당뇨환자의 자기관리행위 및 자기효능감에 대한 추후 전화상담의 효과 (The Effects of Telephone Consulting Follow-up on Self care behaviors and Self-efficacy in Diabetic Patients)

  • This study was conducted to identify the effects of telephone consulting follow- up on self care behaviors and self efficacy in diabetic patients after discharge. The subjects for this study were consisted of 39 patients and twenty subjects assigned to the experimental group received the telephone consulting follow-up after discharge once a week during 4 weeks with diabetic education before discharge and nineteen subjects assigned to the control group received diabetic education before discharge without telephone consulting follow-up. The results of this study are as follows: 1. The experimental group which received the telephone consulting had higher self care behaviors scores than the control group (t=4.92, p=.00). 2. The experimental group which received the telephone consulting had higher self efficacy scores than the control group (t=4.71, p=.00). 3. The relationship between self efficacy and self care behaviors showed a significant correlation in the subjects. In conclusion, the telephone consulting follow-up improved self care behaviors and self efficacy, therefore this intervention can be effective in promoting the care of diabetic patients

Long-term follow-up study and long-term care of childhood cancer survivors

  • The number of long-term survivors is increasing in the western countries due to remarkable improvements in the treatment of childhood cancer. The long-term complications of childhood cancer survivors in these countries were brought to light by the childhood cancer survivor studies. In Korea, the 5-year survival rate of childhood cancer patients is approaching 70%; therefore, it is extremely important to undertake similar long-term follow-up studies and comprehensive long-term care for our population. On the basis of the experiences of childhood cancer survivorship care of the western countries and the current Korean status of childhood cancer survivors, long-term follow-up study and long-term care systems need to be established in Korea in the near future. This system might contribute to the improvement of the quality of life of childhood cancer survivors through effective intervention strategies.

  • Purpose: The purpose of this study was to examine the effect of follow-up care on the social support, self-esteem and maternal confidence in mothers of low birth weight infants. Method: This study applied a quasi-experiment study method to examine the pre-post intervention effects. Eighty-eight mothers whose infants were below 2.5kg of Low birth weight were studied. The period for the data collection was from July to December in 2003. Self Reported Survey and Face to Face Interview by the highly trained home Visiting Nurses were used for this study. Result: Most of the participants reported statistically significant improvements in the score of social support, self-esteem and maternal confidence after they received the intervention. Conclusion: Longitudinal study for the mothers of low birth weight infants might reveal more comprehensive findings. Moreover, follow-up care, which is linked with community health care services, is needed to continue the post hospital care services. Additionally, qualified and professional intervention programs should be provided to consider the characteristics of mothers of low birth weight infants.

  • Purpose: The objective of this study was to identify the effects of a telephone follow-up program on patient compliance, urea kinetic modeling and incidence of infection in patients with continuous ambulatory peritoneal dialysis (CAPD). Method: A pretest-posttest design with a nonequivalent control group was used to examine the effects of a telephone follow-up program for 12 weeks in a group of 39 CAPD patients. The telephone follow-up group of 17 patients received telephone follow-up 10 min once a week for 1 month and 10 min twice a month for 2 months during 12 weeks compared to control group of 22 who patients received usual care. Results: The telephone follow-up group showed no significant changes in patient compliance and urea kinetic modeling, although patients with CAPD receiving a telephone follow-up showed reduced incidence of exit site infection during the intervention compared with the control group. Conclusion: Telephone follow-up program for CAPD patients may decrease the incidence of exit site infection. Future studies with a larger sample be done utilizing diet and exercise reinforcement program combined with telephone follow-up as a nursing intervention for patients with CAPD.

Spiritual Care Training for Mothers of Children with Cancer: Effects on Quality of Care and Mental Health of Caregivers

  • Background: The purpose of this study was to explore the effectiveness of a spiritual care training package in maternal caregivers of children with cancer. Materials and Methods: This study was a quasi-experimental study with pretest and posttest design consisting of a sample of 42 mothers of children diagnosed as having cancer. Participants were randomly assigned to either an experimental or a control group. The training package consisted of seven group training sessions offered in a children's hospital in Tehran. All mothers completed the Spirituality & Spiritual Care Rating Scale (SSCRS) and the Depression, Anxiety and Stress Scale (DASS-21) at pre and post test and after a three month follow up. Results: There was significant difference between anxiety and spiritual, religious, Personalized care and total scores spiritual care between the intervention and control groups at follow-up (P<0.001).There was no statistically significant difference in stress and depression scores between the intervention and the control groups at follow-up. Conclusions: Findings show that spiritual care training program promotes spirituality, personalized care, religiosity and spiritual care as well as decreasing anxiety in mothers of children with cancer and decreases anxiety. It may be concluded that spiritual care training could be used effectively in reducing distressful spiritual challenges in mothers of children with cancer.

  • Advances in neonatal care have been responsible for the improved survival of prematurity but have not resulted in decreased morbidity. Once the high-risk infants is discharged from the hospital, his or her many special care needs do not cease. A well-coordinated multidisciplinary approach is essential in the follow-up care of these infants. Special attention must be given to their growth and nutrition, immunization, vision and hearing, and sequelae of illnesses experienced during the neonatal period. The role of pediatrician in helping these infants attain their full physical, neurodevelopmental, emotional, and psychosocial potential by providing optimal care is invaluable.

체계적 건강관리프로그램이 모성과 미숙아의 건강증진 및 질병예방에 미치는 영향에 관한 연구 (The Effects of the Systemic Follow up Health Care Program on the Health Promotion and the Risk Reduction in Premature Infants and Their Mothers)

  • Purpose: This research was conducted to evaluate the effects of asystemic follow-up care program on health promotion and risk reduction in 64 high-risk infants(HRI) including premature infants and their mothers. Method: The intervention consisted of systemic NICU education, tele-counseling and 3 home visits in 6 months. The subjects were divided into either the intervention group or the control group receiving the conventional NICU education without the tele-counseling and home visiting. Infant health promotion was measured using physical assessment, types of health problems, reflexes, OPD visiting history, DDST, immunization, feeding assessment, Infant death rate, etc. Maternal self-esteem, postpartum depression and family function were measured using the maternal self-report inventory(MRI), EPDS, and family apgar score(Fapgar), retrospectively. Result: All premature infants in the intervention group were in the normal range of growth and development, and the regular vaccination schedule. The health problems in the intervention group were addressed early so not to develop into adverse effects. The follow-up program for 6 months showed beneficial effects on MRI, EPDS, and Fapgar. Conclusion: A systemic follow-up health care program is beneficial on health promotion and risk reduction in 64 HRI including premature infants and their mothers.

  • Purpose: This study was conducted from January to March, 2004 to examine the effect of telephone follow-up on the performance of self-care in cancer patients undergoing chemotherapy and on role stress of family caregivers. Method: Research design was a nonequivalent control group non-synchronized design. Seventy-two participants were assigned to either the experimental group (18 cancer patients, 18 family caregivers) or the control group (18 cancer patients, 18 family caregivers). Data were collected before and after the intervention and were analyzed with paired t-test, t-test, Mann-Whitney U Test & Wilcoxon Signed Ranks Test. Results: Performance of self-care in the experimental group undergoing telephone follow-up was significantly higher than that of the control group (t=8.016, p=0.000). Role stress of family members in the experimental group was also significantly higher than that of the control group (t=2.133, p=0.042). Conclusion: This results suggest that the telephone follow-up is effective for cancer patients undergoing chemotherapy and their family caregivers. Telephone follow-up can be recommended as an effective nursing intervention for self-care performance in cancer patients undergoing chemotherapy and to reduce role stress of family caregivers.

뇌졸중 환자의 추후관리가 주간호제공자의 삶의 질에 미치는 영향 (A Study of the Effects of Follow-up Care for Stroke Survivors on Primary Caregivers' Quality of Life)

  • The purpose of this study was to identify the effect of follow-up care for stroke survivors on primary caregivers' quality of life. Quasi-experimental research was conducted in which an experimental group and a control group-each of 15 stroke survivors and their primary caregivers-were consecutively sampled. Data collected from July to September, 1999 by interview using a structured questionnaire with both the experimental and the control groups. After a month, the two groups were given the same questionnaire. The experimental group was also given a telephone follow-up every week for a month, as well as a home visit. The survey instruments used in this study were Saha and Cooper's "Modified Barthel Index" (11 items) for checking the stroke survivors' level of activities of daily living, and a modified form of Jeong's "Quality of life" (18 items) for primary caregivers' QOL level. The obtained data were analyzed by percentage, t-test, $X^2$-test, Kruscal-Wallis test, Spearman correlation coefficient by SAS/PC program. The results were as follows: 1. There was no significant difference in the stroke survivors' ADL level, though the level of the experimental group was higher than that of the control group. 2. There was a statistical difference in the before and after treatment of the primary caregivers' QOL level. In conclusion, the follow-up care program had a useful effect on the quality of life of primary caregivers.

CQI 활동 후 사후관리 체계 조사연구 (A study on the follow-up management system of Continuous Quality Improvement activity)

  • This study was conducted to determine whether follow-up management is carried out continuously following CQI activity and to analyze the factors behind the success and failure of follow-up management. Past presentations from 1994-1999 of CQI coordinators and lecturers from various institutions who presented at The Korean Society of Quality Assurance in Health Care(KoSQA) on the conditions of follow-up management in each institution were analyzed. The results of this study were as follows; Since the number of subjects on CQI increased each year at symposiums, this has expanded to all medical institutions. Although medical institutions usually conduct 11-20 subjects on CQI per year, there were many such occasions where more than 31 subjects were conducted. Moreover, institutions with less than 800 beds have come up with more projects than those with more than 800 beds, thus 23.3% of these institutions had at least 1 person involved in 4 projects. This had created an overload of responsibilities for specific persons' involvement, prompting them to incline toward formalities in their work rather than substantial activities. Among the projects presented at the symposiums, 51.7% demonstrated that follow-up management could be carried out. In particular, 55.3% of the projects from provincial regions could carry out follow-up management compared to 48.8% in Seoul. Moreover, it was demonstrated that 80% of the projects from institutions with 600-799 beds carried out follow-up management most effectively. With regards to previous presentations, the older they were, it was found that follow-up management could not be effectively carried out. Some institutions that responded that follow-up management was carried out effectively in their institutions were found to have conducted follow-up management without any inspection strategies or the appropriate tools. CQI activities were executed and terminated with no consistency and team members had no real concern for it. The most important factors that contribute to an effective follow-up management are the need for concern and interest from the directors of the hospitals, from the relevant departments and team members in addition to the role of the supervising department, follow-up management through management of target goals, consistency in tasks along with communication between all team members. The biggest problems were perceived to be overload of work due to accumulation of proposed projects in addition to lack of awareness pertaining to follow-up management. CQI is beneficial for all staff for the improvement of the mind and business administration and thus it is believed to be desirable. To carry out follow-up management effectively, leadership, analysis and application of information, follow-up management and planning, as well as quality management are perceived to be essential, on the other hand, the results showed a significant difference. To prevent CQI activities from becoming just an activity, the basic system should be reconstructed and augmented based on the problems derived from the results of this study. Moreover, we hope this study will be used as reference material that would encourage the administration of follow-up management after CQI activities in most hospitals. Furthermore, various studies on follow-up management should be conducted for CQI activities in the future.
