Objectives: The purpose of the study was to investigate the cumulative dental caries rate of pit and fissure sealant in community public health program from 2005 to 2008 follow-up. Methods: The subjects were 4,768 students of 34 elementary schools in Gimcheon. The survey included direct oral examination and cumulative retention rate of pit and fissure sealant from 2005 to 2008. Data were analyzed using PASW statistical package version 18.0 and the level of significance was set at p<0.05. Results: The cumulative retention rate of sealant was 18%, 25%, and 33% after 1 year, 2 years, and 3 years, respectively. In addition, the cumulative retention rate of lower teeth was higher than that of upper teeth. Cumulative dental caries rate increased in sealed group and no sealed group. The cumulative dental caries rate decreased owing to tooth brushing frequency in sealed group and no sealed group. Conclusions: Regular and timely tooth brushing can maintain good oral health condition after tooth sealing. The school based oral health education can prevent loss of sealant and secondary caries.
Objectives: The purpose of this study was to report the long-term efficacy of herbal medicine for treatment of patients with recurrent aphthous stomatitis (RAS). Methods: We reviewed the clinical records of 17 patients with RAS who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from September 1st, 2014 to August 29th, 2017. The patients were treated with herbal medicine for more than 3 consecutive weeks, followed by a telephone survey. We evaluated the clinical characteristics of RAS, and compared the short-term and long-term efficacy of herbal medicine treatment. Results: After treatment with herbal medicine, the clinical characteristics improved in the 17 patients with RAS, especially in terms of the duration and interval of recurrence. Prolonged effects were reported upon the follow-up telephone survey. Conclusion: Herbal medicine treatment may have a long-term effect in patients with RAS.
Purpose: The aim of this study was to develop an appropriate nursing information guideline according to corrected age, after investigating parents' concerns about the growth, development, and diseases of their premature infants after discharge from the neonatal intensive care unit (NICU). Methods: The parents of premature infants (birth weight, <2,500 g; gestational age, <37 weeks) who went to a neonatal follow-up clinic after NICU discharge at Seoul St. Mary's Hospital from January 2005 to December 2009, were asked with regard to their concerns about their infants through a questionnaire survey. The results of physical examinations, including body measurements and neurodevelopmental status at 4, 8, 12, and 18 months of corrected age, were retrospectively reviewed in 390 infants. Results: The most common parental concerns were developmental delay, poor growth, and feeding and nutritional problems. Parental concerns about developmental delay, growth failure in improvement in body weight and length, and overweightness were high in specificity but very low in sensitivity. After NICU discharge, 30% of premature infants experienced infectious diseases before 18 months of corrected age, the most common of which was respiratory tract infection. Conclusion: For guiding of premature infants in outpatient day clinics after NICU discharge, it is necessary to identify the parents' highest concerns, to educate them about the possibilities of growth and neurodevelopmental disabilities in their infants and to provide them with handouts containing guidelines on the management of infectious diseases, especially respiratory infections.
Purpose: The purpose of this study was to evaluate the clinical and radiologic outcome of syndesmosis fixation using TightRope$^{TM}$ (Arthrex, Naples, FL, USA) in acute syndesmosis injuries. Materials and Methods: Twenty-five consecutive patients with acute syndesmosis injuries, treated using TightRope$^{TM}$, were reviewed. Patients were evaluated preoperatively and at the last follow-up (at least 12 months postoperatively). Clinical outcomes were assessed using American Orthopaedics Foot and Ankle Society (AOFAS) ankle-hindfoot score and self-subjective satisfaction survey. Three radiologic parameters were evaluated two times at the preoperative and final follow up from the nonweightbearing ankle anteroposterior radiographs. Results: The mean AOFAS ankle-hindfoot score was 95.5 at the final follow-up. According to the satisfaction survey, 21 patients chose excellent, and four patients chose good. All radiologic parameters, including the mean tibiofibular clear space, mean tibiofibular overlap, and mean medial clear space on nonweightbearing ankle anteroposterior view, significantly improved after surgery. Complications occurred in only one patient who experienced knot irritation with infection. Conclusion: The short-term surgical results of syndesmosis fixation using TightRope$^{TM}$ were good to excellent, both clinically and radiographically. These results suggest that the fixation using TightRope$^{TM}$ is a valid option for acute syndesmosis injury.
The purpose of this study is to suggest a guideline that can be used as a standard for consultation and review by environmental impact assessment consultation and review practitioner to enhance the practical effectiveness of creating alternative habitats. The consultation stage of environmental impact assessment was significantly divided into 1) preparation of draft environmental impact assessment reports and gathering of consensus thereon, 2) preparation of environmental impact assessment reports and consultation thereon, 3) follow-up survey of environmental impacts(under construction) and 4) follow-up survey of environmental impacts(in operation). And it was suggested that the procedures of creating alternative habitat by each consultation stage were linked in order of 1) planning, 2) designing, 3) construction, and 4) post-monitoring and maintenance. The basic principles were also proposed for each stage of consultation on environmental impact assessment and procedure for the creation of alternate habitats. Then, issues and methods to be reviewed by the consulting agency and review agency of environmental impact assessment were presented in detail. The guideline of this study provides important information not only to environmental impact assessment consultation and review practitioner, but also to developers and environmental impact assessment agents who plan, construct, and manage alternative habitats.
목 적 : 가와사끼병의 치료에 면역 글로불린이 도입된 이후 환자의 대부분이 회복기 심초음파 검사에서 정상 관상동맥 소견을 보이게 되었으나 그 장기 예후는 아직 불분명하며 추적관찰 방법도 아직 표준화되지 못한 상태이다. AHA에서는 이 환자군을 위험도 1 또는 2로 분류하고 발병 6-12개월경의 추적 심초음파 검사와 발병 1년까지 외래 추적관찰을 권하고 있다. 본 연구는 이 추적 심초음파 검사의 유용성 정도를 알아보고자 하였으며, 실제 임상에서는 이들 환자가 어떻게 추적관찰되고 있는지 조사하여 그 결과를 비교하였다. 방 법 : 2000년 1월부터 2003년 10월까지 인제대학교 부속 부산백병원 소아과에 가와사끼병으로 내원하였던 환아 147명 중 급성기와 회복기 심초음파 검사결과 AHA위험도 1, 2에 해당되며 발병 후 최소 6개월 이상의 경과를 가진 환아 중에서 심초음파 및 심전도 검사를 시행할 수 있었던 45례를 대상으로 하여 의무기록과 심초음파 검사 결과를 분석하였다. 아울러 국내 소아 심장전문의들에게 관상동맥 병변의 진단 기준 및 AHA 위험도 1, 2군에 속하는 환자들의 추적관찰 방법에 대한 설문조사를 실시하였다. 결 과 : 1) 45례의 대상 환아들의 발병 당시 연령은 5개월에서 7세(평균 29.7개월), 남녀비는 1.4 : 1이었고 평균 발열 기간은 7.3일이었다. 이 중 AHA 위험도 1군은 37례(82.2%), AHA 위험도 2군은 8례(17.8%)이었다. 2) 급성기 심초음파 검사는 평균 발열 후 11.2일에 시행되었다. 24례(53.3%)에서 경한 판막 역류나 소량의 심막액 저류를 보였고 관상동맥 확장은 8례(17.8%)에서 나타났다. 회복기 검사는 평균 발열 8.8주에 시행되었고 모두 심초음파 검사상 정상 관상동맥 소견을 보였으며 경도의 판막 역류가 3례(6.7%)에서 관찰되었다. 추적 검사는 발병 후 평균 22.2개월에 시행되었고 관상동맥 소견은 모두 정상이었다. 5례(11.1%)에서 심낭 삼출 및 판막의 역류가 미량 관찰되었고 1례에서는 심전도에서 이속성 각차단이 나타났으나 심근 경색, 허혈의 소견이나 심기능의 저하는 없었다. 5세 미만과 5세 이상에서 각각 급성기, 회복기, 추적관찰기간의 좌, 우 관상동맥의 평균 내경을 비교하였으나 통계적으로 유의한 변화는 없었다(Table 2, 3). 3) 설문조사의 응답자는 27명이었으며 AHA 위험도 1군에게는 회복기 심초음파 검사까지만 실시한다는 응답이 가장 많았고(48.2%) 외래 추적관찰은 회복기 이후 중단하거나(37.0%), 발병 1년까지만 하는 경우(33.3%)가 대부분이었다. 평균 심초음파 검사 횟수는 2.3회였다. AHA 위험도 2군에게는 회복기 이후 발병 6개월에서 12개월 사이에 1회(37.0%) 또는 2회(51.9%)의 추가 검사를 실시하는 것으로 나타났다. 외래 추적관찰은 1년까지만 하는 경우(33.3%)가 가장 많았으나 다양한 답변이 있었다(Table4). 평균 심초음파 검사 횟수는 3.4회였다. 관상동맥 이상의 판단은 5세 미만 내경 3 mm 이상, 5세 이상에서 내경 4 mm 이상을 병변으로 판단한다는 답변이 가장 많았다(77.8%). 그러나 개인의 선호도에 따라 응답에 차이가 있었다. 결 론 : 본 연구에서 회복기 심초음파 검사상 정상 관상동맥 내경을 보였던 환자들은 발병 6개월 이후 실시한 추적 검사에서도 관상동맥 내경의 변화는 없는 것으로 나타나 AHA에서 권고하는 발병 6-12개월 사이의 추적 심초음파 검사는 유용성이 떨어지는 것으로 판단된다. 임상의들을 대상으로 한 설문조사에서는 AHA 위험도 1군에서는 회복기 이후에 심초음파 추적검사를 하지 않는 병원이 많은 것으로 나타났으나 AHA 위험도 2군에서는 발병 1년까지 추적 심초음파 검사를 하는 병원이 더 많은 것으로 나타났다. 앞으로 초기 심초음파 검사상 정상이었던 가와사끼병 환자군의 장기예후와 추적관찰 방법에 대해 더 많은 수의 증례를 대상으로 한 다기관 조사를 통해 관상동맥 이상의 진단 기준의 표준화 및 적절한 추적관찰 지침을 제정할 필요가 있는 것으로 사료된다.
Purpose: The purpose of this study is to develop guidelines for clinical trial of the total ankle replacement system for premarket approval. Materials and Methods: We selected and analyzed nine peer-reviewed articles whose quality had been proven in a previous phase. Two investigators extracted parameters for guideline criteria, including number of cases, patient age, follow-up period, failure rate, radiographic osteolysis rate, residual pain rate, and percentage of satisfaction. In addition, the inclusion and exclusion criteria were analyzed and developed. Results: Eight level IV studies and one level II study were included. The average number of cases was 159 cases and the mean patient age was 63.5 years. The mean follow-up period was 4.2 years, ranging from two to nine. The average failure rate of total ankle replacement in mid- to long-term follow-up was approximately 13% (2%~32.3%). The rate of osteolysis was approximately 18%. Residual pain was common (21.4%~46%), but overall patient satisfaction was approximately 85.6% (67.5%~97%). Conclusion: The results could be used as criteria for designing the clinical studies, such as number of cases, patient age (over 60 years), and follow-up period (minimum two years). The clinical scoring system and 36-item short form health survey (SF-36) was the most commonly used method for clinical evaluation for total ankle arthroplasty. In addition, the overall results, including failure rate, osteolysis rate, and patient satisfaction, could be used as a parameter of guidelines for premarket approval.

Based on our experience on exoplanet transit observation, we propose the exoplanet science cases with Small Telescope Network. One is the follow-up observation for validation of exoplanet candidates. TESS(Transiting Exoplanet Survey Satellite) is pouring out exoplanet candidates in bright stars(V<15) on all the sky. Since Small Telescope Network will consist of 0.5-1m telescopes, we will expect to produce promising outcomes from the follow-up observation of bright candidates. Next is the transit time observation. By spectroscopy of space and large telescopes during transit event, it can be possible to find the bio signatures in exoplanet atmosphere. So, in terms of cost, it is critical to determine the exact time of transit event. In addition, detecting the variation of transit time can reveal another exoplanet and exomoon in the system. In order to determine the transit time and its variation, the accumulation of transit event data is more important than the quality of photometric data. We expect that it can be a challenging project of Small Telescope Network.
SkyMapper is the largest-aperture optical wide-field telescope in Australia and can be used for transient detection in the Southern sky. Reference images from its Southern Survey cover the sky at δ <+10 deg to a depth of I ~ 20 mag. It has been used for surveys of extragalactic transients such as supernovae, optical counterparts to gravitational-wave (GW) and fast radio bursts. We adopt an ensemble-based machine learning technique and further filtering scheme that provides high completeness ~98% and purity ~91% across a wide magnitude range. Here we present an important use-case of our robotic transient search, which is the follow-up of GW event triggers from LIGO/Virgo. We discuss the facility's performance in the case of the second binary neutron star merger GW190425. In time for the LIGO/Virgo O4 run, we will have deeper reference images for galaxies within out to ~200 Mpc distance, allowing rapid transient detection to i ~ 21 mag.
