Development of fluoride varnish with sustained fluoride release and biocompatibility (불소 지속방출 및 생체적합성을 가진 불소 바니쉬의 개발)

  • The purpose of this study was to fabricate a biocompatible fluoride varnish with sustained fluoride release, and to compare it with commercial fluoride varnishes. For the experimental fluoride varnish, bis-GMA (BG) or two types of rosin (KR-610: K0, KR-612: K2) were used as bases. Either ethyl acetate or ethanol was added as solvent and 5 wt% NaF was used. 5 mg of experimental F- varnishes and 2 commercial products, Cavity shield (CS) and Flor-opal (FO), were applied on a labial surface of bovine teeth ($10mm{\times}7mm$). The amount of fluoride release was measured at 1 hr, 2 hrs, 3 hrs, 4 hrs, 8 hrs, 12 hrs, 1 day, 3 days, 5 days, 10 days, 15 days, 20 days and 30 days. MTT test was done with diluted F- varnishes using ethanol. Statistical analysis was done with one-way ANOVA and Duncan multiple range test (${\alpha}=0.05$). BG showed the highest fluoride release at 1 hr (P<0.05), while that of K0 was highest at 2 hrs (P<0.05). From 1 day to 5 days, experimental fluoride varnishes showed higher fluoride release than the commercial products (P<0.05), and there were no significant differences after 5 days (P>0.05). For MTT test, K0 and FO showed higher cell viability than other experimental groups (P<0.05), with no significant differences with K2 (P>0.05). Considering the sustained fluoride release and cell viability of the experimental rosin-based fluoride varnishes compared with commercial products, it will be appropriate for clinical application.

Effect of Experimental Fluoride Varnish upon the Vickers Hardness of Bovine Teeth (실험용 불소바니쉬가 우치의 비커스 경도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of the experimental and commercial fluoride varnishes on the Vickers hardness of bovine teeth. The experimental fluoride varnishes with 5 wt.% NaF were fabricated using Bis-GMA or rosin as the resin base. The components were mixed with over-head stirrer under warming up in a double boiler for 30 minutes. Four commercial fluoride varnishes (V-varnish, Flor-opal, Cavity shield, Fluor protector) were compared with the experimental fluoride varnishes. Vickers hardness was measured on the labial surface of bovine anterior teeth after applying fluoride varnish on 5th and 30th day. The surface of bovine teeth was observed with a scanning electron microscope before and after applying fluoride varnish and the change of the components on the bovine teeth surface was measured with EDX. In terms of hardness, the experimental rosin-based fluoride varnish showed significantly higher hardness on the 5th and 30th day than the control (bovine teeth without fluoride treatment)(p<0.05). EDX results showed that the fluoride content on the surface of bovine teeth treated with the experimental rosin-based fluoride varnish was highest on the 1st and 10th day. The higher hardness and fluoride content of experimental rosin-based fluoride varnish can be considered to be used as an effective fluoride varnish to prevent dental caries.

Effect of fluoride varnishes on the surface hardness of bovine teeth under demineralization/remineralization cycling

  • We investigated whether fluoride varnishes recover the hardness of bovine teeth under 20 days of demineralization/remineralization cycling. The fluoride varnish groups (two commercial fluoride varnishes [V-varnish (Vericom, Korea) and CavityShield (3M ESPE, USA)] and an experimental fluoride varnish including 5 wt.% NaF were compared with a control group without fluoride varnish. Vickers hardness was measured at baseline, 3 days after immersion in caries-inducing solution, 24 hours after application of a fluoride varnish, and after 10 and 20 days of demineralization/remineralization cycling. Afterward, tooth surfaces were observed by scanning electron microscope. After fluoride varnish application and the cycling 10 and 20 days, the experimental varnish group showed the highest hardness, while the CavityShield and the control groups demonstrated the lowest hardness. The experimental varnish group recovered the hardness of the baseline at 24 hours after application of the varnish, while it was recovered after 20 days of the cycling in case of the V-varnish. However, the CavityShield and the control groups did not recover the hardness even after 20 days of the cycling. The experimental fluoride varnish with fast recovery in the hardness of the baseline can be used as an effective fluoride varnish to resist demineralization and to facilitate remineralization.

EFFECT OF THE CHLORHEXIDINE AND FLUORIDE-CONTAINING VARNISH ON THE LEVEL OF SALIVARY MUTANS STREPTOCOCCI (Chlorhexidine과 fluoride 성분의 varnish가 타액 내 mutans streptococci 수에 미치는 효과)

  • The objective of the present prospective trial was to compare the efficacy of chlorhexidine(Cervitec), Fluoride (Fluorprotector) and combination of chlorhexdine and fluoride varnishes in decreasing the level of salivary mutans streptococci. Forty healthy students of school of dentistry, Chonbuk national university were investigated to evaluate mutans streptococci(MS) counts and randomized into four groups to treat with the experimental varnishes; A) untreated group(n=10), B) chlorhexidine group(n=10), C) fluoride group(n=10), D) chlorhexidine and fluoride group(n=10). Dentocult $SM^{(R)}$ (Orion Diagnostica) strip method was used for measurement of the level of mutans streptococci in saliva. Stimulated saliva were collected at baseline for mutans streptococci counts evaluation (ms1), 12 weeks later the completion of each varnish treatment, mutans streptococci counts were re-evaluated. In varnish group with chlorhexidine, fluoride and combination of chlorhexidine and fluoride, the level of mutans streptococci was lower after 12 weeks than at baseline, but there were no significant differences in saliva(p>0.05), when compared with baseline. After 12 weeks, a remarkable reduction was still found in the subjects with high level of mutans streptococci at baseline, but not different in the low and moderate level of mutans streptococci(p>0.05).

  • 본 연구는 임상에서 사용되어지는 두 가지의 불소 바니쉬를 비교연구 하였다. 또한 각각 치아표면에 10mg의 불소 바니쉬를 도포하였다. 각 대상자들의 타액의 불소이온유리농도는 1,3,12,24시간으로 채취하여 분석하였다. 각각의 불소 바니쉬를 도포한 타액 내 불소이온의 유리 누적량은 차이를 보였다. 본 연구는 현재 사용되어지는 불소 바니쉬를 선택함에 있어 기초자료로 사용될 수 있을 것이라 사료되어진다.

A Convergence Study on the Remineralizing Effect of Domestically Sold Fluoride Varnish Using QLF-D (QLF-D를 활용한 국내 시판 불소 바니쉬의 재광화 효과에 대한 융합적 연구)

  • This study is to analyze seven fluoride varnishes remineralizing effects on early carious lesions in the clinical field. While CS, MI, CW, VV, EP, FP and FN were used for permanent teeth, CS, MI, CW and FP were used for primary teeth. The specimens were demineralized for 4 days after forming a $3{\times}3mm^2$ window with nail varnish, applied with varnishes separately, then were stirred in artificial saliva for 1 week. QLF-D analysis of carious lesion size showed that permanent teeth's ${\Delta}F$ value decreased after applying CS(4,64), MI(4.86), CW(4.50), and EP(5.50), while for primary teeth, FP(3.00) and MI(3.00) displayed a remineralizing effect. Thus, MI can be used effectively for preventing caries for both adults and children in the clinical field.

ANTICARIOGENIC EFFECT OF FLUORIDE VARNISHES AND CHLORHEXIDINE VARNISHES (불소 바니쉬와 클로르헥시딘 바니쉬의 항우식 효과)

  • Dental caries which is one of the most common chronic disease complexly developed by the action of oral bacteria, diet, and host factor. Various prevention program enhance resistance of demineralization and reduce the acidogenecity of oral bacteria have been introduced, representative material is fluoride and chlorhexidine. The purpose of the study was to evaluate and compare effectiveness of fluoride varnish and chlorhexidine varnish in vivo. Bovine tooth specimens were implanted in the lower space maintainers and applied with fluoride varnish and chlorhexidine varnish. After seven days in oral environment, metal mesh was covered to make similar condition of plaque accumulation and induce caries. All specimens were analysed by EPMA to evaluate quantitative change of Ca, P and by polarized microscope to identify histological changes. The results were as follows : After initial artificial caries induction in the mouth, there were remarkable enamel caries lesion in the control group under polarized light microscopy. The highest amount of mineral decrease were showed in control group. No statistically significant mineral decrease were showed in fluoride varnish group, while chlorhexidine varnish group showed only significant decrease of P(P<0.05). In conclusion both fluoride varnish and chlorhexidine varnish seemed to be effective for protecting enamel surface from caries activity, although fluoride varnish has more anticariogenic effect than chlorhexidine varnish.

A Comparison of the Amount of Fluoride Ion Released and Remineralization Effect on the Initial Caries Lesion of the Various Fluoride Varnishes (시판중인 불소 바니쉬의 불소이온 유리량 및 초기우식병소의 재광화 효과 비교)

  • Subjects of this study were : $FluoroDose^{(R)}$ (FD, Centrix Inc., USA), $Enamelast^{TM}$ (EL, Ultradent Product Inc., USA), $Clinpro^{TM}$ white varnish (CW, 3M ESPE, USA), $CavityShield^{TM}$ (CS, 3M ESPE, USA), V $varnish^{TM}$ (VV, Vericom, Korea), MI $varnish^{TM}$ (MI, GC, Japan). The amount of fluoride ion release was measured eight times during 168 hours to see change in accumulation with the course of time using a measuring instrument. And the remineralization rate was measured with Quantitative Light-induced Fluorescence (QLF). V $varnish^{TM}$ group and MI $varnish^{TM}$ group showed high remineralization rates with statistically significance while $CavityShield^{TM}$ group was the lowest rate of remineralization (p < 0.05). After that, several chosen samples were scanned through electron microscope (SEM). Demineralized enamel was observed as the number of enamel crystal was very small; enamel rods and crystals were highly protruding. Remineralized groups with fluoride varnishes show the decreasing tendency of the surface roughness compared to the demineralized enamel.

THE ANALYSIS OF REMINERALIZATION EFFECT IN FLUORIDE VARNISH USING CONFOCAL LASER SCANNING MICROSCOPE (공초점 레이저 주사 현미경을 이용한 불소 바니쉬 재광화 효과의 분석)

  • It is well established that fluoride products play an important role in the prevention and remineralization of carious lesion. Fluoride varnish is a concentrated topical fluoride and varnishes adhere to tooth surface, permitting prolonged fluoride exposure and uptake. In this study, the artificial initial enamel caries was caused on the sound human enamel and divided 60 specimens into three groups. Group 1 and group 2 were treated with the topical application of fluoride varnish and stored in artificial saliva for 1 and 2 weeks. Group 3 was stored in artificial saliva for 2 weeks, which acted as control group. Changes in mineral contents were analysed with the confocal laser scanning microscope. The following results were obtained: 1. In group 1 and group 2, the total fluorescence of the lesion(TFL) was reduced in remineralized area compared to in demineralized area(p<0.05). 2. The total fluorescence of the lesion of remineralized area was more reduced in group 2 than in group 1(p<0.05). 3. The total fluorescence of the lesion was more reduced in group 2 than in control group(p<0.05). 4. Confocal laser scanning microscope can be used in quantitative analysis of remineralization by fluoride varnish.

THE EFFECT OF FLUORIDE VARNISH AND ACIDULATED PHOSPHATE FLUORIDE GEL ON ARTIFICIAL CARIES LESION - A MICROCOMPUTED TOMOGRAPHIC STUDY - (Fluoride Varnish와 Acidulated Phosphate Fluoride Gel이 인공우식 병소에 미치는 영향에 대한 미세전산화 단층 촬영을 이용한 연구)

  • The application of acidulated phosphate fluoride gel(APF) and fluoride varnishes are the most common topical fluoride therapy. The purpose of this in vitro study was to compare the remineralization effects of two topical fluoride agents, APF gel and fluoride varnish with microtomograph and 3D image analyzer without sample preparation and chemical fixation. For the purpose of the study, the artificial caries lesion was caused on the caries-free permanent pre molar and 48 specimens were divided into three groups each containing 16 specimens No application was performed on group 1, which acted as control group. Group 2 was treated with APF gel and was removed after a minute. Group 3 was treated with the topical application of fluoride varnish and removed after 45 minutes. Each specimen was placed into a closed container with 50ml of a artificial saliva during three months and the 3D images of the remineralization area were taken using the SkyScan each month. Using the density-measuring program in V $works^{TM}$, the density value of the remineralization area was measured. The following results were obtained: 1. All groups demonstrated an increase in the density of artificial caries lesion with time. 2. The density was significantly higher in APF gel and fluoride varnish group than control group at 1 month, 2 months, 3 months after the treatment(P<0.05). 3. The difference of the density between that "prior to treatment" and that "1 month after treatment" in Group 2 and Group 3 was significantly higher than that of Group 1 and, the difference of the density between that "1 month after treatmen" and that "2 month after treatment" in Group 3 was significantly higher than that of Group 1 and 2(P<0.05). 4. The fluoride varnish was more effective after 2 and 3 months and continuous than the APF gel.
