무릎외반의 균형 검사 및 보행 중에 근활성도와 발바닥압의 평가 (Evaluation of Muscle Activity and Foot Pressure during Gait, and Balance Test in Patients with Genu Valgum)

  • PURPOSE: This study sought to evaluate muscle activity and foot pressure during gait, and balance in female college students with genu valgum. METHOD: Participants were assigned based on their Q-angle to genu valgum group greater than 20° (GVG, n = 12), unilateral genu valgum group greater than 20° (UVG, n = 11), and control group (CON, n = 13). All subjects were evaluated for balance (Trace length, C90 area, C90 angle, and the Romberg test), muscle activity (gluteus medius; GM, tensor fasciae latae; TFL, vastus medialis; VM, vastus lateralis; VL, biceps femoris; BF, gastrocnemius; GCM and tibialis anterior; TA) and foot pressure (F/F ratio, R/F ratio, Hallux, 2~5 toe, 1st MT, 2~4 MT, 5th MT, Midfoot, M/heel, and L/heel) during gait. RESULTS: Romberg test showed significantly increased loss of balance in the UVG group compared with the CON. In the forward position, the imbalance was significantly increased in the UVG and GVG groups compared to the CON. Muscle activity of VL, GCM, and TA significantly increased in the GVG group compared with the CON. Static foot pressure, 1st MT significantly increased in the GVG compared to the CON group. The 5th MT significantly decreased in the CON compared with the GVG group. The R/F ratio significantly decreased in the GVG compared to the CON group. In dynamic foot pressure, the 2~5 toe significantly increased in the GVG compared with the UVG group. The left 5th MT significantly decreased in the UVG compared with the CON and GVG groups. CONCLUSION: These results indicate that genu valgum has a negative effect on balance, muscle activity, and foot pressure during gait in female college students.

뇌성마비 아동의 대동작 기능에 대한 가정중심치료 효과 : 체계적 고찰 (Effectiveness of home-based therapy on gross motor function in children with cerebral palsy: A systematic review)

  • Background: Although children with cerebral palsy (CP) are able to walk independently, gait imbalance occurs due to abnormal muscle tone, musculoskeletal deformity, loss of balance, and selective motor control impairment. Gait restriction in the community and school is a major problem of rehabilitation in CP. Home-based therapy (HBT) provides a variety of interventions in which the therapist and the parent work together to resolve the activities and problems caused by the child's body structure. Therefore, we investigate the effectiveness of home-centered therapy on gross motor function in CP and try to present the possibility of clinical application. Design: A Systematic Review Methods: Research papers were published from Jan, 2012 to Jan, 2022 and were searched using Medline and PubMed. The search terms are 'family-centered' OR 'home-based' AND 'cerebral palsy'. A total of nine papers were analyzed in this study. The paper presented the quality level based on Physiotherapy Evidence Database (PEDro) scores to assess the quality of randomized clinical trials studies. Results: The results showed that HBT for strengthening exercise in lower extremity has a positive effect on the isokinetic torque and gross motor function. home-based treadmill therapy in CP is effective to perform at least 12 sessions of treadmill HBP in which the therapist determines the treadmill speed every week and the child's own gait pattern is modified. Conclusion: These results suggest that it will be important data for founding evidence on the effectiveness of home-centered therapy on gross motor function in children with cerebral palsy to advance clinical protocols.

하이힐을 자주 착용하는 20대 여성의 허리, 무릎, 발목 및 발가락 통증, 다리 길이 그리고 족저압과의 상관분석 (An Analysis of the Correlation between High Heels and Pain in the Low Back, Knee, Ankle and Toe, Length of Legs, and Plantar Pressure among Women in Their Twenties.)

  • Background: The purpose of this study is to examine the correlation between high heels and body imbalance among female college students in their twenties who mainly wear high heels and prevent associated problems. Methods: The subjects included 89 female college students in their twenties. They were measured in plantar pressure with a gait analyzer. Their legs were measured in length with a tape measure. Their pain intensity and pain frequency were measured in visual analog scale and in pain rating score. Results: There were statistically positive correlations between right leg length and low back pain frequency (p<.05) and negative correlations between the left hindfoot and low back pain frequency (p<.05). There were statistically positive correlations between right leg length and knee pain frequency (p<.05) and positive correlations between the ankle pain intensity and right leg length (p<.05). Conclusion: The stronger the pain was in the ankle, the stronger and more frequent their lumber pain was. When the pressure of the left heel was lower, the frequency of lumbar pain increased.

Effects of Self Exercise Program on Leg Length and Balance in Subjects with Leg-Length Discrepancy

  • Background: If there is a difference in leg length, the center of gravity shifts unilaterally toward the short leg, causing loss of balance and secondary postural imbalance, trunk muscle tone changes, gait abnormalities and pelvic imbalance. Objectives: To investigate effects of self exercise program on leg length, balance in adults with leg-length discrepancy. Design: Single blind randomized controlled trial. Methods: Twenty-eight participants were selected and divided into resistance exercise, flexibility exercise, and core exercise. Each exercise was performed for 40 minutes, 3 times a week for 6 weeks. Leg length and balance before and after exercise were measured and analyzed. Results: Following the interventions, resistance exercise group showed significant improvement in balance, but leg length difference did not show significant results. Flexibility exercise group showed significant improvement in leg length difference, but balance did not show significant results. Core exercise group showed significant improvement in leg length difference and balance. There was no significant difference in the comparison between the three groups. Conclusion: This study suggests that customized exercise according to the patient's level is beneficial to the patients.

A Case Report on the Immediate Effects of Cytoskeletal Manual Therapy on Pain, Muscle Thickness, and Pressure Pain Threshold in a Patient with Scoliosis

  • Objective: Patients with scoliosis complain of various symptoms such as muscle imbalance, dysfunction, back pain, abnormal posture and gait abnormality. The most basic treatment for scoliosis is to observe the progress based on conservative treatment. Therefore, in this case report, the effect of cytoskeletal manual therapy (CMT), a soft tissue mobilization technique, on pain intensity, muscle thickness, and pressure pain threshold (PPT) in a patient with scoliosis was investigated. Design: A case report Methods: A 25-year-old male diagnosed with scoliosis visited the Neuromusculoskeletal Science Laboratory with chronic back pain. In the laboratory, scoliosis was confirmed through the X-ray image used for his diagnosis, and it was confirmed again through Adam's forward bending test. Pain, pressure pain threshold and muscle thickness were measured to compare the immediate effects of CMT applied in the laboratory for 40 minutes. Treatments were visited two weeks after the first visit and outcome measures were assessed after a total of two visits. Results: After receiving CMT up to the second session, the pain intensity decreased by 4 points and the screening angle decreased by 15 degrees. Muscle thickness decreased in all but 10 mm on the dominant side of the thoracic spine. All of the PPTs increased, and the greatest increase was 3.1 lb on the dominant side of the thoracic spine. Conclusions: CMT showed positive improvement in pain during trunk flexion, spinal curvature, muscle imbalance, and pressure pain, which is considered as an ancillary treatment option for scoliosis management.

압력 및 가속도 센서를 활용한 운동 자세 교정 시스템 (Exercise Posture Calibration System using Pressure and Acceleration Sensors )

  • 현대인들의 운동, 건강 관심도가 늘어남에 따라 운동에 관련된 정보 및 기기들의 수요가 늘어나고 있으며 잘못된 자세로 운동할 시 신체 불균형과 부상을 초래할 수 있다. 이에 본 연구에서 사용자들의 올바른 운동 자세를 통한 건강증진 및 부상 예방을 위한 자세교정을 목적으로 한다. 시스템의 주기억 장치로는 Arduino Uno R3와 압력 센서, 가속도 센서를 사용하여 개발하였다. 압력 센서는 스쿼트 자세 판별, 가속도 센서는 일반걸음, 팔자걸음, 안짱걸음 3가지의 걸음걸이 판별을 위해 사용되었다. 데이터를 블루투스 모듈로 스마트폰에 전송하고 App에 표시하여 사용자에게 올바른 운동 자세를 안내해준다. 걸음걸이 판별은 발이 벌어진 각도 20˚를 기준으로 진행하였으며, 올바른 스쿼트 자세는 숙련자의 데이터를 기반으로 전족과 후족의 압력 센서 값의 비율을 비교했다. 따라서 걸음걸이 판별 시 약 90%의 정확도와, 스쿼트 자세 시 압력 센서 값의 비율 7:3을 기준 하에 95%의 정확도를 가지는 실험을 기반으로 사용자가 운동 시 App을 통해 실시간으로 확인하여 올바른 자세로 운동을 할 수 있고, 잘못된 자세로 운동을 진행할 때 교정할 수 있도록 안내해주는 시스템을 구축했다.

Vascularized Osteocutaneous Fibular free Flap for Reconstruction of Mid Foot

  • The foot plays a vital role in standing and gait. This function results from harmonious interaction of bones, joints, and soft tissue. An imbalance or a defect in such structures can lead to impaired function of the foot. The mid foot, composed of cunieforms, navicular and cuboid bone, plays a vital role in maintaining longitudinal and transverse arches and injury or defects to this region can cause instability of the foot. This paper reports a case of complex foot injury; soft tissue defect of dorsum of foot, and medial and intermediate cuneiform bone defect, reconstructed in a single stage using vascularized osteocutaneous fibular free flap. Segmented to fit the defects of medial and intermediate cuneiform bones and a skin paddle providing adequate coverage, restored the stability to the arches and function of the midfoot. The fibula osteocutaneous free flap has appealing characteristics for reconstruction of the foot and the complex mid foot injuries can be considered to the long list of indications.

보행 시 부하의 위치에 따른 발의 안정성 및 운동학적 분석에 관한 연구 (The Studies on the Foot Stability and Kinesiology by Direction of Carry a Load during Gait)

  • Purpose: This study examined how the direction of carrying a load affects the foot stability and kinesiology while walking. Methods: The heel rotation, Hallux stiffness, foot balance, metatarsal load, toe out angle, subtalar joint flexibility were measured in 40 adults (men and women) who carried a load back and forth, walking on a 2-meter-long board. The measurement was carried out three times and the mean of the measurements was used to compare the difference between the front, back and the condition without a load. Results: While walking, heel rotation and hallux stiffness occurred most when a front load was applied compared to a back load or no load condition (p<0.05). A metatarsal load also appeared to be the highest with the frond load, but there was no significant difference in the balance of the whole foot. Both the toe out angle and subtalar joint flexibility appeared to increase significantly (p<0.05). Conclusion: Applying the front load causes subtalar joint instability and increases the plantar foot pressure imbalance during walking.

Effects of Korean Medicine Treatment on a Patient with Functional Scoliosis with an Abnormal Gait Pattern After Hip Injury: A Case Report

  • Scoliosis is a 3-dimensional spinal deformity defined as lateral curvature of the spine in the coronal plane of more than $10^{\circ}$. This study describes a case of functional scoliosis after hip injury. In this case, the patient fell whilst inline skating (June 2015) causing severe tilting of her spine, and left hip pain. She received outpatient treatment from July 14, 2015 to December 28, 2015. For approximately 5 months, acupuncture therapy was performed to relax the tension in both hips, Chuna therapy, and foot orthosis were applied to reduce the body's imbalance. Based on X-ray images, the Cobb angle had decreased from $14.73^{\circ}$ (pretreatment) to $1.90^{\circ}$ (posttreatment). This case report suggested that Korean medicine treatment could be an effective therapeutic choice for functional scoliosis.

Application and Effectiveness of a Program to Promote Adolescent Musculoskeletal Health: A Pilot Study

  • Purpose: Adolescence is a developmental period characterized by the rapid growth of the musculoskeletal system, which is important for maintaining correct posture. Incorrect posture, lack of exercise, and reduced physical activity can cause spine deformities and affect lifelong health. This study was designed to evaluate the application and effect of a program for improving adolescents' musculoskeletal health. Methods: A quasi-experimental pilot study was conducted with 13 male and 20 female middle and high school students, with an average age of 15.39 years. Their general characteristics and physical measurements were obtained. The program consisted of group exercises (60 minutes, once per week), gait pattern monitoring, and online communication. A paired t-test and Wilcoxon signed-rank test were used to examine the program's effect. Results: Overall posture habits improved, and the total musculoskeletal index decreased; however, these results were not statistically significant. Conclusions: The devised program was effective in improving musculoskeletal imbalance. Therefore, effective programs and health devices should be developed to help adolescents maintain correct posture and encourage and support continuous participation in such programs.
