The effect of reduced thickness in different regions on the fracture resistance of monolithic zirconia crowns (다양한 부위에서의 감소된 두께가 지르코니아 크라운의 파절 저항에 미치는 영향)

  • Purpose. This study aims to evaluate the combined effect of reduced thickness in different regions on the fracture resistance of monolithic zirconia crowns. Materials and methods. Seven nickel-chromium dies were generated from a 3D model of mandibular first molar using the digital scanner with the following geometries: 1.5 mm occlusal reduction, 1.0 mm deep chamfer. Based on the abutment model, Zirconia blocks (Luxen Zirconia) were selected to fabricate Sixty-three zirconia crowns with occlusal thicknesses of 0.3 mm, 0.5 mm, and 1.5 mm, and different axial thicknesses of 0.3 mm, 0.5 mm, and 1.0 mm. All crowns were cemented by resin cement. Next, the crowns were subjected to load-to-fracture test until fracture using an electronic universal testing machine. In addition, fracture patterns were observed with a scanning electron microscope (SEM). Two-way ANOVA and the Tuckey HSD test for post hoc analysis were used for statistical analysis (P < .05). Results. The mean values of fracture resistancerecorded was higher than the average biting force in the posterior region. The two-way ANOVA showed that the occlusal and axial thickness affected the fracture resistance significantly (P < .05). However, the effect of axial thickness on fracture resistance did not show a statistical difference when thicker than 0.5 mm. The observed failure modes were partial or complete fracture depending on the severity of crack propagation. Conclusion. Within the limitations of the present study, the CAD-CAM monolithic zirconia crown with extremely reduced thickness showed adequate fracture resistance to withstand occlusal load in molar regions. In addition, both occlusal and axial thickness affected the fracture resistance of the zirconia crown and showed different results as combined.

A Study of Co-Combustion Characteristics of North Korean Anthracite and Bituminous Coal in 2 MWe CFBC Power Plant (2 MWe 순환유동층 발전 플랜트에서 유연탄과 북한 무연탄 혼소시험 특성 연구)

  • In this study, co-combustion characteristics of Chinese bituminous coal and North Korean anthracite were investigated using a 2 MWe scale circulating fluidized bed power plant. At first, the combustion efficiency of bituminous coal of China and Australia as a function of excess air ratio and temperature were observed. The results showed that the combustion efficiency was influenced by particle size and volatile content of coal, the combustion efficiency of Chinese bituminous coal was over 99.5%. The unburned carbon particles from fly ash and bottom ash were a content 5~7% and 0.3%, respectively. The combustion efficiency with the mixture ratio 20% of bituminous coal and anthracite decreased over 5% because of the increase of entrained particles by a small average particle size of anthracite in the combustor. However, the outlet concentration of $SO_2$ and $NO_x$ was not changed remarkably. The concentrations of the typical air pollutants such as $NO_x$ and $SO_2$ were 200~250 ppm($O_2$ 6%), 100~320 ppm($O_2$ 6%) respectively. The outlet concentration of $NO_x$ was decreased to 30~65% with $NH_3$ supplying rate of 2~13 l/min in SCR process. The $SO_x$ removal efficiency was up to 70% by in-furnace desulfurization using limestone with Ca/S molar of approximately 6.5. With wet scrubbing using $Mg(OH)_2$ as absorbent, the $SO_x$ removal efficiency reached 100% under near pH 5.0 of scrubbing liquid.

OCCLUSAL FORCE AND EMG CHANGE OF MANDIBULAR FRACTURE (악골 골절에서 술 후 교합압 및 근전도 변화)

  • Bite force is created by the force of adjacent teeth accompanied with tension of masticatory muscle. The bite force value is greater in male than in female and ha maximum value at first molar. Masseter muscle is associated with bite force and during muscle contraction the electric signal is expressed in EMG form. The aim of the study is to assess recovery time for masseter muscle activity and according to each part of bite force after open reduction with internal fixation when mandibular angle fracture and subcondyle fracture occurred. And to determine the appropriate period for mandibular fracture patients to have normal masticatory activity. 30 patients with normal bite condition was selected for control group and from April, 2007 to September, 2007, 20 patients who visited our department of oral and maxillofacial surgery of Dankook University, were selected for the study and were diagnosed as mandibular angle fracture and subcondyle fracture. For control group, the bite force for incisors, canine, premolars and molars and activity of the masseter muscle was measured and compared for 1, 2, 3, 4, 6 and 8 weeks. That was divided as fracture side and normal side. Mann-Whitney U test was performed for significant difference and the following result was obtained. 1. The maximum voluntary bite force for incisors, canine, premolars and molars portion were 0.113 kN, 0.182kN, 0.295kN and 0.486kN and the masseter muscle activity was 0.192 volts in the control group. 2. The maximum bite force at fracture side was recovered by 4th weeks for incisors, 6th weeks for canine and premolars and 8th weeks for molars and the masseter muscle activity was recovered by 6th weeks in the experimental group. 2. The maximum bite force at normal side was recovered by 4th weeks for incisors, 6th weeks for canine, premolars and molars and the masseter muscle activity was recovered by 3rd weeks in the experimental group. 3. The method for internal fixation by 2.0mm miniplates at both superior and inferior border had no complications according for twenty patients and had a satisfactory recovery. According to the result, patient with mandibular angle fracture and subcondyle fracture, 8 weeks was required for bite force recovery. Therefore, patients with open reduction and internal fixation under general anesthesis, it can be assumed that 8 weeks was needed after operation in order to have normal bite force and masseter muscle recovery.

Factors affecting root curvature of mandibular first molar (하악 제1대구치의 치근 만곡에 영향을 주는 요인)

  • Purpose : To find the cause of root curvature by use of panoramic and lateral cephalometric radiograph. Materials and Methods : Twenty six 1st graders whose mandibular 1st molars .just emerged into the mouth were selected. Panoramic and lateral cephalometric radiograph were taken at grade 1 and 6, longitudinally. In cephalometric radio graph, mandibular plane angle, ramus-occlusal plane angle, gonial angle, and gonion-gnathion distance (Go-Gn distance) were measured. In panoramic radio graph, elongated root length and root angle were measured by means of digital subtraction radiography. Occlusal plane-tooth axis angle was measured, too. Pearson correlations were used to evaluate the relationships between root curvature and elongated length and longitudinal variations of all variables. Multiple regression equation using related variables was computed. Results : The Pearson correlation coefficient between curved angle and longitudinal variations of occlusal plane-tooth axis angle and ramus-occlusal plane angle was 0.350 and 0.401, respectively (p<0.05). There was no significant correlation between elongated root length and longitudinal variations of all variables. The resulting regression equation was $Y=10.209+0.208X_1+0.745X_2$ (Y: root angle, $X_1$: variation of occlusal plane-tooth axis angle, $X_2$: variation of ramus-occlusal plane angle). Conclusion : It was suspected that the reasons of root curvature were change of tooth axis caused by contact with 2nd deciduous tooth and amount of mesial and superior movement related to change of occlusal plane.

Effects of Volatile Fatty Acids on IGF-I, IGFBP-3, GH, Insulin and Glucagon in Plasma, and IGF-I and IGFBP-3 in Different Tissues of Growing Sheep Nourished by Total Intragastric Infusions

  • Twelve Suffolk${\times}$Small-tail-Han male sheep (body weight 21-26 kg), aged four months, were used to study the effects of volatile fatty acids (VFA) on IGF-I (insulin-like growth factor-I), IGFBP-3 (insulin-like growth factor binding protein-3), GH (growth hormone), insulin and glucagon in plasma, and IGF-I and IGFBP-3 in different tissues. The sheep were randomly divided into four groups with 3 sheep in each group. The sheep were sustained by total intragastric infusions and four levels of mixed VFA (the molar proportion of acetic acid, propionic acid and butyric acid was 65:25:10), which supplied 333, 378, 423 and 468 KJ energy/kg $W^{0.75}$/d, were infused into the rumen as experimental Treatments I, II, III and IV, respectively. The experiment lasted 12 days, of which the first 8 days were for pretreatment and the last 4 days for collection of samples. At the end of the experiment, blood samples were taken and then the sheep were slaughtered and tissue samples from the rumen ventral sac, rumen dorsal sac, liver, duodenum and Longissimus dorsi muscle were obtained. IGF-I, IGFBP-3, GH, insulin and glucagon in plasma and IGF-I and IGFBP-3 in different tissues were analysed. Results showed that the concentration of IGF-I, IGFBP-3, GH, insulin or glucagon in plasma and the content of IGF-I and IGFBP-3 in the rumen dorsal sac, rumen ventral sac, liver or Longissimus dorsi muscle were increased with VFA infusion level (p<0.05). No significant differences were found in duodenum IGF-I between Treatments I and II and in rumen dorsal sac IGFBP-3 between Treatments II and III (p>0.05). It was concluded that IGF-I, IGFBP-3, GH, insulin and glucagon in plasma and IGF-I and IGFBP-3 in rumen dorsal sac, rumen ventral sac, liver and Longissimus dorsi muscle were increased significantly with increasing level of ruminal infusion of mixed VFA.

  • The majority of the commonly used protraction headgears for the protraction of small and/or retropositioned maxilla not allow a change in the point of force application or direction of the force delivery to attain predictable results because of the position of the upper and lower lips to avoid discomfort to the patient. The purpose of this study was to investigate the initial reaction of maxillofacial complex according to the change of force variables such as direction and point of force application with designing an antenna type-modified protraction head gear. A macerated human skull with well aligned upper teeth was used to experimental model and the investigation was done by double exposure holographic interferometry. Fringe patterns of each protraction conditions were compared and analized. The results were as follows. (Frontal view) 1. The Counterclockwise rotation of the maxilla was showed by parallel protraction to occlusal plane and the fringe was decreased in number as higher point of force application. 2. Generally, the number of fringe was increased in 500gm of protraction force than in 300gm. 3. When apply the protraction force to the maxilla with rapid palatal expansion, the direction of fringe patterns was differed from the protraction without expansion. 4. In most of cases, the counterclockwise rotation was decreased in case of the direction of the force is $20^{\circ}$ downward to occlusal plane compared to the parallel direction. 5. At the point of force application is 15mm above and the direction of force is 20 downward to occlusal plane , the translation of the maxillary complex was showed. (Lateral view) 6. The direction of fringe patterns of the facial bones were differed each other by the sutures, and showed almost parallel when apply the 300gm and 500gm of protraction force. 7. In case of rapid palatal expansion with protraction of the maxilla, the fringe patterns between the maxillary area and the area from the posterior of the maxillary first molar to the pterygomaxillary fissure were differed. In case without rapid palatal expansion, the changes of direction and point of the force application did not affect to the direction and the number of the fringe patterns.

A study on the change of alveolar crest height following orthodontic treatment (교정치료와 관련된 치조골 높이 변화에 대한 연구)

  • Alveolar crest is the section of interproximal alveolar bone which includes the free edge of the alveolar process. An increase of the normal forces within limits of tolerance leads to deposition of new bone. If forces are beyond the limits of tolerance, resorption of bone will result whether the force produces pressure or tension. This study was designed to evaluate changes of alveolar bone levels in mesial and distal surface of the left, right first molar, by using pre-treatment, post-treatment panorama films. Two hundreds sixteen subjects were divided into adolescent group of 104 subjects and adult group of 112 subjects, to which orthodontic treatment with a bicuspid extraction (adolescent group-50 subjects, adult group-50 subjects) or without a nonextraction (adolescent group-54 !subjects, adult group-62 subjects) was applied by fixed appliances. Pre- and post-treatment Panorama films were traced, and alveolar crest height was measured. Amounts of changes in alveolar crest height by treatment were calculated md compared in terms of side of tooth, extraction, age. The results were as follows ; 1. When pre-treatment alveolar crest bone levels were compared, levels of adult group were significantly lower than those of adolescent group. 2. Post-treatment alveolar crest bone levels were significantly lower than pre-treatment levels. 3. When changes of alveolar crest height were compared, between adolescent and adult group were not significantly. 4. When changes of alveolar crest height were compared, significantly larger changes were noticed in ex윤action than nonextraction cases. 5. When changes of alveolar crest height were compared, significantly larger changes were noticed in maxilla than mandible. 6. When mesio-distally compared, significantly larger changes were observed in the distal than mesial sides of adult group.

Quantitative evaluation of cortical bone and soft tissue thickness in the mandible (하악 피질골과 연조직의 해부학적 두께를 위한 정량적 평가)

  • Objective: The purpose of this study was to provide an anatomical reference for cortical bone and soft tissue thickness, and the attached gingiva width in the mandible. Methods: Fifteen males and fifteen females participated in this study. An acrylic template was fabricated and the radiopaque markers were bonded on the estimated alveolar crest to take measurements of the hard and soft tissue thickness at the same locations. CT images were taken in samples wearing an acrylic template. Cortical bone and soft tissue thickness were measured at 2, 4, 6 and 8 mm from the alveolar crest in interradicular spaces from central incisor to first permanent molar. The attached gingival width was calibrated. Results: Cortical bone thickness was $1.33{\pm}0.38mm$ and soft tissue thickness was $1.49{\pm}0.54mm$. Cortical bone thickness was increased in the posterior area, while it was not the case for the soft tissue thickness. In addition, the total thickness was $2.82{\pm}0.70$. The attached gingival width was wider in the anterior area compared to that in posterior area. Conclusion: These results suggest that the attached gingiva width should be considered upon placement of mini-implants in the mandibular posterior area for orthodontic anchorage.

The Result of Oral Health Care on Primary School Children Using Dental Caries Activity Test (일부 초등학생의 치아우식활성검사를 이용한 계속구강건강관리 효과)

  • The purpose of this study was to evaluate the effectiveness of oral health care including some of dental caries activity test on dental clinics of regular visiting on primary school children. The dental surveys and initial caries control and some of dental caries activity test (Alban's, Lactobacillius, S'mutans, un-stimulation saliva test)were performed by trained dental hygienists with dentist from on 6 to 13 years old patients of S dental hospital in Gwangju Metropolitan Cities. The sample size of 39 children (18 in oral health care group, 21 in control group ) and accepted to their parents. The results of caries activity test score(Alban's and Lactobacillius test)were reduced 1.4 to 1.6 times in oral health care group(OHCG). Oral Hygiene Simplified Index of OHCG was reduced 1.6 times during six months and 1 year. DMFT index of OHCG was reduced 1.4 times more than Control Group(CG) after 1 year. DT index of OHCG was reduced 12 times more than Control Group(CG) after 1 year. Dental health capacity of the first perment molar of OHCG was reduced 1.02 times more than CG after 1 year. From the above results, long life related incremental oral health care system has reinforced to primary school children in dental clinic with oral health professional teams.

Comparison of treatment effects between the modified C-palatal plate and cervical pull headgear for total arch distalization in adults

  • Objective: The purpose of this study was to evaluate the dental and skeletal effects of the modified C-palatal plate (MCPP) for total arch distalization in adult patients with Class II malocclusion and compare the findings with those of cervical pull headgear. Methods: The study sample consisted of the lateral cephalograms of 44 adult patients with Class II Division 1 malocclusion, including 22 who received treatment with MCPP (age, $24.7{\pm}7.7years$) and 22 who received treatment with cervical pull headgear (age, $23.0{\pm}7.7years$). Pre- (T1) and post-treatment (T2) cephalograms were analyzed for 24 linear and angular measurements. Multivariate analysis of variance was performed to evaluate the changes after treatment in each group and differences in treatment effects between the two groups. Results: The mean amount of distalization at the crown and root levels of the maxillary first molar and the amount of distal tipping was 4.2 mm, 3.5 mm, and $3.9^{\circ}$ in the MCPP group, and 2.3 mm, 0.6 mm, and $8.6^{\circ}$ in the headgear group, respectively. In addition, intrusion by 2.5 mm was observed in the MCPP group. In both groups, the distal movement of the upper lip and the increase in the nasolabial angle were statistically significant (p < 0.001). However, none of the skeletal and soft tissue variables exhibited significant differences between the two groups. Conclusions: The results of this study suggest that MCPP is an effective treatment modality for total arch distalization in adults.
