양방향 상행대정맥-폐동맥 단락술 후 폐동정맥루(Pulmonary Arteriovenous Fistula)가 발생한 환자에서 시행한 액와동정맥루 (Axillary Arteriovenous Fistula) 형성술 - 2예 보고 - (Creation of an Axillary Arteriovenous Fistula in Patients with the Pulmonary Arteriovenous Fistula after a Bidirectional Cavopulmonary Shunt - 2 cases -)

  • 양방향 상행대정맥-폐동맥 단락술 후 폰탄 수술의 위험이 큰 2명의 환자에서 청색증의 증가와 운동 능력의 감소를 보여 액와동정맥루 형성술을 시행하였다. 수술 후 두 환자 모두 수술 후 1일째 일반 병실로 전원되었으며, 수술 후 각각 4일, 3일에 퇴원하였다. 두 증례 모두 수술 후 volume loading의 소견은 없었고, 8개월째 추적 관찰 중이며 청색증의 감소와 운동 능력의 향상을 관찰할 수 있었다. 그러나 수술 후 6개월째 시행한 폐관류스캔이나 contrast심초음파 검사에서 폐동정맥루가 아직 충분히 경감되지 않아, 향후 이에 대한 면밀한 추적 관찰이 필요할 것으로 생각한다.

  • Coronary sinus ostial atresia (CSOA) with persistent left superior vena cava (LSVC) in the absence of an unroofed coronary sinus is a benign and rare anomaly that may be taken lightly in most instances. However, if overlooked in patients undergoing univentricular heart repair such as bidirectional Glenn or Fontan-type surgery, fatal surgical outcomes may occur due to coronary venous drainage failure. We report a case of CSOA with a persistent LSVC that was managed through coronary sinus rerouting during a total cavopulmonary connection, and provide a review of the literature regarding this rare anomaly.

양대동맥 우심실기시증의 전교정술 - 45예 보고 - (Total Correction of Double-Outlet Right Ventricle [DORV]: Report of 45 cases)

  • Forty-five patients with double-outlet right ventricle[DORV] underwent complete intracardiac repair between July, 1983 and June, 1989. Patients with complete atrioventricular canal, atrioventricular discordance and uni-ventricular heart were excluded. The 32 male and 13 female patients ranged in age from 3 months to 15 years[mean 4 years]. Thirty-two patients had pulmonary stenosis. The early mortality was 11.ltd[5 /45] None of 27 died after a completely intraventricular repair. The mortality was 20%[1/5] for repair using transannular patch, 20% [1/5] for REV operation, 33.3%[1/3] for repair including extracardiac valved conduit, and 50% [1/2] for Jatene operation, respectively. Two modified Fontan procedures were performed without mortality. One died after Senning operation. Causes of early deaths included high residual right ventricular pressure[one patient] small left atrial and left ventricular volume[one patient], persisting severe pulmonary hypertension [one patient] and low cardiac output of unknown cause [two patients]. Complete heart block developed in one patient. Two late deaths occurred among the 40 operative survivors [5.0Po] from persisting severe pulmonary hypertension and bleeding at reoperation. Our results indicate that significant defects can be repaired with low mortality and morbidity.

  • To understand the local fluid dynamics for different desists of Fontan operation, five models were made out of Pyrex glass to facilitate in-vitro study. Model I, II and III have same position of the center of the anastomosis of the IVC( inferior vena cava) with that of the SVC(superior vena cava), but Model IV and V have 10 mm offset between them. Also the anastomotic junction angles are different(Model I and $IV:90^{\circ}$, Model II and $V:70^{\circ}$, Model $III:45^{\circ}$). These models were then connected to a flow loop for flow visualization study. In Model I any dominant vortex was not seen in the central region of the juntion, but a large unstable vortex was created in the Model II and III. In Model IV and V a significant stagnation region was created in the middle of the offset region. It also showed that the flow direction from the IVC and SVC to the LPA(left pulmonary artery) and RPA(right pulmonary artery) highly depends on the offset of the junction rather than the anastomotic junction angle.

기능적 단심실 환자에 대한 심장내 외측통로 폰탄술식의 중기 수술성적 (Mid-term results of IntracardiacLateral Tunnel Fontan Procedure in the Treatment of Patients with a Functional Single Ventricle)

  • 서울대학교 어린이병원 흉부외과에서는 1990년부터 1996년까지 104례의 기능적 단심실을 가진 선천성심기형 환아에 대하여 심장내 외측통로술을 이용한 완전폰탄술을 시행하였다. 환아의 연령 및 체중분포는 각각 평균 35.9(범위 10-72) 개월, 12.8 (범위 6.5-37.8) kg이었다. 술전진단은 삼첨판폐쇄증(18), 단심실연결을보이는 중복개구심실(53) 및 기타 기능적 단심실을 동반한 복잡심기형(33)이었다, 50례의 환아에 대하여 체폐동맥 단락술 (37), 폐동맥밴딩(13), 외과적 심방중격절제술(15), 동맥전환술(2), 대동맥하 누두부제거술(2), 총폐정맥이상연결증(2), 폐동맥-대동맥봉합술(Damus-Stansel-Kaye, 1) 등의 고식술이 시행되었다. 완전폰탄술식전 19례의 양방향성 체정맥-폐동맥단락술과 1례의 전(全)체정맥-폐동맥단락술(Kawashima procedure)이 진행되었다. 술전 혈역학소견상, 평균 폐동맥압/폐혈관저항은 14.6 (범위 5-28mmHg) / 2.2 (범위0.4-6.9)wood.unit였으며, 폐혈류/체혈류비가 평균 0.9 (범위0.3-3.0)였다. 이완기말심실압은 평균 9.0 (범위 3.0-21.0) mmHg였고 동맥혈의 산소포화도는 평균 76.0 (범위 45.6-88.0) %였다. 수술은 분계능(terminal crest) 2cm 외측으로 우심이부터 우심방-하공정맥경계부에 종절개를 가하고 하공정맥개구부부터 상공정맥개구부 또는 우심이까지 Gore-Tex 인조도관을 이용하여 외측통동을 형성시키는 방법으로 시행하였으며 필요한 경우 통로상에 4-5.5 mm 직경의 구멍을 만들어 주었다. 동시에 시행한 술식은, 폐동맥성형술(22), 심방중격절제술(21), 폐정맥이상연결증 교정(4), 영구적인 인공심박동기거치(3) 등이었고, 32례에 대하여 통로내 구멍을 만들어 주었으며 그중 1례는 조절형(adjustable)으로 시행하였다. 심방-폐동맥 연결 방법으로 시행한 폰탄술 후 4년후에 발생한 재발성 난치성 상심실형 부정맥환아 1례에 대하여 외측통로형의 변환 폰탄술식이 시행되었다. 수술 직후 혈역학 소견상 평균 폐동맥압, 이완기말심실압, 실온에서 동맥혈의 산소포화도가 각각 12.7 (8-21)mmHg, 7.6 (범위4-12)mmHg, 89.9 (범위68-100) %였다. 병원사망율은 6.7 (7/104) %였고 술후 합병증으로 지속적인 늑막삼출(11), 부정맥(8), 유미흉(9), 중추신경계손상(5), 감염 및 염증(5), 급성신부전(4)이 발생하였다. 평균 27.2 (범위1-85) 개월동안의 외래 추적결과 5명의 만기 사망이 있었다. 저자등은 본연구결과를 토대로 심장내 외측통동폰탄술식이 기능적 단심실 환자에 대하여 비교적 낮은 사망율 및 합병증과 우수한 혈역학으로 시행될수 있는 수술방법이란 사실을 입증하였다.

소아 심혈관수술 후의 횡격막마비 (Phrenic Nerve Paralysis after Pediatric Cardiovascular Surgery)

  • From March 1986 to August 1992, 18 patients underwent diaphragmatic plication for the diaphragmatic paralyses complicating various pediatric cardiac procedures. Age at operation ranged from 16 day to 84 months with mean age of 11.8 months. In order of decreasing incidence, the primary cardiac procedures included modified Blalock-Taussig shunt [ 5 ], Arterial switch operation [ 4 ], modified Fontan operation [ 2 ], and others [ 7 ]. The suspicious causes of phrenic nerve injury included overzealous pericardial resection [ 7 ], direct trauma during the procedure [ 6 ], dissection of fibrous adhesion around the phrenic nerve [ 3 ] and unknown etiology [ 2 ]. The involved sides of diaphragm were right in 10, left in 7 and bilateral in one. The diagnosis was suspected by the elevation of hem-idiaphragm on chest x-ray and confirmed by fluoroscopy. The interval between primary operation and plication ranged from the day of operation to 38 postoperative days [mean : 14 days]. The method of plication were "Central pleating technique" described by Schwartz in 16 and other techniques in 2. Five patients expired after plication and the cause of death were not thought to be correlated directly with the plication itself. In the remaining 13 survivors, extubation or cessation of positive ventilation could be done between the periods of the day of plication and 14th postoperative days [mean; 3.8day]. We have made the following conclusions : 1] Phrenic nerve paralyses are relatively common complication after pediatric cardiac procedures and the causes of phrenic nerve injury are mostly preventable; 2] Phrenic nerve palsy is associated with corisiderable morbidity; 3] diaphragmatic plication is safe, reliable and can be applicable in patients who are younger age and require prolonged positive pressure ventilation.ntilation.

CT and MRI for Repaired Complex Adult Congenital Heart Diseases

  • An increasing number of adult congenital heart disease (ACHD) patients continue to require life-long diagnostic imaging surveillance using cardiac CT and MRI. These patients typically exhibit a large spectrum of unique anatomical and functional changes resulting from either single- or multi-stage palliation and surgical correction. Radiologists involved in the diagnostic task of monitoring treatment effects and detecting potential complications should be familiar with common cardiac CT and MRI findings observed in patients with repaired complex ACHD. This review article highlights the contemporary role of CT and MRI in three commonly encountered repaired ACHD: repaired tetralogy of Fallot, transposition of the great arteries after arterial switch operation, and functional single ventricle after Fontan operation.

소아 심혈관 수술 후 발생한 횡격신경마비가 술후 호흡관리에 미치는 영향 (The Effect of Phrenic Nerve Paralysis After Pediatric Cardiac Surgery on Postoperative Respiratory Care)

  • 1990년 1월부터 1995년 12월가지 43명의 환아가 다양한 심혈관수술로 인해 유발된 횡격막 마비로횡 격막 습벽형성술을 시행 받았다. 환아의 평균연령은 11.1개월이었고, 남녀 비율은 31:12였다. 횡격막 마비의 원인이 된 수술로는 변형 Blalock-Taussig단락술 및 활로씨 4징증의 전교정술이 각각 7례로 가장 많았고, 기타 동맥 전환술 6례, 심실중격결손을 동반한 폐동맥 폐쇄증에서의 unifocalization 및 변형 Fontan수술, 심실중격결손 교정 등이 각각 3례씩이었다. 마비된 부위는 우측이 17례, 좌측이 23례, 양측성이 3례였으며, 원인은 대부분 과도한 심낭 절제 및 심낭 절개면 부위의 전기소작으로 추정되었다. 원인이 된 수술후 횡격막 습벽형성술까지의 기간은 수술 당일로부터 98일까지로 대부분 2주이내에 습 벽형성술이 이루어졌다 수술방법은 2례를 제외하고 모두 횡격막 신경분지를 피해서 횡격막을 접어주는 central pleating technique 이 적용되었다. 10명의 환아가 습벽형성술후 사망했으며(조기: 7, 만기: 3), 사망원인은 전례 에서 습벽 형성술과는 무관한 것으로 사료되 었다. 36명의 조기생존자들은 습벽 형성술후 1일에서 24일 사이에 양압호\ulcorner으로 부터 벗어날 수 있었다(평균 4.5일). 6명의 환아가 습벽 형성술후 8개 월에서 52개월 사이에 fluoroscopy를 시행하였으며, 대부분 횡격막의 위치 및 운동성이 양호하였다. 결 론적으로 불가역적인 횡격막 신경 손상이 없다면 횡격막 습벽형성술은 단기적으로는 환아의 양압호흡 의존기 간을 단축시키고 장기적으로는 횡격막 기능의 완전회복을 유도할 수 있다.

Criss-Cross heart 를 동반한 심장기형의 수술 치험 4례 보 (Surgical experience in Criss-Cross heart: report of 4 cases)

  • The criss-cross, a term first used by Anderson and Ando , is a rare cardiac malformation in which the systemic and pulmonary blood streams cross at the atrioventricular level, without mixing. We have surgically experienced four cases of crossed atrioventricular connection, three concordant and one discordant, at Seoul National University Hospital from July 1982 to March 1984. The atrial situs, the atrioventriculoarterial connection, the spatial position of the ventricles, and associated cardiac anomalies were analyzed. We have performed right Blalock-Taussig shunt for case 1, modified Fontan operation for case 2, patch closure of VSD for case 3, and septation of common ventricle with primary closure of ASD for case 4. The relatively good postoperative results in these patients were gratifying and suggest that cardiac anomalies associated with this rare malformation can be successfully repaired without further risk. To our knowledge, this is the first report of surgical experience for criss-cross heart in the Korean literature.

변형 Fontan 술식을 이용한 선천성 심기형증 치험 2 (Direct Atriopulmonary Anastomosis - Report of 2 cases -)

  • Recently, direct atriopulmonary anastomosis was performed in 2 patients without using pericardium or prosthetic material. Case 1 A 13-year-old male patient whose diagnosis was tricuspid atresia [type lb] received direct atriopulmonary anastomosis successfully with good clinical and hemodynamic results. Nine months after operation, he has an increased exercise tolerance. Case 2 A 4-year-old female patient whose diagnosis was DORV with pulmonary atresia in A-V discordance had a formidable surgical challenge. A large-diameter direct atriopulmonary anastomosis was performed after the closure of the right atrioventricular valve. Her postoperative course has been even in 20th postoperative day.
