Investigation on evaluation of functional dyspepsia by using Digital Infrared Thermal Images (기능성 소화불량증에 대한 적외선 체열검사(DITI)의 임상 지표적 의의 연구)

  • Objective: To investigate the significance of temperature differences on two different acupuncture points between functional dyspepsia(FD) group and non-functional dyspepsia(non-FD) group respectively. Methods: We performed this research on 40 patients who came and took D.I.T.I in Kang-nam korean hospital kyung-hee university. We analyzed the averaged temperature of Zhongwon(CV12), Indang(HN1) and, also, investigated the significance of subtraction from Indang to Zhongwon temperature statistically. Results: The temperature differences from Indang to Zhongwon between functional dyspepsia group and non-functional dyspepsia group was significant, it meant that patients who have functional dyspepsia have more significant temperature difference from Indang(HN1) to Zhongwon(CV12). The direct comparisons of mean temperature between Indang and Zhongwon in the FD group and non-FD group were not significant. Conclusion: The study provide hypothesis on the temperature difference from Indang(HN1) to Zhongwon(CV12) and its association with functional dyspepsia. This study can provide a foundation for future studies on the evaluation of functional dyspepsia by using D.I.T.I.

The Effect of Low, High Frequency Electroacupuncture on Functional Dyspepsia: A Randomized, Double Blinded, Two Arm Trial (기능성 소화불량증에 대한 저빈도, 고빈도 전침의 효능 비교 연구: 무작위배정, 이중맹검(피험자, 평가자), 평행군 설계)

  • Objectives : The purpose of this study is to investigate the efficacy of the electroacupuncture with different frequency on functional dyspepsia. Methods : We observed 30 functional dyspepsia patients from May, 2013 to June, 2014. This study was randomized and double blinded. The patients were divided into two groups. One group(group A) was treated with 3 Hz electroacupuncture and the other group(group B) was treated with 300 Hz electroacupuncture. We evaluated the treatment effect of each group by using visual analogue scale(VAS) and pressure algometer. Results : 1. Functional dyspepsia was significantly improved in group treated with 3 Hz electroacupuncture. 2. Functional dyspepsia was significantly improved in group treated with 300 Hz electroacupuncture. 3. As a result of evaluation there were no statistical significance between two groups. Conclusion : Both 3 Hz and 300 Hz electroacupuncture has significant effect on functional dyspepsia. But there is no significant difference between 3 Hz and 300 Hz electroacupuncture.

Study on the Correlation between Swallowing Acid Symptom and Heart Rate Variability in Dyspepsia Patients (소화불량을 주소(主訴)로 내원한 환자에서 탄산(呑酸)증상 유무와 심박변이도와의 상관성 연구)

  • Objectives : The objective of this study was to investigate the correlation between swallowing acid symptom and autonomic nerve system in dyspepsia patients. Methods : The test for heart rate variability (HRV) was done to 50 dyspepsia patients at the Department of Internal Medicine, Kang-Nam Kyung-Hee Korean Hospital. 21 among 50 patients had swallowing acid symptom and 29 didn't. We checked HRV and compared HRV index between the two groups. We also divided the patients into functional dyspepsia and organic dyspepsia groups, and then checked HRV respectively. Results were as follows : 1. In dyspepsia patients with swallowing acid, LF/HF ratio was higher than the dyspepsia patients without swallowing acid but the result was not statistically significant. 2. TP, LF, VLF, and LF/HF ratio was higher in the functional dyspepsia group than in the organic dyspepsia group. Average age in the organic dyspepsia group was significantly higher than in the other group. Both results showed statistically significant difference. 3. In the functional dyspepsia group, LF/HF ratio was significantly higher in patients with swallowing acid symptom than those without. However, in eructation and nausea symptoms, HRV index was not statistically significant. Conclusion : This study shows the ratio of sympathetic and parasympathetic nerve system increased in the swallowing acid patient group compared with the non-swallowing acid patient group in functional dyspepsia patients.

Diagnostic Values of Tongue Coating Thickness and Sterno-costal Angle in Functional Dyspepsia (기능성 소화불량 환자에서 설태후박 및 흉늑각 측정의 진단적 가치)

  • Objectives : The aims of this study were to evaluate the diagnostic values of tongue coating thickness and sterno-costal angle as the quantitative diagnostic indicators in functional dyspepsia. Methods : We surveyed 60 functional dyspepsia patients recruited by the clinical trial, 'Clinical Trial for Evaluation on Availability of tongue diagnosis system (CTS-1000) : a Pilot study'. The patients were classified into three groups according to Rome III criteria for functional dyspepsia, and categorized into five groups according to Instrument of pattern identification for functional dyspepsia. Nepean dyspepsia index-Korean version (NDI-K) score, tongue coating thickness (percentage of tongue coating by tongue diagnosis system (CTS-1000), and weight of tongue coating by microbalance), sterno-costal angle, duration of illness and body mass index (BMI) were investigated. Results : Among the 5 types by instrument of pattern identification for functional dyspepsia, a significant difference of percentage of tongue coating was found. Percentage of tongue coating and weight of tongue coating showed significant correlation with total NDI-K score. Sterno-costal angle showed strong positive correlation with BMI and also showed significant difference between the non-overweight (BMI<23, n=32) and overweight ($BMI{\geq}23$, n=28) groups. Conclusions : Tongue coating thickness showed its potential as a new quantitative diagnostic indicator of functional dyspepsia. Further studies on the sterno-costal angle are anticipated to evaluate its potential as a new quantitative diagnostic indicator.

DA-9701: A New Multi-Acting Drug for the Treatment of Functional Dyspepsia

  • Motilitone$^{(R)}$ (DA-9701) is a new herbal drug that was launched for the treatment of functional dyspepsia in December 2011 in Korea. The heterogeneous symptom pattern and multiple causes of functional dyspepsia have resulted in multiple drug target strategies for its treatment. DA-9701, a compound consisting of a combination of Corydalis Tuber and Pharbitidis Semen, has being developed for treatment of functional dyspepsia. It has multiple mechanisms of action such as fundus relaxation, visceral analgesia, and prokinetic effects. Furthermore, it was found to significantly enhance meal-induced gastric accommodation and increase gastric compliance in dogs. DA-9701 also showed an analgesic effect in rats with colorectal distension induced visceral hypersensitivity and an antinociceptive effect in beagle dogs with gastric distension-induced nociception. The pharmacological effects of DA-9701 also include conventional effects, such as enhanced gastric emptying and gastrointestinal transit. The safety profile of DA-9701 is also preferable to that of other treatments.

A Clinical Case Report of Soyangin Patients with Functional Dyspepsia (기능성 소화불량증이 호전된 소양인 비수한표한병 환자 3례 보고)

  • Objectives The purpose of this study was to report significant improvement of functional dyspepsia in 3 patients, who were diagnosed with 'Soyangin Spleen Cold-based Exterior Cold disease' and applied medication based on Sasang Constitutonal medicine.Methods The patients were treated with herbal medications, according to their own constitution. The objective outcome was measured by body composition analyzer and Gastrointestinal Symptom Rating Scale (GSRS), and the subjective sensation was assessed by using Visual Analogue Scale (VAS).Results The symptoms of functional dyspepsia improved remarkedly in all respects, without any side effects. Original symptoms also changed for the better.Conclusions This results show that it is available that functional dyspepsia is treated with medicines according to Sasang Constitutional medicine.

A Case of the Therapeutic Effect of Nangan-jeon on Refractory Functional Dyspepsia Caused by "Liver-Kidney Deficiency Cold"

  • Objective: This study explored the clinical usefulness of Nangan-jeon decoction to treat refractory functional dyspepsia. Methods: A 61-year-old female had been suffering from severe functional dyspepsia for about 40 years, but the dyspeptic symptoms rapidly improved after taking Nangan-jeon decoction. The clinical outcome was assessed by the numerical rating scale (NRS) and the self-reporting method. Result: After there was no improvement from various herbal treatments in a department of a Korean college hospital, the patient moved to the author's clinic. She had been diagnosed with the pattern identification of "liver-kidney deficiency cold" and prescribed with Nangan-jeon decoction. This herbal drug rapidly improved her dyspeptic symptoms, from NRS 10 to NRS 2 (after 3 weeks) and NRS 1 (after 7 weeks). Conclusion: This case report is the first to present the potential of Nangan-jeon decoction to improve refractory functional dyspepsia.

Effect of herb drug medicine Treatment for Functional Dyspepsia: Controlled Trial (기능성 소화불량증 환자에 대한 한약복합제형간의 비교 임상시험: 대조군 연구)

  • Objective : Functional dyspepsia is a prevalent disease. It impedes subjective quality of life. The purpose of this research is to examine the equivalent effect of herb drug medicine treatment(H-D)and Over the Counter(OTC) for functional dyspepsia. Method : In this controlled study, we compared herb drug medicine(H-D) with Over the Counter(OTC) of functional dyspepsia. 30 volunteers who satisfied the requirements were enrolled in study. Severity of dyspepsia was measured by Nepean Dyspepsia Index(NDI-K) before and after treatments. Result : The results are summarized as follows. 1. In Herb drug medicine and Over the Counter groups, total key symptoms score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. 2. In Herb drug medicine and Over the Counter groups, each symptoms score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. 3. In Herb drug medicine and Over the Counter groups, quality of life score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. Conclusion : Herb drug medicine treatment(H-D) is effective to improve the symptoms and quality of life in patients with functional dyspepsia.

Two cases of the Gastric Dysmotility-like Dyspepsia in functional dyspepsia that were improved by Bojungiki-Tang (보중익기탕으로 호전된 위 운동장애형 기능성 소화불량 환자 2례)

  • Objectives : The aim of this study was to investigate an effect of Bojungiki-Tang in two cases of the Gastric dysmotility-like Dyspepsia in functional dyspepsia Methods : Subjects were 2 patients with Gastric dysmotility-like Dyspepsia. The patients had suffered from a dyspepsia that were not improved by medical treatments. We gave Bojungiki -Tang to the patients 30 minutes after each meal and tried acupuncture and moxibustion 2 times every day. And we had evaluated the condition of patients using a Ryodoraku test and ordinal scales. Results : After the comparison between an admission and discharge date at Ryodoraku test and ordinal scales, we knowed that conditions of two Gastric dysmotility-like Dyspepsia patients had improved. Conclusions : Bojungiki-Tang is effective on the improvement of conditions in the Gastric dysmotility-like Dyspepsia patients.

Could Headache be one of Symptoms induced by Gastric Dysmotility in Patients with Functional Dyspepsia? (기능성 소화불량증 환자의 두통은 위 운동성 장애에 의한 증상이 될 수 있는가?)

  • Objectives : The aim of this study was to investigate the relationship between headache and gastric dysmotility in patients with functional dyspepsia by using electrogastrogram and bowel sounds analysis. Methods : 127 patients(male 40, female 87) with functional dyspepsia were enrolled. By using inquiry and the questionnaire, we examined whether each patient had headache and abdominal pain. All patients were divided into 4 groups according to the above symptoms. Gastric motility was measured during the fasting-postprandial state by using electrogastrogram and bowel sounds analysis. Results : 33 patients complained of headache. There was a significant difference between headache patients and non-headache patients in gastric arrhythmia of the fasting state. Moreover, in headache patients without abdominal pain, postprandial improvement of gastric arrhythmia was poorer than the other groups. So, headache patients without abnormal pain had more severe gastric myoelectrical rhythm disorder than headache patients with abdominal pain. Conclusion : Headache could be one of symptoms induced by gastric dysmotility in patients with functional dyspepsia. Especially, headache in functional dyspepsia was related to gastric arrhythmia.
