The gag reflex is a physiologic reaction which safeguards the airway from foreign bodies. But, an exaggerated gag reflex can be a severe limitation to a patient's ability to accept dental care and for a clinician's ability to provide it. The overactive gag reflex can be due to psychological factors or physiological factors, or both. Psychological factors can include fear of loss of control and past traumatic experiences. A 58-year-old man, scheduled for extraction of left upper second molar, left lower second and third molar and implantation of left upper second molar, and left lower second molar had no specific underlying medical problems. He had exaggerated gag reflex. Dental treatment was successfully performed using intravenous sedation. Intravenous sedation with midazolam and propofol was a useful management technique for reflex control during dental treatment extended to the posterior regions in the oral cavity.

Procedures such as surface anesthesia of the palate and pharyngeral area, sedation, or general anesthesia can be options as behavioral management. But, there are no golden rule for the sever gag reflex patients. We present a case report of propofol intravenous sedation using TCI pump for simple dental treatment. A 44-year-old man, who had past history of general anesthesia for dental treatment because of severe gag reflex, was scheduled intravenous sedation for simple dental treatment. After 8 hour fasting he entered the clinic for persons with disabilities. We explained about intravenous deep sedation and got informed consent. First, we kept intravenous catheter (22G) in the arm and started monitoring ECG, non-invasive blood pressure, pulse oximetry and end-tidal $CO_2$ through nasal cannula. We started propofol infusion with TCI pump at the target concentration of 3 mcg/ml. The patient became sedated, but he showed involuntary movement during dental treatment, so we increased the target concentration to 4 mcg/ml. We finished the dental treatment without complications during 30 min. And after 40 min recovery room stay he was discharged without any complications.
Purpose: Our aim in this study is to investigate efficacy of topical lidocaine spray for sedated esophagogastroduodenoscopy (EGD) in children. Methods: The endoscopy of children aged between 3-18 years who underwent EGD in our endoscopy unit. Intravenous (IV) midazolam and ketamine were used for sedation. Prior to sedation, endoscopy nurse applied topical lidocaine 10% with pump spray at 1 mg/kg dose in group 1, and distilled water via identically scaled pump spray in group 2, in a double blinded fashion. Results: Sedation was not applied in 24.1% of the cases in topical lidocaine spray group (LS group) and in 5.7% of the cases in distilled water spray group (DS group). Gag reflex was observed in 6.5% of cases in LS group and 33.3% of cases in DS group (p=0.024), increased oral secretion was observed in 9.3% of cases in LS group and 51.7% of cases in DS group (p=0.038), sore throat was observed in 3.7% of cases in LS group and 35.6% of cases in DS group (p=0.019) and the difference was statistically significant. Conclusion: The study showed that topical pharyngeal lidocaine reduces both requirement and amount of IV sedation before EGD in children and sore throat, gag reflex and decreased oral secretion increase.
구역반사는 필수적인 기능이며 유용한 생리적 방어기전이지만 심한 구역 반사는 보철 치료의 진행을 어렵게 한다. 본 증례는 구역반사를 일으키는 요인을 파악하고 이를 해결하기 하여 문진표와 환자와의 상담을 통해 환자의 구역반사를 일으키는 요인을 객관화하였다. 이에 따른 구강 내 치과용 기구를 위치시킴으로써 적응하는 체계적 탈감작인 행동조절 및 음주 후 구토습관을 없애는 습관조절 등 다양한 해소방안을 계획하였다. 전신마취하 임플란트 고정체 식립을 제외하고 모든 수복 절차는 통상적인 임플란트 고정성 보철 수복과 동일하게 외래에서 진행하였다. 이러한 과정을 통해 구역반사에 의한 불편감이 감소되었고, 환자의 부분 무치악 부위에 임플란트 고정성 보철 수복을 성공적으로 수행하였기에 이를 보고하는 바이다.
Patients with severe gag reflex (SGR) have difficulty getting the treatment they require in local clinics, and many tend to postpone the start of their treatment. To address this problem, dentists have used behavioral techniques and/or pharmacological techniques for treatment. Among the pharmacological methods available, propofol IV sedation is preferred over general anesthesia because it is a simpler procedure. Propofol in combination with remifentanil is characterized by stable sedative effects and quick recovery, leading to a deep sedation. Remifentanil acts to reduce the pain caused by lipid-soluble propofol on injection. The synergistic effects of propofol-remifentanil include reduction in the total amount of drug required to achieve a desired sedation level and anti-emetic effects. In this case report, we outline how the use of propofol-remifentanil IV sedation enabled us to successfully complete a wide range of dental treatments in a patient with SGR.
다발성 안면 골절의 경우 골절 부위를 원위치로 정복하기 위한 지표가 부족하기에 골절편의 정복 후 교합과 구강 구조의 형태 및 위치가 비정상적으로 변화된 결과를 보이는 경우가 많이 나타난다. 그리고 다발성 안면 골절환자에서 나타난 개구제한과 구역반사는 치과치료의 난이도를 높이고, 외상으로 인한 악골의 위치차이가 심미적, 기능적 회복을 어렵게 한다. 이러한 경우 환자의 불편감과 치과치료의 난이도를 줄이기 위해 최소한의 구내작업을 위한 디지털 워크플로우를 선택할 수 있다. 본 증례는 15년전 발생한 다발성 안면골절로 인한 후천적인 골격성 III급 및 개구제한, 구역반사를 가진 환자에서 상악 전치부 무치악 상태를 임플란트 계획부터 최종 보철물의 제작까지 디지털 워크플로우를 이용하여 수복한 증례이다. 이러한 치과치료의 어려움이 있는 환자에서 디지털 워크플로우의 사용이 환자의 불편감을 최소로 하고 심미적, 기능적으로 적절한 결과를 얻었기에 이를 보고하는 바이다.
이상에서 심한 Gag Reflex를 가친 76명(名)의 환자와 경우를 보았다. 자극의 주된 요소는 상악의치였고, 병력 및 임상조사를 했으며, 두개 측면방사선사진 촬영도 했다. Eysenck Personalty Inuentory도 실시했다. 치료시 레진 training base를 썼고, 기타 Relaxation therapy 및 Heterohypnotic technic을 썼다. 이 결과, (1) 대조군과 비교시 특징적인 유형은 없었다. (2) 몇 환자는 동기유발이 불충분해 치료를 중도에 포기했다. (3) 약간의 환자에서 성공적으로 치료가 되였으나 재발했다.
PURPOSE. To assess removable prosthetic restoration tolerance according to the patient section of the short form of the Gagging Problem Assessment Questionnaire (GPA-pa SF) and the influence of gender, education level and prosthesis type and denture-related mucosal irritation on the GPA-pa SF scores before treatment and over a period of two months after prosthesis insertion. MATERIALS AND METHODS. 130 participants who required removable prosthesis were surveyed with a standard form that included questions regarding age, gender, education level, dental attendance, and prosthetic restoration type. Participants answered the GPA-pa SF before restoration (T0) and 1 day (T1), 2 days (T2), 15 days (T3), 1 month (T4), and 2 months (T5) after prosthesis insertion. RESULTS. Of the 130 participants, 110 participants completed the prosthetic restoration procedure, but only 93 of these were able to use the prosthesis over the two-month period. The mean GPA-pa SF score obtained at T0 was higher than the scores obtained at the other periods in the total of the sample. Significant difference was present between mean scores obtained at T0-T1 and T2-T3 than scores obtained at other periods (P<.05). Female participants and participants with denture-related mucosal irritation had higher GPA-pa SF scores at all time points analysed. Significant difference was present between mean GPA-pa SF scores obtained at T2-T3 than scores obtained at other periods for females and participants with denture-related mucosal irritation (P<.05). Education level and prosthesis type did not significantly influence the GPA-pa SF score at any time point analysed (P>.05). CONCLUSION. GPA-pa SF scores were higher before the restoration procedure began, and decreased over time with the use of prosthesis. Gender and denture-related mucosal irritation affected the GPA-pa SF scores.
A 12-year-old spayed female Yorkshire terrier dog was presented with two-weeks history of lethargy, mental dullness and polydipsia. Neurologic examination revealed proprioceptive defect of all limbs and loss of swallowing gag reflex. The dog revealed persistent dehydration, hypernatremia, hyperosmolarity and hyposthenuria. On magnetic resornance imaging (MRI), the mass were heterogeneous signality on T1 weighted images, hyperintense signality on T2 weighted image with contrast enhancement on hypothalamohypophyseal lesion. Based on these findings, the dog was suspected as having pituitary gland tumor. Through water deprivation test and response to desmopressin acetate (1-deamino-8-D-arginine, DDAVP), this case was diagnosed by central diabetes insipidus (CDI). This paper reports the clinical sign, MRI, response to the exogenous antidiuretic hormone of CDI due to suspected pituitary tumor in a dog and DDAVP administration was evaluated effective therapy to correct hypernatremia induced by CDI.
It is difficult to take intraoral radiographs in some patients who are intolerable to place the film in their mouth. For these patients, Newman and Friedman recommended a new technique of extraoral film placement. Here we report various cases that diagnostic imaging was performed in patients using the extraoral periapical technique. This technique was used to obtain the radiographs for the patients with severe gag reflex, pediatric dental patients, and patients with restricted mouth opening. This technique can be recommended as an alternative to conventional intraoral periapical technique in cases where intraoral film placement is difficult to achieve.
