소아에서의 기면병 (Narcolepsy in Children)

  • Childhood narcolepsy is one of the underdiagnosed diseases even the first symptoms often appear in childhood. Making diagnosis through history taking is not always easy because the symptoms of childhood narcolepsy are different from those of adulthood. Diagnostic laboratory tests such as sleep studies, tests for human leukocyte antigens, cerebrospinal fluid hypocretin measurement should be considered when the child has excessive daytime sleepiness without cataplexy. Treatment approach should be start as early as possible to avoid secondary academic, emotional difficulties. Both pharmacological and non-pharmacological management, and close cooperation between parents and school teachers should be maintained. In the near future, childhood narcolepsy can be a key to understand the pathogenesis of narcolepsy.

Queue Management using Optimal Margin method to Improve Bottleneck Link Performance

  • In network routers, buffers are used to resolve congestion and reduce packet loss rate whenever congestion occurs at bottleneck link. Most of the existing methods to manage such buffers focus only on queue-length-based control as one loop which have some issues of low link utilization and system stability. In this paper, we propose a novel framework which exploits two-loop control method, e.g. queue-length and congestion window size, combined with optimal margin method to facilitate parameter choices. Simulation results in ns-2 demonstrate that bottleneck link performance can be improved with higher link utilization (85%) and shorter queue length (22%) than the current deployed scheme in commercial routers (RED and DropTail).

Delayed Progressive Extradural Pneumatocele due to Incomplete Sealing of Opened Mastoid Air Cell after Micro-Vascular Decompression

  • A case of delayed progressive extradural pneumatocele after microvascular decompression (MVD) is presented. A 60-year-old male underwent MVD for hemifacial spasm; the mastoid air cell was opened and sealed with bone wax during surgery. One month after surgery, the patient complained of tinnitus, and progressive extradural pneumatoceles without cerebrospinal fluid (CSF) leakage was observed. Revision surgery was performed and the opened mastoid air cell was completely sealed with muscle patch and glue. The patient's symptoms were resolved, with no recurrence of pneumatoceles at 6 month follow up. Progressive extradural pneumatocele without CSF leakage after posterior fossa surgery is a very rare complication. Previous reports and surgical management of this rare complication are discussed.

  • This paper presents an experimental investigation on the heat trensfer characteristic of micro pipe (MHP) array with 38 triangular microgrooves. A heat pipe is an effective heat exchanger operating without external power. The heat pipe transfers heat by means of the latent heat of vaporization and two-phase fluid flow driven by the capillary force. The overall size of the MHP array can be put undermeath a microelectonic die and integrated into the electrronic package of a microelectronin device to dissipate the heat from the die. The MHP array is fabricated by micromachining with a silicon wafer and a glass substrate. The MHP was filled with water and sealed. The experimental results show the temperature decrease of 12.1$^{\circ}C$ at the evaporator section for the input power of 5.9 W and the improvement of 28% in the heat transfer rate.

Conceptual design of cryomodules for RAON

  • The heavy ion accelerator that will be built in Daejeon, Korea utilizes superconducting cavities operating in 2 K. The cavities are QWR (quarter wave resonator), HWR (half wave resonator), SSR1 (sing spoke resonator1) and SSR2. The main role of the cryomodule is supplying thermal insulation for cryogenic operation of the cavities and maintaining cavities' alignment. Thermal and structural consideration such as thermal load by heat leak and heat generation, cryogenic fluid management, thermal contraction, and so on. This paper describes detailed design considerations and current results have being done including thermal load estimation, cryogenic flow piping, pressure relief system, and so on.

  • A series hydraulic hybrid vehicle(SHHV) concept has been explored as a potential pathway to an ultra-efficient city vehicle. Improvements in SHHV fuel economy with reduced emissions strongly depend on their supervisory control strategy. Thermostatic control is simple and reliable but it's cause of frequent engine on-off. Therefore, power follower strategy is presented. In this paper, thermostatic control strategy and power follower strategy is compared for the SHHV model developed using AMESim.

A Numerical Study on Flow-Accelerated Corrosion in Two Adjacent Elbows

  • Flow-Accelerated Corrosion (FAC) is a well-known degradation mechanism that attacks the secondary piping in nuclear power plants. Since the Surry Unit 2 event in 1986, most nuclear power plants have implemented management programs to deal with damages in carbon and low-alloy steel piping. Despite the utmost efforts, damage induced by FAC still occurs in power plants around the world. In order to predict FAC wear, some computer programs were developed such as CHECWORKS, CICERO, and COMSY. Various data need to be input to these programs; the chemical composition of secondary piping, flow operating conditions and piping geometries. CHECWORKS, developed by the Electric Power Research Institute (EPRI), uses a geometry code to calculate geometry effects. Such a relatively simple geometry code is limited in acquiring the accuracy of FAC prediction. Recently, EPRI revisited the geometry code with the intention of updating it. In this study, numerical simulations were performed for two adjacent $90^{\circ}$ elbows and the results were analysed in terms of the proximity effect between the two adjacent elbows.

  • Desktop PC CPUs have been significantly required to be the necessity of thermal management while they have satisfied the extensive data and graphic processing requirements. So the cooling systems assembled with heat pipes embedded in a metal cooling plate, and fins are widely used in the desktop PC markets. Due to a number of demands such as the confined space of desktop PCs, higher heat density of CPUs, and acoustic noise, however, there is the main drive to improve continuously cooling systems. This paper presents the flow and thermal behavior of the cooling system by using the computational fluid dynamics(CFD) code.

Case report of the management of the ranula

  • Ranula is a mucocele caused by extravasation of the sublingual gland on the floor of the mouth. The most common presentation is a cystic mass in the floor of the mouth. A portion of the sublingual gland could herniate through the mylohyoid muscle, and its extravasated mucin can spread along this hiatus into submandibular and submental spaces and cause cervical swelling. This phenomenon is called plunging ranula. A variety of treatments for ranula has been suggested and include aspiration of cystic fluid, sclerotherapy, marsupialization, incision and drainage, ranula excision only, and excision of the sublingual gland with or without ranula. Those various treatments have shown diverse results. Most surgeons agree that removal of the sublingual gland is necessary in oral and plunging ranula. Four patients with ranula were investigated retrospectively, and treatment methods based on literature review were attempted.

CFD 모델링을 이용한 화학공장의 안전거리 산정 방법론에 관한 연구 (A Methodology for Determination of the Safety Distance in Chemical Plants using CFD Modeling)

  • As the simple empirical and phenomenological model applied to the analysis of leakage and explosion of chemical substances does not regard numerous variables, such as positional density of installations and equipment, turbulence, atmospheric conditions, obstacles, and wind effects, there is a significant gap between actual accident consequence and computation. Therefore, the risk management of a chemical plant based on such a computation surely has low reliability. Since a process plant is required to have outcomes more similar to the actual outcomes to secure highly reliable safety, this study was designed to apply the CFD (computational fluid dynamics) simulation technique to analyze a virtual prediction under numerous variables of leakages and explosions very similarly to reality, in order to review the computation technique of the practical safety distance at a process plant.
