식품알레르기 아동 부모의 양육부담과 영향요인 (Parental burden of food-allergic children's parents and influencing factors)

  • 식품알레르기는 알레르기 행진의 시작 단계로 원인 식품의 섭취 제한으로 인하여 성장기 아동의 영양 불균형과 아동을 비롯한 가족구성원의 삶의 질을 저하시키는 원인이 될 수 있다. 이에 본 연구는 2세 이상의 영유아 및 학령기 식품알레르기 아동의 주양육자인 어머니를 대상으로 자녀의 식품알레르기로 인한 양육부담을 정량화하여 자녀의 식품알레르기가 부모의 삶의 질에 미치는 영향을 분석하고자 하였다. 식품알레르기 아동 부모의 양육부담은 정서적 고충, 일상생활 제약에 대한 2개의 하부 요인으로 세분화되며, 자녀의 연령 증가, 섭취 제한 식품의 수, 일부 알레르기질환 진단 여부가 양육부담의 감소 및 증가에 유의한 영향을 미치는 것으로 분석되었다. 이처럼 식품알레르기 아동은 식품알레르기가 다른 알레르기질환으로 이행되지 않도록 전문 의료진에 의한 진단과 증상관리가 필수적이며, 환아를 비롯한 가족 구성원에 대한 주변의 지지와 간호 중재가 동반되어야 한다. 또한 성장기의 식품알레르기 아동은 식품 항원에 대한 식품경구유발시험 등의 정확한 임상 진단을 통해 필수 영양소를 포함한 식품의 과도한 섭취 제한을 방지해야 한다. 지역사회와 의료기관은 식품알레르기의 예방 및 증상완화를 위한 맞춤형 영양교육, 대체 식품에 대한 정보 제공 등의 현실적 지원과 정서적 지지를 병행해야 한다. 이처럼 다양한 형태의 사회적 노력과 제도적 방안을 통해 식품알레르기 아동의 삶의 질을 향상시키고 부모의 양육부담 대처능력을 강화해야 할 것이다.

제주지역 초등학교 고학년 학생들의 식품알레르기 유발표시제에 대한 인식도 및 영양교육 운영 실태 조사 (Perception on food allergy labelling and management of nutritional education among higher grade elementary school students in Jeju area)

  • 본 연구에서는 제주지역 6개 초등학교를 임의 선정하여 초등학교 4, 5, 6학년 학생 1,500명을 대상으로 2014년 7월 설문조사를 실시, 최종 1,000부를 분석하여 식품알레르기 유병률을 알아보고 식품알레르기에 대한 지식정도, 학교급식 서비스를 이용하는 학생들의 식품알레르기 유발표시제에 대한 인식도와 그에 따른 학교급식의 대체식 운영현황 및 영양교육의 관리 실태를 조사하였다. 연구결과는 다음과 같다. 1) 조사대상자는 남학생 49.6%, 여학생 44.2%이었고, 4학년 28.4%, 5학년 27.5%, 6학년 44.1% 이었다. 식품알레르기를 경험한 적이 있는 학생이 16.7%이며, 여학생이 19.2%, 남학생이 14.1%로 여학생이 남학생보다 식품알레르기를 경험한 학생이 유의적으로 많은 것으로 나타났다. 이들 중 의사진단을 받은 학생은 26.4%로 나타났다. 2) 식품알레르기의 증상으로는 두드러기, 가려움, 구토, 설사, 호흡곤란 순으로 나타났으며, 알레르기 유발 식품으로는 우유, 게, 새우, 땅콩, 밀과 계란, 대두, 메밀과 돼지고기, 복숭아, 토마토, 아황산염 등의 순으로 조사되었다. 3) 식품알레르기 지식 점수는 여학생이 남학생보다 높은 점수를 기록하였고, 학년별로는 학년이 올라갈수록 지식점수 또한 유의적으로 높아짐을 관찰하였다. 식품알레르기와 관련 있는 영양소에 대한 지식 정답률만이 식품알레르기 경험 여부에 따라 차이를 보였고, 전체적인 식품알레르기 지식점수는 식품알레르기 경험 유무에 따라 차이를 보이지 않았다. 4) 학교급식 식품알레르기 유발표시제 인식도의 평균점수는 5점 만점에 2.25점으로 비교적 낮게 나타났으며, 성별과 학년 그리고 식품알레르기 경험유무에 따른 식품알레르기 유발표시제에 대한 인식도의 차이는 관찰되지 않았다. 5) 성별과 학년에 따른 식습관 평균점수의 유의적인 차이는 없었으나, 식품알레르기 경험 유무에 따라서는 식품알레르기를 경험한 학생의 식습관 점수가 그렇지 않은 학생보다 유의적으로 낮게 나타났다. 6) 식품알레르기 유발 학생들에 대한 학교급식의 관리실태 결과, 20.4%의 학생이 대체식을 제공받고 있었으며, 대체식으로 제공받은 음식으로는 김, 계란후라이, 멸치볶음, 두부 등 조사되었다. 7) 조사대상자 중 30.8%만이 식품알레르기 관련 영양교육을 받은 경험이 있으며, 교육방법으로는 가정통신문과 학교홈페이지 등과 같은 간접적인 방법을 통한 교육으로 나타났다. 이상의 연구결과를 토대로 살펴볼 때, 초등학교 고학년 학생과 학부모 및 담임교사 등으로 대상을 확대하여 학교차원에서의 보다 체계적이고 세심한 식품알레르기 교육이 실시되어야 할 것이며, 학교급식의 식품알레르기 유병학생을 대상으로 성장기에 필요한 영양소들을 충분히 공급할 수 있도록 다양한 대체식의 개발 방안이 모색되어야 할 것으로 사료된다.

Identification of protease-resistant proteins from allergenic nuts using two-dimensional gel electrophoresis and mass spectrometry

  • Nuts are one of the most common sources of allergies in individuals of all ages. In order for a particular protein to render an allergic reaction, it must resist proteolytic digestion by intestinal enzymes. In this study, three well-known allergenic nuts, almonds, cashew nuts, and peanuts, were used as samples, and enzyme digestion with Bacillus protease and porcine pepsin was tested. A proteomic approach using two-dimensional gel electrophoresis and an MS/MS analysis was applied to visualize and identify the proteins that were resistant to enzyme digestion. Among the 150 protein spots tested, 42 proteins were assigned functions. Due to the lack of genomic databases, 41% of the identified proteins were grouped as hypothetical. However, 12% of them were well-known allergens, including AraH. The remainder were grouped as storage, enzymes, and binding proteins.

프로바이오틱스에 의한 알레르기 질환 제어 및 안전성 (Control of Allergy with Probiotics and Its Safety)

  • Allergic diseases, including atopic dermatitis, asthma, allergic rhinitis, and food allergies, could be caused by dysbiosis that results in an immune system imbalance. The incidence of allergic diseases has been increasing and they are now one of the most common diseases throughout the world. Recently, probiotics have been suggested as an alternative intervention for the prevention and treatment of allergic diseases. Probiotics are endogenous microflora with functional effects within hosts. They have various clinical and immunological capacities and have recently been considered as a supplement for the treatment and prevention of allergic diseases. Probiotic bacteria modulate immune cells such as Th1, Th2, and regulatory T cells that are correlated with protection against atopic dermatitis, however, safety concerns for the use of probiotics have been raised. Therefore, further research is needed to clarify the efficacy and safety of probiotics in the treatment of allergic diseases.

식품알레르기 연구를 위한 동물모델의 개발 (Studies on Animal Models of Food Allergy)

  • Food allergy is defined as an immunologically-mediated adverse reaction to food.The food allergy as a clinical entity has been recognized for many years, although there is yet no general consensus as to the incidence of this syndrome. One difficulty in studying food allergies has been the lock of a reasonable animal model in which reactions could be induced by orally administrating foods. It has been generally accepted that the initial target for an immediate reaction to food is the mast cells, within the gastronitestinal mucosa, and such cells are sensitize in vivo by food-specific immunoglobulin(Ig) E. Degranulation of these cells facilitates the entry of an antigenic epitope into the lymphatic system and blood stream, thereby causing further degranulation of the mast cells and basophils throughout the boy. Accordingly, the author attempted to develop an animal model that is indicative of evaluating IgE-mediated immediate hypersensitivity. It is also necessary to evaluate the effects of nutritional envioronments on dietary protein-dependent allergy and the regulatory mechanisms of dietary fats on IgE-mediated immune response. In this review, animal models to evaluate a food ingredient, effects of dietary fats and curcuminoids, milk whey protein hydrolysates on allergic reaction, and effect of dietary fat in splenic immune cells are presented.

Status, Antimicrobial Mechanism, and Regulation of Natural Preservatives in Livestock Food Systems

  • This review discusses the status, antimicrobial mechanisms, application, and regulation of natural preservatives in livestock food systems. Conventional preservatives are synthetic chemical substances including nitrates/nitrites, sulfites, sodium benzoate, propyl gallate, and potassium sorbate. The use of artificial preservatives is being reconsidered because of concerns relating to headache, allergies, and cancer. As the demand for biopreservation in food systems has increased, new natural antimicrobial compounds of various origins are being developed, including plant-derived products (polyphenolics, essential oils, plant antimicrobial peptides (pAMPs)), animal-derived products (lysozymes, lactoperoxidase, lactoferrin, ovotransferrin, antimicrobial peptide (AMP), chitosan and others), and microbial metabolites (nisin, natamycin, pullulan, ε-polylysine, organic acid, and others). These natural preservatives act by inhibiting microbial cell walls/membranes, DNA/RNA replication and transcription, protein synthesis, and metabolism. Natural preservatives have been recognized for their safety; however, these substances can influence color, smell, and toxicity in large amounts while being effective as a food preservative. Therefore, to evaluate the safety and toxicity of natural preservatives, various trials including combinations of other substances or different food preservation systems, and capsulation have been performed. Natamycin and nisin are currently the only natural preservatives being regulated, and other natural preservatives will have to be legally regulated before their widespread use.

광주광역시 광산구 보육시설 유아의 식행동과 식품알레르기 발생 현황 (Dietary Behaviors of Preschool Children and Food Allergy Prevalence of Preschool Childcare Facilities in Gwangsan-gu, Gwangju Metropolitan City)

  • This study aimed to evaluate dietary behaviors and food allergy status of preschool children in Gwangsan-gu, Gwangju Metropolitan city. The survey included 592 preschool children aged 1-5 years old in 31 childcare facilities. General characteristics, feeding type, complementary feeding, food allergy, dietary behaviors based on the NQ-P questionnaire, and dietary habits of their family were considered. Exclusively breast-fed children was 32.2% of the subjects and the breast feeding duration was 6.5 months. Subjects who had complementary feeding within 6 months was 77.2% and starting time of complementary feeding was 6.1 months. Fifteen percent of the subjects had food allergies and foods that induced allergy were instant foods, eggs, milk and dairy products, nuts, seafood. Food allergy was not related to breast feeding nor complementary feeding. The NQ-P score and its 3 factors including 'balance', 'moderation', and 'environment' were 59.9, 61.1, 56.0, 62.6, respectively. There were positive relations between children's dietary behaviors and family dietary habits such as breakfast eating frequency and meal regularity. As age of children increased, instant food intake increased and breakfast eating frequency decreased. Proper nutrition education is needed to children, their parents, and their care givers at childcare facilities to improve children's dietary behavior and health.

IgE-mediated food allergies in children: prevalence, triggers, and management

  • Food allergy (FA) is a serious health problem, and severe FA such as food-induced anaphylaxis can often be life threatening. The incidence of FA has been increasing especially in children. They usually develop early in life and affect up to 10% of children. The 2 most common food allergens worldwide are milk and eggs, while the third one varies depending on the countries: peanuts in the United States and Switzerland, wheat in Germany and Japan, tree nuts in Spain, sesame in Israel, and walnuts in Korea. These common food allergens are different and difficult to identify because of differing study methodologies, population, geography, age, and dietary exposure patterns. The current management of FA relies on the strict avoidance of culprit allergens, the prompt treatment of allergic reactions, including epinephrine use for food-induced anaphylaxis, monitoring, and education to prevent further reactions. Newer approaches for tolerance induction to FA and FA immunotherapy have been under investigation but are not yet ready for real-world application. Thus, consistent and systematic education of patients, caregivers, and food-handling people is of primary importance for the management and prevention of FA reactions. This review assesses and compares IgE-mediated FA in children in Korea and other countries, with a focus on summarizing the prevalence, common triggers, and management of FA.

The Role of the Immune System in the use of Probiotic Lactic Acid Bacteria in Preventing and Treating Allergic Diseases

  • The immune system is generally divided into the innate and the adopted immune systems, both protecting the body from pathogens. Recently, allergies, a disease associated with an imbalanced immune system, have increased rapidly in developed countries. Prevailing symptoms of allergic diseases are eczema, allergic rhinitis, asthma, inflammatory bowel disease, and food allergy. Probiotic bacteria, mainly consisting of lactic acid bacteria, are used in the prevention and treatment of allergic diseases. The function of them is to stimulate the intestinal immune cells and form a complex signal network to activate other immune cells. Beneficial health effects of probiotics are based on the hygiene hypothesis, which suggests that sanitary environment is important for health, but limited exposure to environmental factors increases allergic diseases. An immunoregulatory effect of probiotic bacteria is demonstrated by controlled trial, animal model, in vitro, in vivo and ex vivo designs. However, the immunoregulatory effect of probiotic bacteria is controversial because it depends on probiotic strains, a dose and a type of diseases. In this review, we discussed clinical evidences on immunoregulatory effects of probiotic bacteria.
