The relationship between precarious work and unmet dental care needs in South Korea: focus on job and income insecurity (한국 노동시장 불안정성과 미충족 치과의료의 관련성: 고용과 소득 불안정성을 중심으로)

  • Objectives: The aim of this study was to analyze the accessibility of dental care services among individuals with precarious employment in South Korea. Methods: We used the $9^{th}$ wave of the Korean Health Panel data (2015) and included 7,736 wage and non-wage earners in our study. We determined precariousness in the labor market as a combination of employment relationship and job income, and categorized individuals based on this into the following four groups: Group A comprising those who report job and income security, Group B comprising those who experience job insecurity alone, Group C comprising those who report a stable job but low income, and Group D comprising those who experience both job and income insecurity. Accessibility to dental care services was determined by experience of unmet dental care needs and unmet dental care needs caused primarily by financial burden. Logistic regression analyses were used to assess the effect of precarious work on access to dental care services. Results: Individuals with job insecurity (Group B; OR=1.445; 95% CI=1.22-1.70) and both job and income insecurity (Group D; OR=1.899; 95% CI=1.61-2.24) were more likely to have unmet needs than the comparison group. Both groups B and D were also 2.048 (95% CI=1.57-2.66) times and 4.435 (95% CI =3.46-5.68) times more likely, respectively, to have unmet dental care needs caused by financial burden. Education status, health insurance, and health status were all also effective factors influencing unmet dental care needs. Conclusions: Unstable employment and low income resulted in diminished access to dental care services. Therefore, governments should consider health policy solutions to reduce barriers preventing individuals with employment and income instability from accessing adequate dental care.

  • Before the scholarship loan system was implemented at the Korea Scholarship Foundation, the government's role was strengthened by the direct lending of student funds to banks and other financial institutions. However, the low repayment performance of student loans has raised concerns over the future of student loans and the government's financial burden. Moreover, since student loans are repaid even after graduating from college to support low-income families, it is highly unlikely that the repayment rate of student loans will improve unless the employment rate and income level of the borrower improve. In this paper, the final visualization graph is presented of the repayment amount of the student loan through the collection, storage, processing and analysis phase in the Big Data-based system. This could be the basis for visually checking the amount of student loans to come up with various ways to reduce the burden on the current student loan system.

  • Recently public institutions' debt is growing therefore it became an important issue to the level that the government concerns about the possibility of financial burden to reduce the debt. Especially debt of public enterprises in metropolitan areas was in a serious state where debt in late 2013 was 43.2 trillion, which takes approx. 58.4% of 73.9 trillion of debt of all local public enterprises. Sound financial state of local public enterprises is important to public enterprises in metropolitan areas and it may affect seriously financial stability of local governments when public enterprises have financial problems. However, land supply business to form local industrial complexes or local demand for development of public rental housing business always exist; and vitalizing local economy and creating jobs through these businesses are very necessary to develop the areas. However, for local economic development, industirial land business and public rental housing business are needed. In this study, Gwangju Metropolitan City Corporation Ltd is used as a case study to evaluate the local public financial soundness via debt management assessment i.e.(using) the feasibility analysis in the urban development and housing development. As an improvement measure following the result of analysis, for the enhancement of financial soundness of urban innovation corporation, the government and local government shall evaluate and differentiate market demand, price competitiveness, and infrastructure of new town land development project to improve accuracy of project feasibility analysis. Another important insight is that there should be local government-centered management of liabilities of the local government and local public enterprises with the integrated liability management system to reduce the liability of the corporation and solve the issue of debts for local government. This study is significant in that it has analyzed cases from the theoretical aspect to secure financial soundness of national and local public enterprises.

Feasibility of Tax Increase in Korean Welfare State via Estimation of Optimal Tax burden Ratio (적정조세부담률 추정을 통한 한국 복지국가 증세가능성에 관한 연구)

  • The purpose of this study is to present empirical evidence for discussion of financing social welfare via estimating optimal tax burden in the main member countries of the OECD by using Hausman-Taylor method considering endogeneity of explanatory variables. Also, the author produced an international tax comparison index reflecting theoretical hypotheses on revenue-expenditure nexus within a model to compare real tax burden by countries and to examine feasibility of tax increase in Korea. As a result of the analysis, the higher the level of tax burden was, the higher the level of welfare expenditure was, indicating the connection between high burden and high welfare from the aspect of scale. The results also indicated that the subject countries recently entered into the state of low tax burden. Meanwhile, Korea had maintained low burden until the late 1990s but the tax burden soared up since the financial crisis related to the IMF. However, due to the impact of foreign economy and the tax reduction policy, it reentered into the low-burden state after 2009. On the other hand, the degree of social welfare expenditure's reducing tax burden has been gradually enhanced since the crisis. In this context, the current optimal tax burden ratio of Korea as of 2010 may be 25.8%~26.5% of GDP based on input of welfare expenditure variables, a percent that Korea was investigated to be a 'high tax burden-low ITC' country whose tax increase of 0.7~1.4%p may be feasible and that the success of tax system reform for tax increase might be higher probability when compare to others. However, measures of increasing social security contributions and consumption tax were analyzed to be improper from the aspect of managing finance when compared to increase in other tax items, considering the relatively higher ITC. Tax increase is not necessarily required though there may be room for tax increase; the optimal tax burden ratio can be understood as the level that may be achieved on average when compared to other nations, not as the "proper" level. Thus, discussion of tax increase should be accompanied with comprehensive understanding of models of economic developmental difference from nations and institutional & historical attributes included in specific tax mix.

Has the Copayment Ceiling Improved Financial Protection in the Korean National Health Insurance System? Evidence From the 2009 Policy Change

  • Objectives: To relieve the financial burden faced by households, the Korean National Health Insurance (NHI) system introduced a "copayment ceiling," which evolved into a differential ceiling in 2009, with the copayment ceiling depending on patients' income. This study aimed to examine the effect of the differential copayment ceiling on financial protection and healthcare utilization, particularly focusing on whether its effects varied across different income groups. Methods: This study obtained data from the Korea Health Panel. The number of households included in the analysis was 6555 in 2008, 5859 in 2009, 5539 in 2010, and 5372 in 2011. To assess the effects of the differential copayment ceiling on utilization, out-of-pocket (OOP) payments, and catastrophic payments, various random-effects models were applied. Utilization was measured as treatment days, while catastrophic payments were defined as OOP payments exceeding 10% of household income. Among the right-hand side variables were the interaction terms of the new policy with income levels, as well as a set of household characteristics. Results: The differential copayment ceiling contributed to increased utilization regardless of income levels both in all patients and in cancer patients. However, the new policy did not seem to reduce significantly the incidence of catastrophic payments among cancer patients, and even increased the incidence among all patients. Conclusions: The limited effect of the differential ceiling can be attributed to a high proportion of direct payments for services not covered by the NHI, as well as the relatively small number of households benefiting from the differential ceilings; these considerations warrant a better policy design.

Study on Management Plan of the Financial Supervisory Service According to Increase of Risk of Household Debts (금융권 가계부채 위험증가에 따른 금융감독원 관리방안에 관한 연구)

  • The government adopted activation policy of real estate to overcome low economic growth rate. Real estate activation plan adopted by the government raised credit limit by lowering the regulation, and reduced real estate investment cost by reducing the base rate. Also, delayed transfer tax on multi-house owner to activate real estate investment and resolved purchase right resale. Relief of real estate regulate caused increase of housing sales and price increase, and the real estate market changed to overheating aspect such as premium upon completion of lot sale in a short time. Such market atmosphere greatly increased household debs as owners own houses based on 'financial debt' instead of their income. Since 2017, real estate policy was reinforced to reduce household debts and lending rate was raised due to rise of base rate, accordingly, burden of household debt is expected to increase. This research suggested a plan for the Financial Supervisory Service to efficiently manage the financial world by analyzing the cause and problem of household debs.

Factors Affecting Beneficiary Satisfaction on Financial Aid Program for Cancer Patients in Korea (암환자 의료비 지원사업 대상자의 만족도 관련 요인)

  • Limited research has investigated the satisfaction of patients with cancer. This study was performed to explore patients' satisfaction and the related factors. The data were collected by telephone survey for the participants who were beneficiaries on the national financial aid program for cancer patients between January and October in 2009. Student's t-tests and analysis of variance were performed first to determine if the mean satisfaction score differed by the characteristics of study objects, followed by stepwise multiple regression analyses to examine the factors affecting satisfaction. When comparing the relating factors with patients' satisfaction according to the sociodemographic characteristics, the male, old-aged, higher educated, lower cancer stage, lung cancer group showed a significantly higher level of recognition for satisfaction. A public health center, better public relations, recognized more helpful, don't have expectation, put a person to expense, and don't feel pressured for medical cost groups were showed a higher level of recognition for satisfaction. The result of the multiple regression analysis, short waiting time, well known program policy, recognized helping of policy and had the financial burden had significantly influence on the satisfaction of patients with cancer.

An Analysis of College Tuition and Student Aid Policies in the USA (미국 대학의 등록금 정책과 학자금 정책 분석)

  • The purposes of this study were to examine the college tuition and student aid policies in the USA and to provide implications to Korean higher education. To accomplish these purposes, college tuition policies and student financial aid policies by federal and state governments in the United States were analyzed. Regarding tuition policies, the federal government has tried to minimize the impact of recent tuition increases by using methods like establishing various college affordability acts and government regulations. As a result, the federal government established a new law and was able to publicize a list of higher education institutions charging higher tuition levels and increasing tuition and fees at a higher rate. State governments also have introduced and implemented tuition policies such as tuition caps and linking tuition increase to student financial aid. Regarding student financial aid policies, Obama administration has emphasized grant programs which, unlike loan programs, have no further burden placed upon students. These efforts certainly produced desired results. Related policy implications from this study were provided for tackling tuition issues in Korean higher education.

Effects of an Early Nursing Intervention Program for Infants' Development and Mother's Child Rearing in Poverty (빈곤계층 영유아의 발달과 어머니의 양육을 위한 조기간호중재 프로그램의 효과)

  • Purpose: This quasi-experimental study was performed to evaluate the effectiveness of an early nursing intervention program to support mothers of children aged 0-3 yr living in poverty. Methods: In this study, mothers who received financial support from the government were recruited from one city and assigned to an intervention group (24) and comparison group (18). They completed a baseline questionnaire about depression, child rearing burden, agreement on physical punishment, and child temperament. Also, Denver II screening of the children was performed by the researcher. Mothers in the intervention group received a home visit intervention every two weeks for three months. At 3-months post-baseline, questionnaire and Denver II screening were reused to compare these two groups. Results: Mother's depression, child rearing burden, agreement on physical punishment, and child temperament were not significantly different between the two groups. However, the percentage of depression declined only in the intervention group. Mothers in the intervention group showed higher Home Observation for Measurement of the Environment (HOME) scores than mothers in the comparison group. Conclusion: The findings of the study show that this nursing intervention is an effective parenting program. The early nursing program for mothers with infant and toddlers in poverty is effective in promoting HOME, the child rearing home environment.

The Impact of Work Environment and Work-related Stress on the Mental Health of Workers at a Community Childcare Center (지역아동센터 종사자의 직무환경 및 직무스트레스가 정신건강에 미치는 영향)

  • This study examined the affect of work environment and work-related stress on the mental health of employees. It further sought to provide insights into how employees' work environment and mental health might be improved. Results showed that the relationship between parents of work environment is related to the obsession, depression, working burden is related to the obsession, depression, concern. The lower parts of the relationship with parents and working burden effects every factors. Furthermore, levels of financial compensation and working stability also seemed to have adverse affects on mental health. Secondly, excessive working hours and duty trouble have lots of mutual relationship with the mental health. The lower parts of working stress: excess working hours, duty trouble, and imprecise responsibilities affects the mental health. Especially, the excessive working hours affected all nine parts of mental health level which were examined.
