A retrospective statistical analysis of dental implants (치과 임플란트 환자의 통계 분석에 관한 후향적 연구)

  • Statement of problem: The number of edentulous patients is increasing due to an aging society, which leads to increased demands and interests on implant restorations. Implant restorations are an effective treatment method for both complete and partially edentulous patients, and the success rate has been reported to be high. But because of the increased use of implants in various situations have resulted in frequent reports of failures on implant restorations. Various efforts to overcome these failures have been made. Purpose: The aim of this study was to investigate the statistic survival rate of dental implants on patients who were treated at Dankook University Dental Hospital during the past 10 years based on their charts. Materials and methods :The research was made for two different periods of time; The first investigation was for patients from August, 1998 to August, 2003 and the second was for patients from September, 2003 to April, 2007. Information on the number of implants placed, the location and distribution of implants, the duration between the first and second surgery, and the survival rate of implants were investigated from the patients'charts. Results: 1. According to the first investigation, 1680 implants were placed on 612 patients and the second investigation showed 2438 implants placed on 933 patients. Thus a total of 4118 implants on 1545 patients. 2. Among the 1545 patients, 884 patients were male (57.2 %) and 661 patients were female (42.8%). Out of 4118 implants, 1739 implants (42.2%) were placed on the maxilla, and 2379 implants (57.8%) on the mandible. Implants were placed most frequently in the posterior region of the mandible. A total of 2043 implants (53.2%) were placed in this region. 3. According to the first investigation, 57 out of 1680, implants failed, while from the second investigation, 17 out of 2438 implants were reported as failure. In total, 74 implants failed, which results in a 98.2% survival rate. 4. The average duration between the first and the second surgeries in maxillas decreased from 7.4 months to 6.8 months. The duration also decreased from 5.6 months to 5.0 months in mandibles. Conclusion: As shown in the results, the number of placed implants and the survival rate of implants were higher in the second investigation than that of the first investigation. And the time spent after the first surgery to the second surgery was less in the second investigation. Consequently, it can be presumed that the demand and consumption of dental implants as well as the survival rate will increase in the future.

  • This study analyzes the research trends and evaluates the specialized characteristics of the journal of Korea healthcare architecture. Total 308 papers from 1995(Vol.1) to 2010(Vol.16) were analyzed by following seven criteria: (1) research focus by analyzing the key words of each paper; (2) research method(qualitative and quantitative research); (3) research nature(practical/prescriptive vs. theoretical/normative); (4) research target(healthcare, welfare, and others); (5) researcher characteristics(position, major, and numbers of author); (6) supported research(in-university, out-of-university, and out-of-pocket funds); and (7) international study. The research findings include followings. First, research focus lies in the function of healthcare and welfare facilities. Second, qualitative method is the dominant form of research. Third, practical/prescriptive research continuously increases. Fourth, research target is mainly a healthcare facility, while the research of welfare facility is increasing. Fifth, the considerable numbers of first author are graduate students. Sixth, out-of-university research funds increase. Finally, international studies related to Japan are mostly published.

Prognostic Role of C-reactive Protein in Gastric Cancer: A Meta-analysis

  • Background: A number of studies have investigated the association between increased pretreatment serum C-reactive protein (CRP) levels and the prognosis of gastric cancer. However, due to the inconsistent results, whether the serum CRP level can be a prognostic factor in primary gastric cancer remains controversial. Methods: We searched Medline, PubMed, Embase and the Cochrane Central Register of Controlled Trials for relevant high-quality reports. A meta-analysis was carried out using the included studies to assess the association between pretreatment serum CRP level and overall survival (OS) in patients with gastric cancer. Correlation analyses were conducted to evaluate the relationship between serum CRP and tumor characteristics such as tumor node metastasis (TNM) stage and recurrence. Results: Twelve reports involving 2,597 patients with gastric cancer were included. Primary meta-analysis indicated a significant association between elevated CRP level and poor OS (HR 1.77, 95% CI 1.56-2.00). Subgroup analyses showed no single factor could alter the primary results when we divided the included studies by "number of patients", "max follow-up period", "TNM stage", "treatment" and "cut-off value". Correlation analyses showed that serum CRP level was significantly related to TNM stage (OR 2.96, 95% CI 2.22-3.93) and tumor recurrence (OR 1.81, 95% CI 1.21-2.71). Conclusions: We demonstrated that increased pretreatment serum CRP level (${\geq}10mg/L$) was significantly associated with poor prognosis in gastric cancer patients, either in early or advanced stages.

Clinicopathological observations on the experimental pancreatitis Induced by ligation of pancreatic ducts (췌관 결찰에 의한 실험적 췌장염의 임상병리학적 관찰)

  • Present experiments were undertaken in order to find out the most available diagnostic method for acute pancreatitis. Experimental dogs were divided into experimental group and control group. The experimental dogs were laparotomized and their pancreatic ducts were ligated for the induction of pancreatitis. The control dogs were laparotomized only for comparison. In addition to the complete blood count, serum amylase and lipase activities, serum glucose, total protein and albumin contents were measured daily for 11 days after the operation. Fecal fat droplet count by Sudan III staining and fecal trypsin activity examination by x-ray film digetion test were also undertaken daily. The results obtained were summarized as follows; 1. Serum amylase activities of the experimental group increased to peak on the third day and returned to the preoperative values on the eighth day. 2. Serum lipase activities of the experimental group increased to peak on the first day and returned to the preopertive values within six to eight days. 3. Serum glucose contents of the experimental group showed significant increse only on the first day. 4. Serum albumin contents of the experimental group decreased significantly during the experimental period. Whereas those of the control group significantly decreased only on the first day. 5. The experimental group showed marked leukocytosis, neutrophilia, and lymphopenia for the first 5 or 8 days. Whereas the control group showed only neutrophilia for the first 3 days. 6. The results of fecal fat droplet count showed some diagnostic value on the third and fourth day in only one experimental dog(No 1). No significant changes in the fecal trypsin activity were noticed in all dogs. 7. Histopathologically. all dogs of experimental group showed changes of pancreatitis. However the degree of the pancreatic lesion was not pararell to the degree of the serum amylase or lipase activity changes.

Cut out effect on nonlinear post-buckling behavior of FG-CNTRC micro plate subjected to magnetic field via FSDT

  • This research is devoted to study post-buckling analysis of functionally graded carbon nanotubes reinforced composite (FG-CNTRC) micro plate with cut out subjected to magnetic field and resting on elastic medium. The basic formulation of plate is based on first order shear deformation theory (FSDT) and the material properties of FG-CNTRCs are presumed to be changed through the thickness direction, and are assumed based on rule of mixture; moreover, nonlocal Eringen's theory is applied to consider the size-dependent effect. It is considered that the system is embedded in elastic medium and subjected to longitudinal magnetic field. Energy approach, domain decomposition and Rayleigh-Ritz methods in conjunction with Newton-Raphson iterative technique are employed to trace the post-buckling paths of FG-CNTRC micro cut out plate. The influence of some important parameters such as small scale effect, cut out dimension, different types of FG distributions of CNTs, volume fraction of CNTs, aspect ratio of plate, magnitude of magnetic field, elastic medium and biaxial load on the post-buckling behavior of system are calculated. With respect to results, it is concluded that the aspect ratio and length of square cut out have negative effect on post-buckling response of micro composite plate. Furthermore, existence of CNTs in system causes improvement in the post-buckling behavior of plate and different distributions of CNTs in plate have diverse response. Meanwhile, nonlocal parameter and biaxial compression load on the plate has negative effect on post-buckling response. In addition, imposing magnetic field increases the post-buckling load of the microstructure.

Investigation of 'First-Night Effect' in Normal Young Adult Male Subjects on Polysomnography (젊은 정상인 남자에서 수면다원기록상의 '첫날밤 효과' 분석)

  • Objectives : 'First-night effect' has been a well-known concept since 1960's. It is important because it is one of the major factors to be considered in assessing the reliability of polysomnographic data. However, 'reverse first-night effect' has also been described, resulting in the inconsistency of conceptualization. We attempted to investigate on the first-night effect in adults by having each of them take two nights of polysomnography in a controlled environment. Young healthy adult volunteers were chosen as subjects in order to rule out age- or health-related confounders. Methods : Polysomnography was performed on eight male medical students (mean $age=23.5\;{\pm}\;0.9$) for two nights with Grass model 78 polysomnograph. We scored manually under the standard protocol each epoch of the sleep records. Sleep variables were obtained and compared between the two nights. Results : Sleep period time(SPT) and total sleep time(TST) of the third fraction of night were significantly longer on the first night than on the second night (p<0.05). However, other sleep variables such as percentage of each sleep stage, sleep latency, REM sleep latency, number of waking, and sleep efficiency were not different between the two nights. Conclusion : We could not confirm the existence of first-night effect in this study. In healthy young male adults, it may not happen at all or may happen to a very negligible degree. Young healthy adults may have more adaptability to a new sleep environment. Also, the provision of a reasonably comfortable sleep environment could have helped them with abolition of first-night effect.

Are the educational contents on Family Life field in the primary, middle and high school specified in the contents of the textbooks\ulcorner The results are as follows 1〉Considering the sequence of educational contents on Family Life field in primary school, we can find out that the subject aims have relevance to those of the forth and fifth grade in primary school, and those of the forth grade in primary school are specified in the contents of curriculum of the forth grade and the contents of curriculum of the forth grade are specified in the textbooks for the forth grade in primary school. The subject aims of the fifth grade are specified in the contents of curriculum of the fifth grade and the contents of curriculum of the fifth grade are specified in the textbooks for the fifth grade. 2〉Considering the sequence of educational contents on Family Life field in middle school, we can find out that the subject aims have relevance to those of the first and second grade in middle school, and those of the first grade in middle school are specified in the contents of curriculum of the first grade and the contents of curriculum of the first grade are specified in the textbooks for the first grade in middle school. Age Estimation by Radiological Measuring Pulp Chamber of Mandibular First Molar in Korean Adults

  • Purpose: The purpose of present study was to develop a method for assessing the chronological age of Korean adults based on the relationship between age and size of pulp cavity using dental radiographs of mandibular first molars. Methods: A total of 325 dental radiographs of Korean adults with known age and gender were selected for the study (199 males and 126 females) which were taken in the period between January 2009 and June 2014 at the Pusan National University Dental Hospital. The measurements were carried out on both orthopantomographs (OPGs) and intraoral periapical radiographs of mandibular first molar and the following ratios were calculated: pulp chamber floor height ratio (F/L), pulp chamber ceiling height ratio (R/L), and pulp chamber depth ratio (D/L). Results: The ratios of measurements on intraoral periapical images of mandible first molar generally produce more reliable data than the measurements on OPGs. The pulp chamber floor height ratio and pulp chamber thickness ratio showed significant correlation with age, whereas the pulp chamber ceiling height ratios showed weak correlation with age. It was found that the best correlations between the ratios and age were found for pulp chamber thickness ratios (r=-0.731 to -0.751). The multiple regression models were derived using 3 ratios that were significantly correlated with age. The determination coefficients ($R^2$) of the models ranged from 0.556 to 0.596. Conclusions: Our results indicate that the pulp chamber thickness and pulp chamber floor height in mandibular first molar are an age-dependent variable in adults which can be used to estimate age with reasonable accuracy. The higher image quality of dental radiographs will probably narrow the age estimation error and improve dental age estimation.

Research on the prevention of legal dispute over 119 rescue team (119구급대의 법적분쟁 예방에 관한 연구)

  • Purpose : To check the legal relation between rescue team and patient as well as legal responsibility for patient's damage intentionally or erroneously caused by rescue member, a public official, in the performance of relevant job ; to prevent legal dispute over rescue team and to present program for fair settlement of dispute and equitable and feasible burden of damage. Method : First, the legal principle of Civil Law, Criminal Law and Administrative Law related to the theme of this research will be investigated around research by literature. Second, the case of dispute related to rescue team will be introduced. Result: 1. If 119 rescue members as a public official intentionally or erroneously cause damage to patient in the performance of job, they shall bear civil, criminal and administrative responsibility. They shall bear civil responsibility for indemnity for damage due to default or tort. The typical criminal responsibility includes accidental homicide arising out of duty, preparing falsified official document, dereliction of duty, etc. In the administrative side, the state is responsible for indemnity for peculiar status of the rescue member, public official. 2. Though raising civil petition or legal dispute over unsatisfactory rescue service may be reasonable to guarantee the right of nation, such action may cause stress to rescue member as well as may lead to mental shrinking and defensive attitude only to take the basic first aid treatment which has low possibility of mistake instead of active first aid treatment so as to avoid legal responsibility. 3. The program that may prevent legal dispute over 119 rescue team includes expansion of manpower specialized in first aid treatment, enhancement of education on legal environment, development and application of standard job guideline, formation of mutual trust with patient, detailed explanation, preparing and keeping minute record, improvement of the rescue members' ability of first aid treatment and development of medical instruction mode. Conclusion : The best policy is to prevent legal dispute. If it is impossible to basically exclude the possibility of dispute, however, we need to make effort to minimize the occurrence, settle fairly and divide damage equitably and feasibly. To improve the preventible death rate of our first aid system to the level of advanced country, 119 rescue team which is in charge of the stage before hospital needs to positively enforce special first aid by improving the qualitative level of rescue service and to strive to prevent legal dispute that may occur in the process.

A Study of Patterns of Women's Jackets (Jeogori) in Modern Korea (근대 여자한복 유물의 문양연구)

  • This thesis has three main purposes: first, will show kinds, patterns, types, expression techniques of modern korean women's trimmings by every ten years. Second, it is to trace the effect of each decade on women's Hanbok trimming designs. Third, it will point out factors. that caused the change of modern Korean women's Hanbok trimming design patterns. Henceforth, there comes the result of the research and analysis. Trimming design patterns of modern women's Hanbok can be divided into three categories temporally: The first period of from 1910s to the first half of 1950s saw that design patterns did not changed much from traditional ones. It is because there were many catastrophes such as the invasion of Japanese Empire and Korean War. The compound design had expressed people's wish to earn many sons and babies at the end of Joseon Dynasty. But it turned to wishing luck for individuals affected by individualism and liberalism imported from the west. Realistic designs are the most frequently used and the least lines made up patterns. The second period of from the latter half of 1950s to 1970s saw that women wore their Jeogories and Chimas as suits. Screen printing technique enabled for various design patterns to come out. And the development of textile industry introduced various expression techniques such as making lace, meshing, flocking, burn-out etc cetera. The third period of 1980s saw that Hanbok became a kind of luxurious clothe as people's living standard rose. Hanbok again came into the spotlight, Hanbok designers came forward and their individualities are respected. The characteristic expression technique of grey-dying and geometric design patterns are more frequently used for the simple and modern beauty.
