to the normal group. Feed efficiency ratios and body weight gains of seaweed groups were relatively higher than those of the pectin group. Sea tangle appeared to have an effect of alleviating the typical diabetic symptoms such as polyphasia, polydipsia, polyuria, urinary glucose excretion and hyperglycemia indicating its beneficial acition of improving glucose metabolism even though the degree of effectiveness was less than that with pectin. All the supplemntations of seaweeds and pectin ressulted in the significant changes in gastronitestinal functins ; shortening of GI transit time, increase of fecal volume and the length of intestine. Based on their effects of the significant changes in GI function in may be suggested that seaweeds may influence the process of digestion and absorption of nutrients in diabetic animals.

위장관 증상을 동반한 Henoch-Sch$\"{o}$nlein Purpura 환아의 장관 초음파 소견 및 내시경 소견 (Intestinal Ultrasonographic and Endoscopic Findings in Pediatric Patients with Henoch-Sch$\"{o}$nlein Purpura and Gastrointestinal Symptoms)

  • 목적: 위장관 증상을 갖는 HSP 환아에서 장관초음파소견과 위내시경검사소견을 규명하여 피부자반증없이 위장관 증상만 있는 환아에서 HSP 조기진단에 장관초음파검사와 위내시경검사의 유용성을 알아보고자 본 조사를 시행하였다. 방법: 1999년 1월부터 2001년 4월까지 동강병원 소아과에 입원한 위장관 증상을 동반한 HSP 환아85명 88례를 대상으로 임상증상, 장관초음파소견과 위내시경소견을 후향적으로 조사하였고 동일한 환아에게서 장관초음파검사와 위내시경검사를 동시에 시행한 4례의 환아에서 두 검사의 연관성을 찾아보았다. 결과: 1) 피부자반증이 88례(100%) 모두에서 나타났고 관절통 및 관절증상이 64례(73%), 위장관 증상이 52례(59%), 신장침범소견이 15례(17%), 음낭침범소견이 3례(3%)에서 나타났다. 2) 위장관 증상은 복통이 52례(100%)에서 나타났고 대변잠혈양성반응이 28례(54%), 구역 및 구토가 17례(33%), 복부압통이 17례(33%), 혈변 혹은 흑색변이 12례(23%), 설사 6례(12%), 토혈 4례(8%), 복부반사통과 복부경직이 각각 1례(2%)순이었다. 3) 위장관 증상을 갖는 52례 중 27례에서 복부초음파검사를 시행하였는데 22례(81%)에서 이상소견이 나타났다. 십이지장과 공장벽의 비후소견이 16례(73%)로서 가장 많았고 복수 8례(36%), 장간막임파선종창 8례(36%), 장폐색증 4례(18%), 이상담낭소견 1례(5%)순이었다. 위장관 증상을 갖지 않는 36례의 HSP 환아 중 3례에서 실시한 복부초음파검사는 모두 정상 소견으로 나타났다. 4) 5례의 내시경 소견은 전례에서 위소견은 정상이었고 십이지장에 다발성의 미란성 출혈성 십이지장염의 소견을 보였는데 구부보다 하행십이지장에 더 심했으며 1례에서는 구부소견은 정상이었고 하행십이지장에서만 동일한 소견을 보였다. 이중 2례에서 시행한 내시경적 생검절편의 병리조직학적 소견은 급성 염증반응과 국소미란 및 출혈소견을 보였다. 5) 4례에서 내시경검사와 복부초음파검사를 같이 시행하였는데 정상소견을 보인 1례를 제외하고는 십이지장과 공장벽의 비후소견의 초음파소견을 보여 미란성 궤양성 출혈성 십이지장염의 내시경소견이 초음파검사상 십이지장벽의 비후소견으로 나타나는 것으로 생각되었다. 결론: 위장관 증상을 동반하는 HSP 환아에서 복부초음파검사는 HSP의 위장관변화를 발견하고 경과관찰을 보는 간편하고 비침습적인 유용한 검사방법이며, HSP 환아에서 내시경검사의 적절한 적응증은 아직 확립되어 있지 않으나 하행십이지장의 출혈성 미란성 십이지장염이 일관된 소견으로 생각된다. 피부자반증이 없이 급성 상복부통증을 동반하는 위장관 증상, 대변잠혈 양성반응이 있을 경우, 복부초음파검사상 십이지장과 공장벽의 비후소견이 보이거나 혹은 위내시경상 출혈성 미란성 십이지장염의 소견이 보일 경우 pathognomonic하지는 않지만 HSP의 가능성을 항상 염두에 두고 다른 질환이 배제되면 부신피질호르몬 투여를 고려해보는 것이 좋을 것으로 생각된다.

외래환자의 위장관계 다빈도 질환과 처방 분석 (Analysis of Frequently Diagnosed Gastrointestinal Disorders and Therapeutic Regimens in the Outpatients)

  • The gastrointestinal disorders (GI disorders) is one of the most common diseases in Korea. The community pharmacists are often faced with the complaints of symptoms due to the GI disorders. However the drugs used to treat the GI disorders are frequently abused by the patients themselves because these drugs are easily available and have high placebo effects. Therefore, we have reviewed the digestive diseases statistics of 1996 to find out the frequencies of the GI disorders in the outpatients of Samsung Medical Center. Using these statistic data, we figured out the frequently diagnosed GI disorders and analysed commonly used prescriptions from February 1st to 28th of 1997. In addition, we also evaluated the commonly used drugs in these prescriptions. About twenty thousands of patients visited the hopital because of their GI symptoms in 1996. It was found that dyspepsia, viral hepatitis, and gastric and duodenal ulcer disease are frequently diagnosed in these patients. In a point of view on other GI disorders, gastritis and duodenitis, irritable bowel syndrome, gastroesophageal reflux disease, constipation and diarrhea were commonly detected. And a number of drugs were prescribed to treat the GI disorders, which included the prokinetics, Histamine-2 receptor antagonists, proton pump inhibitor, antacids, tranquillizers, antidepressants, antispasmodics, laxatives and so on. Interestingly, there were many prescriptions composing of the antibiotic regimens to eradicate H. pylori which has been proven to cause peptic ulcers.

A Case of Visceral Autonomic Neuropathy Complicated by Guillain-Barre Syndrome Accompanied with Cyclic Vomiting Syndrome-like Disorder in a Child

  • We present a case of an 8-year-old boy with visceral autonomic neuropathy complicated by Guillain-Barre syndrome. In this pediatric patient, gastroparesis was the major symptom among the autonomic symptoms. Due to the gastroparesis, there was no progress with the oral diet, and nutrition was therefore supplied through a nasojejunal tube and gastrojejunal tube via Percutaneous endoscopic gastrostomy (PEG). After tube feeding for 9 months, the patient's gastrointestinal symptoms improved and his oral ingestion increased. The pediatric patient was maintained well without gastrointestinal symptoms for 3 months after removal of the PEG, had repeated vomiting episodes which lead to the suspicion of cyclic vomiting syndrome. Then he started treatment with low-dose amitriptyline, which resulted in improvement. Currently, the patient has been maintained well for 6 months without recurrence, and his present growth status is normal.

Characteristics of Upper Gastrointestinal Tract Involvement in Korean Pediatric Crohn's Disease: A Multicenter Study

  • Purpose: Crohn's disease (CD) can involve any site of the gastrointestinal tract (GIT). However, the characteristics of upper GIT involvement in CD are unclear, especially in the Eastern pediatric population. This study aimed to estimate the prevalence of upper GIT involvement and identify the clinical features of Korean children with CD. Methods: This was a retrospective multicenter cohort study that included 52 pediatric patients with CD who underwent esophagogastroduodenoscopy and biopsy. The clinical symptoms and endoscopic and histologic features of the upper GIT were identified according to the presence or absence of upper gastrointestinal symptoms. Results: Among the 52 patients, upper GIT involvement was noted in 50.0% (26/52). The mean age at CD diagnosis was $14.1{\pm}2.1$ years. Gastric ulcer was the most common lesion (19.2%) found on upper GIT endoscopy, followed by duodenal ulcers (15.4%). Chronic inflammation was the most common histopathologic feature (75.0%), followed by gastric erosion (17.3%). Granuloma was found in 9.6% of patients. Helicobacter pylori infection was identified in 5.8% of patients. Endoscopic and histologic findings were not significantly different, but the mean values of erythrocyte sedimentation rate ($60.7{\pm}27.1$ vs. $43.0{\pm}27.6mm/h$, p=0.037) and C-reactive protein ($16.5{\pm}28.2$ vs. $6.62{\pm}13.4mg/dL$, p=0.014) were significantly different between patients with and without upper gastrointestinal CD symptoms. Conclusion: Upper GIT involvement was relatively common in pediatric patients with CD irrespective of upper gastrointestinal symptoms, and H. pylori infection was relatively uncommon. The results of this study should aid the establishment of regional guidelines for upper GIT examination.

An investigation of the relationship between cutaneous allodynia and kinesiophobia, gastrointestinal system symptom severity, physical activity and disability in individuals with migraine

  • Background: To investigate the relationship between cutaneous allodynia (CA) and kinesiophobia, gastrointestinal system (GIS) symptom severity, physical activity, and disability, and to determine whether CA, pain, and disability were influencing factors for kinesiophobia, GIS symptoms, and physical activity in individuals with migraine. Methods: The study included 144 individuals with migraine. CA, kinesiophobia, GIS symptoms, physical activity level, and migraine-related disability were evaluated with the Allodynia Symptom Checklist, the Tampa Kinesiophobia Scale (TKS), the Gastrointestinal Symptom Rating Scale (GSRS), the International Physical Activity Questionnaire-7, and the Migraine Disability Assessment Scale (MIDAS), respectively. Results: The CA severity was only associated with TKS (r = 0.515; P < 0.001), GSRS-total (r = 0.336; P < 0.001), GSRS-abdominal pain (r = 0.323; P < 0.001), GSRS-indigestion (r = 0.257; P = 0.002), GSRS-constipation (r = 0.371; P < 0.001), and MIDAS scores (r = 0.178; P = 0.033). Attack frequency (P = 0.015), attack duration (P = 0.035) and presence of CA (P < 0.001) were risk factors for kinesiophobia. Attack frequency (P = 0.027) and presence of CA (P = 0.004) were risk factors for GIS symptoms. Conclusions: There was a relationship between the CA and kinesiophobia, GIS symptoms, and disability. CA and attack frequency were found to be risk factors for kinesiophobia and GIS symptoms. Migraine patients with CA should be assessed in terms of kinesiophobia, GIS, and disability. Lifestyle changes such as exercise and dietary changes and/or pharmacological treatment options for CA may increase success in migraine management.
