Anatomical Review of Lateral Upper Arm Flap for the Oral and Maxillofacial Reconstruction (구강악안면재건을 위한 외측상완피판의 해부학적 고찰)

  • The lateral upper arm flap (LUAF) was initially described by in 1982 by Song et al. as a simple skin flap, addressing the availability of cutaneous nerves for anastomoses. Katsaros et al., reported the use of a lateral upper arm skin flap, but also considered using it as a composite graft. The LUAF for the oral and maxillofacial reconstruction has several advantages over other flaps, such as constant anatomy, good color match and texture, thin design and plasticity. There is no functional limitation in the donor arm, such as strength and extension, and donor defects can be closed primarily with a linear scar, even when a flap of up to 8 cm in width is taken. For a better understanding of LUAF as a routine reconstructive option in moderate defect of maxillofacial region, the constant anatomical findings must be learned and memorized by young doctors during the specialized training course for the Korean national board of oral and maxillofacial surgery. This article review the anatomical basis of LUAF with Korean language.

Structure and Expression Analyses of SVA Elements in Relation to Functional Genes

  • SINE-VNTR-Alu (SVA) elements are present in hominoid primates and are divided into 6 subfamilies (SVA-A to SVA-F) and active in the human population. Using a bioinformatic tool, 22 SVA element-associated genes are identified in the human genome. In an analysis of genomic structure, SVA elements are detected in the 5′ untranslated region (UTR) of HGSNAT (SVA-B), MRGPRX3 (SVA-D), HYAL1 (SVA-F), TCHH (SVA-F), and ATXN2L (SVA-F) genes, while some elements are observed in the 3′UTR of SPICE1 (SVA-B), TDRKH (SVA-C), GOSR1 (SVA-D), BBS5 (SVA-D), NEK5 (SVA-D), ABHD2 (SVA-F), C1QTNF7 (SVA-F), ORC6L (SVA-F), TMEM69 (SVA-F), and CCDC137 (SVA-F) genes. They could contribute to exon extension or supplying poly A signals. LEPR (SVA-C), ALOX5 (SVA-D), PDS5B (SVA-D), and ABCA10 (SVA-F) genes also showed alternative transcripts by SVA exonization events. Dominant expression of HYAL1_SVA appeared in lung tissues, while HYAL1_noSVA showed ubiquitous expression in various human tissues. Expression of both transcripts (TDRKH_SVA and TDRKH_noSVA) of the TDRKH gene appeared to be ubiquitous. Taken together, these data suggest that SVA elements cause transcript isoforms that contribute to modulation of gene regulation in various human tissues.

  • The systematic study of the future began with curiosity and imagination about the future; a unique trait of human mental effort-and this seems to be based on the idea that the future can be different depending on current choices or effort. In this sense, it would be seemly to encourage more interest and academic study on the progress of future fashion. In this study, we examine recent changes of fashion material; e.g. that which science and technology have more impact and importance in futuristic fashion since the 1990�s. The period analysed is from the1990's to the present and related data from recent fashion collections and fashion books has also been included The current prediction of the future is largely based on what was formed between the 19th Century and the 20th Century which has persisted until now and has been influenced by the view that science will play a bigger role in the future. This is especially reflected in fashion which chiefly represents material culture. New materials used for fashion are strong and permanently durable, in addition to being very light, thin, flexible, hygienic, ecological and comfortable to wear-almost like a second skin. These fashionable new materials roughly function in two different ways according to external and internal characteristics. First, they cause external change. Second, they exemplify or allow new functions. Examples of external change are the use of silver color, achromatic color, metallic material, smooth-to-the-touch shiny material and the use of luminous material. Examples of the extended function of clothing through the use of new materials are the use of conducting thread, the use of special material for blocking & opening and the use of material which changes colors as the surroundings change. These days, the use of new material which changes its appearance is a novelty unique to the fashion world but we also expect to witness the debut of diverse new materials with extended inner functions.

  • Industry Foundation Classes(IFC) is the core product schema for ensuring the interoperability of information in Building Information Modeling(BIM) environment. However, since current IFC is mainly focused on building structure. there are limitations in the generation of functional information of components when applied to civil infrastructures. Previous studies have proposed IFC-based new entities for the civil infrastructures, it takes long time to support them in BIM authoring software packages. In this study, we proposed practical rules to apply IFC-based information modeling using BIM authoring software and additional new entities for the civil infrastructure through attributes and information mapping. The availabilities of proposed method were examined using the rail and sleeper information models based on an extended IFC data schema for the railway infrastructures.

Proteomic Approach of the Protein Profiles during Seed Maturation in Common Buckwheat (Fagopyrum esculentum Moench.)

  • Single seeds of common buckwheat cultivar Suwon No. 1 when subjected to SDS-PAGE revealed very high polymorphism. High variation existed for protein or protein subunits with molecular weight 54-47kDa, 45-25kDa and 16-11kDa. The electrophoregram showed variation for globulin as well as other protein fractions. About 300 proteins were separated by two-dimensional electrophoresis in common buckwheat (Fagopyrum esculentum Moench.) seed. Seed maturation is a dynamic and temporally regulated phase of seed development that determines the composition of storage proteins reserves in mature seeds. Buckwheat seeds from 5, 10, 15, 20, and 25 days after pollination and matured stage were used for the analysis. This led to the establishment of high-resolution proteome reference maps, expression profiles of 48 spots. It was identified 48 proteins from MALDI-TOF/MS analysis of wild buckwheat seed storage proteins. The 48 proteins were found identical or similar to those of proteins reported in buckwheat and other plants; it is belonging to 9 major functional categories including seed storage proteins, stress/defense response, protein synthesis, photosynthesis, allergy proteins, amino acid, enzyme, metabolism, and miscellaneous. It appears that the major allergenic storage protein separated played the important role in buckwheat breeding and biochemical characterization.

Effect of Forward-and-Backward Shift Trunk Exercise Using Proprioceptive Neuromuscular Facilitation Diagonal Pattern in Closed Kinematic Chain Exercises on Upper Limb Function and ADL in Stroke Patient -A Single-Subject Design- (닫힌사슬에서의 PNF 대각선 패턴을 이용한 몸통 전·후방 이동운동이 뇌졸중환자의 상지 기능 및 일상생활에 미치는 영향 -단일 사례 연구-)

  • Purpose: The purpose of this study was to investigate the effects of forward-and-backward shift trunk exercise using a proprioceptive neuromuscular facilitation (PNF) diagonal pattern in a closed kinematic chain exercise on the upper limb function and activity of daily living (ADL) in a stroke patient. Methods: One subject participated in this study. The study used a reversal A-B-A' design, where A and A' were the baseline period (no intervention), and B was the intervention period. The intervention was a forward-and-backward trunk shift exercise, using a PNF diagonal pattern on both a stand-on-hand position and a quadruped position of closed kinematic chain exercises, for 20 min per day for 2 weeks. The range of motion (ROM) of the shoulder joint was measured and a Fugl-Meyer assessment of upper extremity (FMA-UE) and a functional independence measure (FIM) were performed to measure upper limb function and activity of daily living (ADL). Results: ROMs of shoulder joint (flexion, extension, abduction, and external rotation) increased in the intervention phase. The FMA-UE score increased (from 28 to 36) in the intervention phase. The FIM score increased (from 20 to 25) in the intervention phase. These increases were maintained after intervention (Baseline II). Conclusion: These results suggest that forward-and-backward shift trunk exercises using a PNF diagonal pattern in a closed kinematic chain exercise have a positive effect on stroke patients' upper limb function and ADL ability.

Modified Bilhaut-Cloquet Method for Correction of Bifid Thumb (Bilhaut-Cloquet 변법을 이용한 이열엄지의 교정)

  • Purpose: As the traditional treatment of the Wassel's type I or II of bifid thumb, Bilhaut-Cloquet, has always been the standard method despite several disadvantages such as tearing of the finger nail, injuries of the growth plate, joint instability, and long visible scarring. To overcome these drawbacks, we applied a modified Bilhaut-Cloquet Method. Methods: The subjects used for the this study were 10 of 20 patients evaluated. The patients underwent modified methods under every type of Wassel's classification. We designed a central wedge Zig-Zag incision and removed the nail and bony tissues in the remaining digit, but not soft tissue if possible, and transferred the ligaments, tendons, and soft tissue to the remaining thumb from the extra digit. We evaluated the patients' lack of extension, the total ROM of the MP and IP joints, the ROM of IP joints, and the lateral deviations of the reconstructed thumb. Results: The results were encouraging, with all patients showing a good functional and aesthetic outcome. Conclusion: The modified method proved a very effective procedure in the treatment of bifid thumb in all types, especially types I or II.

Comparison of Balance and Gait Between Fallers and Non-Fallers in Elderly (넘어진 노인과 넘어지지 않는 노인의 균형과 보행비교)

  • The purpose of this study was to compare the balance and gait between fallers and non-fallers in elderly. A brief questionnaire was used to obtain the fall history. Twenty-seven women subjects were evaluated in this study. Eleven subjects and a mean age of 84.5 years (SD=4.6) were designated as the faller group. Sixteen subjects and a mean age of 80.3 years (SD=5.3) were designated as the non-faller group. The fall-related factors (mental status, balance, range of motion and muscle strength of lower extremity, sensation of foot, and cadence, walking velocity, stride length) were compared between faller group and non-faller group and measured. The results showed that faller group had significantly less range of motion of the hip flexion and knee extension, and strength of the knee extensor and ankle dorsiflexor and plantar flexor than non-faller group. The scores of the Functional reach test and One leg standing were significantly less in faller group than in non-faller group. Faller group showed less walking velocity and stride length compared to non-faller group. However, there was no significant difference in cadence during comfortable waking and fast walking between two groups. There were no significant differences in pressure, position sensory between two groups. These results suggest that exercise for improving the flexibility, muscle strength of the lower extremity and balance may be useful strategies to prevent fall in elderly. Further studies are needed to identify which specific factors are related to fall in the elderly population.

  • When Korea opened its ports and underwent Japanese colonization, many Japanese style houses were built in Korea. Following Korea's independence from Japan, Koreans began to reside in these houses. The objective of this study is to examine the current state of Japanese style residence areas and Japanese style houses in Korea, and to determine the change in the characteristics of dining kitchens that have taken place since Koreans have lived in them. In the process, while assimilation occurred, there was also a conflict between the residential lifestyles of the two cultures, developing into a state where two housing cultures co-existed. The dining kitchens showed the most sensitive adjustments to social changes, facilitating a number of important changes in the process of modernizing houses. In this regard, the intention is to determine how the dining kitchens responded to other areas within the house as they were being transformed. Research for this study is based on previous studies that were carried out in 1991 on Japanese style houses, in order to clearly define the process of change chronologically rather than from a single examination. In consequently, From the process of changes where from a conventional kitchen to DK anger, 1) The public space - wooden floor, living room, etc - had been formed in house spaces. 2) In the lifestyle, privacy secured. It was separated each functional spaces that greeting space for guests and family's space in the lifestyle. 3) The cause of variation could be summarized that differences of living style, a change of life and fuel.

Arthroscopic treatment for symptomatic lateral discoid meniscus in a 26-month-old girl -A case report- (26개월 여아에서 발생한 슬관절 외측 원판형 연골 파열의관절경적 치험례 -증례보고-)

  • In children, the snapping-knee syndrome is strongly suggestive of a diagnosis of discoid menisci. Furthermore the loss of extension of the knee suggests a diagnosis of torn lateral discoid meniscus. Symptomatic lateral discoid meniscus tear in a very young child, especially under the 3-year-old, was very rarely reported and the treatment protocol has not been established. Some therapeutic advancement was achieved in symptomatic lateral discoid meniscus but still technical difficulties exist. We undergone arthroscopic meniscal reshaping in a 26-month-old girl. At the time of postoperatively 2-year 9-month follow up, excellent result has achieved in clinical and functional outcome according to the scale of Ikeuchi without recurrence of symptom or postoperative complication. We reported out experience and results that arthroscopic reshaping procedure was done for management of symptomatic lateral discoid meniscus in a 26-month-old girl.
