Pattern Analysis in Patients with Hypertension grades (고혈압 단계별 변증특성 분석)

  • To investigate and characterize basic patterns of prehypertension. Participants were divided into three groups; ideal blood pressure(n=40), prehypertension(n=62), and stage 1 hypertension groups(n=68) according to the JNC 7 standards. They answered questionnaire asking various symptoms and received clinical examination. Data were collected and analyzed in the focus of three Zheng patterns, cold-heat, deficiency-excess and four major hypertension types. Analysis of variance was used to find differences among groups. In addition, relationship between the cold-heat trends and risk factors of hypertension were analyzed using Pearson's correlation analysis. Three risk factors; age, body mass index (BMI), and fasting blood sugar (FBS) showed intergroup differences. Statistical significances were revealed in the cold-heat pattern and two hypertension subtypes. The cold scores decreased from $4.8{\pm}1.84$ to $3.9{\pm}1.88$ and $3.7{\pm}1.27$ (p=0.022), while the heat increased from $1.9{\pm}1.32$ to $2.8{\pm}1.72$ and $2.8{\pm}1.48$ (p=0.009). Additionally two hypertension subtypes, the excessive liver fire, and the yin-yang deficiency showed significant differences. Cold had negative correlations with blood pressure (both systolic and diastolic), BMI, triglyceride, and FBS. Heat had positive correlations with systolic blood pressure, BMI, triglyceride, and FBS. Prehypertension could be characterized by using the cold-heat patterns. The cold-heat are correlated with Blood pressure, BMI, blood lipids and sugar.

A Study on Changes of Symptoms and Signs Diagnosised by Differentiation of the Pattern Identification in Acute Ischemic Stroke Patients with Collaborative Treatment (한·양방 병행치료를 시행한급성기 뇌경색 환자에게 변증(辨證)별 평가항목 호전도에 대한 연구)

  • Object : The purpose of this study is to observe the changes of symptoms and signs diagnosised by differentiation of the pattern identification in stroke patients. Methods : Seventy subjects were recruited from patients with stroke within ten days of onset. We chose twenty-nine subjects diagnosised as same differentiation of the pattern identification, and who had at least on follow up session. We had investigated change of symptoms and signs diagnosised by differentiation of the pattern identification. Results : There were five symptoms and signs (thick fur, dry fur, difficult defecation, heat vexation and aversion to heat, normal pulse) and ten (heavy-headedness, frequency of defecation, hard defecation, feel heavy, slippery pulsem, dry mouth, bitter taste in the mouth, feel lazy, look lazy) symptoms and signs that were statistically significant improved in fire-heat patterns and dampness-phlegm pattern, respectively. Conclusions : This study provides evidence that collaborative treatment is effective in improving some symptoms and signs in acute ischemic stroke patients diagnosised by fire-heat pattern and dampness-phlegm pattern. Further studies with larger scale, longer observation period would be required.

A Study on the Combustion Characteristics of the Crown of Pine Trees in the Drying Season (건조기 소나무 수관부 부위별 연소특성에 관한 연구)

  • Pine trees, which account for 23% of the forested area of the Republic of Korea, are highly vulnerable to fire in comparison to broad-leaved trees due to the presence of consistent water tube sections throughout the year and resin that is composed of approximately 20% oil. In addition, the pattern of forest fires is determined by weather, topographic conditions, and fluctuation in moisture content. Therefore, when fire breaks out in pine tree forests during the dry season (January to March), it is difficult to extinguish, and it quickly spreads. In this study, the combustion characteristics of pine needles, pine cones, and pine branches in the water tube sections of living pine trees were compared and analyzed in accordance with the moisture content as per the ISO 5660-1. The monthly moisture content was analyzed from January to March, and it was found to be the lowest in March, with 53.6% for pine needles, 51.9% for pine branches, and 10.9% for pine cones. In particular, pine cones were more vulnerable to fire as compared to pine needles and pine branches because their moisture content was more than five times lower than that of pine needles and branches. The ignition time, which affects the speed of flame propagation, was the most rapid in March, and the fastest ignition time was for pine cones, at 19 seconds, followed by 34 seconds for pine needles, and 256 seconds for pine branches. The pine branches were the last to be ignited due to the effect of density, according to the thickness and specific gravity of the specimen. The peak heat release rate, which is a measurable index of fire intensity, was analyzed for pine cones and found to be 184.28 kW/m2 , while the mean effective heat of combustion was 19.79 MJ/kg, and the total heat release rate was 39.7 MJ/m2 , and these values were higher than those of pine branches and pine needles. Thus, we determined that the flame propagation speed and fire intensity according to the moisture content can be used to evaluate the risk of fire to the water tube section of pine trees. It is suggested that because of the combustion characteristics of the pine cone in March, that is when the forest is most vulnerable to fires.

Study for Blood Homocysteine Levels and d-dimer Levels of Cerebral Infarction Patients and Pattern Identification (뇌경색환자의 혈중 homocysteine 및 d-dimer 농도와 한의변증유형에 대한 연구)

  • The aim of this study was to investigate the differences in blood homocysteine levels and blood d-dimer levels of cerebral infarction patients categorized by Pattern Identification. We studied hospitalized patients within 4 weeks after the onset of stroke who were admitted to the Oriental Internal Medical Department at Semyung University Chungju Oriental Medical Hospital from May 2008 to September 2009. We analyzed risk factors and blood homocysteine levels and blood d-dimer levels accordings to Pattern Identification in Cerebral infarction patients. A total of 49 patients were included in the trial. No statistical significance was noted for any characteristics except body weight and body mass index. Body weight and body mass index were significantly higher Dampness-Phlegm pattern. On past history of patients, prevalence of DM was significantly higher in Fire-Heat pattern than that of other patterns. There was no significant difference of blood homocysteine levels and blood d-dimer levels among Pattern Identification. This study investigated the differences in blood homocysteine levels and blood d-dimer levels of cerebral infarction patients categorized by Pattern Identification. The correlation in homocysteine and d-dimer levels and Pattern Identification was not clarified.

A Study of Correlation Between Change in Pattern Identification and Scandinavian Stroke Scale in Ischemic Stroke Patient Who had Receive Traditional Korean Medical Treatment and Conservative Treatment (한·양방 병행치료를 시행한 뇌경색 환자의 변증지표 변화와 Scandinavian Stroke Scale의 상관관계에 대한 연구)

  • Object : The purpose of this study is to observe the correlation between change in pattern identification symptoms and scandinavian stroke scale in ischemic stroke patient who had receive traditional Korean medical treatment and conservative treatment. Methods : 43 subjects were recruited from patients with ischemic stroke within 30 days of onset. We chose the subjects who had at least one follow up session and had checked the score between last follow up session and first session in pattern identification and scandinavian stroke scale. We also assessed the correlation between pattern identification and scandinavian stroke scale. Results : There were significant negative correlation between pattern identification and scandinavian stroke scale in Fire-heat pattern and positive correlation in Dampness-phlegm pattern. Conclusions : This study provides evidence that collaborative treatment maybe effective in improving neurologic symptoms in ischemic stroke patients diagnosed as Fire-heat pattern. Further studies with larger scale and longer observation period, more neurologicscales scales, control group would be required.

  • Objects : This study propose to compare on general characteristics of acute stroke patients between Fire-Heat group and Deficiency of Yin group. Method : We recruited acute stroke patient within 4 weeks after stroke admitted to Department of Internal Medicine of Kyunghee University Oriental Medicial Center, Dongguk University Ilsan Oriental Medical Center, Kyungwon University Songpa Oriental Medical Centar and Kyungwon University Incheon Oriental Medical Center from April 2007 to August 2009. We investigate age, sex, type of stroke, past history, general characteristics, smoking, drinking alcohol, marriage sasang constitution, etc. Result : It was significant different in sex, smoking, dringking alcohol and marriage between Fire-Heat group and Deficiency Yin group. We need more cases for another result that could associate with diagnosis of stroke. Conclusion : We found that Fire heat group was more associated with lifestyle than deficiency yin group. Further study will be needed to analysis for the diagnosis and the treatments of stroke.

  • This study covers pattern differentiation based on Korean medical references, research trend and modern clinical applications about Sleep-Wake disorders of Diagnostic and Statistical Manual of Mental Disorders(DSM-V) published by American Psychiatric Association. Insomnia disorder is mostly caused by yin deficiency of liver-kidney or liver qi depression and main patterns are heart-kidney non-interaction, deficiency-excess complex pattern containing phlegm-heat due to qi stagnation and blood stasis. Hypersomnolence disorder is more due to yang deficiency rather than yin deficiency and it's major pattern is spleen-kidney yang deficiency. Cataplexy is main feature in narcolepsy and corresponds to depressive psychosis or fainting in terms of Korean Medicine and narcolepsy is assumed to be relevant to liver wind. Breathing-related sleep disorders are related with phlegm-fluid retention brought on spleen deficiency with dampness encumbrance. Pattern of circadian rhythm sleep-wake disorders is combined with yin deficiency of liver-kidney or liver qi depression of insomnia disorder and spleen-kidney yang deficiency or dampness-phlegm of hypersomnolence disorder. Yin deficiency with effulgent fire brought on drugs or alcohol is one of main patterns of substance/medication-induced sleep disorder and combined patterns with yin deficiency of liver-kidney and blood stasis or dampness-phlegm-heat are mostly applied clinically. This study drew major and frequently applied patterns of sleep-wake disorders based on Koran medical literature and modern clinical applications. And that can be the groundwork for the task ahead like clinical practice guideline of sleep-wake disorders containing pattern differentiation, diagnosis and prescriptions.

  • Objectives : By analyzing symptoms of tinnitus, we tried to find out the relationship between deficiency and excess(虛實) and visceral pattern identification(臟腑辨證). By testing Tinnitus Handicap Inventory(THI), we evaluated the quality of life of people suffering from various aspects of tinnitus, as well as comparing treatment efficiency. Methods : 52 patients were recruited in this study who was ill with tinnitus. They wrote out the questionnaire about tinnitus and the THI. Results : The vast majority of high-grade tiny noise patients are deficiency type(虛症). The vast majority of low-grade roar patients are excess type(實症). Tinnitus persistence and feeling of ear occlusion were prominent in low-grade roar patients. low-grade roar patients suggest that overall quality of life is lower than high-grade tiny noise patients. In terms of treatment efficiency, spleen-stomach weakness(脾胃虛弱)-type, stomach heat(胃熱)-type and phlegm-fire(痰火)-type was higher than kidney essence depletion(腎精虧損)-type and liver qi depression(肝氣鬱結)-type regardless of the aspect of tinnitus. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation.

A Study on the Three Yin Diseases(三陰病) in the 『Shanghanlun(傷寒論)』 -Focusing on Prognosis Analysis- (『상한론(傷寒論)』 삼음병(三陰病)에 대한 연구(硏究) - 예후 분석을 중심으로 -)

  • Objectives : An accurate judgment of prognosis when treating diseases is crucial. While the 『Shanghanlun(傷寒論)』 deals with the prognosis of the Three Yin Diseases with great importance, full-scale studies have been lacking. This paper aims to study the Three Yin Diseases with a focus on prognosis analysis. Methods : Among the Three Yin Disease verses, those that could provide clues to prognosis were selected and analysed. Conclusions & Results : When Yang pulse patterns such as long(長脈)·floating(浮脈)·rapid(數脈) pulses and Yang symptoms such as fever, vexing heat, mild perspiration, thirst, warmth in hands and feet are present in Yin disease, it could be taken as signs of Yang Qi restoration. In these situations, Yin Cold pattern such as diarrhea and reversal cold disappear and the prognosis is positive. However, despite Yang pulse patterns and symptoms, there are cases where diarrhea happens as a result of cold dampness being eliminated due to Yang Qi restoration. Also, when Yang Qi starts communicating smoothly after its restoration in the Three Yin Diseases, perspiration can happen. When diarrhea and reversal cold, which are patterns of Yin Cold get worse, with pulse patterns such as unfelt(脈不至)·replete(實脈)·fulminating(脈暴出) pulses, false heat symptoms such as fever and hot flashes happen, accompanied with Yang Qi depleted symptoms such as inability to lie down due to agitation, continuous perspiration, sore throat, dyspnea, and exaggerated breathing happen. When fast pulse, fever, and perspiration are present due to depression and stagnation of ministerial fire, symptoms such as bloody stool with pus, purulent abscess, sore throat, and inability to lie down due to agitation show, which signal negative prognosis. In bad cases of Reverting Yin Disease, there is continuous diarrhea and bloody stool with pus, which can be due to either Kidney Yang deficiency or depression and stagnation of ministerial fire. It could also be caused by excessive heat.

  • The purpose of this paper is to analyze the carbonization pattern and operation characteristics of an MCCB. The MCCB is consisted of the actuator lever, actuator mechanism, bimetallic strip, contacts, up and down operator, arc divider or extinguisher, metal operation pin, terminal part, etc. When the actuator lever of the MCCB is at the top or the internal metal operation pin is in contact with the front part, the MCCB is turned on or off. It means trip state if the actuator lever or the internal metal operation pin moves to back side. In the UL 94 vertical combustion test, white smoke occurred from the MCCB when an average of 17~24 seconds elapsed after the MCCB was ignited and black smoke occurred when an average of 45~50 seconds elapsed. It took 5~6 minutes for the MCCB surface to be half burnt and took an average of 8~9 minutes for the MCCB surface to be entirely burnt. In the UL 94 test, the MCCB trip device operated when an average 7~8 minutes elapsed. If the MCCB trip has occurred, it may have been caused by an electrical problem such as a short-circuit, overcurrent, etc., as well as fire heat. From the entire part combustion test according to KS C 3004, it was found that the metal operation pin could be moved to the MCCB trip position without any electrical problems.
