Effectiveness of acupuncture as an adjunctive therapy for lung cancer: A systematic review and meta-analysis

  • Objectives : Lung cancer is one of the most common cancer and the leading cause of cancer deaths worldwide. This study aimed to evaluate the role of acupuncture as an adjunctive therapy for lung cancer. Methods : We conducted a systematic review and meta-analysis on the role of acupuncture therapy in lung cancer treatment by electronic and manual searching in ninedatabases, including PubMed, Cochrane library, Embase, Korean databases, and Chinese medical databases. Results : A total of 21 trials were included in the meta-analysis. The study results showed that acupuncture therapy had significant efficacy in immuneregulation, including CD3 andCD4. Further analysis revealed that acupuncture therapy significant improvements in quality of life, including Karnofsky performance status(KPS)score, functional assessment of cancer therapy-lung cancer subscale (FACT-L) and European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30). In addition, the pooled studies also showed that acupuncture therapy reduced cancer pain and chemotherapy-induced nausea and vomiting. Conclusions : Our study provides moderate evidence of the efficacy of the acupuncture therapy in the treatment of lung cancer.

Evaluation of Clinical Use by Comparative Efficacy and Safety Study of Fine Granvles for Upper Gastrointestinal Disorders Including Symptoms (상부 소화관 질환증상에 대한 제산.소화효소제와 생약제를 함유한 복합제제 세립의 유용성)

  • The purpose of this study was to evaluate the clinical efficacy and safety of NRM which was newly developed as a combination product containing antacids, digestive enzyme and herbal drugs, and OTA powder, called OHTA'S ISAN on the morret, for the treatment of various symptoms in upper gastrointestinal diseases. 63 patients were recruited, 36 as a treated group with NRM and 27 as a controlled one with OTA. Patients were randomly allocated to receive either NRM or OTA t.i.d. for treatment of upper G-1 symptoms and undergone endoscopic, symptomatic and clinical laboratory assessments before and after 2 weeks. The results were as follows; 1. The general improvement rates of subjective symptoms in upper G-1 diseases were observed in 100%(32/32) for NRM and in 92%(23/25) for OTA. 2. The overall effective rates in terms of the symptoms and endoscopic findings were 88.9%(32/36) for NRM and 85.2%(23/27) ,or OTA respectively. 3. In NRM group, no significant side effects by NRM were observed except a mild and transient vomiting in one patient, but in OTA group treatment was discontinued due to nousea in one patient, However, there were no clinically significant changes detected in the laboratory parameters. According to the result of this trial, it is cocluded that NRM was safe and effective and (but not statistically significant) was superior to OTA in the treatment of gastrointestinal diseases caused by gastritis, gastric and duodenal ulcer, functional gastrointestinal disorder, non-ulcer dyspepsia etc.

A case study of third-generation quinolone antibiotics-induced-dyspepsia treated with Beewha-eum and acupuncture (비화음 및 침치료로 호전된 3세대 퀴놀론계 항생제 유발 소화장애 치험례)

  • Objectives: The purpose of this case study is to report the clinical effects of acupuncture and Korean medicine on antibiotics-induced functional dyspepsia. Methods: The patient, 79-year-old, who complained about nausea and vomiting after taking antibiotics was treated by acupuncture and Korean medicine. We measured the amount of meals eaten by patient and quantified the degree of nausea she felt. Results: After the treatment, the symptoms were decreased. Conclusion: Acupuncture and herb medicine may be effective therapy on antibiotic's side effects such as nausea and vomit.

  • The purpose of this study was to develop systematic diagnostic system testing easily, rapidly vestibular function of patients suffered from vestibular syndrome such as nausea vomiting, dizzness, ataxia. Diagnostic system composed of rotatory chair system which rotated sinusoidally patients against their vertical axis for purpose of invoking eye movement by vestibulo-ocular reflex and the softwares which storaged eye movement into computer and analyzed eye movement. Rotatory chair system consisted of comfortable chair and DC servomotor with reducer(1:80) by controlled servo in field of nonlinear motor control, double feedback loops system containing velocity feedback loop and position feedback loop was applied to this sever controlled rotatory chair system. Maximum rotatory velocity of rotatory chair was upto 60 degree per second and frequency range was 0.01 to 0.64 Hz. These above results suggest that clinical rotatory chair system may test easily, rapidly vestibular function and diagnose etiology of dizziness, thus giving effective assistance on the treatment of dizziness patients.

The Improvement Effect of MMSC (DL-Methionine Methylsulfonium Chloride) in Functional Dyspepsia Animal Models (동물모델을 이용한 MMSC(DL-Methionine Methylsulfonium Chloride)의 기능성소화불량증 개선효과)

  • The objective of this study is to investigate the gastric emptying and gastrointestinal transit improvement effect of DL-methionine methylsulfonium chloride (MMSC) in functional dyspepsia animal models. Cisplatin causes nausea, vomiting, and inhibition of gastric emptying. Rats were divided into four groups: G1 (normal group), G2 (gastric emptying induced by cisplatin), G3 (gastric emptying induced by cisplatin with itopride 30 mg/kg pretreatment), and G4 (gastric emptying induced by cisplatin with MMSC 4 mg/kg pretreatment). Immediately after an oral administration of a liquid meal (phenol red), delayed gastric emptying was induced by cisplatin (10 mg/kg (i.p.)). After 20 min in the cisplatin administration, the animals were sacrificed. In rats treated with cisplatin, the gastric emptying rate was significantly reduced. On the other hand, MMSC reversed the reduction of gastric emptying induced by cisplatin. And also, MMSC caused to travel FITC-dextran more significantly longer distance than the control, which is based on the values of the mean geometric center in the atropine driven delayed gastrointestinal transit animal models. Furthermore, MMSC drastically increased the gastrointestinal transit in rats, considerably increased the values of the mean geometric center (MGC), compared to the control, which was comparable to that of mosapride. These results suggest that MMSC could be an effective component for the treatment of functional dyspepsia.

Quality of Life in Malaysian Colorectal Cancer Patients : A Preliminary Result

  • Objective: Rapidly increasing colorectal cancer (CRC) incidence in Malaysia and the introduction of cutting edge new treatments, which prolong survival, mean that treatment outcome measures meed to be evaluated, including consideration of patient's quality of life (QoL) assessment. There are limited data on QoL in CRC patients, especially in Malaysia. Therefore, this study was performed focusing on cancer stages and age groups. Methods: The cross sectional study was conducted from June to September 2011 at three public tertiary hospitals with the EORTC QLQ C-30 questionnaire in addition to face to face interview and review of medical records of 100 respondents. Results: The mean age was 57.3 (SD 11.9) years with 56.0% are males and 44.0% females, 62% of Malay ethnicity, 30% Chinese, 7% Indian and 1% Sikh. Majority were educated up to secondary level (42%) and 90% respondents had CRC stages III and IV. Mean global health status (GHS) score was 79.1 (SD 21.4). Mean scores for functional status (physical, emotional, role, cognitive, social) rangeds between 79.5 (SD 26.6) to 92.2 (SD 13.7). Mean symptom scores (fatigue, pain, nausea/vomiting, constipation, diarrhea, insomnia, dyspnoea, loss of appetite) ranged between 4.00 (SD 8.58) to 20.7 (SD 30.6). Respondents role function significantly deteriorates with increasing stage of the disease (p=0.044). Females had worse symptoms of pain (p=0.022), fatigue (p=0.031) and dyspnoea (p=0.031). Mean insomnia (p=0.006) and diarrhea (p=0.024) demonstrated significant differences between age groups. Conclusion: QOL in CRC patients in this study was comparable to that in other studies done in developed countries. Pain, fatigue and dyspnoea are worse among female CRC patients. Given that functions deteriorates with advanced stage of the disease at diagnosis, a systematic screening programme to detect cases as early as possible is essential nationwide.

  • According to Tokyo protocol which suggests the prevention of global environmental pollution, Korean government establishes the standard of architectural materials emission consistency with best effort to decrease the environmental pollution. But many current architectural materials which are used for constructing and remodeling buildings are composed of a variety of chemicals. These include stimuli bad for the residents' health and safety and harmful discharged air polluting substances such as volatile organic compounds(TVOCs) and formaldehyde(HCHO) that in tern include a variety of carcinogen substances. These discharged substances are also researched into inducing 'sick building syndrome' which induces headache, dizziness, vomiting and concentration failure among residents. But the standard of architectural materials according to the Korean apartment provision is limited to emission factors: HCHO and TVOCs. So the aim of this study is to present a standard of functional material's emission consistency about TVOCs including glues and paints, and a certification grade for green building by instituting a materials standard for green building which has consideration for the residents' safety.

Experience with Conventional Radiofrequency Thermorhizotomy in Patients with Failed Medical Management for Trigeminal Neuralgia

  • Background: To evaluate the results of conventional radiofrequency thermorhizotomy (CRT) for trigeminal neuralgia (TN) in patients with failed medical management. Methods: Patients with Trigeminal neuralgia who were referred to us for 'limited intervention' during the time frame July-2011 to Jan-2013 were enrolled for this study. CRT was administered by the Sweet technique. Pain relief was evaluated by the principle investigator. Results: Eighteen patients were enrolled and completed a mean follow-up of 18.0 months. Pain relief was observed in 14 of 18 (77.8%) patients on the post-operative day, 14 of 18 (77.8%) at 1-month follow-up, 14 of 17 (82.4%) at 3-months follow-up, 12 of 15 (80%) at 6-months follow-up, 7 of 11 (63.6%) at 1-year follow-up and 2 of 6 (33.3%) 1.5 years of follow-up. Four patients required a repeat cycle of CRT; two at six months of follow-up and two at one year of follow-up. One patient was transferred for surgical intervention at six months of follow-up. Side-effects included facial hypoesthesia (n = 6); nausea/vomiting (n = 2), diminished corneal reflex (n = 13) and difficulty in chewing (n = 11). Severity of adverse effects gradually diminished and none of the patients who are beyond 6 months of follow-up have any functional limitation. Conclusions: CRT is an effective method of pain relief for patients with Trigeminal neuralgia. Successful outcome (excellent or good) can be expected in 66.7% of patients after first cycle of CRF. The incidence and severity of adverse effects is less and the procedure is better tolerated by the patients.

The Changes of Quality of Life in Cancer Patients Treated with Chemotherapy (화학치료를 받는 암 환자 '삶의 질'의 변화에 관한 연구)

  • Purpose : It is very important to endow the cancer patients underwent chemotherapy with satisfactory quality of life (QOL). However, little is known about the factors influencing QOL during chemotherapy. Therefore, we designed this study to find out the factors influencing QOL in the cancer patients who underwent chemotherapy. Methods : Ninety-seven cancer patients were studied, prospectively. The patients' characteristics were as follows; median age(range): 48(19{\sim}83) years, male:female; 57:40, PS:0,1/2,3;55/42 patients, diagnosis(number): lymphoma (28), lung cancer (22), gastrointestinal cancer (18), sarcoma (12), breast cancer (12), gynecological cancer (5), Stage: I,II/III.IV;37/60 patients. We used EORTC QLQ-C30 questionnaires to evaluate QOL. EORTC QLQ-C30 scores were performed before the onset of chemotherapy and after the end of 3 cycles of chemotherapy. The correlation of these scores with performance status (PS), diagnosis, disease stage, response to chemotherapy, and regimen related toxicity was evaluated. Results : The responder group (CR, PR) demonstrated marked improvement of social functional and emotional scales to non-responder group (SD,PD) (P=0.024, 0.045). Non-hematologic regimen related toxicity such as mucositis, nausea and vomiting was significantly correlated with pain scale change (P=0.043). Other factors had no notable correlation with QOL changes. Conclusion : Our preliminary study results may suggest as follows. The response to chemotherapy is associated with the change of social functional and emotional scales and the severity of non-hematologic regimen related toxicity is associated with pain scale change.

Comparison of the Clinical Outcomes of a Single Injection Adductor Canal Block with the Concomitant Use of Transdermal Buprenorphine and Continuous Adductor Canal Block after Total Knee Arthroplasty (슬관절 인공관절 전치환술 후 일회 주사 내전근관 차단술 및 경피성 Buprenorphine 병합 요법과 도관 삽입 지속적 내전근관 차단술의 임상결과 비교)

  • Purpose: To compare the clinical outcomes of single injection adductor canal block (SACB), continuous adductor canal block (CACB), and the concomitant use of transdermal buprenorphine after total knee arthroplasty (TKA). Materials and Methods: A total of 125 patients who underwent TKA were divided into three groups and the clinical results were retrospecitively compared. Group I was comprised of patients with pain controlled by SACB (n=41). Group II consisted of patients with pain controlled by both SACB and transdermal buprenorphine (10 ㎍/h) (n=44). Group III contained patients with pain controlled by CACB (n=40). The visual analogue scale (VAS) was used as the pain control indicator and the patients were measured on a VAS for resting on the bed (VAS-Rest) at 12 hours, 24 hours, and 48 hours after surgery. The VAS while doing continuous passive motion (VAS-CPM) on the first and second postoperative day was also measured. In addition, the total amount of medications used (Butopahn, Tridol, and Ketorac) for the intravenous patient controlled analgesia (PCA) was counted for 48 hours after surgery. As the indicator of the functional recovery outcome, the incidence of nausea and vomiting was observed for 48 hours after surgery. The maximum knee joint flexion range and maximum walking distance on the first and second postoperative day, and the total length of stay at the hospital were compared. Results: The VAS-Rest was similar in the three groups at 12 hours after surgery, but at 24 hours and 48 hours after surgery, group II and III a lower VAS-CPM and total amount of medications used for PCA than group I (p<0.05). The three groups showed a low incidence of nausea and vomiting, maximum knee joint flexion range, and similar walking distance and total length of stay at the hospital. Conclusion: The combination of SACB and transdermal buprenorphine has great pain control effect initially. On the other hand, it is not associated with catheter complications and it is convenient to use and safety toward the renal function. Therefore, the concomitant use of SACB and transdermal buprenorphine can be an effective pain control method after TKA.
