Factor Analysis for Foot Classification of Young Men and Women (20대 남녀의 발 유형화를 위한 요인분석)

  • The purpose of this study is to provide foot classification on young men and women by factor analysis. The sample for this work was chosen from data which were collected and measured by Size Korea during two years ($2003{\sim}2004$). In order to analyze and compare features of the foot of young men and women, analysis was performed about 911 subjects (male: 467, female: 444) on 24 parts such as height (8 parts), width (5 parts), thickness (1 part), circumference (3 parts), length (3 parts) and angle (4 parts). The result of this study can be applied in manufacturing and design of shoes and socks. Also, it will enable us to have fruitful information on considerable items during manufacturing and design of shoes and socks.

  • An extremely rare accessory bone of the foot located in the distal portion of the navicular that articulates with the medial cuneiform was observed. Os paracuneiforme is usually located medial to the medial cuneiform or the naviculocuneiform joint. Although os paracuneiforme is often referred to as an accessory bone around the foot, few cases of this type of accessory bone have been reported. This paper reports a patient with a painful Os paracuneiform who underwent surgical excision of the accessory bone.

  • The purpose of this research is to develop functional midsole that can restrain the heightening of plantar pressure in forefoot pressure so as to develop high-heeled shoes that can lessen foot ailments among women. The pumps shoes used for the research were of the same pumps type last. The variant was heel height, 3cm, 6cm, and 9cm, and the shoes were made in both normal midsole and functional midsole. The variant was applied to investigate the changes of foot pressure on forefoot and hindfoot according to heel height.'Heel Cup' was chosen for modification of functional midsolepumps type. to enlarge contact area in hindfoot, and 'Heel Posting Pad' was attached under sustentaculum tali to suppress the weight moving to forefoot pressure. If such functional parts are developed and used, it is possible to lessen the amount of Pmax or Impulse imposed by high-heeled pumps type on forefoot pressure. This can greatly lessen foot ailments, largely caused by high-heeled shoes, among women.

A Study on Changes in Biomechanical Characteristics of the Foot with Respect to Wedge-type Insole Thickness (키높이 인솔두께에 따른 족부의 생체역학적 특성변화에 대한 연구)

  • Recently, functional insoles of wedge-type it is for the young to raise their height inserted between insole and heel cause foot pain and disease. Additionally, these have a problem with stability and excessively load-bearing during gait like high-heel shoes. In this study, we compared the changes in biomechanical characteristics of foot with different insole thickness then we will utilize for the development of the insole with the purpose of relieving the pain and disease. Subjects(male, n = 6) measured COP(center of pressure) and PCP(peak contact pressure) on the treadmill(140cm/s) using F-scan system and different insole thickness(0~50 mm) between sole and plantar surface during gait. Also, we computed changes of stresses at the foot using finite element model with various insole thickness during toe-off phase. COP moved anterior and medial direction and, PCP was increased at medial forefoot surface, $1^{st}$ and $2^{nd}$ metatarsophalangeal, ($9%{\uparrow}$) with thicker insoles and it was show sensitive increment as the insole thickness was increased from 40 mm to 50 mm. Change of the stress at the soft-tissue of plantar surface, $1^{st}$ metatarsal head represents rapid growth($36%{\uparrow}$). Also, lateral moments were increased over the 100% near the $1^{st}$ metatarsal as the insole thickness was increased from 0 mm to 30 mm. And it is show sensitive increment as the insole thickness changed 10 mm to 20 mm. As a result, it was expected that use of excessively thick insoles might cause unwanted foot pain at the forefoot region. Therefore, insole thickness under 30 mm was selected.

Malignant Melanoma of the Foot (족부의 악성 흑색종)

  • Purpose: We reviewed the clinical finding of malignant melanoma of the foot in korean because it's advanced stage and extended lesion at diagnosis. Materials and Methods: Retrospective study was enforced about the 11 cases who has diagnosed to malignant melanoma of the foot from February 1995 to March 2004. The mean follow up period was 61 months. In this study we used age, sex, site, depth, histology, clinical stage, precursor lesion, misdiagnosis, interval to diagnosis, survival time, survival. Results: Average age was 58 years and number of female was six. Common site of involvement were heel of plantar surface (6 cases) and subungual area (2 cases). Depths of involvement were 0.3 to 10 mm, most common histological type was acral lentiginous melanoma (7 cases), stage 5 according to classification of Clark were 5 cases and stage 2 or more according to clinical staging were 8 cases. precursor lesion were benign melanocytic nevi (2 cases) and ill defined (9 cases). Chief complaint were increasing of size, color change, pain and ulceration. Conclusion: Malignant melanoma of the foot usually arise at nonvisible area and is easy to be misdiagnosed or delayed treatment. So it is hard to early diagnosis and have poor prognosis. So we need education and effort to early detection and diagnosis.

Factor Analysis for the Foot and Calf Growth of Primary-School Children (초등학생(初等學生)의 발과 하퇴부(下腿部) 성장(成長)에 관(關) 요인분석(要因分析))

  • For the purpose of investigating the factor of foot and calf growth of primary-school children who are fastgrowing during this period, a group of the 1st graders of primary-school had been the subject of this measurement in 1995 and thereafter they became the subject again in 1997 when they were the 3rd graders. Measurement was carried out in 30 items including height and weight and marthin-type measurer and Footprint were used for this measurement. As the result, in the part of calf, the measuring items of maximum calf circumference and knee circumference, and in the part of foot, the items of instep circumference, heel circumference, instep circumference, measured angle of big toe showed the highest growth. The average growth of length for 2 years appeared about 2cm and the parts of instep circumference, heel circumference had rapidly grown rather than in the part of foot circumference. Height growth of the part of toe was about 0.1 and in the factor analysis of foot measurement of 8-year and 10-year and children, the following findings appeared in both cases that height was related with the items of calf height and foot length and weight was related with calf circumference and breadth, foot circumference items.

A Comparative Study of Gait Characteristics between Single Axis Foot and Energy Storing Foot for Sports in Trans-tibial Amputee (하퇴절단자용 단축식 발과 스포츠용 에너지 저장형 발 보행 특성 비교연구)

  • This study examined the differences in spatio-temporal parameters, joint angle, ground reaction force (GRF), and joint power according to the changes of gait speed for trans-tibial amputees to investigate the features of the energy-storing foot for sports. The subjects walked at normal speed and at fast speed, wearing a single-axis type foot (Korec) and an energy-storing foot for sports (Renegade) respectively. The results showed that Renegade yielded faster gait speed as well as more symmetric gait pattern, compared to Korec. However, as gait speed was increased, there was no significant difference in kinematics, ground reaction force, and joint power between two artificial foots. This was similar to the results from previous studies regarding the energy-storing foot, where the walking velocity and gait symmetry have been improved. Nevertheless, the result of this study differed from the previous ones which reported that joint angle, joint power, and GRF increased as the gait speed increased except spatio-temporal parameters.

Analyses of Plantar Foot Pressure and Static Balance According to the Type of Insole in the Elderly

  • Objective: The purpose of this study was to investigate plantar foot pressure and static balance according to the type of insole in the elderly. Methods: Thirteen elderly (mean age: $67.08{\pm}2.25years$, mean height: $159.63{\pm}9.64cm$, mean body weight: $61.48{\pm}9.06kg$) who had no previous injury experience in the lower limbs and a normal gait pattern participated in this study. Three models of insoles of the normal, 3D, and triangle types were selected for the test. The Pedar-X system and Pedar-X insoles, 3.3 km/h of walking speed, and a compilation of 20 steps walking stages were used to analyze foot-pressure distribution. Static balance test was conducted using Gaitview AFA-50, and balance (opening eyes, closing eyes) was inspected for 20 s. One-way ANOVA was conducted to test the significance of the results with the three insoles. p-value of less than .05 was considered statistically significant. Results: The mean foot pressure under the forefoot regions was the lowest with the 3D insole during treadmill walking (p<.05). The mean value under the midfoot was the highest with the 3D insole (left: p<.05, right: p<.01). The mean value under the rearfoot was the lowest with the 3D insole (p<.001). The maximum foot pressure value under the foot regions was the lowest on both sides of the forefoot with the 3D insole. A statistically significant difference was seen only in the left foot (p<.01). The maximum value under the midfoot was the highest with the 3D insole (p<.001). No statistically significant difference was detected on the values under the rearfoot. In the case of vertical ground reaction force (GRF), statistically significant difference was seen only in the left side rearfoot (p<.01). However, static balance values (ENV, REC, RMS, Total Length, Sway velocity, and Length/ENV) did not show significant differences by the type of insole. Conclusion: These results show that functional insoles can decrease plantar pressure and GRF under the forefoot and rearfoot. Moreover, functional insoles can dislodge the overload of the rearfoot and forefoot to the midfoot. However, functional insoles do not affect the static balance in the elderly.

A Study on the Side Effect of Crude Drugs (상용 한약재의 부작용에 관한 연구)

  • As many crude drugs are used in the oriental medical field problems on the side effects of these drugs come to the front. To conduct delayed-type hypersensitivity we selected 29 kinds of drugs used frequently for therapeutic agents in oriental medical hospitals (Table I). The cell-mediated immune response was evaluated by measuring the foot pad swelling reaction and humoral immune response by measuring the antibody formation to these crude drugs. Mice were given these drugs intraperioneally for sensitization and challenged with same drug as used for sensitization respectively by intral dermal injection on the left and righ hind foot pad 4 days after senstization and then the foot pads were measured with the dial micrometer. The results were as follow; 1) Gentianae Scabrae Radix, Arecae Semen, Corydalis Tuber, and Paeoniae Radix were significant as delayed-type hypersensitivity inducers. 2) None of the crude drugs tested had effect on the induction of humoral immune response.

Recurred Diffuse Pigmented Villonodular Synovitis of Ankle Joint - Case Report - (재발한 족관절의 미만성 색소 융모 결절성 활액막염)

  • Pigmented villonodular synovitis (PVNS) in ankle is relatively uncommon. This disorder results in increased proliferation of synovium causing villous or nodular changes containing histiocytes, fibroblasts, multinucleated giant cell, and hemosiderin. PVNS is classified into two different type : localized and diffuse. Diffuse type of PVNS in ankle is more common than localized type. Also, recurrence of diffuse type is more frequent. We report a case of diffuse type of PVNS which was recurred soon after the excision.
