기본간호학 실습교육에서 웹 기반 학습이 유치도뇨술 수행능력, 지식, 자신감에 미치는 효과 (Effectiveness of Web Based Learning on Competence, Knowledge, and Confidence in Foley-Catheter Management in Basic Nursing Education)

  • Purpose: This study was done to compare the effectiveness of web based learning versus traditional education for learning foley-catheterization in Basic Nursing. Method: This study was a quasi-experimental research. The participants were 60 students who were taking Basic Nursing at A nursing college (3 years) in C city. Thirty students each were assigned to the experimental or control group. Data were collected between October 20 and November 4, 2003. The data were analyzed by descriptive statistics, t-test and ANCOVA. Results: The mean score for competence in foley-catheterization practice in the experimental group was 48.63 and in the control group, 44.67. This result was statistically significant (t=7.655, p=.001). The mean score for knowledge in the experimental group was 63.0, while fur the control group, 64.0. This result was not statistically significant (t=-.330, p=.743). The mean score for confidence in learning in the experimental group was 26.70 for the pre-test and 30.73 for the post-test, and in the control group 27.93 and 28.37 respectively, but this result was not statistically significant (F=.858, p=.358). Conclusion: The Web based learning was found to be effective in nursing practice but not nursing knowledge. It is necessary to continue to develop approaches to teaching nursing and to evaluate these approaches with further research.

Foreign Body Removal in Children Using Foley Catheter or Magnet Tube from Gastrointestinal Tract

  • Foreign body (FB) ingestion of children is a common pediatric emergency requiring medical attention. Pediatric emergency physicians and gastroenterologists often encounter nervous and distressed situations, because of children presenting with this condition in the common clinical practice. When determining the appropriate timing and indications for intervention, physicians should consider multiple patient- and FB-related factors. The utilization of a flexible endoscopy is considered safe and effective to use in these cases, with a high success rate, for the effective extraction of FBs from the gastrointestinal tract of a child. Additionally, a Foley catheter and a magnet-attached Levin tube have been used for decades in the case of FB removal. Although their use has decreased significantly in recent times, these instruments continue to be used for several indications. Using a Foley catheter for this purpose does not require special training and does not necessarily require sedation of the patient or fluoroscopy, which serve as advantages of utilizing this method for foreign object retrieval. An ingested magnet or iron-containing FB can be retrieved using a magnet-attached tube, and can be effective to retrieve an object from any section of the upper gastrointestinal tract that can be reached. Simple and inexpensive devices such as Foley catheters and magnetattached tubes can be used in emergencies such as with the esophageal impaction of disk batteries if endoscopy cannot be performed immediately (e.g., in rural areas and/or in patients presenting at midnight in a facility, especially in those without access to endoscopes or emergency services, or in any situation that warrants urgent removal of a foreign object).

경피적 폴리 카테터 삽입을 이용하여 수술 후 장 누출을 성공적으로 관리하기 위한 기술적 및 임상적 요인들 (Technical and Clinical Considerations for Successful Management of Postoperative Bowel Perforation by Percutaneous Foley Catheter Placement)

  • 목적 본 연구의 목적은 경피적 폴리 카테터 삽입을 이용하여 수술 후 장 누출을 성공적으로 관리하기 위한 기술적 및 임상적 요인을 분석함에 있다. 대상과 방법 이번 후향적 연구에는 32명의 환자가 포함되었다. 수술 후 위장 누출은 컴퓨터단층촬영(CT)을 이용하여 진단되었고, 환자는 Jackson-Pratt 튜브 또는 이미지 유도 방법을 통해 경피적 경로로 누출 위치에 폴리 카테터 삽입을 시행 받았다. 성공은 합병증 없이 폴리 카테터를 성공적으로 제거한 것으로 정의하였고, 임상적 실패와 관련된 인자들에 대하여 분석하였다. 결과 모든 환자에서 경피성 폴리 카테터 배치는 합병증 없이 성공적으로 시행되었다. 폴리 카테터 삽입은 CT에서 누출이 확인된 후 중앙값 10일(범위, 1~68)째에 시행되었다. 임상적 성공은 32명의 환자 중 26명(81%)에서 달성되었다. 통계학적 분석에서, 임상적 실패에 대한 통계적으로 유의미한 위험 인자는 전신적 동반 질환(p < 0.001) 및 구강 섭취 실패(p = 0.015)였다. 결론 경피적 폴리 카테터 삽입은 수술 후 장 누출을 관리하기 위한 안전하고 효과적인 접근방법으로 간주될 수 있다. 폴리 카테터 배치 후, 전신적 동반 질환의 유무 및 성공적인 구강식이를 빠른 시일 내로 시작하는 것은 임상적으로 성공적인 회복에 중요한 요소이다.

  • The main purpose of this study was to develop and evaluate the effectiveness of the standardized patient method in teaching catherization skills, communication skills and students' learning motivation in undergraduate nursing students. The study employed a quasi-experimental method using a non-equivalent control group post-test design. Data were collected over 2 years from August 2000 to September 2001 with 75 sophomore student nurses in one baccaleureate program. The results are summarized as follows ; The scores in catheterization skills and communication skills were statistically significantly higher in the experimental group than the control group. Learning motivation score were also statistically significantly higher in the experimental group than the control group. In conclusion, this study suggests that a standardized patients managed instruction which emphasize authenticity and contex-tualization is very effective in teaching nursing students, the mastery and application of nursing skills, and the utilizing therapeutic communication skills. It is recommended that more standardized patients cases be developed for wider areas of nursing education and this study be replicated with more students using a longitudinal design.

자율실습교육에서 스마트폰 동영상을 활용한 자가평가 학습법이 간호술기 수행능력, 자율실습만족도, 학습동기에 미치는 효과: 유치도뇨를 중심으로 (Effects of Self-evaluation using Smartphone Recording on Nursing Students' Competency in Nursing Skills, Satisfaction, and Learning Motivations: Focusing on Foley Catheterization)

  • Purpose: The purpose of this study was to identify effects of self-evaluation using smartphone recording on competency in nursing skills, satisfaction and learning motivations in nursing students'. The setting was the open laboratory hours. Methods: A quasi-experimental pre/post-test design was conducted in September 2015. The participants were 82 sophomore nursing students from one University. The experimental group did self-evaluation by watching recorded video clips of their performance. The control group did not have any self-evaluation. The primary outcomes were scores from the foley catheterization checklist, scores from a student satisfaction tool, and scores from a learning motivation tool. Data were analyzed using ${\chi}^2$ test, Fisher's exact test, independent t-test, and paired t-test. Results: There was a significant higher mean score in levels of satisfaction by students in the experimental group compared to the control group (t=2.26, p=.027). In addition, improvement in communication skills by the experimental group was significantly higher than improvement in the control group (t=3.96, p<.001). Conclusion: Findings show that self-evaluation using smartphone recordings has positive effects on increasing both communication skills and satisfaction with practice during open laboratory hours. These results indicate that, self-evaluation using smartphone recording is useful as a supplement to traditional open laboratory education.

수직도뇨관환자의 요로감염발생요인에 관한 실험적 연구 -요관삽입기간, 삽입방법, 세척횟수, 회음부소독유무, 항생제사용유무를 중심으로 - (An Experimental Study on the Occurence of Bacteriuria according to Duration of Insertion, Frequency of Bladder Irrigation & Perineal Care, & Administration of Antibiotics in Patients with Indwelling Catheter)

  • This Study was conducted at Intensive Care Unit of H & S Hospitals from Jan 4 to April 7, 1981 on 14mail & 26female adult patients. Each patient was screened and found to have nonbacteriuria in clean catch specimen before catheterization. Clean catch apecimen through Foley catheter were obtained after 24hours, 48hours and 72hours from catheterization. The result of this study is reviewed in a statistical analysis of percentage & Chi Square test to obtain the following findings. 1) The occurenc of bacteriuria in patients according to duration of indwelling catheter. a. 9.1% of the patient showed evidence of bacteriuria 24hours post catheterization specimen and 60% showed 48hours post cathetreization, while 68.4% of the patient showed evidence of bacteriuria 72hours post catheterization specimen. The occurence of bacteriuria in patients were significant differences at 1% level between duration of indwelling catheter. b. Mail patients had no infection 24hours post catheterization, 50% displayed bacteriuria 48hours post catheterization & 62.5% displayed bacteriuria 71hours post catheterization. 11.1% of femail patients displayed infection 24hours post catheterization 66.7% displayed infection 48hours post catheterization and 72.7% displayed infection 72hours post catheterization. There were significant differences at 1% level between bacteriuria occurence of mail & femail patients and the duration of insertion. 2) 56% of those patient who have altered mental state developed bacteriuria, while 40% of those patient who have alear mental state developed bacteriuria. But there was without statistically any significant difference between patient's mental status. 3) The occurence of bacteriuria with the administration of antibiotics in 36 patient was in 50%. The occurence of bacteriuria without the administration of antibiotics in 4 patients was in 50%. But there was without statistically any significant difference between the administration of antibiotics. 4) The occurence of bacteriuria in patients according to frequency of bladder irrigation. 50% of those patient who irrigated twice a day developed bacteriuria, 63.6% of those patient who irrigated once a day developed bacteriuria. The occurence of bacteriuria in patients were significant differences at 1% level between frequency of bladder irrigation. 5) The occurence of bacteriuria in patients who did perineal care once a day was 58.1%, 22.6% of those patient who did perineal care twice a day developed bacteriuria. But there was without statistically any signiticant differences between frequency of perineal care. 6) Most frequent bacteria of all bacterial strains isolated by culture of the urine was E. coli(45%). Enterococci & Staphylococcus were 15% respectively.

급성 신부전 환아에서 유치 도뇨관 제거 후 발생한 신 주위 요낭종 1례 (Spontaneous Perinephric Urinoma after the Removal of a Foley Catheter in a Girl with Acute Kidney Injury)

  • 요낭종이란 신장 및 비뇨기계로부터 누출된 소변이 신장 및 신우 주위에 캡슐상의 낭종을 형성하는 드문 질환으로 자연발생적인 신우의 파열로 인한 누출은 매우 드물며, 대부분 요로와 신우 내강의 압력 증가에 의해 발생한다. 본 증례에서 18세 여자 환아는 내원 7일 전부터 시작된 고열과 핍뇨 및 호흡곤란을 주소로 내원하였다. 환아는 출생시 다운 증후군을 진단받았으며 평소 배뇨는 원활하였다. 입원 후 환아는 패혈증 쇼크 및 급성 신손상, 급성 호흡 곤란 증후군 진단 하에 항생제 및 스테로이드 충격 요법 시행 후 핍뇨, 호흡곤란 및 혈액검사 호전 소견 보였으나 7일 동안 유치된 도뇨관 제거 후 환아는 요 폐색 소견이 관찰되었다. 복부 전산화 단층 촬영상 좌측 신장 주위에 요낭종이 확인되어 좌측 요관에 Double-J catheter가 삽입 되었으며 신우의 파열 부위에 조영제 누출이 확인되었다. 시술 후 요 폐색 소견은 호전되었고, 배뇨성 방광 요도 조영술 상 좌측 신장의 4단계 방광 요관 역류가 관찰되었다. Double-J catheter 제거 3개월 후 좌측 신장 주변의 요낭종은 대부분 소실되었고, 좌측 신장의 방광 요관 역류는 보이지 않았다. 저자들은 급성 신부전 환아에서 유치 도뇨관 제거 후 배뇨곤란 및 요 폐색 소견을 보인 신 주위 요낭종 1례를 경험하였기에 보고하는 바이다.

중환자실내 병원성 요로감염 실태와 전파경로: Imipenem Resistant P. aeruginosa[IRPA]의 분자역학적 특성을 중심으로 (Status of Nosocomial Urinary Tract Infections in the ICU: Molecular Epidemiology of Imipenem Resistant P. aeruginosa)

  • Purpose: This retrospective study was done to evaluate the status of nosocomial urinary tract infections and to determine the risk factors and transmission route of causal IRPA through molecular epidemiology. Method: Two hundred ninety-nine of 423 patients admitted to the internal medicine and surgery ICU at a university hospital incity B had a positiveurine culture. Twelve of the 299 patients who had a urinary tract infection had IRPA strains. The data was collected from November 1, 2004 to January 31, 2005. The following results were obtained after the data was analyzed using percentile and UPGMA. Result: The rate of nosocomial urinary tract infections in the ICU was 10.8%. Therewere 16.8 cases of infection based on the period of hospitalization. There were 16.9 cases of infection based on the use of a foley catheter. The rate of nosocomial urinary tract infection in the ICU and urinary tract infections related to IRPA were higher in patients with the following characteristics: men, old age, admission through the emergency room, longer than seven days admission, severity of admitting causes, disturbance of consciousness, hydration less than 300cc in 24hours, a long course of antibiotics, a long period of foley catheterization and perineal care. Most of the microorganisms that caused the urinary tract infection were gram negative bacilli, among which P. aeruginosa was found in 70 patients (18.5%) and IRPA in 12 (4.0%). Among the 12 IRPA strains that were tested with PFGE, eight showed a dice coefficient higher than 80%, suggesting a genetic relationship. They were related with the period of hospitalization in the same ICU. These patients all received direct care for a urinary tract infection. Conclusion: Through these results, IRPA can be consideredas a contributing factors to urinary tract infections thus, active preventative measures are needed by the medical staff.

중증외상환자의 치료경과 시간과 활력징후에 따른 응급처치 및 간호활동 (Emergency Treatment and Nursing Activities of Severe Trauma Patients according to Elapsed Time and Vital Signs)

  • Purpose: The aim of this study was to identify necessary emergency treatment and nursing activities for severe trauma patients according to elapsed time and vital signs. Methods: A survey was conducted with 121 patients over 15 points ISS on EMR from June 1, 2011 to May 31, 2012. Collected data were analyzed with descriptive statistics, applying McNemar's test using SPSS 12.0. Results: Almost all of the subjects were men and the mean age was 46.9. Run-time for primary diagnosis, treatment decision, and leaving for the hospital room was 0.19, 4.36, and 4.21 hours, respectively, and stayover time was 9 hours. Regardless of vital signs, emergency treatments involving ambu-bagging, intubation, ventilator, and central vein catheterization insertion were offered within an hour. Central venous pressure, Foley catheter/Levin tube preparation and maintenance were performed in cases of unstable vital sign patients within an hour. Unrelated to vital signs, nursing activities for consciousness assessment, skin assesment and wound care, bed sore/fall down assesment and care, intravenous injection insertion and maintenance were conducted for all severe trauma patients within an hour. Foley catheter/Levin tube drainage care was performed for patients who had unstable vital signs within an hour. Conclusion: Emergency treatment and nursing activities for severe trauma patients were specific according to elapsed time and vital signs.

임상실습 전 간호학생에게 적용한 OSCE 프로그램 효과 (The Effects of OSCE Application Before Clinical Practice for Nursing Students)

  • Purpose: The study was done to identify the effects of OSCE program prior to clinical practice for nursing students and to find out the weakest area of fundamental nursing skills. Methods: Study design was methodological triangulation. Qualitative analysis was done to derive nursing students' OSCE experiences using OSCE reflection note. Level of basic nursing skill acquirement was identified by quantitative method. Results: Four themes and 10 sub-categories emerged: (a) confidence and interests in nursing being increased, (b) being encouraged by mentor and evaluator, (c) requiring much effort to learn nursing skill, (d) being aware of themselves and understanding others in their shoes. Results of analysis of OSCE application score record showed as followings ; Intravenous injection(92.6%), Intramuscular injection(89.5%), Foley catheterization(85.2%), Vital sign(81.5%): BP check(63.0%) Respiration check(50.0%), Health assessment: respiration sound auscultation(33.3%) heart sound auscultation(44.4%). Conclusion: OSCE program application before frist clinical nursing practice was effective in terms of fundamental nursing skills learning. It is necessary to reinforce nursing skills based on the study results.
