Scapular muscle endurance, shoulder pain, and functionality in patients with rotator-cuff-related shoulder pain: a matched, case-control study

  • Background: Deficiency in scapular muscle endurance (SME) is a risk factor for rotator-cuff-related shoulder pain (RCRSP). However, the exact relationship among SME, pain, and functionality remains unclear. This study aims to compare SME, pain, and functionality in RCRSP patients to those in age-sex-matched healthy controls. Methods: Twenty-three patients with RCRSP and 23 age-sex matched healthy controls were included in the study. SME was measured using a 1-kg dynamometer. Self-reported pain level was assessed using a visual analog scale. The Functional Impairment Test-Hand, Neck, Shoulder, and Arm (FIT-HaNSA) was also used to assess functional impairment. Results: The control group had higher SME and total FIT-HaNSA scores than the patient group (P<0.05). There was a statistically significant and positive correlation between SME and FIT-HaNSA scores in both groups (P<0.05). Conclusions: SME was affected by RCRSP. Pain and functional impairment were correlated with low SME.

Correlation Analysis between Gastric Emptying Measurement by Ultrasonography and Korean Medical Questionnaires (초음파 위배출 측정과 한의설문 간의 상관성 분석)

  • Objective: This study was designed to investigate the correlation between gastric emptying measurement by ultrasonography and the findings of various questionnaires of functional dyspepsia (FD) or in Korean medicine to explore the possibility of quantification of questionnaires. Methods: Eighty-eight patients (44 patients with FD and 44 healthy controls) were recruited from October 2016 to November 2017. Gastric emptying half-time (T1/2) was measured by ultrasonography at fasting, and at 0, 15, 30, 45, 60, 90, and 120 min after meals (apple juice, 500 ml, 224 kcal). The average half-time (average T1/2) was calculated by averaging all measured half-times. A visual analogue scale (VAS), food retention questionnaire (FRQ), damum questionnaire (DQ), cold and heat questionnaire (CHQ), deficiency and excess questionnaire (DEQ), spleen-qi deficiency questionnaire (SQDQ), Nepean dyspepsia index-Korean version (NDI-K), and quality of life of NDI (NDI-QoL) were filled out by all participants. Correlation between the average T1/2 and questionnaires were analyzed for 79 patients (37 patients with FD and 42 healthy controls) after excluding missing and erroneous data. Results: The FRQ, DEQ, SQDQ, NDI-K, and NDI-QoL scores, especially for some gastrointestinal symptoms and general weakness-related symptoms, showed statistically significant correlations with average T1/2. Conclusions: Our results suggest that gastric emptying measurement by ultrasonography may be a quantitative substitute method for some Korean medical questionnaires or for some dyspepsia-related and deficiency-related items among them. However, further studies using various measurements of gastric emptying will be needed.

  • Caenorhabditis elegans him-6 mutants, which show a high incidence of males and partial embryonic lethality, are defective in the orthologue of human Bloom's syndrome protein (BLM). When strain him-6(e1104) containing a missense him-6 mutation was irradiated with ${\gamma}$-rays during germ cell development or embryogenesis, embryonic lethality was higher than in the wild type, suggesting a critical function of the wild type gene in mitotic and pachytene stage germ cells as well as in early embryos. Even in the absence of ${\gamma}$-irradiation, apoptosis was elevated in the germ cells of the him-6 strain and this increase was dependent on a functional p53 homologue (CEP-1), suggesting that spontaneous DNA damage accumulates due to him-6 deficiency. However, induction of germline apoptosis by ionizing radiation was not significantly affected by the deficiency, indicating that HIM-6 has no role in the induction of apoptosis by exogenous DNA damage. We conclude that the C. elegans BLM orthologue is involved in DNA repair in promeiotic cells undergoing homologous recombination, as well as in actively dividing germline and somatic cells.

A Clinical case study about the Effects of Oryeong-san Gamibang on Case of Functional diarrhea (오령산가미방(五苓散加味方)으로 호전시킨 기능성 설사(泄瀉)환자에 대한 임상적 고찰)

  • Objectives : The purpose of this study is to investigate the effect of Leejung-tang plus Oryung-san Gamibang on functional diarrhea.Method : We treated a 72-year-old male who had severe watery diarrhea and bowel sounds. The patient was treated with Korean medicine including Oryung-san Gamibang from March 31, 2016 to May 23, 2016. Defecation frequency, Bristol stool scale, Bowel sound, Patient's condition were measured.Result : After oriental medical treatment, clinical symptoms of functional diarrhea (defecation frequency, stool form, bowel sounds) were recoverd normally.Conclusions : Leejung-tang plus Oryeong-san Gamibang effects on the functional diarrhea of a patient diagnosed as Deficiency Cold (虛寒). This case study shows that Oryeong-san Gamibang is clinically effective for functional diarrhea. So that, it is helpful in decreasing the symptoms of a patient and in improving quality of life.

Expression of a Functional Type-I Chalcone Isomerase Gene Is Localized to the Infected Cells of Root Nodules of Elaeagnus umbellata

  • A putative type-I chalcone isomerase (CHI) cDNA clone EuNOD-CHI was previously isolated from the root nodule of Elaeagnus umbellata [Kim et al. (2003)]. To see if it encodes a functional CHI, we ectopically overexpressed it in the Arabidopsis (Arabidopsis thaliana) transparent testa 5 (tt5) mutant, which is defective in naringenin production and has yellow seeds due to proanthocyanidin deficiency. Ectopic overexpression of EuNOD-CHI resulted in recovery of normal seed coat color. Naringenin produced by CHI from naringenin chalcone was detected in the transgenic lines like in the wild-type, whereas it was absent from the tt5 mutant. We conclude that EuNOD-CHI encodes a functional type-I CHI. In situ hybridization revealed that EuNOD-CHI expression is localized to the infected cells of the fixation zone in root nodules.

A Novel Behavior, Bang-Sensitive Paralysis, Associated With The shibire Locus of Drosophila melanogaster

  • The Drosophila temperature-sensitive mutant shibire (shi) is paralyzed at restrictive temperature by a reversible block in synaptic transmission. To explore the functional relationship among shi gene products, viability and temperature-sensitive paralytic behavior were quantitaively analyzed for four shi alleles, shi$^{ts1}$, shi$^{ts2}$, shi$^{ts4}$, and shi$^{ST139}$, and their heteroallelic combinations. The hemizygous combination of shi alleles over deficiency was not completely lethal. shi$^{ts2}$ exhibited distinctively higher viability than other alleles. A novel behavior, bang sensitivity, was also found in shi/Df(1). This bang-sensitive paralytic behavior was compared with that of the typical bang-sensitive mutant flies. Heterozygotes, shi/+, are more severe in temperature sensitivity than deficiency hemizygotes, Df(1)/+. Heteroallelic combinations of shi were less sensitive to high temperature than homozygotes. Among all allelic combinations, shi$^{ts2}$/shi$^{ts4}$ showed an unexpected extreme reduction in temperature sensitivity. The results of allelic interactions among 4 shi alleles suggest that the shi mutations examined behave as antimorphic alleles and that the gene product of shi are likely to function in multimeric forms.

A Concentration-Function Basis for Ideal Vitamin C Intake

  • Vitamin C is an essential nutrient involved in many functions. Humans are unable to synthesize vitamin C de novo, because they lack the last enzyme in the biosynthetic pathway. Previous Recommended Dietary Allowances (RDAs) for vitamin C were based on prevention of deficiency with a margin of safety. However preventing deficiency may not be equivalent to ideal nutrient intake. Recommendation should be based on vitamin function in relation to concentration. For this goal, data set of the relationship between wide-range of vitamin C dose and resulting concentrations in plasma and tissues and characterization of functional outcomes in relation to these concentrations should be acquired. This article reviews the current knowledge in these areas and suggest how this knowledge may contribute toward establishing dietary guideline for ideal vitamin C intake.
