Effects of Dynamic Exercise Program Using Thera-Band on Craniovertebral Angle in Adults with Forward Head Posture

  • Background: The alignment of the neck and shoulder is important in people with forward head posture. However, previous studies have mainly conducted fragmentary studies on the neck and shoulders, and studies on the combined movement of the neck and shoulders are incomplete. Objective: To investigate the effects of 6 week dynamic exercise program using Thera-band on craniovertebral angle (CVA) in adults with forward head posture. Design: Quasi-experimental study. Methods: The study was conducted on 24 adults with forward head posture and experimented with neck and shoulder exercises and divided them into groups of neck exercises, shoulder exercises, and neck and shoulder exercises to measure CVA values before and after the experiment. The neck exercise program included flexion and extension muscles of the neck and shoulder exercises included dynamic exercise of the upper extremities such as the trapezius muscles and serratus anterior muscle. The CVA results were measured using PA200. Results: Following the interventions, neck exercise group showed significant improvement in CVA (P<.05), but shoulder exercise group and combined exercise group did not show any significant results (P>.05). However, both groups showed some positive results. Significant differences were seen in the comparisons between the three groups (P<.05), and the results of the post-hoc test showed significant differences in neck exercise group and shoulder exercise, neck exercise and combine exercise group. Conclusion: This study suggested that the Thera-band neck exercise is beneficial for foward head posture patients and is expected to be used in clinical trials.

  • The somatotype classification of this study was to manufacture well-fitted clothes for middle-aged women. The somatotype classification of the upper body of lateral view was based on previous studies, and 4 postures, straight posture, leaning back posture, bent forward posture, and swayback posture were selected for this study. The front of leaning back posture was longer and wider than that of straight posture, and its front neck depth was deeper. Its front interscye breadth was wider and back interscye breadth was narrower. S.N.P. B.P. front waistline length, waist front length, front diagonal length, chest shedder length, front waistline shoulder line length were longer, and S.N.P. scapular back waistline length, back length, back shoulder length, back diagonal length, shoulder line back waistline length were shorter. On the contrary, the front of bent forward posture was shorter and narrower than that of straight posture, and its back neck depth was deeper. The properties of swayback posture were similar to those of bent forward posture. Its front was shorter and narrower, but the results of front neck depth and back neck depth were like those of straight posture.

Efficacy of Forward Head Posture on Scapular Kinematic Changes and Shoulder Pain

  • Objective: Deformation of soft tissues around the neck and scapularcan caused by forward head posture(FHP), which has an uncomfortable effect on biomechanical changes in the scapula as well as functional disorders of the shoulder. However, studies related to direct FHP, biomechanical changes in the scapulafunction, and shoulder pain and disorder have not yet been conducted. Therefore, purpose of this study is to effect of decresedthe FHP on the shoulder function of the sacpular biomechanical examine the change in the shoulder painand disorder. Design: A randomized controlled trial Methods: The participants were 32adults(23.03±3.90 years) recruited and redivided randomly into Forward head posture corrective exercise(FHPCE) vs Control. The FHPCE group was proceeded according to the over load principle through 2steps biofeedback exercise and corrective exercise(n=16). The control (n=16) was TENS did not operated and padding 20 minute. This study was conducted 3 times a week for 4a weeks. Results: FHPCE group is improve in the results of craneocervical angle(p<0.05, 95% CI: 0.352, 4.073). In Mechanical changes of scapula in the shoulder flexion more significant improvement in FHPCE than control group[Axis X(p<0.05), Y(p<0.01), Z(p<0.01)], and shoulder abductionmore significant improvement in FHPCE than control group[xis X(p<0.01)], as well FHPCE showed significant increased in the results in the shoulder pain(p<0.05, 95% CI: -13.244, -1.566) Conclusions: This study suggected that FHP affects the biomechanical changes of the shoulder, and a new method for shoulder pain intervention

어깨 안정화운동과 복부 안정화 운동에 의한 전방머리자세 성인의 머리척추각도와 어깨뼈 정렬 변화 (Change of Craniovertebral Angle and Scapula Alignment in Adults with Forward Head Posture by Shoulder and Abdominal Stabilization Exercise)

  • Purpose: This study aimed to adjust the craniovertebral angle and shoulder alignment through shoulder and abdominal stabilization exercises in adults with a forward head posture. Methods: The study participants were 29 adults with a forward head posture, and they were randomly divided into the following groups: 14 participants in a combined exercise group that used shoulder and abdominal stabilization exercises and 15 participants in a shoulder exercise group that used just shoulder stabilization exercises. The participants performed the stabilization exercises for 30 minutes per day, three times a week for five weeks. Results: There were significant differences in the craniovertebral angle after intervention in the shoulder stabilization exercise group (p < 0.05). There were significant differences in the craniovertebral angle and location of the right root of the spine and both inferior angles before and after intervention in the shoulder and abdominal stabilization exercise group (p < 0.05). There was a significant difference in the location of the right root of the spine and the left inferior angle between the groups at the post-test (p < 0.05), and there was a larger change in the shoulder and abdominal stabilization exercise group. Conclusion: There was a significant difference in the craniovertebral angle and a partially significant difference in shoulder alignment before and after intervention in both groups.

척추 전장 측면 방사선검사 시 손바닥 정면 자세가 척추골반지표에 미치는 영향 (Effect of Palm Facing Forward Posture on SpinoPelvic Parameters on the Whole Spine Lateral Radiography)

  • The purpose of this study was to investigate the effect of cross arms and palms facing forward on spinopelvic parameters during the whole spine lateral radiography. In addition, we would like to present the usefulness of a posture with the palm facing forward during whole spine lateral radiography of the spine using EOS. The subjects of this study were images of a total of 50 patients (18 males, 32 females) who whole spine lateral radiography using the conventional method and the EOS method from October 2020 to March 2021. The posture used in this study was set as 'CAP' for cross arms and 'PUSH' for posture with palms facing forward. In this study, among the spinal stability factors, thoracic kyphosis (thoracic vertebrae 4 to 12), lumbar lordosis (lumbar vertebrae 1 to sacrum 1), sagittal vertical axis, sacral slope, and shoulder flexion angle were compared on average. The mean thoracic kyphosis was 34.52±12.46° for CAP and 28.46±10.81° for PUSH (p<0.01). The lumbar lordosis of CAP was 42.45±17.45°and that of PUSH was 40.56±16.14°(p>0.57). The sagittal vertical axis was 26.59±34.34 mm in CAP and 21.21±35.41 mm in PUSH (p>0.44). In CAP, the sacral slope was 30.96±10.29°, and in PUSH, it was 31.01±10.19° (p>0.98). shoulder flexion angle was 38.31±8.24° for CAP and 26,08±6.71° for PUSH(p<0.01). As a result of this study, the PUSH posture is considered to be a posture that can minimize the shoulder flexion angle and can perform a stable examination while minimizing changes in spino-pelvic parameter.

Can Suboccipital Release Followed by Cranio-Cervical Flexion Exercise Improve Shoulder Range of Motion, Pain, and Muscle Activity of Scapular Upward Rotators in Subjects With Forward Head Posture?

  • Background: For the treatment of forward head posture (FHP) and forward shoulder posture, methods for strengthening scapular retractors and deep cervical flexors and stretching pectoralis and upper cervical extensors are generally used. No study has yet assessed whether suboccipital release (SR) followed by cranio-cervical flexion exercise (CCFE) (SR-CCFE) will result in a positive change in the shoulders and neck, showing a "downstream" effect. Objects: The purpose of this study was to investigate the immediate effects of SR-CCFE on craniovertebral angle (CVA), shoulder abduction range of motion (ROM), shoulder pain, and muscle activities of upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA) and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction in subjects with FHP. Methods: In total, 19 subjects (7 males, 12 females) with FHP were recruited. The subject performed the fifth phase of CCFE immediately after receiving SR. CVA, shoulder abduction ROM, shoulder pain, muscle activities of UT, LT, and SA, and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction were measured immediately after SR-CCFE. A paired t-test and Wilcoxon signed-rank test were used to determine the significance of differences in scores between pre- and post-intervention in the same group. Results: The CVA (p<.001) and shoulder abduction ROM (p<.001) were increased significantly post-versus pre-intervention. Shoulder pain was decreased significantly (p<.001), and LT (p<.05) and SA (p<.05) muscle activities were increased significantly post- versus pre-intervention. The LT/UT muscle activity ratio was increased significantly post- versus pre-intervention (p<.05). However, there was no significant change in UT muscle activity and SA/UT muscle activity ratio between pre- and post-intervention (p>.05). Conclusion: SR-CCFE was an effective intervention to improve FHP and induce downstream effect from the neck to the trunk and shoulders in subjects with FHP.

컴퓨터 작업시 책상과 체간사이 거리가 전방 머리 자세와 목과 어깨 근육들의 근 활성도에 미치는 영향 (Effect of Distance Between Trunk and Desk on Forward Head Posture and Muscle Activity of Neck and Shoulder Muscles During Computer Work)

  • PURPOSE: The purpose of this study was to investigate the effect of distance between trunk and desk on forward head posture and muscle activity of neck and shoulder muscles during computer work. METHODS: Twenty subjects who have healthy conditions were asked to perform computer work in two conditions (distance between trunk and desk was 0 and 15 cm). Forward head angle was analyzed with a SIMI motion analysis system. Surface electromyography recorded the upper trapezius and splenius capitis muscles. The significance of differences in distance between trunk and desk (0cm and 15cm) was tested by paired t-test. The alpha level was set at .05. RESULTS: The results showed that the change of forward head angle was significantly higher during the computer work when the distance between trunk and desk was 15cm than 0cm. The muscle activity of upper trapezius and splenius capitis was also significantly higher during the computer work when the distance between trunk and desk was 15cm than 0cm. CONCLUSION: Our study suggest that the distance between trunk and desk was should minimized for prevention of forward head posture during computer work.

  • PURPOSE: The aim of this study was to investigate the effect of scapular dynamic taping on pain, disability, upper body posture, and range of motion (ROM) in the postoperative shoulder. METHODS: Twenty-two patients who underwent acromioplasty and rotator cuff repair surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, n=11) and a control group (CG, n=11). For the EG, exercise therapy, manual therapy, and scapular dynamic taping were applied for 6 weeks; for the CG, only exercise therapy and manual therapy were applied for 6 weeks. Shoulder pain, disability, upper body posture, and ROM were evaluated at baseline, after 3 weeks of intervention, and after 6 weeks of intervention. Assessment tools included quadruple visual analog scale (QVAS) for level of pain; shoulder pain and disability index (SPADI) for functional disability level; forward head angle (FHA), forward shoulder angle (FSA), and pectoralis minor index (PMI) for upper body posture; and ROM testing. RESULTS: Significant differences were observed between the EG and CG in SPADI total scores; internal rotation and external rotation ROM of the glenohumeral joint ; FSA ; and PMI. All groups showed statistically significant improvement in QVAS; SPADI; flexion; abduction; external rotation and internal rotation ROM of the glenohumeral joint; FSA; and PMI. CONCLUSION: These results suggest that, for patients who have undergone acromioplasty and rotator cuff repair surgery, the addition of scapular dynamic taping during therapy is effective for improvement of shoulder disability level, ROM, and upper body posture.

Effects of Posture Correction Exercise on Muscle Activity and Onset Time during Arm Elevation in Subject with Forward Head and Rounded Shoulder Posture

  • PURPOSE: The aim of this study was to provide evidence for the treatment of Forward Head and Rounded Shoulder Posture (FHRSP) using posture correction exercises by comparing muscle activity and onset time around the neck and shoulder area during an arm elevation task. METHODS: The subjects were divided into FHRSP (21 persons) and non-FHRSP (19 persons) groups to measure muscle activity and onset time of muscle contraction. Wireless surface electromyography was used to assess the muscle activity and onset time of the right and left sternocleidomastoid (SCM), splenius capitis, anterior deltoid, middle deltoid, serratus anterior, upper trapezius, pectoralis major, and infraspinatus during an arm elevation task. After the pre-measurement, the participants performed the postural correction exercises, and then the post-measurement was conducted. RESULTS: After the posture correction exercises, there were significant differences in the muscle activity and onset time of all muscles in the FHRSP group. The results of the comparison of the muscle onset time during an arm elevation task demonstrated that after the postural correction exercises, the muscle onset time was significantly reduced in the right and left SCM and left splenius capitis, but there were no significant changes in the onset time of other muscles. CONCLUSION: The results of this study help us understand the change in muscle activities and muscle contraction onset time in a person with FHRSP when lifting the arm and suggest the relevant basis to apply the posture correction exercise in clinical settings.

동작관찰과 시각적 되먹임 융합 운동이 머리전방자세와 둥근어깨의 정렬, 통증과 기능에 미치는 영향 (The Effects of the Action Observation and Visual Feedback Convergence Exercise on the Alignment, Pain and Function of Forward Head Posture and Round Shoulder Posture)

  • 본 연구는 머리전방자세와 둥근어깨자세에 대한 일반 운동과 동작관찰, 시각적 되먹임의 효과를 알아보고자 한다. 머리전방자세와 둥근어깨자세를 가진 24명으로 하였으며, 일반 운동군(8명), 동작관찰군(8명), 시각적 되먹임군(8명)으로 무작위 배분하였다. 모든 운동군은 주 3회, 총 4주간 운동하였다. 운동 전 후 머리척추각(CVA), 둥근어깨자세(RSP), 시각적 상사척도(VAS), 경부장애지수(NDI)를 평가하였다. 연구결과 CVA는 일반 운동군과 동작관찰군에서 유의한 변화를 보였으며, RSP, VAS, NDI의 경우 모든 운동군에서 유의한 변화를 보였다. 운동군 간 변화량 비교에서는 VAS에서 동작관찰군이 일반 운동군과 시각적 되먹임군보다 큰 변화량을 보였다. 본 연구결과 동작관찰이 머리전방자세와 둥근어깨자세의 개선에 효과가 있을 것이라고 사료된다.
