25세 경산부에서 성교후 특별한 원인을 찾을 수 없는, 질후벽중앙부의 열상으로 shock에 빠진 1예와 29세 경산부에서 성교후 질후궁륭부 열상 및 골반 복막 열상의 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

An urinoma(uriniferous pseudocyst, pararenal pseudocyst) denotes an encapsulated collection of urine in the perirenal or paraureteral space. It was usually reported in relation to trauma and acquired obstructive uropathy but rarely reported in renal infection including renal candidiasis. The mechanism is believed due to rupture of fornix through weakened portion of suppurated kidney and pyelosinus backflow by increased intrapelvic pressure in obstructive uropathy and fungus ball obstruction. We report a case of urinoma in a premature as the first case in Korea which developed as a complication of renal candidiasis.

A dog (Maltese, 4-year-old, intact male) was referred to the hospital because of the multiple cabbage-shaped nodular masses on penis, preputial fornix and inner layer of prepuce with the free-roaming history. Those appearance was similar with the characteristic of transmissible venereal tumor (TVT). As a result, it was tentatively diagnosed as TVT by the veterinarian of a local clinic and treated with vincristine sulfate. However, the lesion did not regress. Histologically, the mass was consisted of fibrovascular connective tissue stalks and keratinized stratified squamous epithelium, and koilocytosis and intranuclear inclusion body were not shown in the epithelial layer. In addition, inflammatory changes were minimal in the tissue of mass. Based on these findings, this case was diagnosed as venereal squamous papilloma. As the treatments of TVT and papilloma differs, practitioners must be cautious with the diagnosis.

Lactobacillus species isolated from posterior fornix of the vagina of 410 Korean women, who visited several local clinics and the Family Health Clinic of the Institute of Reproductive Medicine and Population, Seoul National University in Seoul were identified by their physiological and biochemical characteristics. And the results were as follows. 1) Of the 410 women 297(72.4%) gave the positive culture for Lactobacillus. 2) Isolated 200 strains were differentiated into following species. L. salivarius(43 strains, 18.7%) and L. leichmannii(42, 18.3%) were the most common species followed by L. plantarum(34, 14.8%) L.brevis(22, 9.6%), L. fermenti(19, 8.3%), L. cellobiosus(15, 6.5%), L. acidophilus(14, 6.1%), L. delbrueckii(10, 4.3%) and L. casei(1, 0.4%) and there were 30 strains(13%) which could not have been identified by the methods employed in this study. 3) Age seems to have no influence on the distribution of Lactobacillus species in vagina and it seems like that the overall distribution of Lactobacillus species in vagina does not change during the active reproductive period of women.

Background The objective of this article is to evaluate clinical outcomes of combined orbital floor and medial wall fracture repair using a three-dimensional pre-bent titanium implant in an East Asian population. Methods Clinical and radiologic data were analyzed for 11 patients with concomitant orbital floor and medial wall fractures. A combined transcaruncular and inferior fornix approach with lateral canthotomy was used for the exposure of fractures. An appropriate three-dimensional preformed titanium implant was selected and inserted according to the characteristics of a given defect. Results Follow-up time ranged from 2 to 6 months (median, 4.07 months). All patients had a successful treatment outcome without any complications. Clinically significant enophthalmos was not observed after treatment. Conclusions Three-dimensional pre-bent titanium implants are appropriate for use in the East Asian population, with a high success rate of anatomic restoration of the orbital volume and prevention of enophthalmos in combined orbital floor and medial wall fracture cases.

Purpose: In general, confabulation is defined as confusion of reality with past events without apparent prompting, in association with disruption of the capacity for retrieval and encoding of memory. We report on a patient who showed spontaneous confabulation associated with injury of the Papez circuit following middle cerebral artery (MCA) infarction. Methods: A 67-year-old female patient suffered cerebral infarct resulting from spontaneous MCA territory. After onset of the MCA infarct, she showed severe memory impairment and provoked confabulation. The Papez circuit was reconstructed for evaluation of part of it using diffusion tensor tractography (DTT). Fractional anisotropy (FA), mean diffusivity (MD), and tract volume were measured. Results: The right thalamocingulate tract showed a significant decrement of FA value and tract volume, and an increment of MD value by more than two standard deviations of that of normal control subjects. The tract volume in the left fornix and mammillothalamic tract decreased by more than two standard deviations of that of normal control subjects. Conclusion: Injuries of the Papez circuit were demonstrated in a patient who showed severe memory impairment and provoked confabulation following MCA infarct. We believe that analysis of the Papez circuit tract using DTT is useful in elucidating the cause of provoked confabulation in patients with MCA infarct.

Malakoplakia is an uncommon chronic granulomatous inflammation that usually involves the urinary and gastrointestinal tracts, but rarely affects the female genital tract. We experienced a case of malakoplakia in a cervicovaginal smear in a 54-year-old woman. Colposcopic examination showed a friable, easily bleeding tissue in the uterine cervix and the vaginal fornix. The cervicovaginal smear consisted of numerous isolated histiocytes, polymorphonuclear leukocytes, lymphocytes, and plasma cells. The histiocytes had an abundant, granular, and degenerated cytoplasm with inflammatory cell debris. Michaelis-Gutmann bodies were readily identified.

Eye socket contracture is a well-known late complication of enucleation surgery, and the additional insult of radiotherapy at an early age causes even further fibrosis and scarring of the socket. Management of the contracted socket is challenging, and several methods have been proposed. We report a case of eye socket contracture after enucleation and radiotherapy in which multiple reconstructive procedures failed. The recurrent contracture caused difficulty in housing and retaining the eye prosthesis. We reconstructed the lower eyelid with a facial artery myomucosal flap and nasolabial flap, and the upper eyelid with a Fricke flap following reconstruction of the orbital rims (supraorbital and infraorbital rims with a calvarial bone graft, and further augmentation of the infraorbital rim with a rib bone graft). Cosmesis post-reconstruction was acceptable and the prosthesis was retained very well.

Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, biological mediators. Platelet Rich Plasma have been reported as a biological mediator which regulate activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the possibility of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar. 2 month later experimental group were PRP plus bovine bone and bovine bone only. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus bovine bone group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 4 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during early stage of periodontal tissue regeneration.

가바성 억제 신경세포는 해마의 정상적인 기능을 조절하는데 중요한 역할을 하며 해마 병변을 유발하는 중요한 요소이다. 본 연구는 in vivo 실험법을 사용하여 해마 CA1 영역에서의 전기 생리학적 반응을 측정함으로써 가바성 신경세포의 기능을 분석하고 이를 슬라이스 실험법과 비교하고자 하였다. Fimbria-fonix 전기자극시 전형적인 population spike가 나타났고 $10{\sim}M$ bicuculline 존재하에서는 전기자극에 의해 burst 형태의 population spike가 나타났다. Population spike의 크기는 자극 강도에 비례하였으며 그 숫자도 bicuculline 전극사용시와 같이 동일한 양상을 보였다. CA1 영역의 흥분성 수준을 측정하기 위해 paired-pulse 자극을 하였는데 짧은 자극 간격에서 억제성 반응을 보였고 burst형태의 afterdischarge를 나타내었다. CA1 영역에서 in vivo실험법을 사용한 가바성 신경세포반응의 결과는 추체세포의 흥분성 조절을 효과적으로 분석할 수 있으며 in vitro 실험법에 비해 기능적 평가가 더욱 이상적임을 알 수 있다.
