Study of the oriental medical literature for traditional childbirth (전통적 출산법에 대한 문헌적 고찰)

  • The community of women into society in various fields is expanding, and married late, and accordingly, the first child birth is increasing age. Accordingly, dystocia has also increased. Listed in the literature that the traditional method of birth seemed to oriental medical interpretation of one of the ancient wisdom of the predecessors to explore. The result of this study is summarized as follows. 1. Child birth custom divide into childbirth preparation, childbirth and postpartum care. Postpartum care divide into preparation of the birthplace, food for pregnant woman and birth preparation of the necessary things. 2. Preparation of the birthplace of the court case and that qeen is three months, one month before that in the case of concubines were installed. The difference in manners, and actual number but the configuration of the same item was prepared. In the private, birthplace is installed husband's home or parent's home. 3. Rice and seaweed is food for pregnant woman. San Miguel (产 米), sangwak (山 藿) specifically called was selected by Keep elaborate. 4. 1 month before birth to baby clothing is usually prepared. For the safe of a paturient woman and a baby, magical prescription was practiced in so-guk-so(昭格署) in early chosun dynasty. After so-guk-so(昭格署) abolished magical practice was stopped. Then Wore old clothes based on dong-ui-bo-gam(東醫寶鑑) 5. Im-san-ye-zi-bub(臨产豫智法) specifically to instruct the royal birth is the birth of guidelines. Im-san-ye-zi-bub(臨产豫智法) cosist of birth place, abdominal pain Precautions, food, care after childbirth, remove the plecenta posion, method of cut navel, bath, protection. Through various court records of the situation, according to the guidelines are properly applied based on dong-ui-bo-gam(東醫寶鑑).

Experiences of Korean Women in Choosing the Date of Childbirth (산모의 출산 택일 경험에 관한 융합적 연구)

  • The purpose of this study was to explore the experience of choosing the best days for childbirth and to understand the nature and meaning of the experience. The qualitative method of data collection was used, and in-depth face-to-face interview were conducted. Participants included four women who had given the day of birth set as the desired date. Van Manen's phenomenological method was adopted to analyze data. Four theme clusters emerged from the analysis: "The first gift to my baby"; "Collaboration with others for my baby"; "Choosing a date for childbirth such as blueprint of child"; and "Complex feelings." Our findings helped in understanding the experience of Korean women in choosing the best days for childbirth. Health care providers should offer appropriate advice and show sufficient empathy as well as emotional support by considering cultural expectations.

Effect of Breast-feeding Education and Follow-up care on the Breast-feeding Rate and the Breast-feeding Method - Focused on Home Visit and Phone Counselling - (모유수유교육과 추후간호방법이 산모의 모유수유실천율과 모유수유방법에 미치는 효과 - 가정방문과 전화상담을 중심으로 -)

  • This was a pre-experimental study to confirm the breast-feeding ability and effect of follow up care on the breast-feeding rate and the breast-feeding method by the mother-infant breast-feeding assessment tool for the mothers who got the breast-feeding education. The subjects were 46 mothers who experienced normal delivery of infants at a college hospital in K-city, Kyungsangbuk-do, from July 1 to October 21, 2000. The instrument for data obtainment were The Mother-Infant Breast-Feeding Assessment Tool of Johnson et al. (1999), and The Breast- Feeding Method Measurement Tool of Jeong, Geum-hee(1997). This instrument was reliable, showing Cronbach $\alpha$.751. This study classified them into 3 groups: at high risk for breast-feeding failure, at risk for breast-feeding problems, and at low risk for breast-feeding failure by the mother-infant breast-feeding assessment tool on the day of discharge from the hospital after delivering individual breast-feeding education to the subjects. This study investigated the breast-feeding rate and the breast-feeding method through mail questionnaire at the four week after childbirth, and through the phone counselling and the home visit for follow up care at the first week and the second week after childbirth. The sixth week after childbirth, this study investigated the breast-feeding rate by phone. The data analyzed the hypothesizes by $x^2$-test, paired t-test, ANOVA, Wilcoxon signed rank test, Wilcoxon rank sum test and trend analysis using SPSS/PC+ WIN 10.0 program. The results were as follows : 1) Hypothesis 1-1, "there won't be any difference the breast-feeding rate of a group at risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t= -1.501, p=.270). Hypothesis 1-2, "there won't be any difference the breast-feeding rate of group at low risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t=-1.732, p=.225). 2) Hypothesis 2-1, "there won't be any difference between the breast-feeding method of group at risk for breast-feeding failure for four weeks after childbirth and just after childbirth” was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-7.267, p=.000). Hypothesis 2-2, "there won't be any difference between the breast-feeding method of the group at low risk for breast-feeding failure for four weeks after childbirth and just after childbirth" was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-2.501, p=.012). 3)The 3rd hypothesis, "there won't be any difference between breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure at the 4th week after childbirth and just after childbirth" didn't show any difference between the breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure in the advance test(t=-1.521, p=.130) but there was difference between them in post test (t=-2.012, p=.044). As a result, the 3rd hypothesis was supported by pre test, but it was rejected by post test. In conclusion, this study confirmed breast- feeding education and follow up care just after childbirth were effective for the breast-feeding rate and method. Accordingly, it is proposed that successful nursing intervention of breast-feeding to be necessary by continuously providing follow up care through the mother-infant breast-feeding assessment tool as well as to execute individual breast-feeding education to mothers just after childbirth.

The Moderating Effects of Birth Rate Policies between Korean Women's Values of Children and Their Intentions for Subsequent Childbirth (기혼여성의 자녀가치관과 추가출산의향 간의 관계에서 출산장려정책의 조절효과 분석)

  • This study aims to analyze whether both childcare subsidy and childcare leave policies have moderating effects on the relationship between Korean women's value of children and their intentions for subsequent childbirth. The data are used from the 2015 National Survey on Fertility and Family Health and Welfare. Both hierarchical multiple regression analysis and moderated regression analysis are used for statistical analysis. The findings from the study are as follow. First, the ideal number of children, instrumental values, and emotional values have positive effects on the intentions for subsequent childbirth after controlling for background variables(level of education, income, age, and number of children). Second, childcare leave policy has no significant effect on the intentions for subsequent childbirth while childcare subsidy policy has the negative effect. Third, only childcare subsidy policy has moderating effects on the relationship between instrumental, emotional values, and the intentions for subsequent childbirth. These results suggest that policies enhancing the value of children should be implemented in addition to traditional birth rate policies. Furthermore, new birth rate polices are needed for those married women who have a high possibility of subsequent childbirth.

A Prospective Observational Study on Symptoms of the Postpartum Women with Korean Medicine Treatment during Six Weeks after Childbirth (한의치료를 받은 산모의 산후 6주간 증상에 대한 전향적 관찰 연구)

  • Objectives: This study aims to investigate the changes of women's postpartum symptoms, including their weights, edema index and quality of life (QOL) over the first six weeks after childbirth. Methods: The study participants were 31 postpartum women treated in the Dept. of Obstetrics & Gynecology, at Woo-Suk University Korean Medicine Hospital, from May 20th to August 5th, 2016. In the first 2 weeks of the postpartum period, the main data collection method was to interview each patient and write down all of their complaints, and additionally referring to each participant's PHR (Personal Health Records). At the 3rd, 4th, 5th and 6th week, the researcher interviewed participants by phone once a week. The participants' weight, edema index (ECW/TBW) and the quality of life (by EQ-VAS) were measured at 1st and 2nd week after childbirth. And, EQ-VAS was assessed once more at the 6th week after childbirth. As a last step, the participants responded to a survey on satisfaction regarding their postpartum care with Korean Medicine. Results: 1. In the 1st one week after childbirth, edema was the most frequent complaint. It was about joint pain of the upper limbs in the 2nd and 3rd weeks, sweating in the 4th week, and joint pain of the upper limbs in the 5th and the 6th weeks. 2. In the 1st two weeks of the postpartum period, the weight of the participants decreased from $66.33{\pm}9.30kg$ to $62.60{\pm}8.92kg$ (p<0.001) and the edema index ECW/TBW decreased from $0.399{\pm}0.010$ to $0.385{\pm}0.0 4$ (p<0.001). The EQ-VAS significantly improved from $61.77{\pm}17.72$ to $73.51{\pm}14.67$ (p<0.001). In the last 4-weeks of the postpartum period, the EQ-VAS decreased from $74.30{\pm}14.25$ to $73.63{\pm}13.35$, but this difference was not statistically significant (p=0.749). 3. Regarding the satisfaction with postpartum care with Korean Medicine, 60% of the participants responded that it was 'Excellent' and 40% said it was 'Good'. Conclusion: Over the entire postpartum period, the most frequent complaints were about musculoskeletal symptoms. Postpartum care with Korean Medicine treatments in the early postpartum period improves various postpartum symptoms and the quality of life for postpartum women.

A study on analyzing effectiveness of childbirth education (임부교실 운영효과 분석을 위한 일 연구)

  • The purpose of this study is to provide basic data regarding effective learning opportunities in childbirth education classes. Also analysis of the data indicates the optimum conditions for the welfare and improvements in the promotion of health in childbearing mothers. The results of this study are as follows; 1) The average age of the subjects in this study was 30.6 years and the total number of subjects was 58 pregnant women. The average number of children was one and 84.5% of the subjects were unemployed even though 63.8% of them held over bachelor's degrees. It was found that 22.4% of the subjects were living in an extended family. Also 61.5% of them were living with parents-in-law. The number of pregnancies were calssified as one, two, or three to nine times with the percentages of 58.7%, 22.4% and 18.9%, respectively. Further, 72.4% of the subjects had no abortion experience and 15.5% had one aborion experience. While 89.7% of the subjects planned to feed their babies with breastmilk, mixed feeding were used by only 22.4% of the sample. These data were collected at about 6 months after delivery. Thus one can see that a low rate of breastfeeding was common. 2) The length of one period of childbirth education is four weeks. It was found that 36.2% of the subjects participated in childbirth education only once, where as 13.8% participated four times and 19% of the subjects participated in this class more than four times. pregnant at least once. Further, 75.9% of the participants were participated in this education through their own will. Their motivation for participation developed through information, advertisement and posters which contained information on childbirth education. Those with unplanned pregnancies 92.9% participated after a suggestion by the nurses. The number of participants in terms of percentage according to the childbirth education contents can be classified as following. The most active participation was shown in preparation of delivery(77.6%), postpartrm management(56.9%) fetal development(37.6%) and physiology of pregnancy(17.2%). It was found that 75.9% of the subjects were willing to participate again if they were given a chance. The reason can be summarized as following: The content of the education is very helpful(47.7%). Scientific knowledge can be obtained through this program(20.5%). Participation helps in achieving psychological stability(9.1%). Participation enables one to establish a friendly relationship with other participants(6.8%) of the sample. 24.1% of the participants did not want to participate again. The reasons can be as following: They do not want another baby(42.9%). The first paricipation in childbirth education gave enough knowledge about childbirth(21.4%). Another reason for not want to participate again was because they had a cesarean birth(14.3%). Only 7.1% of them responded with a negative view. A response that they do not need childbirth education after their operation can be traced back to the general belief that childbirth education is the place where one prepares for natural birth through the Lamaze breathing technique. Of the subjects, 91.4% suggested that this program could be recommended to other childbearing mothers, because this program gave educational content along with psychological stability for childbearing women. Of the subjects 41.4% did not see any efforts towards the welfare of the baby, where as 88.2% did. Among the subjects 58.6% made some effort to eliminate the discomfort of labor by breathing and imagination and breathing and walking. Further 41.7% of the 24 subjects did not do anything toward the welfare of the baby, because they did have a cesarean section so that they didn't have a chance even though they had been educated about childbirth. Also 33.3% of the subjects did not do anything toward the welfare of the baby, because they lacked a willingness. After leaving the hospital, only 75.9% of the subjects did some exercises. The subjects who tried participate this program with their husband accounted for 20.7% of the sample. Interviewing with the subjects solved some of the uneasiness and. fear of delivery, increased self-confidence in parenting and active coping in the delivery process.

The Second Childbirth Preference of Married Women with a Child (한 자녀를 둔 기혼여성의 둘째자녀 출산선호에 관한 연구)

  • The purpose of this study was to examine how married women with a child perceived the low-fertility issue in order to increase the success of policy decisions for increasing the fertility rate and to present alternatives. The study subjects were 327 married women with a child who resided in Dongjak-gu, Seoul. After the survey was conducted, the collected data were analyzed with SPSS 11.5 program. The findings of the study were as follows. First, the married women who already had one child found it ideal to have two children. In fact, however, 36.4% had determined not to have another child, and only one parent wanted to have a second child in 26.9%. Second, they took a dismal view of the government's fertility encouragement policies, and they considered it necessary for the government to pursue a more down-to-earth measure. Third, economic aid for child rearing and education was identified as the most critical way to boost the childbirth rate. In conclusion, current national policies aimed at increasing the low-fertility rate are redundant if they are only designed to publicize government action. Actions need to be taken to assist married women in taking better advantage of the existing policies.

A Study on the Transition to Parenthood Expectation on Reward/Cost and Martial Relation of First Pregnant Women (초임부의 부부관계와 부모기전이 보상/비용기대감에 관한 연구)

  • This study focuses on the beginning of transition to parenthood expectation and applies the exchange theory in order to examine reward and cost of transition to parenthood expectation and picks up the main cause effecting it. This study conducted structured questionnaires to total 330 first pregnant women and examined descriptive statistics and t-test F-test analysis by using collected data. And here are the result of research. First the transition to parenthood expectation expected rewards much more than costs and had much importance on individual aspects in parenthood. The more were they aged hardly educated and housewives the higher did they showed expectation of reward. The less did they have high education the more did they expect costs of the transition to parenthood expectation. Second to examined the reward/cost of transition to parenthood expectation according to martial satisfaction the more did they have martial satisfaction the more did they expectation of the rewa ds on transition to parenthood expectation. On the contrary the more did they respect from husbands the less did they cost of transition to parenthood expectation. This study would be helpful to lead family life having childbirth to the positive way be examining the process from childbirth plan to delivery. Also it will be applicable to seek for the methods to help would-be parents as basic data on material on education about transition to parenthood.

The Changing Pattern of Physical and Psychological Health, and Maternal Adjustment Between Primiparas Who Used and Those Who did Not Use Sanhujori Facilities (산후조리원 이용여부에 따른 초산모의 신체적, 심리적 건강상태 및 모성역할적응의 변화양상에 관한 연구)

  • Purpose: The purpose of this study was to compare levels of postpartum fatigue, depression, childcare stress, and maternal identity according to postpartum period between primiparas who used Sanhujori facilities and those who did not. Methods: The research design was a longitudinal descriptive study using self-report questionnaires. Participants were 55 healthy primiparas who delivered at one of 3 hospitals in Chungnam, 21 using Sanhujori facilities and 34 not using these facilities during the first three weeks after childbirth. Data were collected from October 2008 to April 2009 at three measurement points, 2-4 days after childbirth (T1), 4-6 weeks (T2), and 12-14 weeks (T3). Data were analyzed using the SPSS 17.0 WIN program. Results: There was a significant difference in childcare stress between the two groups at 4-6 weeks after childbirth. Postpartum depression and childcare stress at 4-6 weeks were significantly higher than those of the other postpartum periods, while maternal identity was significantly lower. Conclusion: Child care stress is the most important issue among women who use Sanhujori facilities and the 4-6 week period after childbirth is very difficult to primiparas. These results indicate that nursing interventions for primiparas in Sanhujori facilities should focus on reducing childcare stress. Furthermore proper follow-up programs at 4-6 weeks are needed to decrease the difficulties in adjustment by new mothers.

A Study on the Lifestyles of University Students and their Perception of Childbirth with a Focus on the Gyeongsangnam-do Area (대학생의 라이프스타일 유형과 자녀관에 관한 연구 -경상남도 지역을 중심으로-)

  • The purpose of this study was to investigate the impact of lifestyle on the perceived value of children in university students. Questionnaires were distributed to 467 university students with questions covering topics including general personal information and characteristics, lifestyle type, and value of children. First, a cluster analysis based on lifestyle showed that the subjects could be categorized into three groups: negative-leisure types, active-achievement types, and affirmative-fidelity types. The negative-leisure type was highly leisure-oriented and showed a negative self-value; the active-achievement type showed a perspective that valued personal and occupational self-development and a high level of labor-oriented value; and the affirmative-fidelity type favored materialism, seeking occupational aptitude and worth and showing a positive self-value. Second, an investigation of the impact of these three lifestyle types on the students' value of children revealed that the active-achievement type group had high level of belief in the necessity of childbirth, the affirmative-fidelity type group highly valued fulfilling an active parent role, and the active-achievement type group were inclined to delay childbirth. Further, the negative-leisure type group wanted the lowest number of children, while the affirmative-fidelity type group wanted the highest number of children. This study reveals that university students' lifestyle does have an effect on their value of children. It is therefore proposed that a value-focused population education for increasing fertility should relate not only to individuals' value of children but also to their broader lifestyle.
