Usefulness of Myotonometer for Measurement of Tissue Compliance on Medialis Gastrocnemius in Patients with Stroke (뇌졸중 환자에서 내측 비복근의 조직탄성 측정을 위한 Myotonometer의 유용성)

  • The aim of this study was to find useful parameters of the between myotonometer and surface electromyography(sEMG) on the medialis gastrocnemius of stroke patients and investigate between these parameters and modified Ashworth scale(MAS) relationship. 5 years clinical experience physical therapist using the Modified Ashworth Scale(MAS) was selected 15 patients with ankle spasticity and divided randomly MAS2, MAS3, MAS4 groups. Myotonometer and sEMG was measured during relaxed and maximum voluntary contractions of the gastrocnemius muscle. The results of this study, the higher MAS score was the lowered the relaxation and contraction state tissue compliance and muscle activity and in the correlation analysis the higher MAS score during voluntary contraction in the cylinder receiving low-intensity correlation could see that increased than relaxation. Therefore, the myotonometer is a useful clinical and research tool with spasticity muscle and can provide objective quantitative data about the efficacy of physical therapy interventions.

Effect of Dehydroepiandrosterone on Affected and Unaffected Hindlimb Muscles in Rats with Neuropathic Pain Induced by Unilateral Peripheral Nerve Injury (DHEA 투여가 일측성 말초신경 손상에 의해 유발된 신경병증성 통증 쥐의 환측과 정상측 뒷다리근에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of DHEA (Dehydroepiandrosterone) on muscle weight and Type I and II fiber cross-sectional area of affected and unaffected hindlimb muscles in rats with neuropathic pain induced by unilateral peripheral nerve injury. Methods: Neuropathic pain was induced by ligation and cutting of the left L5 spinal nerve. Adult male Sprague-Dawley rats were randomly assigned to one of two groups: The DHEA group (n=10) had DHEA injections daily for 14 days, and the Vehicle group (n=10) had vehicle injections daily for 14 days. Withdrawal threshold, body weight, food intake and activity were measured every day. At 15 days all rats were anesthetized and soleus, plantaris and gastrocnemius muscles were dissected from the both hindlimbs. Body weight, food intake, activity, muscle weight and Type I, II fiber cross-sectional area of the dissected muscles were measured. Results: The DHEA group showed significant increases (p<.05), as compared to the vehicle group for muscle weight of the unaffected plantaris, and in Type II fiber cross-sectional area of the gastrocnemius muscle. The DHEA group demonstrated a higher pain threshold than the vehicle group whereas total diet intake and activity score were not significantly different between the two groups. Conclusion: DHEA administration for 14 days attenuates unaffected plantaris and gastrocnemius muscle atrophy.

Relationship of the Signal Transduction-mediated Proteins and Enzymes to Contractility and Plasticity in Skeletal Muscles (골격근의 수축과 가소성에 대한 신호전달-매개 단백질 및 관련 효소의 상관성)

  • Background: It is generally accepted that skeletal muscle contraction is triggered by nerve impulse and intracellular $Ca^{2+}\;([Ca^{2+}]_i)$ released from intracellular $Ca^{2+}$ stores such as sarcoplasmic reticulum (SR). Specifically, this process, called excitation-contraction (E-C) coupling, takes place at intracellular junctions between the plasma membrane, the transverse (T) tubule L-type $Ca^{2+}$ channel (dihydropyridine-sensitive L-rype $Ca^{2+}$ channel, DHPR, also called tetrads), and the SR $Ca^{2+}$ release channel (ryanodine-sensitive $Ca^{2+}$ release channel, RyR, also called feet) of internal $Ca^{2+}$ stores in skeletal muscle cells. Furthermore, it has been reported that the $Ca^{2+-}$ dependent and -independent contraction determine the expression of skeletal muscle genes, thus providing a mechanism for tightly coupling the extent of muscle contraction to regulation of muscle plasticity-related excitation-transcription (E-T) coupling. Purpose: Expression and activity of plasticity-associated enzymes in gastrocnemius muscle strips have not been well studied, however. Methods: Therefore, in this study the expression and phosphorylation of E-C and E-T coupling-related mediators such as protein kinases, ROS(reactive oxygen species)- and apoptosis-related substances, and others in gastrocnemius muscles from rats was examined. Results: I found that expression and activity of MAPKs (mitogen-activated protein kinases, ERK1/2, p38MAPK, and SAPK/JNK), apoptotic proteins (cleaved caspase-3, cytochrome c, Ref-1, Bad), small GTP-binding proteins (RhoA and Cdc42), actin-binding protein (cofilin), PKC (protein kinase C) and $Ca^{2+}$ channel (transient receptor potential channel 6, TRPC6) was observed in rat gastrocnemius muscle strips. Conclusion: These results suggest that MAPKs, ROS- and apoptosis-related enzymes, cytoskeleton-regulated proteins, and $Ca^{2+}$ channel may in part functionally import in E-C and E-T coupling from rat skeletal muscles.

Effects of Consecutive whole Body Vibration Exercise using Heel Raise Posture on Neuromuscular Response during Single-leg Stance (뒤꿈치 들기 자세를 이용한 전신진동 운동이 외발서기 시 근신경 반응에 미치는 영향)

  • Objective: This study aimed to analyze the effects of consecutive whole body vibration through heel raise posture on the center of pressure and electromyography of anterior tibial muscle, lateral gastrocnemius and soleus muscles during single-leg stance. Method: The subjects of this study included 30 healthy males in their 20's, with the following inclusion criteria: no history of orthopaedic medical history, no participation in regular exercises, no history of whole body vibration exercise, and right leg being the dominant leg. The experimental procedure involved pretreatment measurement of eye open single-leg stance, application of whole body vibration for 30 seconds, post-treatment measurement (3 measurements in total). Static and dynamic movements have been measured over 2 separate experiments, with 72 hours gap between the experiments. Static movement involved maintaining single-leg heel raise posture for 30 seconds while applying whole body vibration, and dynamic movement involved heel raise (15 repetitions over 30 seconds) while applying whole body vibration. The strength of applied whole body vibration was 35 Hz frequency and 2~4 mm amplitude. Results: As the single-leg posture after static heel raise posture, mediolateral velocity of the center of pressure at post 2 and post 3 were significantly reduced compared to the pre-treatment measurement. In addition, the percentage for reference voluntary contraction in anterior tibial muscle and soleus and median frequency at anterior tibial muscle and lateral gastrocnemius muscle at post 3 were significantly decreased compared to the pre-treatment value. As the single-leg posture after dynamic heel raise posture, the mediolateral 95% edge frequency of the center of pressure and median frequency at anterior tibial muscle, lateral gastrocnemius muscle, and soleus muscle at post 3 were significantly reduced compared to the pre-treatment value. Conclusion: Acute whole body vibration via static and dynamic heel raise posture have positive effect on mediolateral posture control during single-leg stance.

Effect of regular exercise during recovery period following steroid treatment on the atrophied hindlimb muscles induced by steroid in rats (스테로이드 치료 후 회복기의 규칙적인 운동이 쥐의 스테로이드 유발성 위축 뒷다리근에 미치는 영향)

  • This study was conducted to determine whether low intensity regular exercise following steroid treatment could attenuate steroid-induced muscle atrophy. Thirty-eight Sprague-Dawley rats weighing $165{\sim}175g$ were divided into six groups ; control group(C), dexamethasone administration group(D), sedentary normal saline administration group(C+Se), exercise after normal saline administration group(C+Ex), sedentary group after dexamethasone administration(D+Se), exercise group after dexamethasone administration(D+Ex). Either dexamethasone(5mg/kg) or normal saline was injected for 7days accordingly. Exercise was started at 10m/min on the $10^{\circ}$ grade treadmill and gradually increased up to 15m/min by the 7th day for 60minutes/day($20min{\times}3$). The data were analyzed by Kruskal-Wallis test and Mann-Whitney U test using the SPSS WIN 9.0 program. Body weight, muscle weight and myofibrillar protein content of both plantaris and gastrocnemius, Type I, II muscle fiber cross-sectional area of plantaris, and Type II muscle fiber cross-sectional area of gastrocnemius in D group were significantly lower than those of C group(p<0.05) respectively. Hindlimb muscle weight, myofibrillar protein content of both plantaris and gastrocnemius. Type I muscle fiber cross-sectional area of soleus and Type I, II muscle fiber cross-sectional area of plantaris in D+Ex group tended to increase compared to those of D+Se group. Myofibrillar protein content of both plantaris and gastrocnemius, Type I muscle fiber cross-sectional area of plantaris in D+Ex group tended to increase compared to those of C+Se group. Based on these results, it is suggested that regular low-intensity exercise during recovery period after steroid treatment might facilitate the recovery from steroid-induced muscle atrophy.

  • Radioactive Phosphorus $(^{32}P)$ was administered intramuscularlly to the newly hatched chicken in the purpose of determination of the uptake and the distribution, as related to sex and hour differences of the several organs of the bodies. $2\;{\mu}\;of\;^{32}P$ was administered to each chick, and the distribution of 32P was observed in 1 hour and 24 hours after administration. In this experiment 80 heads of chicken were used(40 chicken were one day and 40 chicken were 7 days old) and the results obtained as follows: 1. The tissue showed an uptake rate of $^{32}P$ dose per 100 milligram of tissue in one day old chicken, with the following sequence: Males (1 hour): Femur. Liver. G., Muscle. Testis. Brain (24 hour): Femur, Testis, Gastrocnemius Muscle, Liver, Brain. Female(1 hour): Femur, Liver, Gastronemius Muscle, Ovary, Brain. (24 hour): Femur, Liver, Gastrocnemius Muscle, Ovary, Brain. 2. In 1 hour, the uptake rate of $^{32}P$ of the tissues showed significant difference between the male and the female except the gastrocnemius muscle and the brain in one day old group, but they were no significance except the testis and ovary after 24 hours. 3. The distribution of $^{32}P$ of the tissues exhibited higher in 1 hour than in 24 hours except the femur, the brain of the male and female, the brain and gastrocnemius muscle of the female in one day old group. 4. The tissue showed an uptake rate of $^{32}P$ dose per 100 miligram of tissue in 7days old chicken, with the following sequence: Male (1 hour): femur, liver, gastrocmenius muscle, testis, brain. (24 hour): femur, testis, gastrocmenius muscle, liver, brain. Female(1 hour): femur, liver, gastrocmenius muscle, ovary, brain. (24 hour): femur, ovary, liver, gastrocmenius muscle, brain. 5. The distribution of $^{32}P$ of the tissues showed no significant difference between the male and the female except the testis and ovary after 24 hours in 7 days old chicken group. 6. The distribution of $^{32}P$ the tissues exhibited higher in 1 hour in 24 hours except the femur, the brain of the male and the female, the brain and the ovary of the female in 7 days old chicken group.

Effects of an Anabolic Steroid, Nandrolone Phenylpropionate, on Reductions in Body and Muscle Proteins Under the Dietary Regimens of Feeding a Low-Protein Diet and of 50% Food Restriction in Rats

  • The aim of the present investigation was to see whether an anabolic steroid, nandrolone phenylpropionate (NPP), exerts protienanabolic effects under such adverse nutritional conditions as protein deficiency and protein-energy malnutrition in male rats. feeding on a low-protein (8% casein) diet resulted in a marked reduction in body weight gain that was associated with reductions in body protein and protein content of gastrocnemius muscle. Administration of NPP (4 mg/kg body weight) did not alter muscle and body protein depletion induced by a low-protein diet. 50% food restriction caused reductions in body protein and in protein content of gastrocnemius muscle. These reductions were partially prevented by NPP (4 mg/kg body weight). Food restriction did not affect plasma concentration of corticosterone, insulin, or tetosterone plus dihydrotestosterone. On the other hand, neither plasma concentration of corticosterone nor insulin were affected by NPP. The present results show that anabolic steroids do not express anabolic effects under conditions of protein deficiency, but in protein-energy malnutrition, anabolic steroids exert their anabolic effects even in male rats.

Analysis of Muscle Activities of Lower Extremity in Jumping Pattern (점프유형에 따른 하지의 근 활동 형태연구(근전도 데이터 표준화 방법을 중심으로))

  • The purpose of this study was to compare the muscle activities of Double Legged Jump (DLJ) and Single Legged Jump (SLJ) by the normalization of muscle activity. Eight college students without the lower extremity injuries were selected as subjects for collecting EMG data of vastus medialis and gastrocnemius. The entire section of motion was established as eccentric and concentric contractions, and each of the contractions was divided into three sections with equal timing intervals, which becomes a total of 6 phases. The EMG data of each phase was integrated and normalized. The muscle activities of the vastus medialis for both eccentric and concentric contractions were significantly different between DLJ and SLJ(p<.05). The increase in overall muscle activity of SLJ was 33.6%. Approximately, there was an increase of 25.9% in eccentric contraction and 40% in concentric contraction. Moreover, the data of the muscle activity of gastrocnemius was similar to the data of the muscle activity of vastus medialis. In conclusion, this research suggests muscle activity of a certain motion can be normalized for an analysis of another motion.

The Analysis of Muscle Activities on the Lower Limb during Wearing Functional Insole (다기능성 인솔 착용 시 하지의 근활성도 분석)

  • The purpose of this study was to analyze muscle activities on functional insole with diet effect. Originally, ten healthy female subjects with an average age of 23.2 year(S.D=1.1), weight of 49.7 kg(S.D=4.9), height of 163.2 cm(S.D=3.5) and a shoe size of 237.5 cm(S.D=4.9) were participated in this experiment. Ten healthy females walked on a treadmill(speed=about 4.2 km/h) wearing two different insole types. Muscle activities data was collected using the EMG operating system. The surface EMG signal for tibialis anterior(TA), gastrocnemius(GA), vatus lateralis(VL) and biceps femoris(BF) were acquired at the RMS(10 Hz, 350 Hz) using Noraxon Telemyo DTS system(Noraxon inc, USA). This study processed the data using the Windows SPSS ver.17.0 to get an independent t-test, with the setting, p<.05. Analysis of muscle activity were measured and calculated during walking. The results are as follow: Functional insole wearing were increased muscle activities significantly from Tibialis anterior(TA) during total gait cycle. Normal distribution was demonstrated in total step of stances period. One foot standing position showed decreased muscle activity. Two foot standing position was demonstrated with gastrocnemius and biceps femoris. As a result of the analysis, Functional insole will inerease the diet effect in the use of four muscle groups.
