Vaginal Reconstruction with Modified Singapore Flap in MRK Syndrome Patients (MRK증후군 환자에서 변형 Singapore피판술을 이용한 질 재건)

  • Purpose: Mayer-Rokitansky-Kuster syndrome (MRK) is second common cause of primary amenorrhea. It is a syndrome of vaginal aplasia and Mullerian duct anomaly. Vaginal aplasia varies from agenesis of whole vagina to aplasia of upper 2/3. For reconstructing vagina, various methods are introduced. Gracilis myocutaneous flap was the first attempt in that the flap is used in vaginal reconstruction. Various flap-based vaginal reconstruction methods have been introduced. Modified Singapore flap (pedicled neurovascular pudendal thigh fasciocutaneous flap) is one of those methods that used posterior labial artery as pedicle, and pudendal nerve branch as sensory root. As its donor lies on inguinal crease that is easily hidden and there are benefits on sexual intercourse by early sensory recovery, authors think that modified Singapore flap is effective for young MRK syndrome patients. Methods: Eight patients underwent surgery between 2008 and 2010. The flap was designed on both groin area with external pudendal artery branch as a pedicle. All flaps were fixated in pelvic cavity with absorbable suture, and additional compression on neovaginal wall was supplied by polyvinyl alcohol sponge ($Merocel^{(R)}$). Results: All patients were successfully reconstructed without flap related complications such as congestion or partial flap loss. The average size of the flap (each side) was 69.34 $cm^2$. Polyvinyl alcohol sponge ($Merocel^{(R)}$) was inserted into neovagina for 5 days on every patient. One case of rectal laceration was occurred while making pelvic pocket by OBGY team. Other complications such as lumen narrowing, wound contracture or vaginal prolapsed were not reported during 8 months of follow up. Conclusion: Modified Singapore flap is a sensate fasciocutaneous flap that is thinner than other myocutaneous flap such as VRAM, and more durable over skin graft. Therefore this is a good choice for vaginal reconstruction in MRK syndrome. And known complications of Modified Singapore flap could be reduced with careful procedure and mild compression techniques.

Posterolateral Thoracotomy versus Muscle-sparing Vertical Thoracotomy (후측개흉술과 근육보존수직개흉술의 임상고찰)

  • Although posterolateral thoracotomy(PLT) has been a standard thoracic incision in resection surgery of the lung for surgeons to achieve a good surgical field, there remains concern about severing a group of thoracic muscles remains. Muscle-sparing vertical thoracotomy (MVT) is an alternative to PLT, which gives cosmetic result and may preserve motion of the shoulder girdle as well as respiratory function of the patient in the early postoperative period. However, surgeons tend not to perfer it because of limited surgical field from the vertical wound made on the lateral thoracic wall. The purpose of this study is to compare the surgical outcomes of PLT versus MVT. We retrospectively reviewed 29 patients(15 who had PLT and 14 who had MVT, organized into those two groups) who had undergone lung resection surgery in our institute. There were no clinical differences between the two groups in terms of operation time, estimated amount of blood loss during the operation, amount of chest drainage on the first and the second postoperative day, duration of chest tube placement, incidence and amount of transfusion, and postoperative complications. We conclude that, from our limited experience, MVT can be applied to lung resection surgery as safely as PLT and that it may have a beneficial role for the patient with compromised lung function in addition to cosmetic effect.

ITER 블랑켓 일차벽 고유접합법 개발을 위한 표준목업 고열부하 시험

  • 국제핵융합실험로 (ITER)의 블랑켓 일차벽 조달 자격 획득을 위한 검증시험을 수행하여 1단계를 2009년 완료하였고, 2단계는 2011년 예정으로 진행 중이다. 우리나라는 이미 1단계 검증시험에서 2 개의 일차벽 목업 (First Wall Qualification Mockup ; FWQM)을 제출하여 시험을 성공적으로 통과하였고, 2단계 검증 시험을 위해 semi-prototype 제작 기술 및 시험 기술을 개발 중이다. 블랑켓 일차벽 검증 시험 및 제작 기술을 확보하기 위한 고유접합법 개발을 위해서 표준 크기($80\;mm{\times}80\;mm$)의 목업을 제작하여 국내 고열부하 시험 시설에서 접합 방법의 타당성을 확인하였다. 표준목업은 HIP (Hot Isostatic Pressing) 접합법으로 stainless steel과 Cu 냉각부를 제작하고, 다시 $80\;mm{\times}80\;mm$ Be tile을 HIP 방법으로 냉각부에 접합하여 제작한다. 고유접합법 개발을 위해서 Be과 Cu 냉각부 계면에 Cr($1\;{\mu}m$)/Cu($10\;{\mu}m$), Ti($5\;{\mu}m$)/Cu($10\;{\mu}m$) 층을 코팅하여 Be 접합 성능을 개선하였으며, 기존의 접합 계면과 차별화된 기술을 확보하였다. 표준목업의 전체 크기는 $80\;mmW{\times}80\;mmL{\times}84\;mmH$ 이고, 1차로 총 6개, 2차로 4개를 제작하였으며 제작 과정 및 제작 전후에 파괴검사, 비파괴검사를 수행하여 접합의 건전성을 확인하였다. 제작 완료된 표준 목업은 냉각 관로를 장착하여 국내의 고열부하 시험시설인 KoHLT-1에 장착하여 성능 시험을 수행하였다. 고열부하 시험 시설의 냉각수 조건은, 온도 $25^{\circ}C$(실온), 유량 0.15 kg/sec이고, 고열 부하 조건에서는 0.5, 1.0, $1.5\;MW/m^2$의 screening 시험을 거친 후 1.5 MW/m2에서 cycle 시험을 진행하였다. 각 목업의 고열부하 시험을 마친 후 비파괴 검사의 일환으로 UT(Ultrasonic test) 시험을 수행하여 열부하 시험 전후의 목업 건전성을 확인하였다. 고유접합법을 이용하여 개발한 표준 목업의 고열부하 시험을 통해서 접합법의 타당성 및 건전성을 확인하였고, 향후 블랑켓일차벽 조달 검증 2단계 시험에서 semi-prototype 제작 및 고열부하 시험에 대비하고, ITER 관련 핵심 기술 개발 목표를 달성할 것이다.

Isolation and Characterization of Expansin Genes in a Halophyte, Suaeda japonica (칠면초(Suaeda japonica) expansin 유전자의 분리 및 특성 분석)

  • Halophytes are unique land plants that are capable of thriving in a high-salt environment. They are attracting public attention due to their ability to synthesize bioactive substances such as UV protectants or antioxidizing agents. To achieve unaffected growth under high salinity, halophytes may take advantage of the activities of cell growth factors such as expansins. Expansins are well-known cell wall proteins that are responsible for cell enlargement. They loosen cell walls, thereby contributing to actual plant growth. This study aimed to identify positive roles of expansins in the growth of halophytes. Three expansin cDNA clones were isolated from seedlings of Suaeda japonica. Comparing the deduced amino acid sequences of the expansin genes of S. japonica with those of other plant species suggested that the cDNA clones isolated from S. japonica belong to the EXPA (${\alpha}$-expansin) gene family. A phylogenetic tree based on the deduced amino acid sequences revealed that the expansins of S. japonica share a close evolutionary relationship with those of strawberry (Fragaria ananassa) and jujube (Ziziphus jujuba), both of which are woody dicots. SjEXPAs did not show any remarkable change in the gene expression level in different NaCl concentrations, providing a clue to the unaffected seedling growth of S. japonica in a high-salt environment. In conclusion, the present study presents the first report of expansin genes from halophytes and suggests a putative role for these genes in plant growth under high salinity.

Crack Growth Analysis due to PWSCC in Dissimilar Metal Butt Weld for Reactor Piping Considering Hydrostatic and Normal Operating Conditions (수압시험 및 정상운전 하중을 고려한 원자로 배관 이종금속 맞대기 용접부 응력부식균열 성장 해석)

  • This study investigates the crack growth behavior due to primary water stress corrosion cracking (PWSCC) in the dissimilar metal butt weld of a reactor piping using Alloy 82/182. First, detailed finite element stress analyses were performed to predict the stress distribution of the dissimilar metal butt weld in which the hydrostatic and the normal operating loads as well as the weld residual stresses were considered to evaluate the stress redistribution due to mechanical loadings. Based on the stress distributions along the wall thickness of the dissimilar metal butt weld, the crack growth behavior of the postulated axial and circumferential cracks were predicted, from which the crack growth diagram due to PWSCC was proposed. The present results can be applied to predict the crack growth rate in the dissimilar metal butt weld of reactor piping due to PWSCC.

Consideration on the Moat of Wolseong Fortress at Gyuongju (경주 월성의 해자(垓字)에 대한 고찰)

  • Our country traditionally employed a defensive system against the aggression by foreign powers by using a town castle and mountain castle. The moat was one of such a defensive system; however, there is few research work on a moat in comparison with its long history. This study was carried out to see the process of the changes of the Wolseong Fortress at Gyuongju, focused on the moat of the scale and nature of the construction methods to analyze such a Wolseong Fortress at Gyuongju as a result of consideration through bibliographical study, on-site investigation, and interviews, etc. This research discovered some facts as follows: the moats of Wolseong fortress at Gyeongju are roughly divided into three types; the first one is a natural moat flowing curved by the south side of Wolseong using the natural stream[Namcheon]as it is; the second one is a pond-type moat made by digging up plane non-rectangular pond along stereobate of castle wall with lakefront built with stream pebbles, and the last one is masonry moat at the east side of Wolseong with chisel-trimmed granites orderly piled in a plane triangular form. Among these, the pond-type moat was identified at the east-north-west side of Wolseong and the pond slopes from east to west as a separate one constructed with the terraces.

Target Reliability Index of Single Gravel Compaction Piles for Limit State Design (한계상태설계를 위한 단일 쇄석다짐말뚝의 목표신뢰도지수)

  • Target reliability index in the limit state design indicated the safety margin and it is important to determine the partial factor. To determine the target reliability index which is needed in the limit state design, the six design and construction case histories of gravel compaction piles (GCP) were investigated. The limit state functions were defined by bulging failure for the major failure mode of GCP. The reliability analysis were performed using the first order reliability method (FORM) and the reliability index was calculated for each ultimate bearing capacity formulation. The reliability index of GCP tended to be penportional to the safety factor of allowable stress design and average value was ${\beta}$=2.30. Reliability level that was assessed by reliability analysis and target reliability index for existing structure foundations were compared and analyzed. As a result, The GCP was required a relatively low level of safety compared with deep and shallow foundations and the currd t reliability level were similar to the target reliability in the reinforced earth retaining-wall and soil-nailing. Therefore the target reliability index of GCP suggested as ${\beta}_T$=2.33 by various literatures together with the computed reliability level in this study.

Performance Evaluation of Hypersonic Turbojet Experimental Aircraft Using Integrated Numerical Simulation with Pre-cooled Turbojet Engine

  • The effect of Pre-cooled Turbojet Engine installation and nozzle exhaust jet on Hypersonic Turbojet EXperimental aircraft(HYTEX aircraft) were investigated by three-dimensional numerical analyses to obtain aerodynamic characteristics of the aircraft during its in-flight condition. First, simulations of wind tunnel experiment using small scale model of the aircraft with and without the rectangular duct reproducing engine was performed at M=5.1 condition in order to validate the calculation code. Here, good agreements with experimental data were obtained regarding centerline wall pressures on the aircraft and aerodynamic coefficients of forces and moments acting on the aircraft. Next, full scale integrated analysis of the aircraft and the engine were conducted for flight Mach numbers of M=5.0, 4.0, 3.5, 3.0, and 2.0. Increasing the angle of attack $\alpha$ of the aircraft in M=5.0 flight increased the mass flow rate of the air captured at the intake due to pre-compression effect of the nose shockwave, also increasing the thrust obtained at the engine plug nozzle. Sufficient thrust for acceleration were obtained at $\alpha=3$ and 5 degrees. Increase of flight Mach number at $\alpha=0$ degrees resulted in decrease of mass flow rate captured at the engine intake, and thus decrease in thrust at the nozzle. The thrust was sufficient for acceleration at M=3.5 and lower cases. Lift force on the aircraft was increased by the integration of engine on the aircraft for all varying angles of attack or flight Mach numbers. However, the slope of lift increase when increasing flight Mach number showed decrease as flight Mach number reach to M=5.0, due to the separation shockwave at the upper surface of the aircraft. Pitch moment of the aircraft was not affected by the installation of the engines for all angles of attack at M=5.0 condition. In low Mach number cases at $\alpha=0$ degrees, installation of the engines increased the pitch moment compared to no engine configuration. Installation of the engines increased the frictional drag on the aircraft, and its percentage to the total drag ranged between 30-50% for varying angle of attack in M=5.0 flight.

  • Purpose and Method: In this study, a questionnaire interview survey was conducted for the victims living in the temporary house by the Pohang earthquake, and improvements were derived. Results: As a result, major improvements in terms of facilities are as follows. First, in order to expand the toilet and cooking space, the existing inner gate and the wall are removed and the width of the toilet is expanded. Minimize the inconvenience by adding a cooking table as wide as the extended toilet. Second, a separate sleep compartment is set up to secure storage space in a limited area. And the storage closet is installed below and used as a storage space. At this time, the size of the sleeping space is set to double bed size. Third, curtains and blinds are installed on both windows to secure privacy, thereby protecting privacy and psychological stability. Conclusion: If the remodeling of the temporary house proposed in this study is utilized and applied, it is possible to provide a better living environment. In addition, it is expected that it will be possible to improve the efficiency of space and overcome existing spatial limitations by minimizing inconveniences reflecting the needs of the victims.

A Study on Historical Research of Archetype of Kyeong Ju Castle (경주읍성 성곽시설의 원형고증에 관한 연구)

  • The purpose of this study is to examine the archetype of the Gyeongju Castle walls and the facilities around it to bring together previous studies and identify the specific scale and structure of the archetype. The findings can be summarized as follows. First, we verified the exact location and dimensions of the castle through an over-lay of land registration maps from the Japanese colonial period, land registration maps from the present day, cadastral maps, and excavation records. Second, the dimension of the rampart and fort of the castle were discovered by combining and analyzing historical evidence, modern research data, historical site studies from the Japanese colonial era, and maintenance records. Third, the historical records related to the yeojang (rampart sub-wall) formats in the Joseon Dynasty were used to examine the archetype of the yeojang. This study led to the discovery of the archetype of the Gyeongju castle yeojang in the early stage of King Munjong's reign (1451), and the archetype created after the Japanese Invasion (16th century). And finally, although most of the rampart is not present, the structure of the castle can be estimated based on historical research, and research materials modern and contemporary(excavation conservation field survey). In addition, this study presented a archetype restoration plan for each site, to be used as foundational data for future restoration projects.
