Clinical Results of the Arthroscopic "Multiple Pulled Suture" Technique for Large or Comminuted Bony Bankart Lesion

  • Background: Arthroscopic fixations for large and comminuted bony Bankart lesions are technically difficult. We developed an arthroscopic multiple pulled suture (MPS) technique to restore large and comminuted bony Bankart lesions. Methods: Ten patients (mean age, 49.8 years; range, 31-79 years) underwent bony Bankart repair using the illustrated MPS technique and were then followed for a mean of 27.3 months. A plain radiograph series and three-dimensional computed tomography scans were taken at the initial clinical evaluation and 3 months postoperatively. Outcome measurements included the American Shoulder and Elbow Surgeons (ASES) score, Rowe score, University of California at Los Angeles (UCLA) score, and subjective patient satisfaction, along with surgical complications. Results: Union of an osseous fragment with the glenoid rim was confirmed in all patients on a computed tomography scan 3 months after operation. The osseous fragment was restored to proper articular congruence and reduction. The affected shoulder was stable in nine of the 10 patients. One patient presented with a redislocation after a sports injury 3 years postoperatively. The ASES, Rowe, and UCLA scores improved at the final evaluation, and median patient satisfaction at the final follow-up was 9 of 10 points (range, 6-10 points). Conclusions: The arthroscopic MPS technique for bony Bankart lesions with large or comminuted osseous fragments was a relatively easy and safe method for stable fixation of the osseous fragment. Therefore, the arthroscopic MPS technique resulted in good restoration of stability with high patient satisfaction and low complication rates.

Evaluation of Compost Maturity by Physico-chemical Properties and Germination Index of Livestock Manure Compost (축분 퇴비의 이화학적 특성과 발아지수를 이용한 부숙도 평가)

  • This study was conducted to evaluate the humification grade of compost, based on physico-chemical property and phyto-toxicity during the composting with three kinds of livestock manures and saw dust. The ratios of the compost, which was mixed with pig manure(P) and sawdust(S) were 4 : 6(PS-1), 5 : 5(PS-2) and 6 : 4(PS-3); poultry manure(PO) and sawdust(S) were 4 : 6(POS-1), 5 : 5(POS-2) and 6 : 4(POS-3); cow manure(C) and sawdust(S) were 4 : 6(CS-1), 5 : 5(CS-2) and 6 : 4(CS-3) by volume to volume, and they were decomposed for 60 days. In the result, the temperature in all treatments during composting rapidly increased above 65.4C, and then gradually decreased to around 40C. At 60 day, after the treatment, pH 5.9 ~ 8.0 at the incipient stage increased to 7.6 ~ 8.5, and the C/N ratio was 13.8 ~ 21.1 at the final composting stage. Germination Index(G.I.) showed in the range of 75.1 ~ 94.6 in all treatments at day 60. Therefore, it is likely recommended to take the best humification grade when the temperature maintains above 65C longer than a day at minimum, in the range of pH 6.5 ~ 8.5 for the final compost, under 20 of C/N ratio, and G.I. above 80. The level of G.I. above 80 should be the mature compost which could be used in the field without gas demage to crops.

  • The existing subtract image based intrusion detection system for CCTV digital images has a problem that it can not distinguish intruders from moving backgrounds that exist in the natural environment. In this paper, we tried to solve the problems of existing system by designing real - time intrusion detection system for CCTV digital image by combining subtract image based intrusion detection method and background learning artificial neural network technology. Our proposed system consists of three steps: subtract image based intrusion detection, background artificial neural network learning stage, and background artificial neural network evaluation stage. The final intrusion detection result is a combination of result of the subtract image based intrusion detection and the final intrusion detection result of the background artificial neural network. The step of subtract image based intrusion detection is a step of determining the occurrence of intrusion by obtaining a difference image between the background cumulative average image and the current frame image. In the background artificial neural network learning, the background is learned in a situation in which no intrusion occurs, and it is learned by dividing into a detection window unit set by the user. In the background artificial neural network evaluation, the learned background artificial neural network is used to produce background recognition or intrusion detection in the detection window unit. The proposed background learning intrusion detection system is able to detect intrusion more precisely than existing subtract image based intrusion detection system and adaptively execute machine learning on the background so that it can be operated as highly practical intrusion detection system.

A Comparative Study of Consumption Propensity and Determinants of Purchasing According to Cultural Differences between Baby Boomers and Millennials: Application to Franchise Customers

  • Purpose: The baby boomers and the millennials, who make up the largest share of the market, are showing the gap in their differentiated cultural styles. The purpose of this study is to study the consumption propensity and purchasing determinants of the baby boomers generation and the millennials generation, which have become a very important key pillar in shaping the market based on the cultural dimension model theory of h(=>H)opstead, and to identify the difference Research design, data, and methodology: In order to verify the research hypotheses, The data was collected from the baby boomers and the millennials using online questionnaires. The pre-test was conducted from October 10 to 12, 2019, and the main survey was conducted from October 15 to 25. A total of 230 copies were recalled, and the final 219 copies were used for the final analysis. Results: The consumption propensity between the baby boomers and the millennials differed in resource saving purchase propensity and impulse buying propensity. The baby boomers showed higher resource saving purchase propensity than the millennials, while the millennials showed higher impulse buying propensity. There was a difference in the determinants to purchasing only in the evaluation factor. Results of gender differences by generation, baby boomers differed in their planned purchase propensity, and women were higher. The millennials generation differed in resource saving purchase propensity and awareness propensity for others, all higher in men. In determinants to purchasing, the baby boomers did not differ in gender, and the millennials did differ in product factors. A canonical correlations analysis of the relationship between the baby boomers and the millennials showed a significant relationship between consumption propensity and determinants to purchasing. Also, the baby boomer generation has the biggest link between impulse buying propensity and evaluation factor. The millennials generation showed the biggest link between resource saving purchase and product factors. Conclusions: This study compared consumption propensity and determinants to purchasing of baby boomers and millennials on the basis of Hopstead's cultural dimension model theory to identify differences between generations, presenting practical and theoretical implications for establishing a correct understanding and specific marketing strategy among generation.

Quality of clinical nursing education for new graduate nurses: A concept analysis with a hybrid model (혼종모형을 이용한 신규간호사 임상간호교육의 질에 대한 개념분석)

  • Purpose: The study aimed to examine the concept and attributes of the quality of clinical nursing education for new graduate nurses. Methods: This study adopted a hybrid model introduced by Schwartz-Barcott and Kim. In the theoretical stage, the meaning and attributes of the quality of clinical nursing education for new graduate nurses were determined by analyzing eight articles. In the fieldwork stage, data were collected using semi-structured interviews with five new graduate nurses and seven experienced nurses. The data were analyzed by qualitative content analysis methods developed by Elo and Kyngӓs. In the final analysis, a final result was arrived at comparing, contrasting, and integrating the attributes of the concepts derived in the theoretical and field-work stages. Results: The quality of clinical nursing education for new graduate nurses was identified as excellence or the standard of education for new graduate nurses that would support them in adapting to clinical settings and transitioning to professional nurses. The attributes of the quality of clinical nursing education were founded to possess three dimensions, six categories, and 18 attributes. The multidimensional attributes of the quality of clinical nursing education for new graduate nurses were confirmed as education resources, design, method, content, evaluation, interaction, and outcome under the three dimensions of input, process, and output. Conclusion: The concept and nature of the quality of clinical nursing education observed in this study can be utilized as a basis for the future development, evaluation, and improvement of clinical nursing education for new graduate nurses in healthcare organizations.

  • This study is a design and development study that developed instructional strategies based on distributed cognitive theory for science classes using technology according to procedures that ensured reliability and validity. To develop instructional strategies, development study and validation study were conducted according to design and development research methodology procedures. In the development study, an initial instructional strategy was developed through prior literature review and prior expert review. In the validation study, the instructional strategy was validated using internal validation (expert validation, usability evaluation) and external validation (field application evaluation) methods, and the final instructional strategy was developed. The final instructional strategy consisted of 3 instructional principles, 9 instructional strategies, and 38 detailed guidelines. Through this study, the researcher suggested the suitability of instructional strategies for science classes using technology, the usefulness of blocks and teaching and learning processes, the possibility of using technology as a cognitive tool, the need for teachers' efforts to cultivate teaching capabilities using technology, and the needs lesson plan that takes into account conditions affecting the application of instructional strategies.

Functional Outcome after Reimplantation in Patients Treated with and without an Antibiotic-Loaded Cement Spacers for Hip Prosthetic Joint Infections

  • Purpose: A staged revision with placement of a temporary antibiotic-loaded cement spacer after removal of the implant is the "gold standard" for treatment of chronic prosthetic joint infection (PJI). It enables local delivery of antibiotics, maintenance of limb-length and mobility, easier reimplantation. However, bacterial colonization of spacers and mechanical complications can also occur. The aim of this study is to evaluate functional results and infection control in two-stage treatment of total hip arthroplasty (THA) PJI with and without a spacer. Materials and Methods: A retrospective review of 64 consecutive patients was conducted: 34 underwent two-stage revision using a cement spacer (group A), 30 underwent two-stage revision without a spacer (group B). At the final follow-up, functional evaluation of patients with a THA in site, without PJI recurrence, was performed using the Harris hip score (HHS). Measurement of limb-length and off-set discrepancies was performed using anteroposterior pelvic X-rays. Results: Most patients in group B were older with more comorbidities preoperatively. Thirty-three patients (97.1%) in group A underwent THA reimplantation versus 22 patients (73.3%) in group B (P<0.001). No significant differences in limb-length and off-set were observed. The results of functional evaluation performed during the final follow-up (mean, 41 months) showed better function in patients in group A (mean HHS, 76.3 vs. 55.9; P<0.001). Conclusion: The use of antibiotic-loaded cement spacer seems superior in terms of functional outcomes and reimplantation rate. Resection arthroplasty might be reserved as a first-stage procedure in patients who are unfit, who might benefit from a definitive procedure.

  • International research has actively explored and analyzed the implementation of e-learning platforms by libraries in response to the advancement of online learning models. In contrast, within Korea, while the significance and necessity of online library services and education are acknowledged, there remains a paucity of studies providing detailed methodologies for their implementation. This study aims to propose a practical e-learning platform for public libraries, detailing design guidelines and developing a prototype. Utilizing the Rapid Prototyping methodology, this study reviewed theoretical backgrounds and prior research, selecting Moodle LMS for initial design guidelines. The validity and reliability of the evaluation were assessed by experts and users. Based on the evaluation results, the final design guidelines and prototype of the e-learning platform for libraries were established. The final guidelines consist of four main functions, nine detailed functions, and forty specific instructions.

Development of Evaluation Criteria for the Gifted Curriculum in Visual Arts (미술영재 교육과정 평가준거 개발)

  • The purpose of this study was to develop and validate the evaluation criteria for gifted curriculum in visual arts. For this purpose, a draft of evaluation criteria for gifted curriculum in arts were first developed through literature review and opinions of experts in educational assessment, gifted education, visual art education. Then, the Delphi method was employed to validate the evaluation criteria. Delphi survey was carried out twice with 30 panelists consisting of experts in educational assessment, experts in gifted education, experts in arts(gifted) education and administrators in gifted education. Delphi survey was analyzed using the mean, content validity ratio, and the degree of consensus. Results were as followed. First, 58 evaluation criteria for gifted curriculum in arts were developed through literature review and expert opinions. The evaluation criteria included 12 context evaluation criteria, 25 input evaluation criteria, 12 process evaluation criteria, and 9 product evaluation criteria. Second, through the first and second Delphi survey, 2 evaluation criteria were not validated and finally were removed. The final evaluation criteria included 12 context evaluation criteria, 25 input evaluation criteria, 10 process evaluation criteria, and 9 product evaluation criteria. The evaluation criteria developed by this study can be used as a standard to establish the institution assessment system at the national level and evaluate institutions and programs.

Computerized occlusal analysis: correlation with occlusal indexes to assess the outcome of orthodontic treatment or the severity of malocculusion

  • Objective: The aims of our study were to verify the validity of the T-Scan III system (Tekscan) as an objective occlusal evaluation tool, and to assess the differences between two occlusal indexes - the peer assessment rating (PAR) index and the American Board of Orthodontics objective grading system (OGS) - by comparing the scores derived from the T-Scan III system with the two occlusal indexes and analyzing the correlations between them. Methods: The final study sample included 48 adult volunteers (39 men and 9 women, mean age 24.14±3.16years), after excluding 29 volunteers whose occlusion could not be evaluated by the T-Scan III system due to severe skeletal or occlusal problems. PAR index and OGS scores were assessed using dental study models, and measurements of centric occlusion, protrusive movement, and lateral excursion movement were obtained via the T-Scan III system. The results were analyzed to determine correlations. Results: Occlusal analysis by the T-Scan III system was clinically reliable (p < 0.05), and the PAR index and OGS scores were significantly correlated with several measurements obtained with the T-Scan III system (p < 0.05). Conclusions: The T-Scan III system is a quantitative and reliable method for occlusal evaluation, and represents a potential substitute for occlusal indexes. Compared to the PAR index, the OGS scores of more variables were significantly correlated with the T-Scan measurements.
