Limited Impact of Music Therapy on Patient Anxiety with the Large Loop Excision of Transformation Zone Procedure - a Randomized Controlled Trial

  • Background: Many studies have pointed to strategies to cope with patient anxiety in colposcopy. Evidence shows that patients experienced considerable distress with the large loop excision of transformation zone (LLETZ) procedure and suitable interventions should be introduced to reduce anxiety. This study aimed to investigate the effects of music therapy in patients undergoing LLETZ. Materials and Methods: A randomized controlled trial was conducted with patients undergoing LLETZ performed under local anesthesia in an out patient setting at Ramathibodi Hospital, Bangkok, Thailand, from February 2015 to January 2016. After informed consent and demographic data were obtained, we assessed the anxiety level using State Anxiety Inventory pre and post procedures. Music group patients listened to classical songs through headphones, while the control group received the standard care. Pain score was evaluated with a visual analog scale (VAS). Statistical analysis was conducted using Pearson Chi-square, Fisher's Exact test and T-Test and p-values less than 0.05 were considered statistically significant. Results: A total of 73 patients were enrolled and randomized, resulting in 36 women in the music group and 37 women in the non-music control group. The preoperative mean anxiety score was higher in the music group (46.8 VS 45.8 points). The postoperative mean anxiety scores in the music and the non-music groups were 38.7 and 41.3 points, respectively. VAS was lower in music group (2.55 VS 3.33). The percent change of anxiety was greater in the music group, although there was no significant difference between two groups. Conclusions: Music therapy did not significantly reduce anxiety in patients undergoing the LLETZ procedure. However, different interventions should be developed to ease the patients' apprehension during this procedure.

The Recognition Level of Food Contamination with Residual Pesticides and Hazardous Heavy Metals in Taejon Area (식품의 잔류농약 및 유해중금속의 오염인식도 -대전지역을 중심으로-)

  • This study was to investigate the recognition level of food contamination with residual pesticides and hazardous heavy metals from 365 male and female adults in Taejon area using questionnaires. Among the recognizing level of residual pesticides in overall foods, 69.1% were 'serious', 25.6% were 'average' and 5.3% were 'not serious'. Over 94.7% of the subjects recognized residual pesticides pollution infoods. for hazardous heavy metals in food, 47.8% responsed as 'serious', 40.5% as 'average' and 11.8 as ' not serious' . Over 88.3% of the subjects recognized contaminated pesticides seriously in fruits, 72.1% in vegetables and 51.7% in cereals, whereas 55.7% of the subjects recognized hazardous heavy metal contamination seriously in fruits, 53.4% in vegetables, 40.8% in fishes and shellfishes and 35.0% in seaweeds. The subjects recognized residual pesticides contamination more seriously in overall foods, cereal, potatoes and starches, bean, vegetables and fruits, whereas hazardous heavy metal contamination was recognized more seriously in fishes and shellfishes, and seaweeds food groups. Comparisons were shown based on individuial's occupation. Farmer, forester, iner and fisher showed the lowest recognizing level of food contamination in most food groups. The mean score of the dietary effect by mass media's information on food contamination from residual pesticides and hazardous heavy metals were 3.51±0.96 out of 5 points. By Duncan's multiple range test, sex, age, marriage, food cost per month, concerns about health and nutrition knowledge showed significant differences in the mean effect score at p<0.05. According to a pesticides contamination in several food groups were affected by food cost per month, mass media's information on food contamination, health status, and concerns about health, But a recognition level of hazardous heavy metals in food were affected by income and, food cost per month mass media's information on food contamination, health status, concern about health and nutrition knowledge. People who need to take extreme precautions of food contamination were in order of producers, government officials, homemakers, the consumer's association and consumers.

Daytime Napping and Nighttime Sleep During Pregnancy and Preterm Birth in Iran

  • Objectives: This study investigated the relationship between sleep quality during pregnancy and preterm birth. Methods: This longitudinal study was conducted between August 2018 and May 2019. The participants were 150 pregnant women who had been referred to 7 healthcare centers in the city of Qazvin, Iran and met the inclusion criteria. The Petersburg Sleep Quality Index, the Epworth Sleepiness Scale, and 2 questions about daytime sleep status and a demographic questionnaire were administered at 14-18 weeks and 28-32 weeks of gestation. Data were analyzed using the Mann-Whitney test, the Fisher exact test, and univariate and multivariable logistic regression. Results: In the present study, poor sleep quality affected 84.7% of the participants at 14-18 weeks and 93.3% at 28-32 weeks of gestation. The final model for preterm birth prediction incorporated age and the Petersburg Sleep Quality Index score in the second and third trimesters. Preterm birth increased by 14% with each unit increase in age. With each unit increase in the Petersburg Sleep Quality Index score in the second and third trimesters, preterm birth increased by 42% and 28%, respectively, but the p-values of these factors were not significant. Conclusions: Although a significant percentage of pregnant women had poor sleep quality, no significant relationship was found between sleep quality during pregnancy and preterm birth.

Initial Serum Ammonia as a Predictor of Mortality in Patients with Acute Glufosinate Ammonium Herbicide Poisoning (글루포시네이트 암모늄 제초제 중독환자에서 혈중 암모니아 농도와 사망률과의 연관성에 대한 연구)

  • Purpose: The association of the initial serum ammonia level with in-hospital mortality in patients with acute glufosinate-ammonium herbicide poisoning was studied. Methods: This retrospective cohort study was conducted between March 2012 and August 2019 in the emergency department after glufosinate-ammonium herbicide poisoning. Survivors and non-survivors were analyzed using a Mann-Whitney U test and Fisher's exact test. Multivariate logistic regression analysis was performed to determine the independent risk factors for mortality. Results: One hundred and six patients were enrolled; 11 died, yielding a mortality of 10.4%. The serum bicarbonate level was significantly lower in the non-survival group than the survival group. Age, serum ammonia, blood urea nitrogen, creatinine levels, SOFA score, and APACHE II score were significantly higher in the non-survival group than the survival group. Age, serum ammonia, and creatinine level were independent risk factors for mortality in multivariate logistic regression analysis. Conclusion: The initial serum ammonia level is associated with mortality in patients with acute glufosinate herbicide poisoning.

A Comparative Study on the Nursing Dependency of Suspected COVID-19 Patients and General Patients in the Emergency Department (응급실에 내원한 COVID-19 의심환자와 일반환자의 간호의존도 비교 연구)

  • Purpose: This study was conducted to investigate the nursing needs and workload of nurses according to nursing dependency for effective placement of nursing staff in the emergency department (ED). Methods: In June 2020, 256 adult patients who visited the ED were classified as two groups, suspected COVID-19 patients and general patients. The participants'electronic medical records were analyzed using descriptive statistics, t-test, 𝑥2-test, and Fisher's exact test using the SPSS. Results: The patient dependence score showed a significant difference between the two groups, with an average of 13.99±1.85 for the suspected COVID-19 patient group and 10.58±2.10 for the general patient group (t=12.42, p<.001). There were statistically significant differences in communication (t=3.28, p=.001), mobility (t=3.29, p=.001), nutrition, elimination, and personal care (t=7.34, p<.001) among the six domains of nursing dependency. In the domains of environment, safety, health, and social needs, the dependency score was 3 for all suspected COVID-19 patients and 1 for all general patients. Conclusion: The results of this study confirmed that infection control activities of emergency patients who need isolation affect the patients' nursing dependency on nursing care.

Risk Factors and Clinical Outcomes of Brain Reperfusion Injury after Mechanical Thrombectomy for Ischemic Stroke (기계적 혈전제거술을 시행한 허혈성 뇌졸중 환자의 뇌재관류 손상 위험요인과 임상결과)

  • Purpose: The aim of this study was to investigate the risk factors for brain reperfusion injury in ischemic stroke patients and to analyze the clinical outcomes. Methods: A retrospective study was conducted in 168 patients who underwent mechanical thrombectomy. The data were analyzed using descriptive statistics, t-test, Mann-Whitney U test, Chi-Square test, Fisher's exact test, and logistic regression with IBM SPSS/WIN 24.0. Results: Brain reperfusion injury occurred in 67 patients (39.9%) with a low favored outcome (𝛘2=6.01, p=.014). On multivariable analysis, blood urea nitrogen (Odds ratio [OR]=1.14, 95% Confidence interval [CI]=1.06-1.23), aphasia (OR=6.16, CI=1.62-23.40), anosognosia (OR=4.84, CI=1.13-20.79), presence of both aphasia and anosognosia (OR=7.33, CI=1.20-44.60), and time required to achieve targeted blood pressure (OR=1.00, CI=1.00-1.00) were identified as risk factors for brain reperfusion injury. A statistically significant difference was detected in clinical outcomes, including hemorrhagic transformation (𝛘2=6.32, p=.012), intensive care unit length of stay (Z=-2.08, p=.038), National Institute of Health Stroke scale score at discharge (Z=-3.14, p=.002), and modified Rankin Scale score at discharge (Z=-2.93, p=.003). Conclusion: This study identified the risk factors and presented the clinical outcomes of brain reperfusion injury. It is necessary to consider these risk factors for evaluating the patients and to establish nursing interventions and strategies.

Investigation of the efficacy and safety of ultrasound-standardized autologous blood injection as treatment for lateral epicondylitis

  • Background: There are various conservative treatment options for lateral epicondylitis (LE). The aim is to evaluate pain, daily functioning, and complications after ultrasound-standardized autologous blood injections in patients with LE. Methods: For this prospective cohort study, consecutive patients (>18 years) diagnosed with LE were included. Autologous blood was injected using a medical device containing an injection disposable with 12 small needles (Instant Tennis Elbow Cure [ITEC]) device. Patient-Rated Tennis Elbow Evaluation (PRTEE), subjective elbow score (SES), palpation and provocation pain, satisfaction, and complications of treatment were measured at baseline and two months after treatment. Paired t-tests and Fisher's exact tests were used for calculating the difference between pre- and post-treatment outcomes. Results: Fifty-five elbows were analyzed. Mean time between pre- and post-treatment was 11.1 weeks (standard deviation [SD], 8.9 weeks). The mean PRTEE score decreased from 68.2 (SD, 15.7) before surgery to 53.2 (SD, 25.9; p<0.001) after. The mean SES improved from 36.9 (SD, 20.8) to 51.7 (SD, 27.4; p<0.001). Despite this improvement, only 44.7% of patients showed relevant clinical improvement in PRTEE, and 37.3% showed significant clinical improvement based on SES. Four patients reported a complication and the injection disposable failed three times. Conclusions: Ultrasound-standardized autologous blood injection using the ITEC device is not an effective tool in reducing symptoms related to LE. This study showed that only half of all patients experienced a positive effect. In this heterogeneous cohort of patients, we showed no added value of ultrasound standardization.

Factors Associated with Mortality in Elderly Patients Admitted to Intensive Care Unit due to Acute Drug Intoxication from Suicide Attempts (중환자실에 급성약물중독으로 입실한 자살시도 노인의 사망 관련요인)

  • Purpose : This study aimed to determine the mortality rate among elderly patients admitted to the intensive care unit (ICU) for acute drug intoxication resulting from suicide attempts. It also compared the characteristics of survivors and decedents to identify factors associated with mortality. Methods : This retrospective descriptive study included 150 patients aged 65 years or older who were admitted to the ICU of a tertiary university hospital in Gwangju due to acute drug intoxication, with the period spanning January 1, 2018 to December 31, 2020. The collected data were analyzed using descriptive statistics, independent t-tests, Chi-squared tests, Fisher's exact test, and multiple logistic regression analysis. Results : The mortality rate among elderly individuals admitted to the ICU for acute drug intoxication was 19.3%. The likelihood of death was significantly higher in patients with an acute physiology and chronic health examination (APACHE) III score of 70 or above (OR=23.75, 95% CI=3.78-149.46, p<.001) and those with metabolic acidosis on initial acid-base results (OR=3.73, 95% CI=1.12-12.43, p=.032). Conclusion : These findings underscore the need for developing and implementing systematic education and targeted nursing interventions for ICU nurses caring for acutely drug-intoxicated elderly adults, particularly considering the APACHE III score and the presence of metabolic acidosis.

Comparison of the Effects on Sleep and Vital Signs of the Elderly Between the Hand Bath Group and the Foot Bath Group (수욕(手浴)과 족욕(足浴)이 노인의 수면과 활력징후에 미치는 효과비교)

  • Purpose: The purpose of this study was to identify the effects of hand and foot baths on sleep and body temperature of the elderly. Methods: The research was designed for a nonequivalent control group before and after the test design. The subjects of this study totaled 43, aged 65 and over (23 people in hand bath group and 20 people in a foot bath group), who received a written voluntary consent to participate in experimental treatment and are in the W care center, S Nursing Home, and H care center in Seoul. Data were collected with questionnaires and observations, and were analyzed on the basis of frequency, percentage, mean, standard deviation, ${\chi}^2-test$, Fisher exact test, t-test and paired t-test. Results: There are no significant differences in homogeneity test of a dependent variable between groups (hand bath, foot bath). The differences between 2 groups on the sleep quality score and systolic blood pressure, diastolic blood pressure temperature were not significant statistically except on the pulse. Conclusion: Based on the results of this study, hand bath is as efficient as foot bath. Therefore, a hand bath can be a nursing intervention in order to enrich sleep quality for the elderly because a hand bath is more convenient than foot bath.

The Effects of Seogeum Therapy on Nasal Eosinophil, Nasal Symptoms, and Rhinitis Related Quality of Life in College Students with Allergic Rhinitis (서금요법이 알레르기 비염 대학생의 비호산구, 비염증상, 비염관련 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of Seogeum therapy on nasal eosinophil, nasal symptoms, and rhinitisrelated quality of life in college students with allergic rhinitis. Methods: A nonequivalent control group pre-post test design was used. There were 26 subjects in the experimental group and 26 subjects in the control group. The experimental group received Seogeum therapy on both hands 12 times for three weeks. The collected data were analyzed with SPSS 21.0 version using χ2-tests, Fisher's exact test, independent t-test, and repeated measure of the ANOVA Results: After three weeks the positive rate of nasal eosinophils (p= .048) significantly decreased in the experimental group than in the control group. The experimental group demonstrated significantly lower total nasal symptom score (p= .032) and higher rhinitis-related quality of life (p< .001) compared to the control group. Conclusion: These findings showed that Seogeum therapy is effective as well as beneficial in relieving nasal symptoms and improving the rhinitis-related quality of life in college students with allergic rhinitis. Thus, Seogeum therapy may be an alternative nursing intervention for those suffering from allergic rhinitis.
