고유수용성 신경근 촉진법 하지 패턴에 기초한 탄력밴드 훈련이 만성 편마비 환자의 보행에 미치는 영향 (The Effect of Elastic Theraband Exercise Based of PNF L/E Pattern on the Gait of the Chronic Hemiplegic Patients)

  • Purpose : The purpose of this study were to determine the effect of a Elastic Theraband Exercise Based of PNF L/E pattern on the gait of the chronic Hemiplegic Patients. Methods : We selected the 20 chronic Hemiplegic Patients not given treatment now and divided them into two groups of both 10 Elastic Theraband group and 10 Self Exercise. The first group went through a Elastic Theraband Exercise Based of PNF L/E pattern 30 minutes a day, 5 times a week, for 6 weeks. Exercise used to blue elastic band which 2 patterns of PNF by 1) hip extension - abduction - internal rotation with knee extension. 2) hip flexion - adduction - external rotation with knee flexion. The latter group experienced Self Exercise, 30 minutes a day, 5 times a week, for 6 weeks. Firstly, we measured the absolute improvement of gait velocity(m/s), cadence(steps/min) among walking characters. Secondly, we measured the functional walking ability such as Functional Ambulatory Category(FAC, score out of 5), Modified Motor Assesment Scale(MMAS, score out of 6). Data analysis was performed with using SPSS 12.0 win program. The descriptive analysis was used to obtain average and standard deviation. The independent t-test and the paired t-test were used to compare both the groups about pre and post training test. Treatment effects were established by pre and post assessment. Subjects tolerated the training well without side-effects. Therefore, the results of this study were as follows; Results : 1. There was a more significant improvement of Gait velocity(0.12m/s) Elastic Theraband group(p<.05). 2. There was a more significant improvement of cadence(9.40steps/min) Elastic Theraband group(p<.05). Conclusion : As we can see from above, the findings suggest that Elastic Theraband may be more effective than the Self Exercise for improving some gait parameters such as Gait velocity and Cadency. This conclusion also suggest that Elstic Theraband is more effective for the improvement of gait of chronic Hemiplegic Patients.

외상성 뇌손상 청소년 대상 리듬청각자극(RAS) 보행 훈련 시 선율 적용 사례 (Exploring the Use of Melody During RAS Gait Training for Adolescents with Traumatic Brain Injury: A Case Study)

  • 본 연구는 외상성 뇌손상 청소년을 대상으로 리듬청각자극(RAS) 적용 시 선율의 유무에 따른 보행 훈련 효과를 비교하고, 보행인자의 변화를 알아보고자 시행하였다. 이를 위해 3명의 외상성 뇌손상 청소년을 대상으로 사전 사후 평가를 제외한 총 8회의 보행훈련을 진행하였다. 회기별 중재 시 선율 유무에 따른 보행 속도 차이를 비교하였고, 훈련 효과에 따른 시공간적 지표 및 운동 형상학적 지표 변화 비교를 위해 VICON 370 Motion Analysis System을 사용하여 보행 지표를 측정하였다. 연구 결과 RAS 보행 훈련 시 제공한 음악 자극 형태에 따라 보행 속도에 미치는 유의미한 영향이 없으므로, 선율의 사용 유무는 보행 훈련 효과와 직접적인 관련이 없음을 알 수 있었다. 또한 사전사후 검사 결과, RAS 보행 훈련 후 분속수, 보행 속도, 활보장, 활보장 시간, 보행 대칭성이 공통적으로 개선되었으며, 대상자 모두 공통적으로 고관절 및 슬관절의 움직임에서 필요 이상의 과도한 굴곡이나 신전이 감소함에 따라 양측의 비대칭성 감소 및 움직임의 안정성이 증가한 모습을 보였다. 리듬 적용과 리듬 자극에 선율을 반영한 음악 자극 적용에 따른 보행 속도 간 차이를 비교한 본 연구 결과는 운동기능을 위한 RAS 적용 시, 일정한 간격의 리듬자극에 의한 리듬-운동 동조화가 핵심 기제로 작용함을 시사한다. 또한, 본 연구는 외상성 뇌손상 청소년을 대상으로 RAS 보행 훈련을 통한 보행 기능뿐만 아니라 보행 패턴의 개선을 살펴본 것에 의의가 있다.

팔걸이가 편마비환자의 보행에 미치는 영향 (Effect of an Arm Sling on Gait with Hemiparesis)

  • Purpose: This study was to investigate the effect of an arm sling on gait with hemiparesis. Methods: Fifteen patients(8 male, 7 female) with hemiparesis participated in this study and walked self-selected speed over 10m walkway, randomly without arm sling, with Single strap hemisling and Rolyan humeral cuff sling. It were filmed by 5 video camera and used with 3-dimensional motion analyzer system. The following gait variables were analyzed: temporo-spatial parameters, kinematic parameters. Results: In the comparison of temporo-spatial parameters each trial, walking velocity and single support time on affected side was significantly increased and step length on affected side, step length asymmetry ratio, single support time asymmetry ratio was significantly decreased in the Single strap hemisling and Rolyan humeral cuff sling. In the comparison of kinematic parameters each trial, maximal angle of the hip flexion on affected side was significantly increased in the Single strap hemisling and Rolyan humeral cuff sling and maximal angle of the knee flexion on affected side was significantly increased in the Rolyan humeral cuff sling and maximal angle of the ankle dorsiflexion on affected side was significantly increased in the Single strap hemisling. Conclusion: An arm sling improved walking velocity and decreased asymmetry and increased maximal angle of hip, knee, ankle flexion on affected side with hemiparesis caused by stroke.

Study on the Gait Pattern of the Aged with Lower Limbs Orthosis

  • The purpose of this study was to analyze the gait motion of the aged with a lower limbs orthosis. The gait motion was analyzed with and without lower limbs orthosis using APAS 3D Motion Analysis System. The pattern of lower limbs motion was tracked based on four targets attached to the body of the subject. The targets were positioned at hip, knee, ankle, and foot. The parameters measured were the displacement, the velocity, and the acceleration of the four targets. The improvement in the measured values on the displacement and the velocity of the four targets were small with the orthosis due to inconvenience of wearing it, but the increase in the acceleration was large due to the elastic force of the rubber actuator. Especially, the increase of the acceleration of foot with lower limbs orthosis seems to help the gait motion of the elderly.

The Effectiveness of the Use of Custom-Made Foot Orthotics on Temporal-Spatial Gait Parameters in Children With Spastic Cerebral Palsy

  • This study examined the effects of custom-made foot orthotics on the temporal-spatial gait parameters in children with cerebral palsy. Twenty spastic bilateral cerebral palsy (spastic CP) children (11 boys and 9 girls) participated in this study. GAITRite was used to examine the velocity, cadence, step length differential, step length, stride length, stance time, single support time, double support time, base of support, and toe angle while walking with and without foot orthotics. The differences in temporal-spatial parameters were analyzed using paired t-test. The significance level was set at .05. The velocity, cadence, both step lengths, both stride lengths, both bases of support and right toe angle significantly increased when the children with spastic CP with foot orthotics compared to without foot orthotics (p<.05). The step length differential between the two extremities, left stance time and left single support time, significantly decreased with foot orthotics (p<.05). Right stance time, right single support time, both double support times and left toe angle showed little change (p>.05). This study demonstrated that foot orthotics were beneficial for children with spastic CP as a gait assistance tool.

좌·우 기울기각도를 이용한 남자 성인의 보행안정성 평가 (Evaluation of Gait Stability using Medio-Lateral Inclination Angle in Male Adults)

  • Human body is hard to be in perfect balance during walking. Most of time the trunk is supported by one leg and the center of mass(COM) falls to the contralateral side. Thus, dynamic variables such as the velocity of the COM should be considered when gait stability is evaluated. The purpose of this study was to investigate whether the extrapolated center of mass(XCom) which utilized the COM position and its velocity, is appropriate to evaluate gait stability. Ten healthy adults participated in this study and performed 3 different types of gaits(normal(NG), hands on waists(HWG), and hands on shoulders(HSG)) onto 4 different types of obstacle(obstacle height: 0%, 30%, 40% and 50% of leg length). Medio-lateral Com-CoP and XCom-CoP inclination angle were calculated during support phase. For all condition, greater M-L XCoM-CoP inclination angles were found(p<.05) compared with those of matched obstacle height CoM-CoP. Especially, M-L XCoM-CoP inclination angle at 50% height revealed the best condition for monitoring dynamic stability. Significantly increased in M-L XCoM-CoP inclination angle was found(p<.05) as obstacle height increased on NG and HWG.

수중물리치료를 통한 뇌졸중환자의 치료적 중재효과 (The Effect of Therapeutic Intervention by Aquatic Physical Therapy on Stroke Patient)

  • Purpose : The purpose of this study is to compare the rehabilitation effects of hydrotherapy and Bobath therapy. Methods : The study was performed with patients of hemiparesis caused by cerebral stroke. The participants were divided into two groups based on random sampling method. One group received hydrotherapy while the other received Bobath therapy. Each rehabilitation program lasted 40 minutes a day, 5 days a week, for 6 weeks. Pertinent indicators-Berg's balance scale (BBS), gait parameter, and static balance analysis-were recorded before andafter the programs, as well as every 2 weeks during the rehabilitation programs Results : There was showed a significantly increase of BBS score. Static open and close showed statistically significant in interaction by time and groups. There was significant differences of gait velocity. Conclusion : These findings in this study that the hydrotherapy was effective therapy in improving balance and gait velocity.

대퇴절단자와 정상인 걸음걸이의 운동학적 요인과 발목관절 강성 비교 (Comparisons of Kinematic Factors and Stiffnesses of the Lower-limb Joints between Transfemoral Amputees and Normal Adults)

  • The purpose of this study was to compare kinematic variables and stiffnesses of ankle joints between normal person and transfemoral amputee gait in order to develop or fit prosthetic leg. Twenty subjects (ten normal persons and ten transfemoral amputees) participated in this experiment, and walked three trials at a self-selected pace. The gait motions were captured with Vicon system and variables were calculated with Visual-3D. The velocity, stride length, stride width, cycle time, double limb support time and right swing time of gaits were statistically significant. Because coefficients of variability of normal persons on velocity, double limb support time and swing time were greater than transfemoral amputees, normal persons controlled these gait variables effectively. The stiffnesses of ankle joints were not statistically significant, but patterns of stiffnesses of ankle joints during three rockers were absolutely different. The negative correlations between stiffnesses of ankle joints and cycle time and swing time were presented. These differences suggest that developing and fitting prosthetic leg were demanded. Further studies should develop fitting program and simulator of prosthetic leg.

PNF 치료가 성인 편마비 환자의 기능회복에 미치는 영향 (The Effect of PNF Method in Functional Restoration of Adult Hemiplegic Patients)

  • Objectives : The purpose of this study is to identify influence that PNF method have an effect in function restoration of the impaired patients of central nervous system. Methods : The data were collected by 15 adult stroke patients. The treatment was based on proprioceptive neuromuscular facilitation techniques. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. Results : In the comparison of functional ambulation profile(FAP) before and after experiment, the FAP was significantly increased in the PNF method. The gait velocity, cadence and single support time asymmetry ratio was significantly increased in the PNF method. The Motor Assessment Scale was significantly increased in the PNF method. Conclusions : Based on these results, it is concluded that the forced PNF method for 6 weeks can be improve the temporal-spatial gait parameters including FAP in hemiplegic patients. Therefore, the forced PNF method is useful to improve the function restoration in hemiplegic patients. Further study should be done to analyze the effects of intervention duration of treatment, optimal time to apply the treatment in more long peried.

하퇴절단자용 단축식 발과 스포츠용 에너지 저장형 발 보행 특성 비교연구 (A Comparative Study of Gait Characteristics between Single Axis Foot and Energy Storing Foot for Sports in Trans-tibial Amputee)

  • This study examined the differences in spatio-temporal parameters, joint angle, ground reaction force (GRF), and joint power according to the changes of gait speed for trans-tibial amputees to investigate the features of the energy-storing foot for sports. The subjects walked at normal speed and at fast speed, wearing a single-axis type foot (Korec) and an energy-storing foot for sports (Renegade) respectively. The results showed that Renegade yielded faster gait speed as well as more symmetric gait pattern, compared to Korec. However, as gait speed was increased, there was no significant difference in kinematics, ground reaction force, and joint power between two artificial foots. This was similar to the results from previous studies regarding the energy-storing foot, where the walking velocity and gait symmetry have been improved. Nevertheless, the result of this study differed from the previous ones which reported that joint angle, joint power, and GRF increased as the gait speed increased except spatio-temporal parameters.
