FES를 이용한 편마비 환자의 보행증진에 대한 연구 (Study for gait improvement of hemiplegic patients using Functional Electrical Stimulation)

  • The purpose of this study was to determine the effects of FES on the clinical test patients who had disturbance gait due to cereberal vascular accident. The subjects for study were 16 hemiplegic patients (9 Male and 7 Female) who can gait with or without device. Their average age was 55 and they received average of 20.19 month of treatment collected data analysis was completed by using one-way analysis variable(ANOVA), Pearson ($-1{\leq}r{\geq}1$). The results were as follows : 1) There was difference in four variable (stride length,gait speed,gait cadence) between at the biginning and at the end of the treatment of FES (p<0.01). 2) There was relationship in capacity of activity between MAS and stride length (r = 0.751), gait speed (r = 0.689) but no relationship gait cadence (r = 0.236). 3) Age revealed relationship of stride length (r = -0.727), gait speed (r = -0.725), gait cadenc (r = -0.362). 4) There was no relationship in months post-CVA with MAS (r = 0.171), stride length (r = -0.110), gait speed (r = -0.096), gait cacedce (r = -0.154).

Relationship between Gait, Static Balance, and Pelvic Inclination in Patients with Chronic Stroke

  • PURPOSE: This study examined the correlations between gait, static balance, and pelvic inclination in patients with chronic stroke. METHODS: Twenty-two chronic stroke patients were included in this study. The subjects participated in gait, static balance, and pelvic inclination tests. In the gait measurement, the cadence and gait velocity were measured, and the average of three trials was calculated and recorded. The static balance was measured using a force platform. The data was captured for ten seconds, and five successful trials were recorded. Pelvic inclination in the sagittal plane was measured using a palpation meter. For data processing, a KolmogorovSmirnov test was used to determine the type of distribution for all variables. Pearson's correlation coefficient was used for correlation analysis. The correlations among the gait, static balance, and pelvic inclination was calculated. The level of significance was .05. RESULTS: Significant negative correlations were observed between the gait variables (cadence, velocity) and static balance variables (COP path length, COP average velocity, and 95% confidence ellipse area) (p < .05). On the other hand, there was no significant correlation between pelvic inclination and gait or between the pelvic inclination and static balance variables. CONCLUSION: Significant correlations were observed between the gait function and static balance. On the other hand, there were no significant correlations between the pelvic inclination and gait and static balance. These results suggest that the pelvic inclination is not an important consideration for increasing the gait function and static balance.

3차원 관절 전기측각기를 이용한 정상성인의 보행분석결과 (Three Dimensional Gait Analysis of Normal Adults with Electrogoniometer Domotion)

  • Background: The aim of this study is to present the basic reference data of kinematic gait analysis of normal Korean adults with 3 dimensional electrogoniometer, $Domotion^{(R)}$. Method: The basic kinematic gait parameters of hip, knee and ankle joints on the sagittal plane were obtained from 10 healthy adults with 5 repetition for each. Three-dimensional gait analysis was performed with $Domotion^{(R)}$ electrogoniometer in 10 meters long flat floor. Each data collected was processed with IBM PC equipped with gait analysis program. Results: Mean maximal hip flexion was $23.05^{\circ}{\pm}4.62^{\circ}$and mean maximal hip extension was $6.46^{\circ}{\pm}1.30^{\circ}$. Knee flexion was observed with two peak values. The first peak knee flexion was $6.50^{\circ}{\pm}2.07^{\circ}$ at 20.4% of gait cycle and the second peak flexion was $50.34^{\circ}{\pm}2.23^{\circ}$ at 75.8% of gait cycle. Mean maximum ankle dorsiflexion was $5.57^{\circ}{\pm}1.19^{\circ}$ at 44% of gait cycle and mean maximum ankle plantar flexion was $15.51^{\circ}{\pm}1.73^{\circ}$ at 68.5% of gait cycle. Conclusion: We concluded three dimensional gait analysis with electrogoniometer $Domotion^{(R)}$ offers a valid and reliable kinematic data and the application of this tools for clinical gait evaluation will be helpful in management of pathological gait.

Effects of Treadmill Gait Training According to Different Inclination on Postural Balance in Patients with Chronic Stroke

  • Purpose: This study was to examine the effects of treadmill gait training at different controlled inclinations on the standing balance of hemiplegic patients caused by cerebrovascular injury. Methods: The study's subjects were 44 patients with chronic stroke, randomly divided into three experimental groups: $0^{\circ}$ treadmill gait training (n=14), $5^{\circ}$ treadmill gait training (n=15), and $10^{\circ}$ treadmill gait training (n=15). In addition to conventional physical therapy, the subjects underwent gait training on a treadmill with one of three different inclinations for 30 min per session five times per week for six weeks. The Biodex balance system SD, Berg balance scale, and timed up and go tests were used to measure the postural balance of the patients. Results: This study showed that gait training on a treadmill at controlled inclinations of $0^{\circ}$, $5^{\circ}$, and $10^{\circ}$ positively influenced the stroke patients' standing balance. The $5^{\circ}$ and $10^{\circ}$ inclination gait training groups showed significantly more improvement than the $0^{\circ}$ inclination group. Conclusion: Gait training on a treadmill with inclinations of $0^{\circ}$, $5^{\circ}$, or $10^{\circ}$ can be said to positively affect the postural balance of hemiplegic patients due to stroke. In particular, $5^{\circ}$ and $10^{\circ}$ inclination gait training offered more significant improvement than the $0^{\circ}$ inclination gait training group. When added to regular treatment routines, gait training at controlled inclinations is an effective intervention for improving hemiplegia due to postural balance.

  • Purpose: To examine the correlation between the gait speed and muscle activation or foot pressure in stroke patients. Methods: Twenty five functionally ambulant stroke patients (male/female: 15/10, mean age: $57.65\pm2.30$) were enrolled in this study. The patients were asked to walk on a plate at a self-selected and comfortable speed. Three walking trials were obtained and then averaged for data analysis. The gait speed and foot pressure were measured from a RS-Scan system. Activation of the quadriceps femoris muscle and biceps hamstring muscle (%RVC) were recorded using ProComp $Infiniti^{TM}$. Results: There was a significant positive correlation between Hamstring muscle activation (%RVC) and gait speed. The gait speed correlated with the foot pressure of the lateral metatarsal zone (M3-5) in the affected side. There was a correlation between the gait speed and the foot pressure of the lateral metatarsal (M3-5) and heel (medial, lateral) zone in the less-affected side. Conclusion: The gait speed is related to hamstring muscle activation and the characteristics of foot pressure. This information was observed in both the affected and less-affected sides, suggesting that rehabilitation programs should be implemented on both sides.

고령자 보행보조로봇 제어기법 연구 (Research of Elderly Gait-assistant-robot Control System)

  • In this study, the control method of assistive robot was developed for the elderly. The control method of gait-assistant-robot was proposed considering the change of COP (Center of Pelves), BOS (Base of Support) and comparative analysis of the moving velocity for the elderly. We analyzed the movement of COP of the body and its velocity of the elderly equipped with manual walker and gait-assistant-robot. As a result, change in COP was greater from left to right than from anterior to posterior; also, the average velocity of the movement of COP and manual walker for manual walker gait was 0.7(m/s). Therefore, it is necessary to concern more on the left-right balance and synchronization of the velocity of COP.

이상근 증후군에서 보툴리눔 독소 주사 후 보행의 호전 (Gait Improvement after Botulinum Toxin Injection in a Patient with Piriformis Muscle Syndrome)

  • Piriformis muscle syndrome is a condition that causes direct muscle pain around piriformis muscle or sciatica from irritated sciatic nerve and the diagnosis remains debatable. The main treatment is symptomatic relief from conservative therapy such as medication and piriformis stretching exercise, and various therapeutic injections including local anesthetic, corticosteroid, botulinum toxin can be considered for diagnostic and therapeutic purposes. In this case, a 54-year-old male who had sciatica and gait disturbance showed piriformis muscle hypertrophy in the pelvis MRI. From imaging studies, electrodiagnostic study and physical examination, he was diagnosed with piriformis muscle syndrome. He underwent trigger point injection and botulinum toxin injection into the piriformis muscle, and pain and gait disturbance significantly improved. This case reports a case of piriformis muscle syndrome with clinical symptom of gait disturbance, which was improved by botulinum toxin injection.

Clinical Feasibility of Wearable Robot Orthosis on Gait and Balance Ability for Stroke Rehabilitation: A Case Study

  • Purpose: The emphasis on gait rehabilitation after stroke depends on training support through the lower limbs, balance of body mass over the changing base of support. However, muscle weakness, lack of control of lower limb, and poor balance can interfere with training after stroke. For this case study report, a wearable robot orthosis was applied to stroke patients in order to verify its actual applicability on balance and gait ability in the clinical field. Methods: Two stroke patients participated in the training using the wearable robot orthosis. Wearable robot orthosis provides patient-initiated active assistance contraction during training. Training includes weight shift training, standing up and sitting down, ground walking, and stair up and down Training was applied a total of 20 times, five times a week for 4 weeks, for 30 minutes a day. Gait ability was determined by Stance phase symmetry profile, Swing phase symmetry profile, and velocity using the GAITRite system. Balance ability was measured using the Biodex balance system. Results: Subjects 1, 2 showed improved gait and balance ability with mean individual improvement of 72.4% for velocity, 19.4% for stance phase symmetry profile, 9.6% for swing phase symmetry profile, and 13.6% for balance ability. Conclusion: Training utilizing a wearable robot orthosis can be useful for improvement of the gait and balance ability of stroke patients.

  • In this study, we wolud be developed the fuzzy controlled PGO that controlled the flexion and the extension of each PGO's joint using the bio-signal and FSR sensor. The PGO driving system is to couple the right and left sides of the orthosis by specially designed hip joints and pelvic section. This driving system consists of the orthosis, sensor, control system. An air supply system of muscle is composed of an air compressor, 2-way solenoid valve(MAC, USA), accumulator, pressure sensor. Role of this system provide air muscle with the compressed air at hip joint constantly. According to output signal of EMG sensor and foot sensor, air muscles and assists the flexion of hip joint during PGO gait.

  • In this paper, a transfemoral prosthesis with a microprocessor controlled pneumatic knee developed at KOREC is presented. The resistance of the knee is changed automatically via a microprocessor as the amputee's gait speed changes, so that the prosthetic side of the amputee can follow the sound limb. Gait analysis has been conducted to evaluate the performance of the developed prosthesis and the improvement of the gait pattern including the gait symmetry was observed.
