Effect of regional emergency medical access on the death rate of elderly individuals with ischemic heart disease (지역별 응급의료접근성이 노인의 허혈성 심장질환 사망률에 미치는 영향)

  • Purpose: This study aimed to investigate the relationship between emergency medical service accessibility in different regions and the sudden death rate in elderly patients with ischemic heart disease using data analysis techniques and suggest improvements in regional emergency medical services. Methods: The study collected data from the NEDIS database and Statistics Korea. Data on a total of 75,867 patients aged ≥65 years were reviewed among patients with ischemic heart disease who visited emergency medical institutions in 2018. Frequency analysis, chi-square test, multiple logistic regression analysis, and simple logistic regression analysis were performed using SPSS PC Window 25.0. Results: With an emergency medical resource per 100km2, there was a concomitant reduction in the risk of death. There was a decrease in the death rate by 0.967, 0.970, 0.997, and 0.391 times with the increase in the presence of a fire department, an ambulance, a paramedic, and a regional medical center, respectively. Furthermore, a decrement in the death rate was witnessed 0.844, 0.825, and 0.975 times with the initiation of a local emergency medical center, a local emergency medical institution, and an angiography device, respectively(p <.001). Conclusion: To improve the accessibility of emergency medical services, the population and geometric area of the region should be considered essential factors when deploying emergency medical resources.

  • Since hot toxic gas included in the fire is the most principal reason of the death and has the fluid mechanical characteristics unlike air, smoke control method appropriate for the figure and structure of a fire room is needed and each unit of the smoke control system requests high performance according to this characteristics, for which performance test procedures and evaluation criteria must be established. Domestic criteria involved with the smoke control consist of the pre-investigation by documents and the part inspection, which has lots of problems because they are far from the performance based evaluation method compared with the references of developed countries. Consequently, domestic and international references were compared and analyzed, problems being emerged and standardization scheme of the test procedure was presented.

Post-traumatic Stress Experienced by Firefighters and Paramedics (소방공무원의 외상 후 스트레스 경험)

  • Purpose: The purpose of this study was to describe the post-traumatic stress experiences of firefighters and paramedics. Methods: The participants were 20 fire fighters and paramedics who belong to the Busan Fire Department. Data were collected through in-depth, unstructured audio-taped interviews by the investigator over a six-month period. The participants were asked to describe their post-traumatic stress. The data were analyzed according to Giorgi's method for phenomenological analysis. Results: The interview data were organized by theme into 7 categories that emerged from the analysis. Participants faced various post-traumatic stresses working as fire fighters or paramedics. The categories were suffering from disastrous situations, feeling of fear and helplessness in the face of death, re-experiencing vividly the past traumatic situation, avoiding painful reminders of the trauma, emotional numbing as time passes, suffering from emotional arousal and increased anxiety, and struggling to cope with the post-traumatic stress. Conclusion: This study provides a better understanding of post-traumatic stress experiences from fire fighters and paramedics and the knowledge gained from this study will help in developing appropriate post-traumatic stress management programs.

  • The smoke and toxic gases generated from the building fires are proved to cause human death. Therefore, the necessity and significance of smoke control have been emphasized, and lots of studies for developing improved smoke management system have been carried out. In this study, the experiments were conducted to evaluated a function and performance in newly developed smoke management system using mechanical pressurization. As a result of this experiment, the differential pressure was 40Pa${\sim}$60Pa and the air velocity through the door was 0.7m/s between safety zone and fire zone. The functional pressure control equipment which could make proper pressure and maintain differential pressure between safety zone and fire zone was developed. And it will give a lot of helps to evacuation activity for peoples in building and fire fighting.

A Numerical Study for Fire Safety Evaluation of the Multi-story Residential Buildings -The Effects of the Openings of Stairwell on Fire Characteristics- (다세대주택의 화재안전평가에 대한 수치해석 연구 -계단실 개구부의 개폐가 화재특성에 미치는 영향-)

  • In the events of a fire in the residential building, highly flammable polyurethane foam sofa produce toxic smokes. In this type of fire, the residents of the building can be gotten into the difficulties of evacuating from the fire places or may be to death due to a lot of hot toxic gases. In this study, CFD simulations were carried out to study the effects of the openings of stairwell on the fire characteristics of fire room and stairwell. Also, analysis of fire hazard based on the tenability limits of fire and FED(fractional effective dose) was performed to evaluate the life safety of the residents of the building. In the fire room, maximum temperature was about $290^{\circ}C$, maximum CO concentration was about 4,740 ppm, and the time to incapacitation of residents in fire room was about t=144 s. In the stairwell, temperature and CO concentration in the condition of openings to be open were even lower than those in it to be closed. Time to the tenability limit with respect to smoke visibility in the stairwell with openings, which was open, was shorter than that of it without openings to be open. It has been shown from this study that opening the stairwell openings is able to decrease the fire hazards to the life safety in the multi-story residential building fire.

A Study on the Estimation of Human Damage Caused by the LP Gas Flame in Enclosure using Probit Model

  • The energetic and environmental problems have been getting serious after the revolution of modern industry. Therefore, demand of gas as an eco-friendly energy source is increasing. With the demand of gas, the use of gas is also increased, so injury and loss of life by the fire have been increasing every year. Hence the influence on flame caused by Vapor Cloud Explosion in enclosure of experimental booth was calculated by using the API regulations. And the accident damage was estimated by applying the influence on the adjacent structures and people into the PROBIT model. According to the probit analysis, the spot which is 5meter away from the flame has nearly 100% of the damage probability by the first-degree burn, 27.8% of the damage probability by the second-degree burn and 14.5% of the death probability by the fire.

  • Lately, In case of domestic fire situation, Suffocation due to inflammables has shown higher than direct disaster of the fire among the statistics of death caused by disaster. According to study, Lightweight Hybrid Panel as using the inner or outer wall is made with Polysilicon of the inorganic material, PA and vermiculite, so we make progress to performance experiment and review the density, thermal conductivity properties.

  • The land transportation is the most common way to transport passengers as well as freight among other mode of transportations and consequently more likely to be constructed for faster and convenient travel In this regard, the demand for tunnel constructions will be increasing and the safety inside the tunnel will be considered major concern more than ever. In this paper, we show probabilistic methodology to calculate the personal risk of each evacuee starting from a different location in a tunnel on fire. Passenger evacuation time and smoke spread time are both assumed to be continuous random variables having specific distributions. The evacuation of passengers at each location and the safety facilities inside the tunnel are also crucial factors to calculate the probability of death.

  • Poet, painter and publisher, Roland $Gigu{\grave{e}}re$ is one of Quebec's outstanding figures, inspired by both Surrealism and Quebec nationalism. He participated in contemporary artistic movement 'Phases' and influenced collective self-awareness and political ferment, 'Quiet Revolution'. In La Main au feu(1973), his poetry represent a landscape dominated by darkness in contrast with red color of fire from the volcanic crater. The world is immersed in darkness of despair which allude to the Great Darkness of Quebec society. Acts of violence assume many different forms: crows, black rain, dark flow, frenzy of knife blows. Both things and humans are in the state of absence or lack. Life falls into opacity of death. In the background of dark landscape, we discover Miror, a singular character. Similar to chain of mountains and to bare forest, he is a creature that shape the tragic inner world of poet. He is as like as seismograph that record the tremble of being. Finally, in order to fight the darkness of environment, the poet attempt to use the power of fire of volcanoes. The flow of magma become paintings of his dream and the flame of eruption, poetry of cry toward the sky. 'La main au feu' means the will to resist injustice and repression in the world. The tragic reality is replaced by a dream that become second reality out of reach of the force of hostile external circumstances.

Experience of a Disaster Medical Assistant Team activation in the fire disaster at Jecheon sports complex building: limitation and importance of rescue (제천 스포츠복합건물 화재 재난에서의 권역재난의료지원팀 활동 경험 고찰: 한계점과 구조의 중요성)

  • Objective: This study was designed to report on the progress of the fire at Jecheon sports complex and to assess the adequacy of Disaster Medical Assistant Team (DMAT)'s activities in response to the fire disaster. Methods: We conducted a retrospective review based on camera recordings and medical records that were recorded at the disaster site for assessment of activities. We cooperated with firefighters, police officers, local hospital medical staffs and public health personnel in Jecheon in order to classify patients in the disaster field and to understand the patients' progress. Results: At 15:53, the first request for emergency rescue came to the 119 general emergency call center, and a request for DMAT activation came at 16:28. DMAT arrived at the site at 17:04 and remained active until the following day at 00:43. The total number of casualties was 60, including 27 minimal (Green) patients, 29 expectant (Black) patients, three delayed (Yellow) patients, and one immediate (Red) patient. There were 32 patients who received on-site care by DMAT. Two patients were transferred from a local hospital to Wonju Severance Christian Hospital for hyperbaric oxygen therapy. Conclusion: Twenty-nine victims were found in the sports complex building, and there were 31 mildly to moderately injured patients in this fire disaster. The main cause of death was thought to be smoke suffocation. Although DMAT was activated relatively quickly, it was not able to provide effective activity due to the late rescue and difficulty with fire suppression.
