Serum Gamma-glutamyltransferase Levels and the Risks of Impaired Fasting Glucose in Healthy Men: A 2-year Follow-up (건강한 남성에서 혈중 감마지티피 수준과 공복혈당장애 (IFG)의 발생 위험도: 2년 추적 연구)

  • Objectives: An increase in the serum gamma-glutamyltransferase (GGT) concentration has been regarded as a marker of alcohol drinking or liver disease. Some reports, however, have suggested that the serum GGT may be a sensitive and early biomarker for the development of prediabetes and diabetes. In this study we investigated whether serum GGT is a reliable predictor of the incident impaired fasting glucose (IFG), including diabetes. Methods: We performed a prospective study for two years (2002-2004). We analyzed the periodic health examination data from a total of 4,711 men. The examinations were done in the years 2002 and 2004. The analyzed data included a self-questionnaire, a physical examination and the laboratory results. Both IFG and diabetes were defined as a serum fasting glucose concentration of more than 100 mg/dL and 126 mg/dL, respectively. Results: A total of 738 cases (15.7%) of incident IFG and 13 cases (0.3%) of diabetes occurred. The mean serum GGT concentrations were quite different between the normal (38.0 IU) and incident IFG groups (50.3 IU), and the incident diabetes group (66.0 IU) (p<0.001). After multivariable adjustment, the relative risks for incident IFG or diabetes across the baseline GGT categories (<10th, 10th-20th, 30th-40th, 50th-60th, 70th-80th and >90th percentile) were 1.0, 1.172 (0.769-1.785), 1.107 (0.725-1.689), 1.444 (0.934-2.232), 2.061 (1.401-3.031) and 2.545 (1.784-3.631) (p-value for trend: <0.001). The risks significantly increased with increasing levels of GGT for 2 years; when comparing the increased groups (<10%, 10-20%, >20%) versus the decreased over 20% group of GGT, the risks for IFG or diabetes were 1.334 (1.002-1.776), 1.613 (1.183-2.199) and 1.399 (1.092-1.794). Conclusions: Our findings suggest that serum GGT concentrations within its normal range may be an early predictor of the development of IFG and diabetes. As serum GGT is a relatively inexpensive test and a reliable marker, it might have important implications in public health promotion.

Mercury Exposure in Association With Decrease of Liver Function in Adults: A Longitudinal Study

  • Objectives: Although mercury (Hg) exposure is known to be neurotoxic in humans, its effects on liver function have been less often reported. The aim of this study was to investigate whether total Hg exposure in Korean adults was associated with elevated serum levels of the liver enzymes aspartate aminotransferase (AST), alanine transaminase (ALT), and gamma-glutamyltransferase (GGT). Methods: We repeatedly examined the levels of total Hg and liver enzymes in the blood of 508 adults during 2010-2011 and 2014-2015. Cross-sectional associations between levels of blood Hg and liver enzymes were analyzed using a generalized linear model, and nonlinear relationships were analyzed using a generalized additive mixed model. Generalized estimating equations were applied to examine longitudinal associations, considering the correlations of individuals measured repeatedly. Results: GGT increased by 11.0% (95% confidence interval [CI], 4.5 to 18.0%) in women and 8.1% (95% CI, -0.5 to 17.4%) in men per doubling of Hg levels, but AST and ALT were not significantly associated with Hg in either men or women. In women who drank more than 2 or 3 times per week, AST, ALT, and GGT levels increased by 10.6% (95% CI, 4.2 to 17.5%), 7.7% (95% CI, 1.1 to 14.7%), and 37.5% (95% CI,15.2 to 64.3%) per doubling of Hg levels, respectively, showing an interaction between blood Hg levels and drinking. Conclusions: Hg exposure was associated with an elevated serum concentration of GGT. Especially in women who were frequent drinkers, AST, ALT, and GGT showed a significant increase, with a significant synergistic effect of Hg and alcohol consumption.

The Relationship between Lipid Peroxidation of Red Blood Cell Membrane, and Mean Corpuscular Volume and Liver Enzyme Markers in Alcohol Dependence Patients (알코올 의존 환자의 적혈구막 지질과산화 정도와 적혈구 평균 용적 및 간 효소 지표와의 관련성)

  • Objectives : Alcohol-induced oxidative stress has been known to injure various tissues or organs. This stress is related with free radicals which are produced as the result of long-term alcohol consumption. Malonyldialdehyde(MDA) is produced by the interaction of free radicals and cell membrane lipids, and indicates the degree of lipid peroxidation indirectly. The purpose of this study was to investigate the relationship between red blood cell(RBC) membrane lipid peroxidation by free radicals, and associated hepatic injuries and hematologic changes. Methods : Thirty-three subjects diagnosed as alcohol dependence according to DSM-IV diagnostic criteria were evaluated within 72 hours after discontinuing alcohol drinking. Clinical characteristics were evaluated by CAGE questionnaire and Korean Michigan Alcoholism Screening Test(MAST). RBC membrane MDA level was measured as the marker of RBC membrane lipid peroxidation. Aspartate aminotransferase(AST), alanine aminotransferase(ALT) and gamma-glutamyltransferase(GGT) were used as the biochemical markers of liver damage due to alcohol ingestion. The alcohol-induced hematologic change was assessed by mean corpuscular volume(MCV). Results : The results were as follows. Clinical characteristics were not different between two groups having normal and abnormal levels of AST, ALT, GGT or MCV. The levels of MDA were not correlated with the clinical characteristics and serum levels of AST, ALT and GGT. However, there was a significant correlation between the levels of MDA and the value of MCV(p=0.017). Conclusions : These findings suggest that oxidative stress in alcohol dependence may not be reflected in liver enzyme markers such as AST, ALT and GGT, but may be reflected in MCV.

Effects of Providing Health Education to Workers for the Management of Liver Disease Screened by Periodic Health Surveys (일개 사업장 간장질환 유소견자 보건교육의 효과)

  • Purpose: This study was conducted to compare the changes in the levels of liver enzymes after providing health education to workers. Methods: Among 909 electronics-manufacturing workers, 96 (10.6%) workers had abnormal liver functions. Of these, male workers were randomly assigned into either the health education experimental group (48 [52.2%]) or the control group (44 [47.8%]). Depending on the level of participation in the health education, workers in the experimental group were classified into the participation (32 [34.8%]) and non-participation groups (16 [17.4%]). Changes in the levels of liver enzymes were compared among three groups. Results: The changes in the levels of gamma-glutamyltransferase (r-GTP) in the participation, non-participation, and control groups were $-25.3{\pm}54.5$ IU/L, $-4.4{\pm}24.1$ IU/L, and $-5.3{\pm}38.8$ IU/L, respectively (p=.036). Aspartate transaminase, alanine transaminase, waist circumference, body mass index, daily alcohol consumption, weekly exercise, and changes in smoking habits in the 3 groups did not differ significantly. In the multiple linear regression analysis, the variable of education participation revealed a significant regression coefficient of -25.10 when the change in r-GTP levels was the dependent variable. Conclusion: A brief health education targeted towards the management of liver disease among workers improved r-GTP levels.

The Association of Central Obesity with Type 2 Diabetes among Koreans according to the Serum Gamma-Glutamyltransferase Level: Korean Genome and Epidemiology Study (혈청 Gamma-Glutamyltransferase에 따른 복부비만과 제2형 당뇨병간의 관련성: Korean Genome and Epidemiology Study)

  • Objectives : This cross-sectional study was performed to examine if the serum gamma-glutamyltransferase (GGT) level that is within its normal range is associated with the risk of type 2 diabetes and if the association between the waist hip ratio (WHR) and type 2 diabetes is different depending on the serum GGT levels. Methods : The study subjects were 23,436 persons aged 40 years or older and who participated in regular health check-ups at 11 hospitals (males: 5,821, females: 17,615). The gender-specific quintiles of the serum GGT and WHR were used to examine the associations with type 2 diabetes. Results : The serum GGT levels within their normal range were positively associated with type 2 diabetes only in women. The adjusted odds ratios (ORs) were 1.0, 1.0, 1.4, 2.1, and 2.5 according to the quintiles of the serum GGT ($p_{trend}$<0.01). The WHR was more strongly associated with the prevalence of diabetes among the women with a high-normal serum GGT level as compared with those with a low-normal serum GGT level (p for interaction=0.02). For example, the adjusted ORs for women with a low normal serum GGT level were 1.0, 1.2, 1.5, 2.2, and 2.4 according to the quintiles of the WHR, while those figures were 1.0, 2.4, 3.6, 5.0, and 8.3 among the women with a high normal serum GGT level. However, in men, the serum GGT was very weakly associated with type 2 diabetes and the association between the WHR and type 2 diabetes was not different depending on the serum GGT level. Conclusions : Serum GGT within its normal range was positively associated with type 2 diabetes, and central obesity was more strongly associated with the prevalence of type 2 diabetes when the serum GGT level was highnormal. However, these associations were observed only in women, which is different from the previous findings. The stronger relation between central obesity and type 2 diabetes among women with a high-normal serum GGT level can be useful for selecting a group that is at high risk for type 2 diabetes irregardless of whatever the underlying mechanism is.

  • Pretreatment of acidic polysaccharide of Korean red ginseng (AcPS) for two weeks remarkably lowered the elevated content of lipid peroxide and levels of aminotransferases, sorbitol dehydrogenase, ${\gamma}$-glutamyltransferase, alkaline phosphatase and lactate dehydrogenase in liver intoxicated by acetaminophen (AA) . Pretreatments of AcPS also strengthen the liver function of glutathione related detoxication system indicated by glutathione contents and activities of glutathione S-transferase and glutathione reeducates which were affected by AA treatments. Activity of ${\gamma}$-glutamylcysteine syntheses was not changed by AcPS pretreatment whereas the activity of flu tathione reeducates was increased significantly. These results collectively indicate that the treatments of AcPS can promote the metabolism of lipid and reduce the production of peroxide in acetaminophen-intoxicated animals.

The Therapeutic Effects of Korean Red Ginseng on Carbon Tetrachloride- and Galactosamine-induced Hepatotoxicity in Rats (홍삼의 사염화탄소 및 갈락토사민 유발 간독성에 대한 치료효과)

  • In this study, we investgated the effect of Red Ginseng (KRG) on liver damage induced by carbon tetrachloride (CTC) and galactosamine (GalN) in rats using indicator enzymes such as serum alanine/aspartate aminotransferases, sorbital dehydrogenase, lactate dehydrogenase, and ${\gamma}$-glutamyltransferase. Treatment of KRG restored these enzyme activities to near normal level compared to CTC or GalN treatment alone. Treatment of KRG also enhanced hepatic microsomal enzyme system, malondialdehyde formation, and depletion of reduced glutathione content, which were reduced by CTC or GalN. We also found that the decreased activities of glutathione S-transferase and glutathine reductase but not ${\gamma}$-glutamycysteine synthetase after KRG treatment restored to normal level. These results indicate that KRG has potent therapeutic activity against CTC- and GalN-induced hepatotoxicity in rat.

Comparison of Blood Metabolites and Enzyme Activities at Different Slaughter Ages of Hanwoo Cattle

  • This study was conducted to investigate changes of blood metabolites and enzyme activities at different slaughter ages of Hanwoo cattle. Blood samples were taken from six hundred thirty two steers (24~33 months of age) and one hundred twenty eight bulls (17~24 months of age) by venipuncture from caudal vein immediately prior to slaughter. Glucose concentrations were linearly increased (P<0.001) with increases of the slaughter age in steers, and blood urea nitrogen (BUN) concentrations were linearly decreased (P<0.001) with increases of the slaughter age in steers. Likewise, glucose and albumin concentrations in bulls were linearly decreased (P=0.009 and P<0.001, respectively) with increases of the slaughter age. BUN, cholesterol, triglycerides, and total cholesterol concentrations were linearly increased (P<0.001, 0.018 and 0.002, respectively), and creatinine concentrations were quadratically increased (P=0.009) with increases of the slaughter age. Activity of gamma glutamyltransferase (${\gamma}$-GT) and aspartate aminotransferase (AST) was linearly increased (P<0.001) with increases of the slaughter age in both steers and bulls, whereas alanine aminotransferase (ALT) was not affected by increasing slaughter age. In addition, the concentrations of ${\gamma}$-GT and AST were higher (P<0.05) for bulls than those of steers at 24 months of age. Increased levels of ${\gamma}$-GT and AST indicate impaired liver function; it may be associated with increases of concentrate level which is a concomitant of the extended fattening periods in Hanwoo feeding. In conclusion, the results in the present study may be one of the useful information for diagnosis of the metabolic disorder in Hanwoo cattle.

Biological Activities of Ursi Fel's Component Ursodeoxycholic Acid and Its Derivatives (웅담 성분 Ursodeoxycholic Acid 유도체들의 생물활성)

  • Ursi Fel's component ursodeoxycholic acid (UDCA), a traditional medicine, is used for the treatment of hepatic diseases. UDCA derivatives prepared by conjugation with antioxidant moiety such as maltol, sesamol, eugenol, mesitol and 3,4-(methylenedeoxy)aniline were expected to have various biological activity caused by synergistic effect of UDCA. Therefore, in this study, it was conducted the study of the manufacture of the UDCA derivatives and their biological activity. As a result, UDCA derivatives showed weak antioxidant activity in TBA method in vitro compared to original agents. SJ-505, SJ-502 and SJ-504 showed the effect of reducing ALT, AST, sorbitol dehydrogenase and ${\gamma}-glutamyltransferase$ in $CCl_4-induced$ liver injury experiment in vivo, even if the effects are weaker than UDCA and silymarin of the control group.

  • During the serial attempts to identify the chemical and biological characteristics of Ailanthi Cortex Radicis, the root bark of Ailanthus altissima (Simaroubaceae), we find out the serious toxic effect on kidney by chloroform fraction of the methanolic extract of the herb drug. The toxicities were revealed as the increase of urea nitrogen amount in blood and lactate dehydrogenase and ${\gamma}-glutamyltransferase$ activities in urine and the decrease of the concentration of glutathione and both of protein bound and non-protein bound -SH in kidney tissue.
