식도 중복 낭종 -수술 치험 1례- (Esophageal Duplication Cyst -A Case Report-)

  • Esophageal duplication cysts are uncommon, benign lesions of the esophagus. They are rare congenital foregut anomalies. To be considered an esophageal duplication cyst, a lesion must meet the following criteria:1 the cyst in the esophageal wall 2 the cyst is covered with a muscularis propria,generally of two layers and 3 the cyst has an epithelial lining consistent with that of the 4 week embryo, which may be columnar or pseudostratified columnar, and may be ciliated. Herein we report a case of an esophageal cyst located within the thoracic cavity, which is, to the best of our knowledge, the first case reported in Korea. Surgery is generally the treatment of choice for esophageal cyst, and was indicated in this case for the diagnosis and management of symptoms attributable to the cyst.

기관지성 낭종 -1례 보고- (Bronchogenic Cyst - 1 case report -)

  • Bronchogenic cysts are uncommon congenital lesions which are derived from primitive foregut. Most of the bronchogenic cysts may occur at the tracheal bifurcation, both main bronchi, the lung parenchyme and the mediastinum. We experienced a case of bronchogenic cyst with a esophageal stalk. The diagnosis was made by simple chest x-ray and confirmed by bronchoscopy and chest CT. On the chest CT findings, 6.8X4.8 cm-sized oval shaped mass was located on the right posteroinferior side of the carina, elevating the right main bronchus and the thin wall of the mass was enhanced with contrast materials. On the operative findings, the esophagus and the cyst were connected with a stalk and the cyst was filled with mucinous materials. And on the histological findings, the mass was lined with pseudostratified ciliated columnar epithelium. Thus we report this case of bronchogenic cyst with review of literatures.

  • Communicating bronchopulmonary foregut malformation (CBPFM)은 매우 드문 선천성 기형으로 격리된 호흡계 조직과 식도 또는 위장 사이의 누관이 특징적이다. 원시위장관에서 기원한 부속의 폐조직이 모든 종류의 bronchopulmonary furegut malformation 발생의 중요한 요인이 된다. 반복되는 폐렴이 있는 환자의 chest X-ray에서 낭성 병변이 보인다면 CBPFM을 생각해봐야 하며, 진단적 방법으로 식도조영술, 기관조영술, 컴퓨터 단층촬영, 자기공명영상 그리고 혈관조영술 등이 도움이 된다. 치료는 수술적 제거가 권장되며 예후도 좋다. 좌측 완전 심낭결손증과 동반되었으며, 이중식도에 연결된 외엽성 폐격리증과 기관지성 낭종을 보인 CBPFM 1예를 보고한다.

  • The bronchogenic cysts result from an abnormal budding or branching of the tracheo-bronchial tree from the primitive foregut. Bronchogenic cysts may be pulmonary or mediastinal. The most common location for a mediastinal bronchogenic cyst is in relation to the carina. They are usually solitary. They usually are thin walled and contain fluid that is most often clear. The cyst is lined by ciliated columnar epithelium. The bronchogenic cyst i~ usually asymptomatic. They can cause pressure symptoms. When they become infected, symptoms do occur. We report 3 cases of bronchogenic cysts experienced at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. Case I, a man of 20 year-old, had a cyst at the site between aortic arch and left pulmonary artery. He complained cough and dyspnea. The cyst wasn`t communicated with tracheobronchial tree. Case II was 55-year-old male who had had hemoptysis. A huge cyst was located within the lower lobe of left lung and removed by pulmonary lobectomy. There were not any symptoms in Case III that was 6-year old girl. That cyst was located just next to the right main bronchus.

승모판막 폐쇄부전을 유발한 기관지성 낭종 (Bronchogenic Cyst Causing Mitral Regurgitation)

  • 기관지성 낭종은 태아기에 원시전장으로부터 분화하는 비교적 드문 선천성 질환이며, 대부분 기관분기부, 양측 주기관지, 폐실질 그리고 종격동에서 발생한다. 40세 남자 환자로 호흡곤란과 흉부압박감을 주소로 입원하여, 컴퓨터 단층촬영 검사상 경계가 명확하고 크기가 7.2$\times$7.9 cm이며, 좌심방을 압박하는 균질의 종양이 발견되었고 심초음파 검사상 중등도의 승모판막 폐쇄부전을 보였다. 수술은 낭종을 완전히제거하였으며, 조직학적 검사상 기관지성 낭종으로 확진되었다. 추적관찰 결과 환자는 증상없이 잘 지내고 있었으며, 심초음파 검사상 승모판막 폐쇄부전은 경도로 호전되었다. 본 국립의료원에서는 승모판막 폐쇄부전증을 유발한 기관지성 낭종 1례를 치험하였기에 보고하는 바이다.

Bronchogenic Cyst of the Right Hemidiaphragm Presenting with Pleural Effusion

  • Bronchogenic cysts are developmental foregut anomalies usually located within the mediastinum or lung parenchyma. An isolated bronchogenic cyst of the diaphragm is very rare. Our case was a 56-year-old female patient who presented with pleuritic chest pain in her right chest. Chest and abdominal computed tomography revealed a large lobulated cystic mass that was accompanied with pleural effusion in the right lower hemithorax. The tumor showed focally calcified areas in the wall and abutted against the diaphragm. We performed complete excision of the cyst including a portion of the diaphragm attached to it. The pathological diagnosis was established as the bronchogenic cyst originating from the diaphragm. We report this case with a review of the literature.

산전 진단된 폐외분리증 2예 (Prenatally Diagnosed Extrapulmonary Sequestration - 2 cases -)

  • Congenital thoracic malformations such as intra- and extra-pulmonary sequestration, cystic adenomatoid malformation, congenital pulmonary airway malformation, malinosculation, bronchogenic cyst, reduplication cyst, and foregut cyst are frequently detected on routine prenatal ultrasound. There are some controversies about treatment for postnatally persistent pulmonary sequestration. Some authors recommend expectant long term follow up but most authors advocate elective surgical excision because of complication such as respiratory distress, infection, intrathoracic bleeding, haemoptysis, cardiac failure, and potential risk of malignancy. We experienced 2 cases of prenatally diagnosed extrapulmonary sequestration which were located in the subdiaphragmatic retroperitoneum. Resections were performed at 2 months and 4 months of age using intraabdominal approach. There were no complications. In conclusion, if the prenatally diagnosed extrapulmonary sequestration remained postnatally, early operation might reduce morbidity related to extrapulmonary sequestration and parental anxiety without any postoperative complication.

경부절게를 통한 기관지원성낭종 치료 (Bronchogenic cysts treated with the transcervical approach)

  • Bronchogenic cysts are congenital malformations of the bronchial tree, a type of bronchopulmonary foregut malformation. The presentation of the bronchogenic cyst is variable, making pre-operative diagnosis difficult. They aremostly asymptomatic orarefound incidentally when the chest is imaged. They can present as lower neck massesor mediastinal masses that may enlarge. They cause mass effect due to local compression and may result in tracheo-bronchial obstruction leading to air trapping and respiratory distress. The treatment is somewhat controversial, and in general,these lesions are treated using the transcervical or transbronchial approach. When these cysts arelocalized in the upper mediastinum, it may be possible to removethemusing the transcervical approach. In our three cases, the patientscomplained of mild dysphagia, foreign body sensation, and dyspnea. We report three cases of a large bronchogenic cyst in the lower neck and the upper mediastinum treated using the transcervical approach.

선천성 낭성 폐질환의 외과적 치료 (Surgical treatment of congenital cystic lung disease -Report of 47 cases-)

  • Congenital Cystic Lung Disease is a spectrum of closed related anomalies that arise during an early stage of embryonic lung bud maturation-namely bronchogenic cyst, congenital lobar emphysema, pulmonary sequestration and congenital cystic adenomatoid malformation. And they show similar surgical strategies. So they are called as the term bronchopulmonary-foregut malformations, firstly proposed by Gerle[1968]. From Aug. 1979 to Aug 1989, 47 patients were operated upon on Dept. of Thoracic & Cardiovascular Surgery at the CUMC. There were 21 females and 26 males ranging in age from age of 21 day to age of 56 year [15 cases under 15 years old]. 30 patients had bronchogenic cysts - 23 of intrapulmonary type, 7 of mediastinal type in location. Affected lobes and locations were as follows: 11 in upper lobe, 3 in middle lobe, 11 in lower lobe and anterosuperior, middle, and posterior mediastinal type were 3, 2, 2 respectively. There were 9 pulmonary sequestrations[all intralobar type] with the distribution of 5 in right lower lobe and 4 in left lower lobe. And associated anomalies were presented with arterial supply originating from thoracic aorta[8 cases], abdominal aorta[1 case] and with venous drainage into azygos vein[1 case]. They all were operated upon lower lobectomy [8 case], pneumonectomy[1 case] in case of pulmonary hypoplasia Congenital lobar emphysema and congenital cystic adenomatoid malformation had 4 cases respectively. Their affected lobes were as follows: the former were 3 in upper lobes, 1 in middle lobe and the latter were 3 in upper lobe, 1 in lower lobe. They were treated with lobectomy and segmentectomy. Diagnosis was aided by chest X - ray, bronchography, aortography, DSA and CT scan, They all were confirmed by pathologic exams. There were no hospital death but few minor morbidities such as, atelectasis-pneumonia[2], wound infection[2], prolonged chest tube placement[2]. We experienced surgical treatments of 47 cases for 10 years and reported them with literature review.

  • 저자들은 14세 여아에서 호흡곤란과 흉통이 발생하였으나, 긴장성 기흉등으로 잘못 인식되었던 선천성 낭종성 선종성 기형(CCAM)을 진단하여 수술적 방법으로 치료한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
