A Case Report Regarding a Treatment Includes Lots of Different Version of Samchulkunbi-tang (蔘朮健脾湯) to Two Pediatric Patients Diagnosed as Sik-gan (食癎) (삼출건비탕가미방(蔘出健脾湯加味方)을 이용한 식간(食癎) 환아 치험 2례에 대한 증례보고)

  • Objectives The purpose of this study is to report the cases of two pediatric patients diagnosed as Sik-gan (食癎), a kind of epileptic seizure thought to be caused by uncontrolled consumption of food, treated by formula variation of Samchulkunbi-tang (蔘朮健脾湯). Methods Two pediatric patients diagnosed as Sik-gan (食癎) according to traditional Korean medical terms were administered by variety of Samchulkunbi-tangs (蔘朮健脾湯) while correcting unhealthy eating habit. To measure the degree of the patients' process, the number and exact symptoms of seizure events, and gastrointestinal symptoms were recorded. Results The treatment of various Samchulkunbi-tang (蔘朮健脾湯) was not only extended remission period of seizure, but also improved gastrointestinal symptoms on both of the patients. Conclusions Pediatric patients who have past medical conditions for epileptic seizure as well as unhealthy eating habits or gastrointestinal malfunctions are prone to have a specific form of seizure called the Sik-gan (食癎). In this report, we have proven that variety of Samchulkunbi-tang (蔘朮健脾湯) can considerably be effective in improving the patients' gastrointestinal symptoms and preventing recurring seizure events.

Clinical Evaluation of Granules Containing Antacids, Digestive Eneymes and Herb Drugs in Patients with Various Symptoms of Upper Gastrointestinal Disorders (상부 소화관 질환증상에 대한 제산, 소화효소 및 생약 세립의 유용성)

  • The purpose of this study is to assess the clinical efficacy and safety of NRM granules which were newly developed as a combination product containing antacids, digestive enzymes and herb drugs for the treatmet of various symptoms of upper gastrointestinal disorders taking SGN granules of similiar but different composition as a control. One hundred and twenty one patients were randomly allocated to receive one pack of either NRM granule(80 cases) or SGN granules(41 cases) three times a day after meal and undergone symptomatic, endoscopic and clinical laboratory assessment before and after two weeks of treatment. The results were as follows; 1. All symptoms except anorexia showed improvements in more than 90% of total cases with each symptom in the NRM group. 2. The general improvement rate was 97.5% in NRM group, and was superior to the(92.5%) of SGN group. 3. Two cases of NRM group and one case of SGN group developed mild nausea, and onecase of SGN group developed mild epigastric soreness. No cases of the two groups developed laboratory(blood, biochemical, urine) abnormalities. 4. The overall clinical efficacy rate was 95.0% in NRM group, 87.8% in SGN group. kccording to the result of this trial, it was concluded that NRM granule was an effective and safe drug and(but not statistically significant) was slightly more efficient than SGN for treatment of various symptoms in upper gastrointestinal disorders.

Effect of Cancer Symptoms and Fatigue on Chemotherapy-related Cognitive Impairment and Depression in People with Gastrointestinal Cancer (암증상과 피로가 소화기 암 환자의 항암화학요법 관련 인지기능 감퇴와 우울에 미치는 영향)

  • Purpose: The purpose of this study was to test a hypothetical model of chemotherapy-related cognitive impairment (CRCI) and depression in people with gastrointestinal cancer. Methods: A purposive sample of 198 patients undergoing chemotherapy was recruited from November 2014 to July 2015. The instruments were Everyday Cognition (ECog), Hospital Anxiety Depression Scale (HADS), Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F), and M. D. Anderson Symptom Inventory-Gastrointestinal Cancer Module. Data were analyzed using descriptive statistics, correlation, and path analysis. Results: CRCI was directly affected by cancer symptoms (${\beta}=.19$, p=.004) and fatigue (${\beta}=.56$, p<.001)($R^2=47.2%$). Depression was directly affected by fatigue (${\beta}=.48$, p<.001) and CRCI (${\beta}=.27$, p<.001). However, The impact of cancer symptoms on depression was confirmed through the mediating effect of CRCI. Conclusion: Results indicate that in patients with gastrointestinal cancer undergoing chemotherapy along with the direct physiologic effects (fatigue, symptoms) of cancer treatment may have altered cognitive function leading to depression.

A Clinical Study on the Correlation between Gastrointestinal Symptoms, Depression, and Anxiety in Patients with Irritable Bowel Syndrome (과민성 장증후군 환자의 위장관 증상과 우울, 불안과의 상관관계 연구)

  • Objective: This study was designed to assess the correlation between gastrointestinal symptoms, depression, and anxiety in patients with irritable bowel syndrome (IBS). Method: New patients who visited the ${\bigcirc}{\bigcirc}$ Oriental Hospital completed standardized questionnaires. These consisted of questionnaires for IBS based on Rome III criteria, the Gastrointestinal Symptom Rating Scale (GSRS), the Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI). Results: A total of 103 patients participated. Among them, IBS was observed in 42 patients. There was a weak correlation between GSRS and BDI, but it was not statistically significant. There was a statistically significant weak correlation between GSRS and BAI. The main symptom factors affecting BAI were borborygmus, increased flatus, sucking sensations in the epigastrium, and abdominal distension. Conclusion: There was a statistically significant correlation between gastrointestinal symptoms and anxiety in patients with irritable bowel syndrome.

The Analysis on Clinical Characteristics of Out-Patients with Gastrointestinal Symptoms in a Korean Medicine Hospital (일개 한방병원에 위장관 증상을 주소로 내원한 외래환자의 사상의학적 특성에 관한 고찰)

  • Objectives The aim of this study was to make an analysis of clinical characteristics and status of Sasang constitutional medicine of patients with gastrointestinal symptoms who visited a Korean medicine hospital. Methods We studied 287 patients with gastrointestinal symptoms who visited a Korean medicine hospital from March 1, 2010 to February 30, 2012. We retrospectively collected medical history and characteristics of subjects using data of EMR (electronic medical record). Results The proportion of female was higher than male. Most patients was suffered from chronic gastrointestinal disorder. And they were not under western medicine treatment. Among the 287 patients, 139 persons (48.43%) were Soyangin, 88 persons (30.66%) Taeeumin and 60 persons Soeumin (20.91%). The proportion of Soyangin was higher than other Sasang types. In the distribution of prescription according Sasang Constitutional Medicine symptomatology, the proportion of exterior disease's prescription was higher than interior disease in Soyangin (93.53%). And the proportion of interior disease's prescription was higher than exterior disease in Taeeumin (75.86%) and Soeumin (95.00%). The majority of disease were Gyolhyung-symptom (51.54%) and Mangeum-syndrome (46.15%) of exterior disease in Soyangin, Dry-febrile symptom (73.56%) of interior disease in Taeeumin, and Taeeum-syndrome (95.00%) of interior disease in Soeumin. Conclusions By analyzing on clinical characteristics and distribution of Sasang Constitutional Medicine symptomatology could contribute to the development of guideline for treatment of gastrointestinal disease of Sasang Constitutional Medicine.

Which Alarm Symptoms Are Associated With Abnormal Gastrointestinal Endoscopy Among Thai Children?

  • Purpose: Alarm symptoms (red flag signs) are crucial indications for management decisions on pediatric gastrointestinal endoscopy. We aimed to identify items in the alarm symptoms and pre-endoscopic investigations that predict abnormal endoscopy results. Methods: A retrospective descriptive study was conducted among children aged under 18 years undergoing endoscopy. The patients were classified into normal and abnormal endoscopic groups. The incidence of alarm symptoms and pre-endoscopic investigations were compared between the groups. Univariate and multivariate logistic regression analyses were performed to determine independent risk factors for abnormal endoscopy. Results: Of 148 participants, 66 were classified in the abnormal endoscopy group. Compared with the normal group, the abnormal group had a significantly higher prevalence of alarm symptoms. Moreover, hematemesis/hematochezia, anemia, low hemoglobin level, hypoalbuminemia, rising erythrocyte sedimentation rate, increased serum lipase, and blood urea nitrogen/creatinine ratio were significantly higher in the abnormal endoscopy group than in the normal group. Multivariate logistic regression analysis indicated that hematemesis/hematochezia and low hemoglobin level were independent risk factors for abnormal endoscopy. Conclusion: The alarm symptoms and pre-endoscopic investigations were evaluated using predictive factors for abnormal pediatric endoscopic findings. According to multivariate logistic regression analysis, hematemesis/hematochezia and low hemoglobin levels were independent risk factors for abnormal endoscopy.

Objectives However there was a report that dyspepsia occurs more frequently in Soeumin, there has not been any clinical research if structural changes such as gastritis occurs differently according to Sasang Constitution. So in this research, we investigated to find out if there are differences in the degree and types of gastritis and gastrointestinal symptoms according to Sasang Constitution and gender difference. 2. Methods Subjects were 30 men and 38 women, who had visited Kyung-Hee Medical Center for physical examination and took gastroscopy and differential diagnosis of Sasang Constitution in March, 2003. We investigated the degree and types of gastritis and gastrointestinal symptoms. 3. Results and conclusions Gender difference han no relation with types of gastritis, but had significant relation with the degree of gastritis and gastrointestinal symptoms. But Sasang Constitution had no significant relation with the degree and types of gastritis and gastrointestinal symptoms.

Two cases of Atopic Dermatitis with Gastrointestinal Disorders treated with Soshiho-tang (소시호탕(小柴胡湯)으로 호전을 보인 소화기계 문제를 동반한 아토피피부염의 치험 2례)

  • Objectives : The purpose of this study is to report the effectiveness of Soshiho-tang on the patients who suffered from atopic dermatitis with gastrointestinal disorders.Methods : We diagnosed the symptoms and the signs of the patients who suffered from atopic dermatitis with gastrointestinal disorders as Soshiho-tang syndrome(嘔而發熱) according to 'Treatise on Cold Damage Disease(傷寒論)'. Patients are treated only with Soshiho-tang. The severity of Atopic dermatitis was evaluated by SCORAD index, visual analogue scale(VAS) and pictures. Gastrointestinal disorders(mainly food allergy, indigestion) were evaluated by patients' subjective complaint.Results : After the treatment, not only the severity of atopic dermatitis but gastrointestinal disorders were also improved.Conclusions : Soshiho-tang has improved the symptoms of Atopic dermatitis with gastrointestinal disorders in this study.

  • Gastrointestinal (GI) symptoms in children and adolescents are influenced by diverse psychiatric components such as psychosocial stresses, familial environment, school-related situations, and comorbid psychiatric conditions. Absolutely psychiatric symptoms of pediatric patients are also affected by problems of GI system. Lots of symptoms including anorexia, dyspepsia, nausea and so on are commonly originated from either GI or psychiatric causes or both. Sometimes the negative interactions between GI and psychiatric problems aggravate the severity and eventually decline the functions of children and adolescents with GI symptoms. We summarized the common GI and psychiatric conditions which have GI and psychiatric associations. To a clinician who manages pediatric GI disorders, psychiatric considerations can be beneficial to understand the clinical manifestations of patients and to find the way to relieve them. This short and somewhat superficial review may help to have a bird's-eye view on this topic.

A Case Report of Abdominal Pain with Acute Kidney Injury and Elevated Pancreatic Enzymes (췌장 효소 상승을 동반한 급성 신손상 환자의 복통 치험 1례)

  • Objectives: The aim of this study is to report the effectiveness and safety of herbal medicine treatment for abdominal pain with acute kidney injury. Methods: A 80-year-old female patient presented with abdominal pain. Blood test results showed increased blood urea nitrogen, serum creatinine, amylase, and lipase. The patient was treated with acupuncture and herbal medicine, specifically Hyangsayukgunja-tang-gahwangryeon for 4 days and Gagam-gunbi-tang for 11 days. Gastrointestinal symptoms were assessed using the Numerical Rating Scale, Gastrointestinal Symptom Rating Scale, and abdominal examination. Results: Gastrointestinal symptoms improved after taking Korean herbal medicine. Additionally, blood urea nitrogen, serum creatinine, amylase, and lipase levels showed improvement compared to values before treatment. Conclusions: Korean medicine treatment can improve clinical symptoms without damaging the kidneys of patients with acute kidney injuries.
