Gait Pattern of Hemiplegic Patients with Swimming Aqua-noodles

  • Objective: The purpose of this study was to investigate the effects of aqua-noodles on the practice of underwater walking in patients with hemiplegia. Method: After an oral explanation and signing an IRB approved consent form 10 participants ($66.8{\pm}10.75yr$, $165.3{\pm}8.79cm$, $73.6{\pm}46kg$) agreed to participate in this study. Each of the participants was required to walk with the aqua noodles and without the aqua-noodles in a swimming pool. Each participant was asked to walk a distance of 5 m a total of 10 times, 5 with and 5 without the aqua-noodles. The depth of the swimming pool was at 1.3 m, approximately chest height. The following variables were calculated for analysis; height of the knee (m), knee joint ROM ($^{\circ}$), ankle joint ROM ($^{\circ}$), knee joint maximum angular velocity ($^{\circ}/sec$), and ankle joint maximum angular velocity ($^{\circ}/sec$). Results: First, there was a significant increase in time (s) for the maximum knee height to reach as well as the maximum knee height (m) increased when the participant used the aqua-noodles. Second, there was a statistically significant decrease in stride length when the aqua-noodles were used. Conclusion: This study helps to verify that the effect of underwater walking exercise can provide a suitable walking exercise environment. The results of this study provide systematic scientific information about how walking in water can be used for the rehabilitation of patients and the elderly.

Application and Effectiveness of a Program to Promote Adolescent Musculoskeletal Health: A Pilot Study

  • Purpose: Adolescence is a developmental period characterized by the rapid growth of the musculoskeletal system, which is important for maintaining correct posture. Incorrect posture, lack of exercise, and reduced physical activity can cause spine deformities and affect lifelong health. This study was designed to evaluate the application and effect of a program for improving adolescents' musculoskeletal health. Methods: A quasi-experimental pilot study was conducted with 13 male and 20 female middle and high school students, with an average age of 15.39 years. Their general characteristics and physical measurements were obtained. The program consisted of group exercises (60 minutes, once per week), gait pattern monitoring, and online communication. A paired t-test and Wilcoxon signed-rank test were used to examine the program's effect. Results: Overall posture habits improved, and the total musculoskeletal index decreased; however, these results were not statistically significant. Conclusions: The devised program was effective in improving musculoskeletal imbalance. Therefore, effective programs and health devices should be developed to help adolescents maintain correct posture and encourage and support continuous participation in such programs.

Reconstruction of Triceps Tendon Avulsion Using Mesh Graft and Krackow Suture in a Border Collie

  • A 3-year-old, 24-kg intact female Border Collie was referred for a toe-touch weight-bearing stance, intermittent weight-bearing lameness, and moderate pain reaction of the right forelimb on physical examination and right humerus olecranon avulsion fracture on diagnostic imaging examination. Surgical repair was performed using tension band wiring to re-attach the triceps tendon and distal olecranon. Migration of the distal olecranon fragment was observed due to comminuted fracture of the fragment 5-days after surgery, and revision surgery was performed. The tension-relieving sutures were passed through the pre-drilled hole in the olecranon, and the polyester mesh was augmented to the suture region, covering the triceps tendon and olecranon drilling hole using the Krackow suture pattern. The elbow joint was immobilized using a type IA transarticular external fixator, which was removed 8 weeks after surgery. Fourteen weeks after surgery, no lameness was observed on gait evaluation. At follow-up after 7 months, the distal olecranon fragment had stabilized, and no lameness was observed.

트레드밀 보행시 인솔 형태 변화에 따른 족저압력 분석 (Analysis of Plantar Foot Pressure according to Insole Types during Treadmill Gait)

  • Objective : The purpose of this study was to analyze the foot-pressure distribution of 2D(2 dimensional form) & 3D(3 dimensional form; a customized arch-fit for posture correction) insoles for assessing their biomechanical functionality. Background : Recently there has been increased interest in both foot health and foot pain patients. Analysis of the plantar pressure was often used to solve the problems of the foot displayed by such people as rheumatoid arthritis patients. Method : Subjects who participated in this study were 17 female university students who had no previous injury experience in lower limbs and a normal gait pattern. The shoe size of all subjects was 240 mm. Two models of insoles of 2D(typical flat insole - 2 dimensional form) and 3D(special production - 3 dimensional form) were selected for the test. Using the Pedar-X system and Pedar-X insoles, 4.0 km/h of walking speed, and a compilation of 50 steps walking stages were used to analyze foot-pressure distribution. Results : Results of the foot-pressure distribution and biomechanical functionality on each insole were as follows; analyses of mean plantar pressure, maximum plantar pressure, maximum vertical GRF, and plantar pressure curve shape all showed overall low plantar pressure and GRF. Conclusion : This can be evaluated as an excellent insole for low levels on the plantar pressure and GRF. Therefore, it is possible to conclude that according to this analysis the 3D Customized Arch-fit Insole was better than 2D insole on the basis of these criteria.

조명의 색이 육계의 행동과 생산성에 미치는 영향 (The Effects of Light Colors on the Behavior and Performance of Broiler Chickens)

  • 본 연구에서는, 3개의 다른 색(white, blue 및 red/ 23L-1D)의 조명이 육계 암수별 행동 및 생산성에 미치는 영향에 대해서 조사하였다. 시험계는 초생추 육계를 6개 pan(pan당 암컷 16수, 수컷 16수)으로 나누어서, 3가지 색의 조명에서 실험이 실시되었다. 실험계는 3일간의 조명에 대한 적응 기간을 거친 후, 동일한 사양 조건에서, 암수 각각 3수씩, pan당 6마리의 focal birds를 중심으로 pecking, resting, standing 및 walking 행동이, 1일 120분간(pan당 20분/일), 주 5일간, 직접관찰법으로 5주간 시행되었다. 뿐만 아니라 증체량과 사료 섭취량 등은 주별로 계산되었고, 시험계 19일령과 33일령의 2회에 걸쳐서 TI-반응과, 33일령의 1회에 걸쳐서 gait score, foot pad score 및 horn burn score가 조사되었다. 조명의 색은 증체량, 사료 섭취량에는 유의한 영향을 미치지 않았다(P>0.05). 그러나 사료 효율에는 유의한 영향을 미쳐서(P<0.05), blue 조명의 경우가 white 및 red 조명의 경우보다 높았다(P<0.05). Resting 행동 빈도는 전체의 행동 빈도에서 주요한 행동으로 나타났으며, 이는 암수간 또는 조명의 색깔에 따라서 영향을 받지 않았다. 성장 초기에는 red 조명에서 resting 행동 빈도가 낮게 나타나는 경향이 있었다. 뿐만 아니라 standing 행동 빈도는 4~18일령에, walking 행동 빈도는 전 시험 기간 동안 red 조명에서 높게 나타나는 경향이 있었다. 본 연구 결과로부터 육계 사육장내 조명의 색은 생산성, 행동 빈도 및 복지에 영향을 미칠 가능성이 생각되었다.

Analyses of Plantar Foot Pressure and Static Balance According to the Type of Insole in the Elderly

  • Objective: The purpose of this study was to investigate plantar foot pressure and static balance according to the type of insole in the elderly. Methods: Thirteen elderly (mean age: $67.08{\pm}2.25years$, mean height: $159.63{\pm}9.64cm$, mean body weight: $61.48{\pm}9.06kg$) who had no previous injury experience in the lower limbs and a normal gait pattern participated in this study. Three models of insoles of the normal, 3D, and triangle types were selected for the test. The Pedar-X system and Pedar-X insoles, 3.3 km/h of walking speed, and a compilation of 20 steps walking stages were used to analyze foot-pressure distribution. Static balance test was conducted using Gaitview AFA-50, and balance (opening eyes, closing eyes) was inspected for 20 s. One-way ANOVA was conducted to test the significance of the results with the three insoles. p-value of less than .05 was considered statistically significant. Results: The mean foot pressure under the forefoot regions was the lowest with the 3D insole during treadmill walking (p<.05). The mean value under the midfoot was the highest with the 3D insole (left: p<.05, right: p<.01). The mean value under the rearfoot was the lowest with the 3D insole (p<.001). The maximum foot pressure value under the foot regions was the lowest on both sides of the forefoot with the 3D insole. A statistically significant difference was seen only in the left foot (p<.01). The maximum value under the midfoot was the highest with the 3D insole (p<.001). No statistically significant difference was detected on the values under the rearfoot. In the case of vertical ground reaction force (GRF), statistically significant difference was seen only in the left side rearfoot (p<.01). However, static balance values (ENV, REC, RMS, Total Length, Sway velocity, and Length/ENV) did not show significant differences by the type of insole. Conclusion: These results show that functional insoles can decrease plantar pressure and GRF under the forefoot and rearfoot. Moreover, functional insoles can dislodge the overload of the rearfoot and forefoot to the midfoot. However, functional insoles do not affect the static balance in the elderly.

12주간 복합운동이 여성 노인의 족저압력에 미치는 영향 (Effects of 12-Week Complex Training Program on Foot-Pressure Patterns of the Elderly Women)

  • 이 연구는 12주간 복합운동이 여성 노인의 보행동작 시 발의 압력분포에 미치는 영향을 알아보기 위하여 규칙적으로 운동에 참여하지 않는 65세 이상의 노인여성 15명을 대상으로 하였다. 연구대상에게 보행속도 2.4km/h로 직선보행동작과 $45^{\circ}$ 방향전환보행동작을 실시하게 한 후 보행 시 족저압력분포를 측정한 후 평균 족저압력과 최대 족저압력을 분석한 결과 다음과 같은 결론을 얻었다. 첫째, 12주간의 복합운동이 직선보행동작과 $45^{\circ}$ 방향전환 보행동작 시 평균 족저압력을 유의하게 감소시켜 효과적인 보행동작을 수행하는데 도움을 주는 것으로 분석되었다. 둘째, 12주간의 복합운동이 직선보행동작과 $45^{\circ}$ 방향전환 보행동작 시 최대 족저압력을 유의하게 감소시켜 효과적인 보행동작을 수행할 수 있도록 도움을 주는 것으로 분석되었다. 셋째, 12주 복합운동 전 후의 족저압력분포와 압력중심 이동곡선의 경로 분석결과 복합운동 전보다 복합운동 후의 족저압력분포가 보다 폭넓게 낮은 족저압력분포를 나타낸 것으로 관찰되었으며, 압력중심 이동곡선도 복합운동 전보다 복합운동 후 안정적인 곡선을 나타내는 것으로 분석되었다.

사지마비를 주소로 하는 길랑바레 증후군 환자에 대한 증례보고 1례 (A Case Report of a Patient with Guillain-Barre Syndrome Who Complained of Quadriplegia)

  • Purpose: The aim of this study was to report the improvement of Guillain-Barre syndrome after long-term combination treatment with Korean medicine. Methods: A patient was diagnosed with Guillain-Barre syndrome and treated with herbal medicine, acupuncture, pharmacopuncture, moxibustion, and exercise, including quadruped walking after three hospital admissions. To evaluate muscle strength and weakness, we measured manual muscle function, gait pattern, and the speed of quadruped walking. Results: The patient's muscle weakness in the extremities and gait stance were improved. The speed of quadruped walking was increased. Conclusion: We consider that combined treatment with Korean medicine might be effective for the muscle weakness of Guillain-Barre syndrome with a poor prognostic factor. To verify the effectiveness of this treatment, further research is needed.

Transcallosal Ipsilateral Motor Pathway from the Unaffected Hemisphere in a Patient with Traumatic Brain Injury

  • Purpose: We presented with a patient with traumatic brain injury who showed a transcallosal ipsilateral motor pathway from unaffected motor cortex to affected extremities, as evaluated by diffusion tensor tractography (DTT). Methods: One patient and six age-matched normal subjects were enrolled. A 42-year-old left hemiparetic male who suffered from brain injury by motor vehicle accident 9 years ago showed a leukomalactic lesion in the right corona radiata and parieto-temporal lobe. His left extremities were completely paralyzed initially, but recovered slowly over 2 years. At the time of the evaluation, he was able to grasp and release an object, and to walk with spastic gait pattern. DTT was performed using 1.5 T with a Synergy-L Sensitivity Encoding head coil. DTT was obtained with termination criteria of FA <0.2 and an angle change > $45^{\circ}c$. Results: The motor tracts of the unaffected (left) hemisphere of the patient and control subjects originated from the motor cortex and descended along the known corticospinal tract without any transcallosal tract. By contrast, the tract of the affected (right) hemisphere originated from the left premotor cortex, descended through the left corona radiata, and then crossed the mid-portion of the corpus callosum. The tract then descended through the known corticospinal tract pathway to the right medulla. Conclusion: We conclude that the transcallosal ipsilateral motor pathway from the unaffected hemisphere appeared to contribute to the motor recovery in this patient.

정상족과 편평족의 Arch Support 사용에 따른 운동역학적 분석 (Biomechanical Analysis of Arch Support Devices on Normal and Low Arch)

  • The purpose of this study was to the kinetic variables effects from the use of arch support inserts on low-arched people. We selected 10 people for the research and separated them into 2 groups, 5 people for the normal arched group and 5 people for the low arched group. Each group wear shoes which have a 3 step convertible arch support (level 0, level 2, level 5) and we measured their foot pressure and 3D motion analysis data. As a result, we found that the mean pressure at the heel of the low arched group was decreased when using the arch supports. The arch support induced the correct grounding area for the foot and dispersion of foot pressure. 3D motion analysis found that as the height of the arch support was increased, the movement of the Y-axis(inversion-eversion) was increased to relieve the shock to the heel. The arch support insert limited the range of motion(ROM) of the Z-axis(abduction-adduction) of the low arched person's ankle joint and prevented ankle injury caused by the excessive eversion when walking. Low arched people are seen to be easily tired due to the ineffective shock absorption of the knees and abnormal walking motion. In order to improve the problems, a 3 step convertible arch support(level 5) insert would improve the low-arched people's walking ability. In other words, the low arched people should be expected to walk as well as normal arched people when they wear shoes with the arch support insert.
