Comparative Analysis between Total Disc Replacement and Posterior Foraminotomy for Posterolateral Soft Disc Herniation with Unilateral Radiculopathy : Clinical and Biomechanical Results of a Minimum 5 Years Follow-up

  • Objective : To compare the clinical outcomes and biomechanical effects of total disc replacement (TDR) and posterior cervical foraminotomy (PCF) and to propose relative inclusion criteria. Methods : Thirty-five patients who underwent surgery between 2006 and 2008 were included. All patients had single-level disease and only radiculopathy. The overall sagittal balance and angle and height of a functional segmental unit (FSU; upper and lower vertebral body of the operative lesion) were assessed by preoperative and follow-up radiographs. C2-7 range of motion (ROM), FSU, and the adjacent segment were also checked. Results : The clinical outcome of TDR (group A) was tended to be superior to that of PCF (group B) without statistical significance. In the group A, preoperative and postoperative upper adjacent segment level motion values were $8.6{\pm}2.3$ and $8.4{\pm}2.0$, and lower level motion values were $8.4{\pm}2.2$ and $8.3{\pm}1.9$. Preoperative and postoperative FSU heights were $37.0{\pm}2.1$ and $37.1{\pm}1.8$. In the group B, upper level adjacent segment motion values were $8.1{\pm}2.6$ and $8.2{\pm}2.8$, and lower level motion values were $6.5{\pm}3.3$ and $6.3{\pm}3.1$. FSU heights were $37.1{\pm}2.0$ and $36.2{\pm}1.8$. The postoperative FSU motion and height changes were significant (p<0.05). The patient's satisfaction rates for surgery were 88.2% in group A and 88.8% in group B. Conclusion : TDR and PCF have favorable outcomes in patients with unilateral soft disc herniation. However, patients have different biomechanical backgrounds, so the patient's biomechanical characteristics and economic status should be understood and treated using the optimal procedure.

Breast Conserving Therapy and Quality of Life in Thai Females: a Mixed Methods Study

  • Background: To explore factors that influence quality of life (QOL) in patients receiving breast conserving therapy (BCT). Materials and Methods: In this sequential mixed methods study, 118 women from Songklanagarind Hospital were included. We used participants' characteristics, Body Image Scale (BIS), and Functional Assessment of Cancer Therapy with the Breast Cancer Subscale (FACT-B) for analysis. The BIS transformed into presence of body image disturbance (BID). Factors that influenced QOL were determined by stepwise multiple linear regression. Forty-one participants were selected for qualitative analysis. Our female researcher performed the semi-structured interviews with questions based on the symbolic interaction theory. Final codes were analysed using thematic analysis along with investigator triangulation methods. Results: Ninety percent had early stage breast cancer with post-completed BCT, for an average of 2.7 years. The median BIS score and FACT-B score were 2 (IQR=10) and 130 (IQR=39). In the regression analysis, an age of more than 50 years and BID were significant factors. As for the value of conserved breasts, two themes emerged: a conserved breast is an essential part of a participant's life and also the representation of her womanhood; the importance of a breast is related to age. Conclusions: Body image influenced QOL in post BCT participants. The conserved breasts also lead to positive and better impact on their body image as an essential part of their life.

  • Supracricoid partial laryngectomy (SCPL) with cricohyoidoepiglottopexy (CHEP) or cricohyoidopexy (CHP) involves the removal of the whole thyroid cartilage, both true and false vocal cords, the ventricles, and the paraglottic spaces, sparing the cricoid cartilage, hyoid bone, and at least one functional and mobile cricoarytenoid unit. Reconstruction is performed by suturing of the cricoid cartilage up tightly to the hyoid bone, so trachea-releasing procedures are needed to prevent leakage at anastomosis site. In case of advanced tranglottic cancer invading tracheal tracheal wall, we need to perform additional circumferentrial circumferential tracheal wall resection. However, when we perform SCPL, circumferential resection of tracheal wall is limited because SCPL procedure itself needs releasing of tracheal length. We report a case of advanced transglottic cancer involving tracheal wall treated with induction chemotherapy and SCPL including tracheal wall resection with reconstruction of tracheal defect by sternocleidomastoid muscle flap covered with skin graft.

Genotype-phenotype correlations in pediatric patients with myotonic dystrophy type 1

  • Purpose: Myotonic dystrophy, also known as dystrophia myotonica (DM), is an autosomal dominant disorder with 2 genetically distinct forms. DM type 1 (DM1) is the more common form and is caused by abnormal expansion of cytosine/thymine/guanine (CTG) repeats in the DM protein kinase (DMPK ) gene. Our study aimed to determine whether the age of onset is correlated with CTG repeat length in a population of pediatric patients with DM1. Methods: We retrospectively identified 30 pediatric patients with DM1 that underwent DMPK testing, of which the clinical data of 17 was sufficient. The cohort was divided into 2 subgroups based on the clinical phenotype (congenital-onset vs. late-onset) and number of CTG repeats (<1,000 vs. ${\geq}1,000$). Results: We found no significant difference between the age of onset and CTG repeat length in our pediatric patient population. Based on clinical subgrouping, we found that the congenital-onset subgroup was statistically different with respect to several variables, including prematurity, rate of admission to neonatal intensive care unit, need for respiratory support at birth, hypotonia, dysphagia, ventilator dependence, and functional status on last visit, compared to the late-onset subgroup. Based on genetic subgrouping, we found a single variable (poor feeding in neonate) that was significantly different in the large CTG subgroup than that in the small CTG subgroup. Conclusion: Clinical variables exhibiting statistically significant differences between the subgroups should be focused on prognosis and designing tailored management approaches for the patients; our findings will contribute to achieve this important goal for treating patients with DM1.

Effect of dietary supplementation of Sargassum meal on laying performance and egg quality of Leghorn layers

  • Objective: Seaweeds could be an alternative and functional feed resource. The purpose of this experiment is to investigate the effect of dietary supplementation of Sargassum meal on laying performance and egg quality of layers. Methods: Two hundred 36-wk-old layers were divided into five treatment groups. Each treatment had four replicates with 10 hens per experimental unit. The corn-soybean meal basal diet was formulated as control group. Sargassum meals were included 0%, 1%, 2%, 3%, or 5% to diets for five treatment groups, respectively. Treatment groups were isocaloric-isonitrogenous diets. Laying performance and egg quality were measured for eight weeks. Results: Sargassum meal supplementation did not affect daily feed intake. Supplementation 1% to 3% of Sargassum meal in diets increased daily laying rate and egg mass compared with those from control group (p<0.05). Egg qualities among five groups were all similar. Supplementation of 3% Sargassum meal increased the lightness of egg yolk (p<0.05). Eggs produced from layers fed 1% and 2% Sargassum meal had a higher consumer's acceptability than the control group (p<0.05). In blood characteristics, contents of glucose, nitrogen, triiodothyronine (T3) and thyroxine (T4) increased as the increase of supplementation ratio of Sargassum meal (p<0.05). In serum antibody titers, supplementation of 2% Sargassum meal stimulated a higher immunoglobulin M (IgM) level than that from control group (p<0.05). However, IgM content of layers fed diets with Sargassum meal ≥3% were decreased (p<0.05). There was no difference in IgA and IgG titers among groups. Conclusion: Supplementation of 1% to 3% Sargassum meal has shown to increase egg laying rate and egg mass of Leghorn layers. However, high supplementation (5%) would negatively affect laying performance. In consideration of laying performance, egg quality, consumer responses, and blood antibody, supplementation of Sargassum meal was suggested 2% in the diet for layers.

수동폐확장과 능동호흡강화 기법이 건강한 성인 폐기능에 미치는 영향 (The Effect of Passive Lung Expansion Technique and Active Respiration Enhancement Technique on Lung Function in Healthy Adults)

  • Purpose : This study was conducted to investigate the effect of positive active pressure technique and active breathing technique on lung function in healthy adults. Methods : In this study, the passive lung expansion technique and active respiration enhancement technique using an air mask bag unit were conducted in 30 normal adults to observe changes in pulmonary function with forced vital capacity (FVC), Forced expiratory volume at one second (FEV1). In order to observe the change in the level of respiratory function, we would like to investigate the peak expiratory flow (PEF) and the forced expiratory flow (FEF 25-75 %). Results : As a result of this study, there was no significant difference in comparison between the passive lung expansion technique and the active breathing enhancement technique (p>.05). The passive lung expansion technique effectively increased the effortful expiratory volume and the median expiratory flow rate of 1 second (p<.05). And the passive lung expansion technique effectively increased the effortless lung capacity and the maximum expiration flow rate (p<.05). Conclusion : The passive lung expansion technique effectively increases the range of motion of the lungs and chest cages, intrathoracic pressure, and elasticity of the lungs, and the active breathing technique increases the muscle functions such as the diaphragm and the biceps muscles. It is expected that it will be able to selectively improve the respiratory function of patients with respiratory diseases or functional limitations as it is found to be effective.

서비스기업의 고객경험관리역량이 직무만족 및 고객지향성에 미치는 영향 : 은행서비스를 중심으로 (Effect of Customer Experience Management Capability on Job Satisfaction and Customer Orientation of Service Firms : Focus on the Bank Service)

  • 본 연구는 서비스기업 중 하나인 은행 구성원을 대상으로 은행의 고객경험관리역량이 조직구성원의 직무만족 및 고객지향성에 어떤 영향을 미치는지 확인하고자 한다. 이를 위해 고객경험관리역량, 직무만족, 고객지향성에 대한 이론을 설명하고 이들 간의 관계를 이론 및 실증적으로 검증하였다. 본 연구를 위하여 은행에서 근무하는 접점직원 321명을 대상으로 SPSS23.0, AMOS 23을 이용해 구조방정식 분석을 실시하였다. 본 연구의 분석결과를 살펴보면 은행의 고객경험관리역량의 요소 중 구성원교육, 임파워먼트, 구성원평가, 교차검정은 직무만족 및 고객지향성에 긍정적 영향이 있는 것으로 나타났다. 이에 반해 채널통합은 직무만족과 고객지향성 모두 영향을 미치지 못했으며, 성과관리는 직무만족을 매개로하여 고객지향성에 영향을 미쳤다. 이러한 결과는 조직단위에서 고객경험관리역량을 확인할 수 있는 시사점을 제시하지만, 서비스기업이 은행으로 한정되었다는 한계도 존재한다.

Environmental test campaign of a 6U CubeSat Test Platform equipped with an ambipolar plasma thruster

  • The increasing interest in CubeSat platforms ant their capability of enlarging the frontier of possible missions impose technology improvements. Miniaturized electrical propulsion (EP) systems enable new mission for multi-unit CubeSats (6U+). While electric propulsion systems have achieved important level of knowledge at equipment level, the investigation of the mutual impact between EP system and CubeSat technology at system level can provide a decisive improvement for both the technologies. The interaction between CubeSat and EP system should be assessed in terms of electromagnetic emissions (both radiated and conducted), thermal gradients, high electrical power management, surface chemical deposition, and quick and reliable data exchanges. This paper shows how a versatile CubeSat Test Platform (CTP), together with standardized procedures and specialized facilities enable the acquisition fundamental and unprecedented information. Measurements can be taken both by specific ground support equipment placed inside the vacuum facility and by dedicated sensors and subsystems installed on the CTP, providing a completely new set of data never obtained before. CTP is constituted of a 6U primary structure hosting the EP system, representative CubeSat avionics and batteries. For the first test campaign, CTP hosts the ambipolar plasma propulsion system, called Regulus and developed by T4I. After the integration and the functional test in laboratory environment, CTP + Regulus performed a Test campaign in relevant environment in the vacuum chamber at CISAS, University of Padua. This paper is focused on the test campaign description and the main results achieved at different power levels for different duration of the firings.

건설사와 입주민의 공동주택 하자분쟁 쟁점 및 개선 방안 (Major Issues and Improvement Measures for Disputes between Construction Companies and Residents Related to Defects in Multi-Family Housing Complexes)

  • 입주민은 육안으로 구별이 되는 하자를 중심적으로 보며, 건설사는 공동 주택의 구조적, 기능적 문제에 영향이 가는 하자를 중심적으로 본다. 그에 따라, 건설사와 입주민은 공동주택의 하자를 기준으로 서로 다른 관점을 가지고 있다. 이를 해결하기 위해 본 연구에서는 서로 다른 하자의 관점차이로 인하여 발생하는 공동주택 하자분쟁의 쟁점과 개선 방안을 목표로 진행하였다. 먼저, 국토교통부와 법원의 서로 다른 하자판정기준 및 보수기준을 분석하였다. 과거 청구소송 하자판결 사례분석을 통하여 문제점을 도출 하였으며, 여러 건설사의 하자 데이터를 기반으로 건설사와 입주민의 관점차이를 도출 하였다. 이를 해결하기 위해 공동주택 하자 데이터베이스의 개선방안을 제안하였다.

Effects of Pahs and Pcbs and Their Toxic Metabolites on Inhibition of Gjic and Cell Proliferation in Rat Liver Epithelial Wb-F344 Cells

  • The liver progenitor cells could form a potential target cell population fore both tumor-initiating and -promoting chemicals. Induction of drug-metabolizing and antioxidant enzymes, including AhR-dependent CYP1A1, NQO-1 and AKR1C9, was detected in the rat liver epithelial WB-F344 "stem-like" cells. Additionally, WB-F344 cells express a functional, wild-type form of p53 protein, a biomarker of genotoxic events, and connexin 43, a basic structural unit of gap junctions forming an important type of intercellular communication. In this cellular model, two complementary assays have been established for detection of the modes of action associated with tumor promotion: inhibition of gap junctional intercellular communication (GJIC) and proliferative activity in confluent cells. We found that the PAHs and PCBs, which are AhR agonists, released WB-F344 cells from contact inhibition, increasing both DNA synthesis and cell numbers. Genotoxic effects of some PAHs that lead to apoptosis and cell cycle delay might interfere with the proliferative activity of PAHs. Contrary to that, the nongenotoxic low-molecular-weight PAHs and non-dioxin-like PCB congeners, abundant in the environment, did not significantly affect cell cycle and cell proliferation; however both groups of compounds inhibited GJIC in WB-F344 cells. The release from contact inhibiton by a mechanism that possibly involves the AhR activation, inhibition of GJIC and genotoxic events induced by environmental contaminants are three important modes of action that could play an important role in carcinogenic effects of toxic compounds. The relative potencies to inhibit GJIC, to induce AhR-mediated activity, and to release cells from contact inhibition were determined for a large series of PAHs and PCBs and their metabolites. In vitro bioassays based on detection of events on cellular level (deregulation of GJIC and/or proliferation) or determination of receptor-mediated activities in both ?$stem-like^{\circ}{\times}$ and hepatocyte-like liver cellular models are valuable tools for detection of modes of action of polyaromatic hydrocarbons. They may serve, together with concentration data, as a first step in their risk assessment.
